Ace\u0219ti autoanticorpi sunt denumi\u021bi generic <em>anticorpi antinucleari<\/em> (ANA) datorit\u0103 faptului sunt \u00eendrepta\u021bi predominant \u00eempotriva antigenelor nucleare, dar ei includ \u0219i anticorpi anticitoplasmatici.<\/p>\n<p>Detectarea ANA are o mare sensibilitate, de aceea este considerat cel mai bun test de screening pentru SARD. Prevalen\u021ba ANA \u00een aceste afec\u021biuni se situeaz\u0103 \u00eentre 20 \u0219i 100%. Pentru identificarea bolilor reumatismale este foarte important\u0103 diferen\u021bierea ANA<sup>1;2;3<\/sup>. Astfel, sunt descrise \u00een prezent dou\u0103 categorii principale de anticorpi antinucleari:<\/p>\n<ol>\n<li>Autoanticorpi ce au drept \u021bint\u0103 antigenic\u0103 ADN-ul \u0219i histonele;<\/li>\n<li>Autoanticorpi ce au drept \u021bint\u0103 antigene nucleare solubile (extractable nuclear antigens = ENA).<\/li>\n<\/ol>\n<p>Varietatea autoantigenelor \u021bint\u0103 ale anticorpilor anti-nucleari este extrem de mare de aceea \u00een prezen\u021ba unui rezultat pozitiv la examenul prin imunofluorescen\u021b\u0103 indirect\u0103 este necesar\u0103 detectarea de anticorpi specifici care s\u0103 orienteze diagnosticul c\u0103tre o anumit\u0103 boal\u0103 autoimun\u0103. Utilizarea tehnicii imunoblot prezint\u0103 avantajul test\u0103rii unor profile extinse de autoanticorpi, cum este \u0219i cazul celui efectuat \u00een laboratorul nostru care permite diferen\u021bierea ANA fa\u021b\u0103 de 16 antigene<sup>4<\/sup>.<\/p>\n<p><strong><u>Anticorpii anti-U1nRNP \u0219i Sm<\/u><\/strong> sunt indrepta\u021bi \u00eempotriva unor antigene care apar\u021bin grupului de ribonucleoproteine mici (snRNP). Acestea sunt complexe formate din ARN cu un con\u021binut \u00eenalt de uridin\u0103 (U-RNA) \u0219i diverse proteine av\u00e2nd o greutate molecular\u0103 de 9-70 kDa. Prin cromatografie au fost puse \u00een eviden\u021b\u0103 5 tipuri de U-RNA: U1, U2, U4, U5 \u0219i U6. Particulele de U-nRNP con\u021bin 6 proteine core sau Sm (B, B&#8217;, D, E, F, G), iar U1-RNP con\u021bine \u00een plus proteine specifice (70K, A, C). Astfel, anticorpii anti &#8211; U1 RNP reac\u021bioneaz\u0103 cu una sau mai multe proteine specifice \u00een timp ce anticorpii anti-Sm reac\u021bioneaz\u0103 cu una sau mai multe proteine core. Ac\u021biunea snRNP este esen\u021bial\u0103 pentru eliminarea intronilor din pre-ARN-ul mesager (procesul de splicing), rezult\u00e2nd ARNm care va fi exportat din nucleu \u0219i implicat \u00een procesul de transla\u021bie de la nivelul ribozomilor.<\/p>\n<p>Titruri crescute de anticorpi anti-U1 RNP se \u00eent\u00e2lnesc cu o prevalen\u021b\u0103 de 95-100% \u00een boala mixt\u0103 de \u021besut conjunctiv (sindromul Sharp), constituind unul din criteriile de diagnostic al acestei afec\u021biuni. Titrul de anticorpi se coreleaz\u0103 de asemenea cu gradul de activitate a bolii. Anticorpii anti-U1 RNP mai pot fi \u00eent\u00e2lni\u021bi \u0219i la pacienti cu LES (15-40%), scleroz\u0103 sistemic\u0103 \u0219i polimiozit\u0103<sup>1;7<\/sup>.