{"id":12133,"date":"2020-05-05T23:16:50","date_gmt":"2020-05-05T21:16:50","guid":{"rendered":"https:\/\/www.synevo.ro\/shop\/hpv-adn-detectie-tipuri-cu-risc-crescut-genotipare-16-18-cobas%e2%80%8b%e2%80%8b-hpv\/"},"modified":"2026-02-19T00:47:57","modified_gmt":"2026-02-18T22:47:57","slug":"hpv-adn-detectie-tipuri-cu-risc-crescut-genotipare-16-18","status":"publish","type":"product","link":"https:\/\/old.synevo.md\/ru\/shop\/hpv-adn-detectie-tipuri-cu-risc-crescut-genotipare-16-18\/","title":{"rendered":"\u0414\u041d\u041a \u0412\u041f\u0427 \u2013 \u0432\u044b\u044f\u0432\u043b\u0435\u043d\u0438\u0435 \u0442\u0438\u043f\u043e\u0432 \u0432\u044b\u0441\u043e\u043a\u043e\u0433\u043e \u043e\u043d\u043a\u043e\u0433\u0435\u043d\u043d\u043e\u0433\u043e \u0440\u0438\u0441\u043a\u0430: \u0433\u0435\u043d\u043e\u0442\u0438\u043f\u0438\u0440\u043e\u0432\u0430\u043d\u0438\u0435 16, 18"},"content":{"rendered":"<p style=\"text-align: justify;\"><span style=\"font-size: 12pt;\"><strong><em>Informa\u021bii generale<\/em><\/strong><\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-size: 12pt;\">Testul <strong>HPV ADN &#8211; detec\u021bie tipuri cu risc crescut: genotipare 16, 18 (cobas\u00ae HPV) <\/strong>este disponibil\u00a0at\u00e2t ca serviciu izolat pentru screening-ul primar al cancerului cervical, c\u00e2t \u0219i ca test reflex pentru pacientele care au solicita anterior Laboratorului Synevo <span style=\"text-decoration: underline;\">Citologie cervico-vaginala Babes &#8211; Papanicolaou in mediu lichid<\/span> \u0219i\u00a0pentru care exist\u0103 recomandarea de a efectua testul HPV.\u00a0 <\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-size: 12pt;\">\u00cen cazul\u00a0\u00een care testul HPV este utilizat ca test reflex,\u00a0acesta va putea fi solicitat \u00eentr-un interval de <strong><span style=\"text-decoration: underline;\">3 s\u0103pt\u0103m\u00e2ni<\/span><\/strong> de la data recolt\u0103rii probei pentru citologia cervico-vaginal\u0103, flaconul Sure Path fiind men\u021binut \u00een aceast\u0103 perioad\u0103 \u00een laborator \u00een condi\u021bii de refrigerare.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-size: 12pt;\">Testul HPV efectuat pe platforma automat\u0103 Cobas 6800 (Roche Diagnostics) detecteaz\u0103 printr-o <strong>metod\u0103 real-time PCR,<\/strong> genotipurile 16, 18.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-size: 12pt;\">Testul a fost recent aprobat de FDA (US Food and Drug Administration) pe baza datelor furnizate de studiul ATHENA (Addressing THE Need for Advanced HPV Diagnostics) ce a \u00eenrolat peste 47 000 femei din SUA (cu v\u00e2rst\u0103 \u226521 ani) cu ocazia efectu\u0103rii screening-ului de rutin\u0103 pentru cancerul cervical. Unul din obiectivele-cheie ale acestui studiu a fost <span style=\"text-decoration: underline;\">validarea clinic\u0103<\/span> a noului test HPV pentru triajul pacientelor cu citologie ASC-US (Atypical Squamous Cells of Undetermined Significance Population). <\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-size: 12pt;\">Prevalen\u021ba citologiei ASC-US \u00een studiul ATHENA a fost de 4.1%; la pacientele cu acest tip de modificare s-au efectuat, al\u0103turi de testarea HPV, colposcopie \u0219i biopsie cervical\u0103 (chiar \u0219i \u00een absen\u021ba unor leziuni vizibile colposcopic). Validarea clinic\u0103 a testului HPV la pacientele cu ASC-US