{"id":52413,"date":"2023-06-09T14:49:08","date_gmt":"2023-06-09T11:49:08","guid":{"rendered":"https:\/\/www.synevo.md\/?post_type=product&#038;p=52413"},"modified":"2026-02-18T22:56:37","modified_gmt":"2026-02-18T20:56:37","slug":"hpv-detectie-tipuri-cu-risc-crescut-genotipare-extinsa-2","status":"publish","type":"product","link":"https:\/\/old.synevo.md\/ru\/shop\/hpv-detectie-tipuri-cu-risc-crescut-genotipare-extinsa-2\/","title":{"rendered":"MD ADN HPV &#8211; Genotipare extinsa"},"content":{"rendered":"<p><strong><em>Informa\u021bii generale \u0219i recomand\u0103ri<\/em><\/strong><\/p>\n<p>Conform datelor actuale infec\u021bia persistent\u0103 cu genotipuri HPV de risc crescut (oncogene, hrHPV) reprezint\u0103 condi\u021bia necesar\u0103 pentru dezvoltarea cancerului cervical \u0219i a leziunilor sale precursoare. Practic, prezen\u021ba tipurilor HPV oncogene a fost demonstrat\u0103 \u00een aproape toate cazurile de cancer cervical.<\/p>\n<p>Genotipul HPV 16 este considerat cel mai carcinogen, fiind responsabil de aproximativ 55-60% din toate cancerele cervicale. Este urmat de genotipul HPV 18 ce este implicat \u00een 10-15% din cazuri, mai frecvent \u00een cancerele glandulare, adenocarcinoame \u0219i carcinoame adenoscuamoase dec\u00e2t \u00een carcinoamele cu celule scuamoase. Aproximativ alte 10 genotipuri HPV cauzeaz\u0103 restul de 25-35% din cancerele cervicale<sup>1<\/sup>.<\/p>\n<p>\u00cen 2012 Agen\u021bia Interna\u021bional\u0103 pentru Cercetare \u00een Domeniul Cancerului (IARC) a concluzionat c\u0103 sunt dovezi suficiente privind implicarea \u00een carcinogeneza cervical\u0103 a 12 tipuri HPV de risc crescut (HPV16, HPV18, HPV31, HPV33, HPV35, HPV39, HPV45, HPV51, HPV52, HPV56, HPV58 and HPV59), acestea fiind considerate carcinogene 1A. Pentru HPV68 exist\u0103 mai pu\u021bine dovezi, motiv pentru care a fost considerat carcinogen 2A (probabil carcinogen). \u00cen plus, alte 7 \u00a0tipuri HPV \u00eenrudite filogenetic (HPV26, HPV53, HPV66, HPV67, HPV70, HPV73 \u0219i HPV82) au fost identificate \u00een infec\u021bii HPV unice \u00een anumite cazuri rare de cancer cervical, fiind considerate astfel posibil carcinogene (carcinogene 2B). \u00centr-un studiu recent, Halec et al au ar\u0103tat c\u0103 cele 8 tipuri HPV probabil\/posibil carcinogene (grupurile 2A si 2B) sunt biologic active \u00een infec\u021biile unice \u0219i afecteaz\u0103 acelea\u0219i c\u0103i celulare ca \u0219i tipurile HPV considerate pe deplin carcinogene (grupul 1A). Cercet\u0103torii au constatat de asemenea c\u0103 ad\u0103ugarea la grupul celor 13 tipuri HPV cu risc crescut (carcinogene 1 \u0219i 2A) a celor 7 tipuri HPV posibil carcinogene a crescut cu 2.6% procentul cancerele cervicale HPV pozitive. Din acest motiv, s-ar impune o nou\u0103 clasificare a tipurilor HPV carcinogene. Cu toate acestea, av\u00e2nd \u00een vedere faptul c\u0103 sunt foarte rar implicate \u00een cancerele cervicale nu este necesar ca genotipurile HPV din categoria 2B s\u0103 fie incluse \u00een testele de screening sau vaccinuri<sup>2;3<\/sup>.