{"id":45683,"date":"2022-05-23T10:14:09","date_gmt":"2022-05-23T07:14:09","guid":{"rendered":"https:\/\/www.synevo.md\/?post_type=product&#038;p=45683"},"modified":"2025-05-06T11:15:26","modified_gmt":"2025-05-06T08:15:26","slug":"borrelia-anticorpi-igg-igm-blot","status":"publish","type":"product","link":"https:\/\/old.synevo.md\/ru\/shop\/borrelia-anticorpi-igg-igm-blot\/","title":{"rendered":"\u0411\u043e\u0440\u0440\u0435\u043b\u0438\u0438 - \u0430\u043d\u0442\u0438\u0442\u0435\u043b\u0430 IgG, IgM (\u0418\u043c\u043c\u0443\u043d\u043e\u0431\u043b\u043e\u0442)"},"content":{"rendered":"<p><em><strong>Informa\u0163ii generale Borrelia-Anticorpi IgG, IgM (Blot)<\/strong><\/em><\/p>\n<p>Borrelia burgdorferi (B.b.) face parte din ordinul Spirochaetales, familia Spirochaetaceae, genul Borrelia \u015fi a fost izolat\u0103 \u00een SUA, in anul 1982, din intestinul c\u0103pu\u015fei Ixodes dammini, de catre Dr. Willy Burgdorfer, al c\u0103rui nume \u00eel poart\u0103<sup>2<\/sup>.<\/p>\n<p>Studiul diferen\u0163elor antigenice dintre tulpinile izolate \u00een America \u015fi Europa (izolat\u0103 din capu\u0219a Ixodes ricinus) a permis recunoa\u015fterea implic\u0103rii \u00een patologia uman\u0103 a trei genospecii care alc\u0103tuiesc\u00a0<u>complexul Borrelia burgdorferi sensu lato<\/u>: Borrelia burgdorferi sensu stricto (ce cuprinde toate tulpinile izolate \u00een America de Nord) dar \u0219i B. garinii \u015fi B. afzelii.<\/p>\n<p>\u00cen Europa circul\u0103 toate cele trei genospecii, dar predomin\u0103 B. garinii \u015fi B. afzelii1;<sup>2;4<\/sup>. \u00cen plus fa\u0163\u0103 de cele 3 genospecii men\u0163ionate s-a constatat recent implicarea, \u00een unele cazuri din Europa, \u015fi a unei alte specii: Borrelia spielmanii3. Odata cu descifrarea genomului bacterian al Borreliei s-au izolat numeroase antigene cu rol important \u00een patogenie \u015fi diagnostic<sup>2<\/sup>.<\/p>\n<p>Acest test este folosit ca\u00a0<strong>metod\u0103 de confirmare\u00a0<\/strong>(confirm\u0103 prezen\u0163a anticorpilor IgM \u015fi IgG specifici fa\u0163\u0103 de bacteriile apar\u0163in\u00e2nd grupului Borrelia burgdorferi sensu lato), a rezultatelor pozitive \u015fi echivoce ob\u0163inute prin screeningul serologic, realizat cu teste imunoenzimatice<sup>1;2;3;4<\/sup>.<\/p>\n<p>Antigenele specifice sunt produse prin recombinare, prin folosirea unor tulpini de E coli. \u00cen compara\u0163ie cu lizatul conven\u0163ional, folosirea proteinelor recombinante prezint\u0103 un avantaj, deoarece este mimat\u0103 exprimarea antigenic\u0103 in vivo \u015fi este oferit\u0103 \u00een acela\u015fi test si o combina\u0163ie de antigene, provenite de la cele patru genospecii de Borrelia.<\/p>\n<p>Spre deosebire de testele screening, testul de confirmare permite o detec\u0163ie selectiv\u0103 a anticorpilor prezen\u021bi \u00een diferitele etape ale infec\u0163iei. Astfel, prezen\u021ba de antigene tip OspC \u015fi p41 este asociat\u0103 cu r\u0103spunsul imun de tip IgM, \u00een timp ce antigenele tip OspB, p39 \u015fi p58 sunt biomarkeri cert asocia\u0163i cu r\u0103spunsul imun de tip IgG. De asemenea, producerea de anticorpi sintetizati \u00eempotriva antigenului de tip VlsE este detectat\u0103 ca \u0219i r\u0103spuns imun de tip IgG<sup>2;3;4<\/sup>.<\/p>\n<p><strong>Preg\u0103tire pacient\u00a0<\/strong>\u2013 preferabil \u00e0 jeun (pe nem\u00e2ncate)<sup>3<\/sup>.<\/p>\n<p><strong>Specimen recoltat\u00a0<\/strong>\u2013 s\u00e2nge venos<sup>3<\/sup>.<\/p>\n<p><strong>Recipient de recoltare\u00a0<\/strong>\u2013 vacutainer f\u0103r\u0103 anticoagulant cu\/f\u0103r\u0103 gel separator<sup>3<\/sup>.