Pe m\u0103sur\u0103 ce boala progreseaz\u0103, poate fi observat\u0103 la microscopul optic \u00eengro\u0219area membranei bazale glomerulare, de unde provine \u0219i denumirea ini\u021bial\u0103 de glomerulonefrit\u0103 membranoas\u0103<sup>2<\/sup>.<br \/>\nInciden\u021ba anual\u0103 raportat\u0103 a bolii este de 5\u201310 cazuri\/1 milion de persoane \u00een Europa de Nord. Aproximativ 20% din cazurile de sindrom nefrotic la adult sunt generate de nefropatia membranoas\u0103. Evolu\u021bia acestei afec\u021biuni este variabil\u0103; circa o treime din pacien\u021bi va progresa c\u0103tre insuficien\u021b\u0103 renal\u0103 terminal\u0103 \u00een decurs de 10 ani, o alt\u0103 treime va manifesta proteinurie cronic\u0103 \u0219i simptome de sindrom nefrotic, \u00een timp ce o alt\u0103 treime va prezenta remisiune spontan\u0103<sup>2<\/sup>.<br \/>\nNefropatia membranoas\u0103 poate s\u0103 survin\u0103 secundar unui num\u0103r de condi\u021bii clinice, cele mai importante fiind infec\u021biile (hepatita B, hepatia C, sifilisul, malaria), bolile autoimune (lupusul eritematos sistemic, artrita reumatoid\u0103, sindromul Sj\u00f6gren, pemfigoidul bulos), neoplaziile \u0219i toxicitatea medicamentoas\u0103 (antinflamatoare nesteroidiene, aur, mercur, penicilamin\u0103)<sup>3<\/sup>.<br \/>\nNefropatia membranoasa ce apare independent de alte sindroame clinice reprezint\u0103 nefropatia membranoas\u0103 primar\u0103<sup>1<\/sup>. Se consider\u0103 c\u0103 85% din nefropatiile membranoase sunt primare, restul de 15% fiind secundare<sup>4<\/sup>. Au fost asociate cu nefropatia membranoas\u0103 primar\u0103 sexul masculin, grupa de v\u00e2rst\u0103 40 \u2013 70 de ani, HLA-DQA15.<br \/>\nNefropatia membranoas\u0103 primar\u0103 a fost clasic descris\u0103 ca idiopatic\u0103 deoarece antigenul \u021bint\u0103 era necunoscut. \u00cen 2009, grupul de cercet\u0103tori condus de David J. Salant a detectat o glicoprotein\u0103 de 185 kDa ca fiind antigenul major fa\u021b\u0103 de care sunt \u00eendrepta\u021bi autoanticorpii prezen\u021bi la 70-80% din pacien\u021bii cu nefropatie membranoas\u0103 primar\u0103. La analiza prin spectrometrie de mas\u0103 glicoproteina a fost identificat\u0103 drept <strong>receptorul de tip M pentru fosfolipaza A2 (PLA2R)<sup>6<\/sup><\/strong>.<br \/>\nPLA2R este exprimat pe suprafa\u021ba podocitelor \u0219i interac\u021bioneaz\u0103 cu fosfolipaza A2. Se presupune c\u0103 legarea autoanticorpilor de PLA2R are ca rezultat formarea de complexe immune in situ \u00een regiunea membranei bazale glomerulare ce va declan\u0219a activarea local\u0103 a complementului, cu producerea \u00een exces de collagen tip IV \u0219i laminin\u0103. Distruc\u021bia citoscheletului va afecta podocitele, conduc\u00e2nd la alterarea func\u021biei de filtru a glomerulului \u0219i la apari\u021bia proteinuriei<sup>7;8<\/sup>.<br \/>\nAnticorpii anti-PLA2R sunt predominant de tip IgG<sup>4<\/sup>. \u00cen practic\u0103, determinarea lor are ca principal\u0103 utilitate diferen\u021bierea nefropatiei membranoase primare de formele secundare de boal\u0103, aceasta influen\u021b\u00e2nd \u0219i strategia terapeutic\u0103<sup>9<\/sup>. \u00cen timp ce managementul pacien\u021bilor cu nefropatie membranoas\u0103 secundar\u0103 se axeaz\u0103 pe tratarea bolii de baz\u0103, pacien\u021bii cu nefropatie membranoas\u0103 primar\u0103 sunt \u00een general trata\u021bi cu imunosupresoare. Trebuie men\u021bionat totu\u0219i c\u0103 un mic procent de pacien\u021bi cu nefropatie membranoas\u0103 secundar\u0103 pot fi pozitivi pentru anti-PLA2R. Semnifica\u021bia clinic\u0103 a acestei descoperiri este neclar\u0103; prezen\u021ba anti-PLA2R ar putea fi o simpl\u0103 coinciden\u021b\u0103 sau ace\u0219tia ar putea avea un anumit rol \u00een dezvoltarea \u0219i progresia bolii<sup>2<\/sup>.