s-a ob\u021binut prin stabilirea performan\u021belor caracteristice \u00een detectarea leziunilor \u2265 CIN2 \u0219i a celor \u2265CIN3 \u0219i prin compararea cu testul considerat standard \u00een SUA. Astfel, sensibilitatea pentru leziunile cervicale \u2265 CIN2 \u0219i a celor \u2265CIN3 a fost de 90% \u0219i respectiv 93.5%, iar specificitatea pentru leziunile cervicale de grad \u00eenalt a fost de 71% &#8211; performan\u021be comparabile cu testul HPV standard. <\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-size: 12pt;\">Pe de alt\u0103 parte, \u00een compara\u021bie cu testul HPV standard, noul test prezint\u0103 avantajul identific\u0103rii separate a genotipurilor HPV 16 \u0219i 18 <span style=\"text-decoration: underline;\">\u00een cadrul aceleia\u0219i rul\u0103ri a probei<\/span>, ceea ce aduce o informa\u021bie suplimentar\u0103 \u00een management-ul pacientelor\u00a0cu ASC-US prin <span style=\"text-decoration: underline;\">detectarea femeilor care prezint\u0103 riscul cel mai mare de a dezvolta leziuni premaligne<\/span>.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-size: 12pt;\">HPV-16 \u0219i HPV-18 sunt responsabile de aproximativ 70% dintre cancerele invazive de col uterin. \u00cen studiul ATHENA s-a constatat c\u0103 genotiparea specific\u0103 pentru tipurile 16 \u0219i 18 a avut un impact dramatic asupra riscului absolut \u0219i a riscului relativ de leziuni \u2265CIN2 \u0219i \u2265CIN3 la pacientele cu ASC-US. Astfel riscul absolut de leziuni \u2265CIN2 la pacientele cu HPV-16+ a fost de 31.5%, iar cel pentru leziuni \u2265CIN3 de 20%. Riscul relativ pentru \u2265CIN2 la pacientele cu HPV-16+ comparativ cu pacientele av\u00e2nd un genotip HPV de risc crescut altul dec\u00e2t 16 sau 18 a fost de 3.7, iar riscul relativ pentru \u2265CIN3 de 4.5. Pe de alt\u0103 parte, pozitivitatea doar pentru HPV-18 nu a conferit un risc relativ semnificativ crescut de leziuni \u2265CIN2 \u0219i \u2265CIN3 comparativ cu grupul celorlate 12 tipuri HPV oncogene. Aceast\u0103 constatare ar putea fi consecin\u021ba faptului c\u0103 \u00een studiile prospective leziunile intraepiteliale de grad \u00eenalt asociate cu HPV-18 se dezvolt\u0103 mai lent sau r\u0103m\u00e2n clinic oculte (probabil \u00een canalul endocervical) \u00een compara\u021bie cu cele asociate tipului HPV 16. Efectul genotipului 18 va fi investigat \u00een continuare \u0219i definit mai bine \u00een faza de urm\u0103rire la 3 ani a studiului ATHENA.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-size: 12pt;\">Implica\u021biile clinice ale riscului crescut de leziuni \u2265CIN2 asociate cu HPV-16 sunt considerabile. De\u0219i riscul de leziuni\u00a0 \u2265CIN2 la pacientele cu ASC-US \u0219i o infec\u021bie cu alt tip HPV oncogen este suficient de mare pentru a face necesar\u0103 colposcopia, totu\u0219i, datorit\u0103 sensibilit\u0103\u021bii reduse a colposcopiei (dovedit\u0103 recent), \u00a0pacientele cu ASC-US \u0219i HPV-16+ la care colposcopia ini\u021bial\u0103 nu indic\u0103 leziuni\u00a0 \u2265CIN2 pot beneficia de o monitorizare mai intensiv\u0103. \u00cen concluzie, aceste date noi privind riscul dependent de genotipul HPV ar putea fi foarte\u00a0 utile dac\u0103 se va lua \u00een considerarea revizuirea ghidurilor actuale de management al femeilor cu ASC-US.