Informa\u021bii generale <\/strong><\/em><\/p>\n

Klebsiella pneumoniae (bacilul Friedlander)<\/em> este un bacil Gram-negativ, imobil, incapsulat, lactozo-pozitiv, ce se gaseste in flora normala a cavita\u021bii bucale, tegument si tract gastrointestinal. Membrii genului Klebsiella exprima 2 tipuri de antigene pe suprafa\u021ba: antigenul O, care este un lipopolizaharid, si antigenul K, un polizaharid capsular, ambele contribuind la patogenitate.<\/p>\n

Sunt patogeni oportunisti, principalul rezervor de infec\u021bie fiind reprezentat de tractul gastro-intestinal al pacien\u021bilor si mainile personalului medical din spitale3<\/sup>.<\/p>\n

Aceste bacterii determina, in general, pneumonii dobandite in comunitate, dar frecvent se pot raspandi rapid, conducand adesea la apari\u021bia unor focare nosocomiale. Spectrul de sindroame clinice determinate de acest agent patogen include pneumonie, bacteriemie, tromboflebita, infec\u021bii ale tractului urinar (ITU), respirator superior, plagilor, colecistita, colangita, diaree, osteomielita endocardita, endoftalmita si meningita. Prezen\u021ba dispozitivelor invazive, contaminarea echipamentului de suport respirator, utilizarea de catetere urinare, starea precara de sanatate si tratamentul intr-o unitate de terapie intensiva sunt factori care cresc riscul de infec\u021bie nosocomiala cu specii de Klebsiella. Folosirea pe scara larga de antibiotice cu spectru larg la pacien\u021bii spitaliza\u021bi a condus la cresterea portajului de Klebsiella si, ulterior, dezvoltarea unor tulpini multirezistente care produc \u03b2-lactamaze cu spectru extins. Aceste tulpini sunt extrem de virulente, prezinta antigen capsular de tip K55 si au o capacitate extraordinara de a se raspandi. Infec\u021biile determinate de aceste specii au devenit o problema majora in majoritatea spitalelor din cauza rezisten\u021bei multiple la antibiotice1;3<\/sup>.<\/p>\n

Mai mult decat atat, unul dintre aspectele cele mai interesante al patogenita\u021bii Klebsiellei pneumoniae este rolul acestui microorganism in etiopatologia afec\u021biunilor reumatice asociate cu HLA-B27 (spondilita anchilozanta-AS). Modul de asociere dintre Klebsiella, HLA-B27 si AS este neclar.<\/p>\n

La sfarsitul anilor 1970 au aparut primele rapoarte care au sugerat o posibila legatura intre Klebsiella si AS, insa doar recent s-a demonstrat conexiunea dintre anumite Enterobacteriaceae si afec\u021biunile reumatoide. Studii bine documentate care au analizat asocia\u021bia dintre artrita reactiva si infec\u021biile acute cu enterobacterii (Yersinia enterocolitica, Shigella, Salmonella, Klebsiella) au condus la ipoteza ca AS ar putea fi o sechela a infec\u021biei cu microorganismele enterice1<\/sup>.<\/p>\n

In 1978, Ebringer et al. si mai tarziu si al\u021bi cercetatori au raportat o crestere a colonizarii intestinului cu Klebsiella in timpul fazei active a bolii la pacien\u021bii cu AS.<\/p>\n

Intr-un studiu care vizeaza detectarea anticorpilor anti-bacterieni in serul pacien\u021bilor cu AS, Trull et al. (in 1983) au aratat ca titrurile de anticorpi IgA anti-Klebsiella au fost semnificativ crescute in serul pacien\u021bilor cu forme active de boala comparativ cu serul pacien\u021bilor cu forma inactiva sau martorii sanatosi.<\/p>\n

Diverse teorii au fost dezvoltate pentru a explica modul de asociere intre HLA-B27, Klebsiella si etiopatogenia AS.<\/p>\n

Teoria mimetismului molecular, propus pentru prima data de Ebringer et al., implica reactivitatea incrucisata intre antigenele Klebsiellei si anumi\u021bi epitopi de la nivelul moleculei de HLA-B27. Aceasta teorie este sus\u021binuta de observa\u021biile conform carora anticorpi anti-Klebsiella se leaga de celulele HLA-B27 pozitive si anticorpii anti-HLA-B27 la antigenele Klebsiella. In plus, o secven\u021ba de sase aminoacizi (QTDRED) a fost detectata atat la nivelul Klebsiellei cat si in structura HLA-B* 2705, iar anticorpi specifici pentru acest hexapeptid au fost detecta\u021bi in serul pacien\u021bilor cu AS.<\/p>\n

Teoria receptorului dezvoltata de Zinkernagel si Doherty se bazeaza pe ipoteza conform careia complexul major de histocompatibilitate clasa I prezinta antigene virale si bacteriene. S-a sugerat ca HLA-B27 se comporta ca un receptor pentru polipeptidele prelucrate artritogenic, probabil derivate din Klebsiella. Ca urmare, celulele T citotoxice indreptate impotriva acestor peptide ataca, de asemenea, proteine similare structural, de la nivelul articula\u021biilor, prezentate de moleculele de HLA-B271<\/sup>.<\/p>\n

In ciuda cercetarii intensive in acest domeniu, legatura dintre Klebsiella si bolile reumatoide ramane necunoscuta, deoarece teoriile men\u021bionate mai sus au fost par\u021bial infirmate de rezultatele contradictorii ale diferitor grupuri de cercetatori.<\/p>\n

Studii recente legate de raspunsul anticorpilor la polizaharide capsulare provenite de la Klebsiella in serul pacien\u021bilor cu AS sau cu alte afec\u021biuni reumatice au analizat pentru prima oara toate cele 77 serotipuri. Serul pacien\u021bilor cu AS si HLA-B27-pozitiv a prezentat titruri ridicate de IgG ca raspuns la polizaharidele capsulare ale serotipurilor K26, K36, K50 in compara\u021bie cu nivelul IgG al persoanelor HLA-B27-negative.<\/p>\n

Este de remarcat faptul ca toate cele trei tipuri de antigene capsulare (tipuri de K26, K36 si K50) con\u021bin polizaharide recunoscute de macrofage printr-un mecanism lectinic. Acest tip de legare ar putea juca un rol ini\u021bial in patogeneza AS. Dintre cele 77 de serotipuri cunoscute de Klebsiella, doar 16 tipuri capsulare con\u021bin secven\u021be care permit o interac\u021biune specifica cu receptorul manozic al macrofagelor. Nu este exclus ca serotipurile men\u021bionate mai sus sa con\u021bina antigene specifice, probabil peptide, care sa induca activitatea artritogenica dupa ingestia si distrugerea bacteriei de catre macrofage. Prelucrarea si prezentarea acestui antigen ar putea activa o cascada imunologica specifica, dar inca necunoscuta, ce determina apari\u021bia artritei reactive. Cu toate acestea, sunt necesare studii suplimentare care vizeaza elucidarea interac\u021biunii dintre aceste serotipuri si celule relevante ale pacien\u021bilor cu AS si HLA-B27-pozitiv1<\/sup>.<\/p>\n