<\/p>\n<p><strong><u>Anticorpii anti-Sm<\/u><\/strong> sunt \u00eendrepta\u021bi asupra antigenului Sm, denumit astfel dup\u0103 primul pacient (Stephanie Smith) la care a fost pus \u00een eviden\u021b\u0103. Antigenul Sm face parte din grupul snRNP implicate \u00een procesul de splicing al ARN-ului mesager. Anticorpii anti-Sm sunt \u00eent\u00e2lni\u021bi la 5-40% din pacien\u021bii cu LES av\u00e2nd o specificitate \u00eenalt\u0103 (peste 90%) pentru boala lupic\u0103. \u00cempreun\u0103 cu anticorpii anti-ADN dublu catenari pot fi considera\u021bi patognomonici pentru aceast\u0103 condi\u021bie clinic\u0103 \u0219i fac parte din criteriile ACR de diagnostic al LES<sup>6<\/sup>.<\/p>\n<p><strong><u>Anticorpii anti-SS-A (Ro)<\/u><\/strong> sunt \u00eendrepta\u021bi \u00eempotriva unui antigen care face parte tot din grupul snRNP &#8211; soluble substance A sau Robert antigen. Acesta este compus dintr-o molecul\u0103 ARN (Y1, Y2, Y3, Y4 sau Y5) \u0219i o protein\u0103 de 60 kDa. Complexul poate asocia \u0219i o protein\u0103 de 52 kDa (Ro52), dar acest lucru este \u00eenc\u0103 controversat \u00een literatura de specialitate. Diverse studii au ar\u0103tat c\u0103 serurile pozitive pentru SS-A con\u021bin \u00eentotdeauna anticorpi \u00eempotriva proteinei native SS-A de 60 kDa \u0219i adi\u021bional pot avea \u0219i anticorpi \u00eendrepta\u021bi \u00eempotriva proteinei Ro52. Diferen\u021bierea de anticorpii anti-Ro52 are importan\u021b\u0103 diagnostic\u0103, deoarece ace\u0219tia sunt nespecifici, put\u00e2nd fi detecta\u021bi \u00eentr-o multitudine de afec\u021biuni.<\/p>\n<p>Anticorpii anti-SS-A sunt asocia\u021bi cu diverse boli autoimune: sindrom Sj\u00f6gren (40-95% din cazuri), LES (20-60%), ciroz\u0103 biliar\u0103 primitiv\u0103 (20%). Trebuie men\u021bionat c\u0103 ace\u0219ti autoanticorpi apar \u00een 100% din cazurile de lupus neonatal. Pot fi \u00eent\u00e2lni\u021bi de asemenea \u00een hepatitele autoimune sau virale.<\/p>\n<p><strong><u>Anticorpii anti-SS-B (La)<\/u><\/strong> sunt \u00eendrepta\u021bi \u00eempotriva unei proteine ce de\u021bine un rol \u00een transcrip\u021bia ARN polimerazei III &#8211; soluble substance B sau Lane antigen. La om reprezint\u0103 probabil un factor de finalizare a transcrip\u021biei care se leag\u0103 de cap\u0103tul 3&#8242; al produ\u0219ilor de transcrip\u021bie nou-genera\u021bi ai ARN polimerazei III. Anticorpii anti-SS-B apar de obicei \u00eempreun\u0103 cu anti-SS-A, fiind asocia\u021bi cu acelea\u0219i condi\u021bii clinice<sup>1;3;4<\/sup>.<\/p>\n<p><strong><u>Anti-Ro52<\/u><\/strong> sunt \u00eendrepta\u021bi \u00eempotriva unui antigen de 52 kDa \u0219i au fost descri\u0219i ca autoanticorpi nespecifici asocia\u021bi at\u00e2t cu boli reumatice c\u00e2t \u0219i non-reumatice. Sunt prezen\u021bi \u00eentr-un procent de 100% la mamele fe\u021bilor sau nou-n\u0103scu\u021bilor cu bloc atrio-ventricular congenital sau lupus neonatal<sup>1<\/sup>.<\/p>\n<p><strong><u>Anticorpii anti-Scl-70<\/u><\/strong> sunt \u00eendrepta\u021bi \u00eempotriva unei proteine non-histonice de 70 kDa, izolat\u0103 ini\u021bial din nuclei de celule hepatice provenite de la \u0219obolan. Ulterior, s-a demonstrat c\u0103 aceast\u0103 protein\u0103 este un produs de degradare al ADN topoizomerazei I, o clas\u0103 de enzime localizate \u00een nucleoplasm\u0103 \u0219i nucleoli, av\u00e2nd func\u021bia de a men\u021bine integritatea informa\u021bional\u0103 a ADN (prin capacitatea de a scinda \u0219i de a reuni la loc unul din cele dou\u0103 lan\u021buri ADN).