<br \/>\nS-a dovedit c\u0103 nivelul seric de anticorpi anti-PLA2R se coreleaz\u0103 cu activitatea clinic\u0103 a bolii, la fel ca \u0219i nivelul proteinuriei \u0219i are de asemenea valoare prognostic\u0103<sup>4<\/sup>. Astfel, la pacien\u021bii cu niveluri reduse de anti-PLA2R insuficien\u021ba renal\u0103 a fost raportat\u0103 mai pu\u021bin frecvent, iar la cei cu niveluri \u00eenalte se produc foarte rar remisiunile spontane. De asemenea, pacien\u021bii cu niveluri \u00eenalte ale anticorpilor necesit\u0103 mai frecvent terapie imunosupresoare comparativ cu cei av\u00e2nd niveluri joase. \u00cen plus, s-a constatat c\u0103 anticorpii se coreleaz\u0103 cu r\u0103spunsul la terapia imunosupresoare. Mai multe studii au raportat un declin al nivelului anti-PLA2R la pacien\u021bii ce primesc medica\u021bie imunosupresosre, \u00een timp ce \u00een cursul unei rec\u0103deri nivelul acestora cre\u0219te. Date recente sugereaz\u0103 faptul c\u0103 nivelul anti-PLA2R la sf\u00e2r\u0219itul terapiei are valoare predictiv\u0103 asupra evolutiei ulterioare<sup>2<\/sup>.<br \/>\nNefropatia membranoas\u0103 reapare la circa 40% din pacien\u021bii ce au suferit transplant renal. Detectarea unui nivel crescut de anti-PLA2R \u00eenainte de efectuarea transplantului confer\u0103 un risc mai mare de recuren\u021b\u0103 a bolii<sup>2;4<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><strong>Recomand\u0103ri pentru determinarea anticorpilor anti-receptor fosfolipaza A2:<\/strong><\/p>\n<p style=\"text-align: justify;\">&#8211; diferen\u021bierea nefropatiei membranoase primare de cea secundar\u0103;<br \/>\n&#8211; monitorizarea evolu\u021biei nefropatiei membranoase primare \u0219i a r\u0103spunsului la tratament;<br \/>\n&#8211; estimarea riscului de recuren\u021b\u0103 a bolii la pacien\u021bii care vor primi transplant renal<sup>9<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><strong>Preg\u0103tire pacient<\/strong> \u2013 nu este necesar\u0103 o preg\u0103tire special\u0103<sup>10<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><strong>Specimen recoltat<\/strong> \u2013 s\u00e2nge venos<sup>10<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><strong>Recipient de recoltare<\/strong> \u2013 vacutainer f\u0103r\u0103 anticoagulant, cu\/f\u0103r\u0103 gel separator<sup>10<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><strong>Prelucrare necesar\u0103 dup\u0103 recoltare<\/strong> \u2013 se separ\u0103 serul prin centrifugare; se lucreaz\u0103 serul proasp\u0103t; dac\u0103 acest lucru nu este posibil, serul se p\u0103streaz\u0103 la 2-8\u00b0C<sup>10<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><strong>Volum prob\u0103<\/strong> \u2013 minim 1 mL ser<sup>10<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><strong>Cauze de respingere a probei<\/strong> \u2013 specimen intens hemolizat sau lipemic<sup>10<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><strong>Stabilitate prob\u0103<\/strong> \u2013 serul separat este stabil 7 zile la 2-8\u00b0C<sup>10<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><strong>Metoda<\/strong> \u2013 imunofluorescen\u021ba indirect\u0103; testul utilizeaz\u0103 ca substrat celule transfectate ce exprim\u0103 la suprafa\u021b\u0103 PLA2R<sup>10<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><strong>Valori de referin\u021b\u0103 \u0219i interpretarea rezultatelor<\/strong><\/p>\n<p style=\"text-align: justify;\">\u00a0Rezultatele se exprim\u0103 sub forma unui titru.<br \/>\n&lt;1\/10: Negativ.<br \/>\nTitrul anticorpilor poate fi folosit \u00een monitorizarea bolii. O cre\u0219tere\/descre\u0219tere a titrului sau o dispari\u021bie a anticorpilor precede \u00een general modificarea statusului clinic<sup>4<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><strong>Limite \u0219i interferen\u021be<\/strong><\/p>\n<p style=\"text-align: justify;\">Rezultatele acestui test trebuie coroborate cu alte informa\u021bii clinice (simptomatologie, examen fizic, alte teste serologice).