<\/p>\n<p><strong>Prelucrare necesar\u0103 dup\u0103 recoltare\u00a0<\/strong>\u2013 se separ\u0103 serul prin centrifugare<sup>3<\/sup>.<\/p>\n<p><strong>Volum prob\u0103\u00a0<\/strong>\u2013 minim 2 mL ser<sup>3<\/sup>.<\/p>\n<p><strong>Cauze de respingere a probei\u00a0<\/strong>\u2013 specimen intens hemolizat, hemolitic sau contaminat bacterian<sup>3<\/sup>.<\/p>\n<p><strong>Stabilitate\u00a0\u00a0 prob\u0103\u00a0\u00a0\u00a0<\/strong>\u2013\u00a0 1 s\u0103pt\u0103m\u00e2n\u0103\u00a0 la\u00a0 2-8\u00b0C;\u00a0\u00a0 timp \u00eendelungat la\u00a0 -20\u00b0C;\u00a0 evita\u0163i decongelarea\/ recongelarea repetat\u0103.<\/p>\n<p><strong>Metod\u0103\u00a0<\/strong>-\u00a0<u>Microblot Array<\/u><\/p>\n<p>Testul utilizeaz\u0103 antigene relevante pentru diagnosticul serologic de infec\u021bie cu Borrelia si\u00a0Anaplasma phagocytophilum:<\/p>\n<ul>\n<li><strong>VlsE Ba, Bg, Bs<\/strong>: proteine de fuziune, reprezint\u0103 epitopii imunodominan\u0163i tip VlsE ai diferitelor genospecii<\/li>\n<li><strong>p83:\u00a0<\/strong>proteina extracelular\u0103 rezultat\u0103 din clivarea proteinei p100<\/li>\n<li><strong>p58<\/strong>: transportor membranar (OppA-2 oligopeptid permeaza), biomarker de boala Lyme<\/li>\n<li><strong>p41 Ba, Bs<\/strong>: protein\u0103 flagelar\u0103 provenit\u0103 de la burgdorferi sensu stricto, specific\u0103 pentru stadiul precoce al infec\u021biei<\/li>\n<li><strong>p39 (BmpA)<\/strong>: proteina 39 asociat\u0103 membranei, specific\u0103 pentru stadiul avansat al infec\u021biei<\/li>\n<li><strong>OspB:\u00a0<\/strong>proteina de suprafa\u021b\u0103, specific\u0103 pentru stadiul avansat al infec\u021biei, se asociaz\u0103 cu artrita Lyme<\/li>\n<li><strong>OspA Ba, Bg, Bs<\/strong>: protein\u0103 major\u0103 de suprafa\u0163\u0103<\/li>\n<li><strong>OspC Ba, Bg, Bs, Bsp<\/strong>: proteine de suprafa\u0163\u0103 provenite de la: garinii, B sensu stricto, B. afzelii \u015fi B. se pot ob\u021bine urm\u0103toarele rezultate semicantitative:<\/p>\n<p><strong>&lt; 6 AU (<\/strong>intensitate spot<strong>) \u2013 Negativ<\/strong><\/p>\n<p><strong>6 \u2013 7 AU \u2013 Echivoc<\/strong><\/p>\n<p><strong>&gt; Proteina VlsE este un biomarker precoce pentru r\u0103spunsul de tip IgG, dar se identific\u0103 adesea \u015fi in cadrul r\u0103spunsul imun ce \u00eenso\u021beste manifest\u0103rile tardive ale borreliozei.<\/p>\n<p>Prezenta anticorpilor anti-Borrelia \u00een LCR este sugestiv\u0103 pentru afectarea neurologic\u0103 din boala Lyme<sup>3<\/sup>.<\/p>\n<p><strong>Limite \u015fi interferen\u0163e<\/strong><\/p>\n<p>Prin folosirea unor antigene recombinante selective, din structura Borrelia, se exclud reac\u0163iile \u00eencruci\u015fate care ar putea s\u0103 apar\u0103 \u00een cadrul unor infec\u021bii, precum : sifilis, febr\u0103 recurent\u0103 sau leptospiroz\u0103.<\/p>\n<p>Totu\u015fi, \u00een cazul unei infec\u0163ii active sau al unor antecedente de infec\u021bie cu Treponema pallidum, serurile pot s\u0103 con\u021bin\u0103 uneori anticorpi \u00eendrepta\u021bi \u00eempotriva antigenelor tip p41 sau p41\/i, \u00een aceast\u0103 situa\u0163ie fiind necesare teste de excludere a acestei infec\u021bii (VDRL \u015fi TPHA)<sup>2<\/sup>.<\/p>\n<p>Este obligatoriu ca testele serologice s\u0103 fie corelate \u00eentotdeauna cu anamneza \u015fi datele clinice.<\/p>\n<p>Dac\u0103 exist\u0103 totu\u015fi suspiciune clinic\u0103 de boal\u0103 Lyme, iar rezultatul test\u0103rii este negativ sau echivoc, se recomand\u0103 repetarea recolt\u0103rii, dup\u0103 3 s\u0103pt\u0103m\u00e2ni<sup>1<\/sup>.<\/p>\n<p><strong>\u0411\u0438\u0431\u043b\u0438\u043e\u0433\u0440\u0430\u0444\u0438\u044f:<\/strong><\/p>\n<ol>\n<li>Bettina Wilske, Barbara J.B.