<\/p>\n<p>Stabilirea leg\u0103turii cauzale \u00eentre HPV \u0219i cancerul cervical al\u0103turi de \u00een\u021belegerea epidemiologiei \u0219i a evolu\u021biei naturale a infec\u021biei HPV a condus la un nou model de carcinogenez\u0103 cervical\u0103: achizi\u021bionarea HPV, persisten\u021ba HPV (vs clearance), progresia c\u0103tre leziuni premaligne \u0219i invazia. Cele mai multe infec\u021bii HPV sunt tranzitorii fiind eliminate de c\u0103tre sistemul imun al gazdei \u00een decurs de 1-2 ani. Infec\u021biile persistente se asociaz\u0103 cu un risc crescut de apari\u021bie a leziunilor precanceroase. Astfel, persisten\u021ba timp de 1 an sau 2 ani, \u00een special a HPV16, prezint\u0103 un risc de 20-30% de dezvoltare a CIN3 sau de leziuni mai avansate (CIN3+); CIN3 netratat are 30% probabilitate de a conduce la cancer cervical invaziv \u00een decurs de 30 ani, \u00een timp ce doar 1% din CIN3 tratate vor deveni invazive<sup>1<\/sup>.<\/p>\n<p>Scopul fundamental al screening-ului cancerului cervical este de a preveni morbiditatea \u0219i mortalitatea prin acest timp de cancer. O strategie optim\u0103 de screening este aceea de a identifica leziunile precursoare capabile s\u0103 progreseze c\u0103tre cancer invaziv \u0219i \u00een acela\u0219i timp de a evita detectarea infec\u021biilor HPV tranzitorii. Examenul citologic Babe\u0219-Papanicolaou a redus mult inciden\u021ba cancerului cervical \u00een \u021b\u0103rile care au adoptat programe de screening eficiente. Cu toate acestea, s-au descris limit\u0103ri importante ale citologiei, printre care se numar\u0103 o sensibilitatea redus\u0103 (50-60%) \u0219i de asemenea o specificitate scazut\u0103, ceea ce conduce la necesitatea repet\u0103rii screening-ului la intervale scurte de timp<sup>1;4<\/sup>.<\/p>\n<p>Introducerea \u00een clinic\u0103 a test\u0103rii HPV a avut ca scop \u00eembun\u0103t\u0103\u021birea ratei de detec\u021bie a leziunilor premaligne \u0219i a cancerului cervical. Pentru un screening adecvat este necesar\u0103 doar testarea grupelor de HPV cu risc crescut \u0219i nu a genotipurilor HPV cu risc sc\u0103zut<sup>1<\/sup>.<\/p>\n<p>Ini\u021bial testarea hrHPV a fost utilizat\u0103 pentru triajul citologiilor echivoce (ASC-US), ulteori s-a introdus \u00een SUA cotestarea citologie Pap + hrHPV ADN la femeile \u2265 30 ani, iar \u00een ultimii anit, mai multe studii au ar\u0103tat poten\u021bialul testelor hrHPV de a fi utilizate \u00een screening-ul primar al cancerului cervical datorit\u0103 sensibilit\u0103\u021bii crescute \u00een detectarea leziunilor scuamoase intraepiteliale de grad \u00eenalt \u0219i a valorii predictive negative foarte bune. Cu toate acestea, \u00een studiul Horizon s-au constatat diferen\u021be considerabile \u00eentre rezultatele ob\u021binute prin diverse hrHPV \u00een screening-ul primar al cancerului cervical la femei peste 30 ani<sup>5<\/sup>.