<\/p>\n<p>Anticorpii anti-Scl-70 se g\u0103sesc la 40% din pacien\u021bii cu scleroz\u0103 sistemic\u0103 forma difuz\u0103 \u0219i la 10% din pacien\u021bii cu scleroz\u0103 sistemic\u0103 forma limitat\u0103, fiind un element de prognostic nefavorabil<sup>1;3;4;5<\/sup>.<\/p>\n<p><strong><u>Anticorpii anti PM-Scl<\/u><\/strong> sunt \u00eendrepta\u021bi \u00eempotriva proteinelor complexului nucleolar PM-Scl, compus din 11-16 polipeptide \u00een care componentele antigenice au greut\u0103\u021bi moleculare cuprinse \u00eentre 20 kDa \u0219i 110 kDa. Principalele antigene sunt 2 polipeptide de 75 \u0219i respectiv 100 kDa, cunoscute <u>ca PM-Scl 75<\/u> \u0438 <u>PM-Scl 100<\/u>. Cele 2 antigene sunt independente unul de cel\u0103lat \u0219i nu reac\u021bioneaz\u0103 \u00eencruci\u0219at. Acest complex de proteine este implicat \u00een biosinteza ribozomilor. Anticorpii anti-PM-Scl se asociaz\u0103 frecvent cu sindroamele overlap sclerodermie-polimiozit\u0103\/dermatomiozit\u0103.<\/p>\n<p><strong><u>Anticorpii anti-Jo-1<\/u><\/strong> sunt \u00eendrepta\u021bi \u00eempotriva enzimei histidil-t-ARN sintetaza. <em>In vitro<\/em> ace\u0219ti autoanticorpi se leag\u0103 de epitopii conforma\u021bionali ai enzimei \u0219i \u00eei inhib\u0103 activitatea catalitic\u0103. Antigenul este sub form\u0103 dimeric\u0103 \u0219i este localizat \u00een citoplasm\u0103; masa molecular\u0103 a subunit\u0103\u021bilor este 50 kDa. Anticorpii anti-Jo-1 constituie un marker serologic pentru dermatomiozit\u0103\/polimiozit\u0103, av\u00e2nd o prevalen\u021b\u0103 de 20-40%. Prezen\u021ba acestor autoanticorpi se asociaz\u0103 cu o form\u0103 mai sever\u0103 de boal\u0103, cu un prognostic rezervat \u0219i rec\u0103deri mai frecvente. Autoanticorpii pot fi detecta\u021bi precoce, uneori preced\u00e2nd debutul simptomelor.<\/p>\n<p><strong><u>Anticorpii anti-CENP B<\/u><\/strong> sunt \u00eendrepta\u021bi \u00eempotriva unei proteine de 80 kDa localizat\u0103 \u00een zona de ancorare a fibrelor fusului mitotic \u00een cursul mitozei. Toate serurile care con\u021bin anticorpi anti-centromer reac\u021bioneaz\u0103 cu CENP-B. Anticorpii anti-CENP B constituie un marker serologic pentru sindromul CREST, un subtip de scleroz\u0103 sistemic\u0103 (prevalen\u021b\u0103 70-90%). Pot ap\u0103rea \u0219i la pacien\u021bii cu ciroz\u0103 biliar\u0103 primitiv\u0103.<\/p>\n<p><strong><u>Anticorpii anti-PCNA<\/u><\/strong> (proliferating cell nuclear antigen) sunt \u00eendrepta\u021bi \u00eempotriva unei proteine de 36 kDa ce constituie un cofactor al ADN polimerazei \u0219i particip\u0103 astfel la procesul de replicare \u0219i reparare a ADN-ului. Acest antigen se mai nume\u0219te \u0219i ciclina I, av\u00e2nd un rol \u0219i \u00een reglarea ciclului celular. Aspectul \u00een imunofluorescen\u021ba indirect\u0103 este variabil (granular\/p\u0103tat), jum\u0103tate din nucleii celulelor \u00een interfaz\u0103 av\u00e2nd o intensitate sc\u0103zut\u0103. Anti-PCNA sunt detecta\u021bi la 3% din pacientii cu SLE<sup>1;4<\/sup>.