Confirmarea diagnosticului de infectie acuta se poate baza pe detectarea unor secven\u021be unice din ARN-ul viral (prin testarea amplific\u0103rii acidului nucleic \u2013 RT-PCR) sau detectarea proteinelor virale (teste antigenice) din probe provenite de la persoane infectate<sup>2<\/sup>. Antigenele virale pot fi identificate la nivelul tractului respirator doar in momentul cand virusul se replica activ, de aceea testele de detectare a antigenelor sunt utile in clinica medicala pentru identificarea infec\u021biei acute sau precoce. Studiile actuale sugereaz\u0103 ca testul antigenic ar trebui efectuat \u00een primele 5-9 zile de la data debutului simptomatologiei specifice COVID-19 sau de la momentul expunerii (contact cu persoane confirmate). Dac\u0103 data expunerii nu este cunoscut\u0103, testul antigenic ar trebui efectuat c\u00e2t mai cur\u00e2nd posibil<sup>1;2;3;4;5;6<\/sup>. Testarea antigenului este indicata in perioada de timp \u00een care s-a observat ca exista cea mai mare \u00eenc\u0103rcatura viral\u0103 la persoanele infectate cu SARS \u2013 CoV \u2013 2. Prin urmare, testele antigenice sunt cel mai sensibile \u00een faza ini\u021bial\u0103 infec\u021biei, in primele zile de la debutul simptomelor<sup>8<\/sup>. Aceste teste pot face parte, de asemenea, din recomandarile de testare periodica pentru identificarea \u0219i izolarea persoanelor infectate, inclusiv a celor asimptomatici<sup>1;2;3;4;5;6<\/sup>.<\/p>\n<h5>Recomandari pentru determinarea Antigen \u2013 SARS-COV-2:<\/h5>\n<p>Utilizarea testelor pentru detec\u021bia antigenului SARS-CoV-2 poate fi luat\u0103 \u00een considerare pentru persoanele cu simptome compatibile cu COVID-19 \u00een conditii de prevalenta crescuta, cand\u00a0<strong>rata de pozitivitate este mare sau foarte mare<\/strong>, de exemplu \u226510%. Aceste teste pot ajuta la reducerea transmiterii ulterioare, prin detectarea timpurie a cazurilor cu contagiozitate ridicat\u0103, permi\u021b\u00e2nd \u00eenceperea rapid\u0103 a supravegherii epidemiologice a contac\u021bilor<sup>7<\/sup>.<\/p>\n<p>Pentru majoritatea cazurilor cu prevalen\u021b\u0103 sc\u0103zut\u0103, ECDC recomand\u0103 \u00een continuare utilizarea RT- PCR pentru cre\u0219terea valorii predictive pozitive. Testele antigenice pot fi, de asemenea, luate \u00een considerare \u00een situa\u021bii specifice, unde nu exist\u0103 deloc capacitate de\u00a0<a href=\"https:\/\/www.synevo.md\/programari-analize\/\" rel=\"nofollow noopener\" target=\"_blank\"><strong>testare prin RT-PCR<\/strong><\/a>.<\/p>\n<p>Testarea se recomanda urmatoarelor categorii de persoane\/pacienti:<\/p>\n<p>A. Persoane nevaccinate \u00eempotriva SARS-CoV-2, persoane cu schema de vaccinare incomplet\u0103, persoane care au trecut prin boal\u0103, dar la care au trecut mai mult de 180 zile de la data primului test pozitiv, persoane care nu au avut niciodat\u0103 boala si care se incadreaza in oricare din urmatoarele categorii:<\/p>\n<ol>\n<li>Persoane simptomatice<\/li>\n<li>Contac\u021bi direc\u021bi ai cazurilor confirmate, \u00een a 3-a \u2013 a 5-a zi de la contact, inclusiv personal medico-sanitar si auxiliar<\/li>\n<li>Pacien\u0163i asimptomatici cu imunosupresie \u00een contextul bolii sau indus\u0103 medicamentos, cu maximum 48 de ore \u00eenainte de internarea \u00een spital \u00een cazul testarii RT-PCR, iar \u00een cazul testarii cu teste antigenice