Determinarea Biotinidazei din ser sau plasm\u0103 este recomand\u0103 pentru evaluarea activit\u0103\u021bii biotinidaze, \u00een screeeningul neonatal sau \u00een diagnosticarea \u0219i urm\u0103rirea deficitului acesteia.<\/strong><\/p>\n

Informa\u021bii generale<\/strong><\/em><\/p>\n

Deficitul de biotinidaz\u0103 este o tulburare autosomal\u0103 recesiv\u0103 cauzat\u0103 de muta\u021bii ale genei biotinidazei (BTD). V\u00e2rsta de debut \u0219i fenotipul clinic variaz\u0103 \u00eentre indivizi, \u00een func\u021bie de cantitatea de activitate rezidual\u0103 a biotinidazei. Deficitul profund de biotinidaz\u0103 apare la aproximativ 1 din 137.000 de na\u0219teri vii \u0219i deficitul par\u021bial de biotinidaz\u0103 apare la aproximativ 1 din 110.000 de na\u0219teri vii, rezult\u00e2nd o inciden\u021b\u0103 combinat\u0103 de aproximativ 1 din 61.000. Frecven\u021ba purt\u0103toare a deficitului de biotinidaz\u0103 \u00een popula\u021bia general\u0103 este de aproximativ 1 din 1201.<\/p>\n

Deficitul profund de biotinidaz\u0103 netratat\u0103 se manifest\u0103 de obicei \u00een primul deceniu de via\u021b\u0103 prin convulsii, ataxie, \u00eent\u00e2rziere \u00een dezvoltare, hipotonie, pierderea auzului senzorineural, probleme de vedere, erup\u021bii cutanate \u0219i alopecie. Deficitul par\u021bial de biotinidaz\u0103 este asociat cu o prezentare clinic\u0103 mai u\u0219oar\u0103, care poate include simptome cutanate f\u0103r\u0103 implicare neurologic\u0103. Anumite acidurii organice, cum ar fi deficitul de holocarboxilaz\u0103 sintetaz\u0103, deficitul izolat de carboxilat sintetaz\u0103 \u0219i 3-metilcrotonilglicinuria, prezint\u0103 similarit\u0103\u021bi cu deficien\u021ba de biotinidaz\u0103. Determinarea nivelurilor serice de biotinidaz\u0103 pot ajuta la excluderea acestor tulbur\u0103ri. Tratamentul cu biotin\u0103 are succes \u00een prevenirea caracteristicilor clinice asociate cu deficitul de biotinidaz\u0103. La pacien\u021bii simptomatici, tratamentul va avea efecte benefice asupra unor caracteristici clinice, cu excep\u021bia \u00eent\u00e2rzierii dezvolt\u0103rii, a vederii \u0219i a complica\u021biilor auditive. Ca urmare, deficitul de biotinidaz\u0103 este inclus \u00een majoritatea programelor de screening pentru nou-n\u0103scu\u021bi. Acest lucru permite identificarea timpurie \u0219i tratamentul pacien\u021bilor presimptomatici1,2,3.<\/p>\n

Testele moleculare constituie baza testelor de confirmarea genei BTD. C\u00e2nd activitatea biotinidazei este deficitar\u0103, secven\u021bierea \u00eentregii gene BTD permite detectarea muta\u021biilor cauzatoare de boli la pacien\u021bii afecta\u021bi. Identificarea muta\u021biilor familiale permite testarea membrilor familiei cu risc. \u00cen timp ce corela\u021biile genotip-fenotip nu sunt bine stabilite, s-a constatat c\u0103 anumite muta\u021bii sunt asociate cu deficit profund de biotinidaz\u0103, \u00een timp ce altele sunt asociate cu deficit par\u021bial. Aceste test\u0103ri enzimatice se realizeaz\u0103 pentru diagnosticarea deficitului de biotinidaz\u0103, de obicei \u00een urma unei dezvolt\u0103ri anormale a nou-n\u0103scutului. Deficitul de biotinidaz\u0103 se poate corecta cu suplimente orale de biotin\u0103. Persoanele care sunt diagnosticate presimptomatic (de exemplu, prin screening pentru nou-n\u0103scu\u021bi) \u0219i care sunt tratate cu suplimente de biotin\u0103 nu dezvolt\u0103 caracteristicile clinice asociate deficitului de biotinidaz\u01033.<\/p>\n

    Deficien\u021be par\u021biale de biotin\u0103 pot s\u0103 apar\u0103 \u0219i la limita minim\u0103 a intervalului de referin\u021b\u0103. Valori sub 3,5 U \/ L sunt foarte rar \u00eent\u00e2lnite la pacien\u021bii asimptomatici3.<\/p>\n

    Limite si interferen\u021be<\/strong><\/p>\n

    Suplimente de biotin\u0103 \u2013 Aporturile foarte mari de biotin\u0103 pot interfera cu testele de diagnostic care utilizeaz\u0103 formatul biotin\u0103-streptavidin\u0103, utilizate \u00een mod obi\u0219nuit pentru a m\u0103sura nivelurile de hormoni (cum ar fi hormonul tiroidian) \u0219i al\u021bi anali\u021bi, cum ar fi 25-hidroxi-vitamina D, produc\u00e2nd rezultate fals normale sau anormale. C\u00e2teva studii de caz recente au descris constat\u0103ri care indic\u0103 \u00een mod fals boala Graves \u0219i hipertiroidism sever, la pacien\u021bii care consum\u0103 10-300 mg biotin\u0103 pe zi, inclusiv \u0219ase copii c\u0103rora li s-au administrat doze mari de biotin\u0103 (2-15 mg \/ kg greutate corporal\u0103 pe zi) pentru tratarea bolilor metabolice mo\u0219tenite4.<\/p>\n

    Chiar \u0219i o singur\u0103 doz\u0103 de 10 mg de biotin\u0103 a interferat cu testele func\u021biei tiroidiene, realizate dup\u0103 24 de ore de la administrarea suplimentului. Un studiu efectuat pe \u0219ase adul\u021bi s\u0103n\u0103to\u0219i, care au consumat 10 mg\/zi de suplimente cu biotin\u0103 timp de 1 s\u0103pt\u0103m\u00e2n\u0103, au constatat interferen\u021be \u00een cazul mai multor teste de determinare a biotinei, inclusiv niveluri fals sc\u0103zute ale hormonului stimulator tiroidian (care ar putea duce la un diagnostic gre\u0219it de tirotoxicoz\u0103) \u0219i de peptid natriuretic tip B5. Biotina poate interac\u021biona cu anumite medicamente, iar unele medicamente pot avea un efect advers asupra concentra\u021biilor de biotin\u0103. Persoanele care iau suplimente cu biotin\u0103 \u0219i alte medicamente \u00een mod regulat, ar trebui s\u0103 anun\u021be acest aspect cu furnizorii de servicii de sanatate6.<\/p>\n

    Anticonvulsivantele \u2013 \u00centr-un studiu efectuat pe 264 de persoane cu epilepsie, tratamentul anticonvulsivant timp de cel pu\u021bin 1 an a fost asociat cu niveluri semnificativ mai mici de biotin\u0103 seric\u0103 dec\u00e2t la pacien\u021bii din grupul de control6. Motivul ar putea fi acela c\u0103 tratamentul anticonvulsivant cre\u0219te catabolismul biotinei, ceea ce duce la reducerea concentra\u021biei de biotin\u0103 \u0219i inhibarea absorb\u021biei intestinale a biotinei7.<\/p>\n