<\/p>\n<p><strong><u>Anticorpii anti-ADN dublu catenar<\/u><\/strong> reac\u021bioneaz\u0103 cu principalii epitopi ai acidului dezoxiribonucleic (ADN) nativ, dublu spiralat. Ace\u0219ti anticorpi reprezint\u0103 un marker de \u00eenalt\u0103 specificitate pentru LES, fiind prezen\u021bi la aproximativ 60% (30-90%) dintre pacien\u021bii cu boal\u0103 lupic\u0103. Constituie de asemenea un instrument de monitorizare a evolu\u021biei clinice a acestei boli, deoarece nivelele serice de anticorpi anti-ADN se coreleaz\u0103 cu gradul de activitate a bolii (\u00een special pentru nefrita lupic\u0103). Se mai pot asocia \u00eentr-un procent redus cu lupusul indus medicamentos, artrita reumatoid\u0103 \u0219i scleroza sistemic\u0103<sup>1;7<\/sup>.<\/p>\n<p><strong><u>Anticorpii anti-nucleozomi<\/u><\/strong><\/p>\n<p>Nucleoproteinele (cromatina) sunt compuse din 40% ADN, 40% histone \u0219i 20% proteine non-histonice. Histonele sunt organizate de-a lungul ADN-ului \u00een unit\u0103\u021bi repetitive numite nucleozomi. Fiecare nucleozom este alc\u0103tuit dintr-un miez ce con\u021bine 8 histone (2 din clasa H2A, 2 H2B, 2 H3 \u0219i 2 H4) \u0219i este \u00eenf\u0103\u0219urat prin 2 rota\u021bii complete de o secven\u021b\u0103 ADN (165bp). Nucleozomii sunt lega\u021bi \u00eentre ei printr-un segment de ADN liber av\u00e2nd forma unui \u0219irag de m\u0103rgele. Aceast\u0103 structur\u0103, men\u021binut\u0103 str\u00eens de histona H1 ce leag\u0103 2 nucleozomi vecini, realizeaz\u0103 o \u00eempachetare a ADN-ului de 10 ori. Anticorpii anti-nucleozomi reac\u021bioneaz\u0103 cu partea expus\u0103, adic\u0103 cu ADN-ul din cromatin\u0103. Studiile clinice au demonstrat c\u0103 ace\u0219ti anticorpi apar \u00eenaintea anticorpilor anti-ADN dublu catenar la pacien\u021bii cu LES<sup>8<\/sup>.<\/p>\n<p><strong><u>Anticorpii anti-histone<\/u><\/strong> reac\u021bioneaz\u0103 \u00eempotriva histonelor ce constituie \u00eempreun\u0103 cu ADN-ul componentele esen\u021biale ale cromatinei nucleare. Histonele sunt alc\u0103tuite din proteine bogate \u00een aminoacizii arginin\u0103 \u0219i lizin\u0103, iar una din func\u021biile lor importante este implicarea \u00een organizarea structural\u0103 a cromatinei \u00een nucleozomi .<\/p>\n<p>Toate cele cinci clase de histone (H1, H2A, H2B, H3, H4) ca \u0219i complexele ADN-histone pot fi \u021bint\u0103 pentru producerea de autoanticorpi. Anticorpii antihistone apar la aproximativ 95% dintre pacien\u021bii cu lupus eritematos indus medicamentos \u0219i reprezint\u0103 cel mai important criteriu de diagnostic. Cele mai frecvente medicamente implicate \u00een apari\u021bia lupusului medicamentos sunt: procainamida, hidralazina, izoniazida, carbamazepina, penicilamina, clorpromazina, alfa metildopa, chinidina, salazosulfapiridina, minociclina. Anticorpii anti-histone pot persista ani dup\u0103 \u00eentreruperea medicamentului incriminat.<\/p>\n<p>Anticorpii anti-histone pot s\u0103 apar\u0103 \u0219i \u00een alte afec\u021biuni, cum ar fi LES \u0219i artrita reumatoid\u0103<sup>1;3;8<\/sup>.<\/p>\n<p><strong><u>Anticorpii anti-proteina P ribozomal\u0103<\/u><\/strong> sunt \u00eendrepta\u021bi \u00eempotriva a 3 fosfoproteine ribozomale: P0 (38 kDa), P1 (19 kDa) \u0219i P2 (17kDa). Ace\u0219ti anticorpi prezint\u0103 un patern citoplasmatic \u00een imunofluorescen\u021ba indirect\u0103 \u0219i constituie un marker pentru afectarea neuro-psihic\u0103 \u00een LES; nu se coreleaz\u0103 cu activitatea bolii<sup>4<\/sup>.