Informa\u021b<\/i><\/b>ii generale <\/i><\/b><\/p>\n

Pemfigoidul include un grup de boli autoimune localizate la nivelul tegumentelor \u0219i mucoaselor ce se caracterizeaz\u0103 prin prezen\u021ba unor vezicule situate subepidermal sau subepitelial \u0219i anticorpi IgG anti-membran\u0103 bazal\u0103 \u00een ser. Din aceast\u0103 categorie fac parte: pemfigoidul bulos<\/i>, herpesul gestationis<\/i> \u0438 pemfigoidul cicatricial<\/i>.<\/p>\n

Pemfigoidul bulos este o boal\u0103 cronic\u0103 pruriginoas\u0103 debilitant\u0103, ce apare \u00een principal la persoanele \u00een v\u00e2rst\u0103 \u0219i este caracterizat\u0103 prin dezvoltarea unor vezicule pe o zon\u0103 eritematoas\u0103 sau urticarian\u0103. Erup\u021bia este de obicei generalizat\u0103. Au fost descrise mai multe asocieri ale pemfigoidului bulos cu alte afec\u021biuni, dar probabil c\u0103 acestea sunt \u00eent\u00e2mpl\u0103toare. De asemenea nu a putut fi demonstrat\u0103 asocierea cu procese maligne1<\/sup>.<\/p>\n

Anticorpii IgG anti-membran\u0103 bazal\u0103 se g\u0103sesc \u00een serul a 80% dintre pacien\u021bi, dar \u0219i local la nivelul membranei bazale, unde se pot observa depozite lineare de IgG \u0219i C31;2;3<\/sup>.<\/p>\n

\u00cen pemfigoid, anticorpii IgG se leag\u0103 de proteine specifice ce intr\u0103 \u00een alc\u0103tuirea unor complexe de adeziune celula-matrice – hemidesmozomi – \u0219i activeaz\u0103 complementul. In urma reac\u021biei dintre anticorpi \u0219i moleculele din componenta jonc\u021biunii celulare sunt eliberate numeroase substan\u021be chemotactice care atrag la acest nivel celulele inflamatorii; celulele elibereaz\u0103 proteaze \u0219i diver\u0219i mediatori, care degradeaz\u0103 hemidesmozomii \u0219i determin\u0103 o separare dermo-epidermic\u0103. Tinta autoanticorpilor serici sunt proteinele transmembranare BP230 \u0219i BP180 (BPAG1 \u0219i BPAG2), denumite astfel deoarece greutatea molecular\u0103 a acestor antigene este 230 kD \u0219i respectiv, 180 kD. Se consider\u0103 c\u0103 BP180 reprezint\u0103 \u021binta direct\u0103 a autoanticorpilor datorit\u0103 localiz\u0103rii sale de-a lungul membranei bazale, \u00een timp ce anticorpii anti-BP230 sunt produ\u0219i secundar1;2<\/sup>.<\/p>\n

Pemfigoidul cicatricial afecteaz\u0103 \u00een principal cavitatea bucal\u0103 (gingivita descuamativ\u0103) \u0219i zona ocular\u0103 (conjunctivita la debut care dezvolt\u0103 \u00een timp aderen\u021be fibroase conjunctivale, obliterarea sacului conjunctival, xeroftalmie, opacifierea \u0219i perfora\u021bia corneei \u0219i \u00een final orbire). Pacien\u021bii prezint\u0103 acela\u0219i tip de anticorpi IgG circulan\u021bi ca \u0219i cei cu pemfigus generalizat1<\/sup>.<\/p>\n

De\u0219i diferitele boli cutanate buloase au anumite caracteristici specifice care le disting, de cele mai multe ori manifest\u0103rile clinice sunt atipice \u0219i se suprapun cu alte afec\u021biuni. Depistarea anticorpilor anti-membran\u0103 bazal\u0103 \u00een ser este extrem de sensibil\u0103 \u0219i specific\u0103 pentru pemfigoid \u0219i reprezint\u0103 un element important \u00een diagnosticarea acestei afec\u021biuni.<\/p>\n

Diagnosticul \u00a0prompt \u0219i precis este util \u00een stabilirea tratamentului, minimizarea sechelelor bolii, comorbidit\u0103\u021bii \u0219i disconfortului pacientului.<\/p>\n

Identificarea antigenelor BP180 \u0219i BP230 a condus la dezvoltarea de teste ELISA cu sensibilitate \u0219i specificitate \u00eenalte2<\/sup>.<\/p>\n

Recomand\u0103ri pentru determinarea anticorpilor anti-BP180<\/i><\/b> <\/b>– <\/b>diagnosticul \u0219i monitorizarea r\u0103spunsului terapeutic al pacien\u021bilor cu pemfigoid2<\/sup>.<\/b><\/p>\n

Preg\u0103tire pacient <\/i><\/b>– <\/b><\/i>nu este necesar\u0103 o preg\u0103tire special\u01033<\/sup>.<\/p>\n

Specimen recoltat <\/i><\/b>– <\/i>\u0432\u0435\u043d\u043e\u0437\u043d\u0430\u044f \u043a\u0440\u043e\u0432\u044c3<\/sup>.<\/p>\n

La cei mai mul\u021bi dintre pacien\u021bi nivelurile de anticorpi se coreleaz\u0103 cu activitatea bolii, pacien\u021bii cu forme severe prezent\u00e2nd \u00een general concentra\u021bii ridicate ale anticorpilor. De asemenea, ameliorarea clinic\u0103 este asociat\u0103 cu sc\u0103derea nivelului de anticorpi2<\/sup>.<\/div>\n