<\/p>\n<p>Aceste diferen\u021be se datoreaz\u0103 faptului c\u0103 testele HPV variaz\u0103 \u00een capacitatea lor de a detecta infec\u021biile tranzitorii vs infec\u021biile clinic relevante (infec\u021bii asociate cu leziuni CIN2+ sau av\u00e2nd un risc crescut de conduce la CIN2+, vezi figura de mai jos).<\/p>\n<p><img decoding=\"async\" class=\"aligncenter wp-image-20111 ls-is-cached lazyloaded\" src=\"https:\/\/www.synevo.md\/wp-content\/uploads\/2016\/04\/fig-1.png\" alt=\"fig 1\" width=\"685\" height=\"335\" data-src=\"\/wp-content\/uploads\/2016\/04\/fig-1.png\" \/><\/p>\n<p>Figura 1: Rolul HPV \u00een oncogeneza cervical\u0103 \u2013 tipuri de infec\u021bie HPV<br \/>\n(<em>Adaptare dup\u0103\u00a0<\/em><em>Meijer C. Regulation of HPV tests, Melbourne 2015<\/em>)<\/p>\n<p>Din aceast\u0103 perspectiv\u0103, testele HPV\u00a0 pot fi clasificate \u00een 2 categorii:<\/p>\n<ul>\n<li>teste cu sensibilitate \u0219i specificitate analitic\u0103 crescut\u0103: detecteaz\u0103 toate infec\u021biile cauzate de hrHPV (at\u00e2t cele tranzitorii c\u00e2t \u0219i pe cele cu relevan\u021b\u0103 clinic\u0103);<\/li>\n<li>teste cu sensibilitate \u0219i specificitate clinic\u0103 crescut\u0103: detecteaz\u0103 doar infec\u021biile HPV clinic relevante.<\/li>\n<\/ul>\n<p>\u00cen practic\u0103, selectarea testelor HPV se va face \u00een func\u021bie de scopul urm\u0103rit:<\/p>\n<ul>\n<li>testele cu sensibilitate \u0219i specificitate analitic\u0103 crescut\u0103 sunt utile \u00een studii epidemiologice, pentru stabilirea eficacit\u0103\u021bii vaccinurilor \u0219i monitorizarea prevalen\u021bei HPV<\/li>\n<li>teste cu sensibilitate \u0219i specificitate clinic\u0103 crescut\u0103 sunt utile \u00een\u00a0<u>screening-ul cancerului cervical<\/u>, triajul citologiei echivoce, monitorizarea post-interven\u021bie pentru CIN2+.<\/li>\n<\/ul>\n<p>Astfel, pentru un screening adecvat al cancerului cervical este esen\u021bial s\u0103 se utilizeze teste HPV care s\u0103 detecteze infec\u021biile clinic relevante \u0219i s\u0103 ignore infec\u021biile tranzitorii (asociate cu niveluri joase ale \u00eenc\u0103rc\u0103turii virale).<\/p>\n<p>Un consor\u021biu international a stabilit cerin\u021bele pe care trebuie s\u0103 \u00eendeplineasc\u0103 un test HPV pentru a fi util \u00een screening-ul cancerului cervical:<\/p>\n<div>\u2192 s\u0103 prezinte o sensibilitate clinic\u0103 pentru CIN2+ de cel pu\u021bin 90% din cea a testului de standard<\/div>\n<div>\u2192 s\u0103 demonstreze o specificitate clinic\u0103 pentru CIN2+ de cel pu\u021bin 98% din cea a testului standard<\/div>\n<div>\u2192 s\u0103 prezinte o bun\u0103 reproductibilitate a rezultatelor intra- laborator \u0219i s\u0103 demonstreze o concordan\u021b\u0103 de minimum 87% inter-laboratoare (\u00een acest scop este necesar\u0103 testarea a cel pu\u021bin 500 de probe, dintre care o treime s\u0103 fie pozitive la un test HPV validat clinic)<sup>6;7<\/sup>.