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-size: 12pt;\">De\u0219i nu au fost \u00eenc\u0103 publicate, exist\u0103 disponibile date privind riscul de leziuni\u00a0 \u2265CIN2 la pacientele \u2265 30 ani cu citologie negativ\u0103 \u0219i rezultate pozitive la testul HPV Cobas 6800. Astfel, \u00een prezen\u021ba unui rezultat pozitiv pentru un tip HPV oncogen riscul de leziuni \u2265CIN2 este 7.9 ori mai mare dec\u00e2t pentru un rezultat negativ. Riscul este de 14.4 ori mai mare pentru HPV-16 \u0219i\/sau HPV-18 comparativ cu un rezultat negativ \u0219i de 2.4 ori mai mare comparativ cu un rezultat pozitiv pentru celelalte 12 genotipuri cu risc crescut. <\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-size: 12pt;\">Rezultatele studiului ATHENA indic\u0103 faptul c\u0103 1 din 10 femei \u226530 ani testat\u0103 pozitiv pentru HPV 16 sau 18 a avut leziune intraepitelial\u0103 de grad \u00eenalt cu toate c\u0103 rezultatele citologiei \u00een mediu lichid au fost negative.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-size: 12pt;\">Importan\u021ba identific\u0103rii genotipului 16 sau 18 la femeile cu citologie negativ\u0103 a fost demonstrat\u0103 \u0219i de studii anterioare, astfel c\u0103 ghidul ASCCP (American Society for Colposcopy and Cervical Pathology) din 2006 recomand\u0103 utilizarea unui test de genotipare HPV pentru tipurile 16 \u0219i 18 la pacientele \u2265 30 ani cu citologie negativ\u0103 \u0219i ADN-HPV detec\u021bie tipuri cu risc crescut pozitiv. Scopul acestui test este de a tria pacientele pozitive pentru HPV 16 sau 18 care trebuie trimise imediat la colposcopie de cele negative, la care se recomand\u0103 repetarea citologiei \u0219i ADN-HPV detec\u021bie tipuri oncogene peste 12 luni. \u00cenc\u0103 o dat\u0103 trebuie men\u021bionat beneficiul adus de HPV Cobas 6800 prin includerea \u00eentr-un singur test a genotip\u0103rii specifice\u00a0 HPV 16 \u0219i 18 al\u0103turi de detectarea celorlalte tipuri cu risc crescut.\u00a0<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-size: 12pt;\">Limitele de detec\u021bie pentru HPV-16 \u0219i HPV-18: 300 \u0219i respectiv 600 copii sau celule\/mL. Testul HPV include obligatoriu controlul celular reprezentat de gena beta-globinei; \u00een absen\u021ba amplific\u0103rii acesteia testul nu poate fi validat (este foarte probabil vorba de o mas\u0103 redus\u0103 de celule cervicale datorit\u0103 unei recolt\u0103ri incorecte a probei).<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-size: 12pt;\">Performan\u021bele testului <span style=\"text-decoration: underline;\">nu<\/span> au fost stabilite la persoanele vaccinate anti-HPV \u0219i nici la cele imunodeprimate.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-size: 12pt;\">A\u0219a cum au confirmat studiile efectuate p\u00e2n\u0103 \u00een prezent, utilizarea combinat\u0103 a citologiei Papanicolau \u00een mediu lichid \u0219i a testului HPV permite detectarea leziunilor intraepiteliale cu risc crescut cu o sensibilitate \u0219i o specificitate mai mare. Astfel, protocoalele de screening care includ ambele teste beneficiaz\u0103 de sensibilitatea crescut\u0103 \u0219i valoarea predictiv\u0103 negativ\u0103 a testului HPV \u0219i de specificitatea crescut\u0103 a citologiei cervico-vaginale.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-size: 12pt;\">Este de asemenea evitat disconfortul legat de repetarea recolt\u0103rii unei noi probe de celule cervicale pentru test\u0103ri ulterioare.