rapide, testare \u00een ziua internarii sau cu o zi anterior intern\u0103rii<\/li>\n<li>Pacien\u021bi oncologici asimptomatici afla\u021bi \u00een curs de chimioterapie \u0219i\/sau radioterapie:<\/li>\n<\/ol>\n<ol>\n<li>\n<ul>\n<li>Pacien\u021bi oncologici asimptomatici afla\u021bi \u00een curs de chimioterapie \u2013 cu maximum 48 de ore inainte de fiecare cur\u0103, respectiv de fiecare prezentare la spital pentru monitorizare, \u00een cazul test\u0103rii RT-PCR, iar \u00een cazul test\u0103rii cu teste antigenice rapide, testare \u00een ziua intern\u0103rii sau cu o zi anterior intern\u0103rii<\/li>\n<li>Pacien\u021bi oncologici asimptomatici afla\u021bi \u00een curs de radioterapie \u2013 \u00eenainte de prima \u0219edin\u021b\u0103 \u0219i apoi la 14 zile, p\u00e2n\u0103 la terminarea curei, indiferent de testul utilizat<\/li>\n<\/ul>\n<\/li>\n<\/ol>\n<ol>\n<li>Pacien\u021bi oncologici asimptomatici \u00eenainte de interven\u021bii operatorii sau manevre invazive, cu maximum 48 de ore \u00eenaintea interven\u021biei \/manoperei, \u00een cazul test\u0103rii RT-PCR, iar \u00een cazul test\u0103rii cu teste antigenice rapide, testare \u00een ziua internarii sau cu o zi anterior intern\u0103rii<\/li>\n<li>\u00censo\u021bitorii pacien\u021bilor copii de la punctele 3., 4. \u0219i 5., precum \u0219i \u00eenso\u021bitorii copiilor cu COVID-19, \u00een ziua intern\u0103rii sau cu o zi anterior intern\u0103rii<\/li>\n<li>Pacien\u021bi hemodializa\u0163i asimptomatici \u2013 de 2 ori pe lun\u0103, indiferent de testul utilizat<\/li>\n<li>Persoane institu\u021bionalizate asimptomatice, inclusiv pacien\u021bii cronici psihiatrici cu durata de internare care dep\u0103\u0219e\u0219te o lun\u0103, la internare, persoane din penitenciare, la admiterea \u00een unitate \u0219i ulterior cu frecven\u021ba men\u021bionat\u0103<\/li>\n<li>Personalul de \u00eengrijire din centre reziden\u021biale, sec\u021biile de psihiatrie cronici \u0219i personalul din penitenciare<\/li>\n<li>Gravidele asimptomatice care se afl\u0103 \u00een carantin\u0103 la domiciliu (avand istoric de calatorie sau care au fost contact direct cu un caz confirmat), \u00een ziua a 3-a \u2013 a 5-a.<\/li>\n<\/ol>\n<p>B. Personalul medico-sanitar si auxiliar asimptomatic care face parte din una din categoriile de mai jos:<\/p>\n<ul>\n<li>nevaccinat \u00eempotriva SARS-CoV-2 sau cu schema de vaccinare incomplet\u0103 si care nu a avut niciodat\u0103 boala;<\/li>\n<li>nevaccinat \u00eempotriva SARS-CoV-2 sau cu schema de vaccinare incomplet\u0103 si care a trecut prin boal\u0103, dar la care au trecut mai mult de 180 zile de la data primului test pozitiv<\/li>\n<\/ul>\n<p><strong><em>Specimen recoltat\u00a0<\/em><\/strong>-\u00a0<strong>Exsudat nazofaringian\u00a0<\/strong>recoltat conform procedurilor standard, \u00een recipient special (cu mediu de transport).<\/p>\n<p><strong>Exsudat nazofaringian \u2013\u00a0<\/strong>Solicita\u021bi pacientului s\u0103 tu\u0219easc\u0103 pentru a mobiliza secre\u021biile c\u0103tre faringele posterior. Inclina\u021bi spre spate capul pacientului \u00eentr-un unghi de aproximativ 70 de grade. Tamponul se introduce cu grij\u0103 \u00een narin\u0103 si se coboar\u0103 pe plan\u0219eul inferior al acesteia p\u00e2n\u0103 c\u00e2nd se ajunge \u00een nazofaringe. Tamponul se r\u0103suce\u0219te de 2-3 ori, dup\u0103 care se extrage u\u0219or.