<\/div>\n<p><strong>Noul test de screening BD<\/strong><strong> detecteaz\u0103 15 tipuri HPV cu risc crescut cu raportarea <u>tipurilor in grupuri de 2,4 sau 6 virusuri<\/u>: Grup 1: 16\/31\/33\/35\/52\/58; Grup 2: 18\/39\/45\/59; Grup 3: 6\/11, si genotiparea individuala a tipurilor HPV 51\/56\/68<\/strong><strong>. <\/strong><\/p>\n<p><strong><em>\u041f<\/em><\/strong><strong><em>reg\u0103tire paciente<\/em><\/strong>\u00a0\u2013 se va evita recoltarea probei \u00een perioada menstrual\u0103. De asemenea, 48 ore \u00eenainte de prelevarea probei, pacientele vor evita:<\/p>\n<ul>\n<li>raporturile sexuale;<\/li>\n<li>efectuarea du\u0219urilor vaginale;<\/li>\n<li>folosirea tampoanelor intravaginale, contraceptivelor locale, a diverselor geluri, creme \u0219i tratamente vaginale<sup>10<\/sup>.<\/li>\n<\/ul>\n<p><strong><em>Specimen recoltat<\/em><\/strong>\u00a0\u2013 celule ale<em>\u00a0zonei exocervicale<\/em>,\u00a0<em>endocervicale<\/em>\u00a0\u0219i de tranzi\u021bie dintre acestea (<em>zona de transformare<\/em>). Zona de transformare reprezint\u0103 sediul cel mai frecvent al leziunilor preneoplazice (turn-over crescut, expus\u0103 microtraumatismelor, sensibilitate crescut\u0103 la ac\u021biunea HPV), acolo unde epiteliul scuamos al exocolului se continu\u0103 cu cel de tip glandular de la nivel endocervical prin intermediul unei zone de metaplazie scuamoas\u0103; recoltarea se efectueaz\u0103 cu ajutorul periu\u021bei cervicale cu cap deta\u0219abil (vezi fig.1). Pentru recoltarea \u00een mediu lichid sunt recomandate dispozitivele din plastic. De obicei nu se recomand\u0103 \u0219tergerea colului uterin \u00eenainte de recoltare, dar la nevoie se poate tampona u\u0219or cu un tifon. Cu partea central\u0103 fix\u0103, se rote\u0219te complet periu\u021ba\u00a0<strong><em>\u00een sensul acelor de ceasornic de cinci ori<\/em><\/strong>. Reactivii sunt con\u021binu\u021bi \u00een 3 tuburi (G1, G2 \u0219i G3) fiind capabili s\u0103 detecteze 15 hrHPV (6, 11, 16, 18, 31, 33, 35, 39, 45, 51, 52, 56, 58, 59, \u0219i 68) precum \u0219i un control intern reprezentat de un fragment ADN din gena beta-globinei. De asemenea, la fiecare rulare a probelor se utilizeaz\u0103 HPV Positive Control \u0219i\u00a0 HPV Negative Control. Genotipurile HPV vor fi raportate <strong><u>in grupuri de 2,4 sau 6 virusuri<\/u>: Grup 1: 16\/31\/33\/35\/52\/58; Grup 2: 18\/39\/45\/59; Grup 3: 6\/11, si genotiparea individuala a tipurilor HPV 51\/56\/68<\/strong><strong>.<\/strong><\/p>\n<p><strong><em>Valori de referin\u021b\u0103 \u0219i raportarea rezultatelor<\/em><\/strong><\/p>\n<p>Valori de referin\u021b\u0103: HPV ADN pentru 15 genotipuri de risc crescut \u2013 nedetectabil.<\/p>\n<p>\u00cen cazul unui rezultat HPV ADN pentru 15 genotipuri de risc crescut pozitiv se va raporta genotipul sau grupul de genotipuri pentru care s-a ob\u021binut un rezultat pozitiv<sup>10<\/sup>.<\/p>\n<p><strong><em>Limite<\/em><\/strong><strong><em>\u00a0\u0219i interferen\u021be<\/em><\/strong><\/p>\n<p>Prezen\u021ba de s\u00e2nge sau mucus \u00een prob\u0103 nu interfer\u0103 cu testul<sup>10<\/sup>.<\/p>\n<p><strong>\u0411\u0438\u0431\u043b\u0438\u043e\u0433\u0440\u0430\u0444\u0438\u044f<\/strong><\/p>\n<ol>\n<li>Saslow D, Solomon D, Lawson HW, Killackey M, Kulasingam SL, Cain J,et al.