<\/p>\n<p><strong><u>Anticorpi anti-mitocondriali AMA M2<\/u><\/strong> sunt \u00eendrepta\u021bi asupra subunit\u0103\u021bii E2 a complexului piruvat dehidrogenazei (PDC-E2). Titrurile crescute vin \u00een sprijinul diagnosticului de ciroz\u0103 biliar\u0103 primitiv\u0103 (PBC) \u0219i pot avea valoare predictiv\u0103 pentru dezvoltarea ulterioar\u0103 de PBC la pacien\u021bi cu semne de colestaz\u0103. Anti-M2 mai pot fi detecta\u021bi \u00een sindroame overlap ciroz\u0103 biliar\u0103 primitiv\u0103 \u2013 hepatit\u0103 autoimun\u0103 (PBC-AIH), scleroza sistemic\u0103 (6%) \u0219i sindromul Sj\u00f6gren<sup>1;5;9<\/sup>.<\/p>\n<p><strong><u>Anticorpi anti-DFS70 <\/u><\/strong>sunt \u00eendrepta\u021bi \u00eempotriva unui un co-activator al transcrip\u021biei denumit DFS70 (engl. dense fine speckles 70 antigen, 70 kDa), p75 sau factorul de cre\u0219tere derivat din epiteliul cristalinului (engl. lens epithelium-derived growth factor, LEDGF)<sup>10<\/sup>. Anticorpii anti-DFS70 au fost pentru prima dat\u0103 identifica\u021bi la un pacient cu cistit\u0103 intersti\u021bial\u0103<sup>11<\/sup>, ulterior fiind asocia\u021bi cu diferite condi\u021bii patologice, cum ar fi dermatita atopic\u0103 \u0219i cancerul de prostat\u01031<sup>2;13<\/sup>. Prevalen\u021ba cea mai mare a anticorpilor anti DFS70 a fost raportat\u0103 la pacien\u021bii cu sindrom Vogt-Harada (66.7%), tiroidit\u0103 autoimun\u0103 (55%), dermatit\u0103 atopic\u0103 (30%), \u00een timp ce \u00een SARD aceasta este semnificativ sc\u0103zut\u0103 (\u2264 2%)<sup>12<\/sup>. Mai mult, nici unul dintre indivizii s\u0103n\u0103to\u0219i cu anticorpi anti DFS70 pozitivi nu a dezvoltat SARD \u00een decurs de 4 ani de la evaluarea ini\u021bial\u0103<sup>14<\/sup>. \u00cen studiul condus de Watanabe et al, efectuat pe un lot de 597 voluntari s\u0103n\u0103to\u0219i, au fost identifica\u021bi anticorpii anti-DFS70 cu o prevalen\u021b\u0103 de 11% \u00een popula\u021bia analizat\u0103 \u0219i de 54% \u00een cazul rezultatelor ANA pozitive. La examenul prin imunofluorescen\u021b\u0103 indirect\u0103 pe substratul conven\u021bional Hep-2 anti-DFS70 au fost asocia\u021bi cu un aspect granular\/p\u0103tat fin dens, prezent at\u00e2t \u00een nuclei \u00een interfaz\u0103 c\u00e2t \u0219i \u00een cromozomii din metafaz\u0103<sup>15<\/sup>. Av\u00e2nd \u00een vedere aceste date de prevalen\u021b\u0103, detec\u021bia izolat\u0103 a anticorpilor anti DFS70 a fost propus\u0103 ca test de excludere a diagnosticului de SARD, inclusiv a lupusului eritematos systemic, la persoanele cu rezultat ANA pozitiv<sup>14;15;16<\/sup>. De men\u021bionat c\u0103 c\u0103 peste 20% dintre indivizii s\u0103n\u0103to\u0219i pot s\u0103 prezinte un test ANA pozitiv, care nu pare s\u0103 aib\u0103 leg\u0103tur\u0103 cu v\u00e2rsta, ceea ce limiteaz\u0103 relevan\u021ba diagnostic\u0103 pentru SARD a autoanticorpilor ANA<sup>17;18<\/sup>. Ce anume indic\u0103 prezen\u021ba anticorpilor antiDFS70 r\u0103m\u00e2ne \u00eenc\u0103 un fapt neelucidat. Recent, s-a formulat ipoteza conform c\u0103reia, \u00een func\u021bie de contextul \u00een care sunt detecta\u021bi, ace\u0219tia ar de\u021bine un rol protector sau dimpotriv\u0103, unul patogenic, sau ar servi ca \u201esenzori\u201d pentru stresul oxidativ crescut<sup>19<\/sup>.<\/p>\n<p><strong><em>Recomand\u0103ri pentru determinarea profilului ANA extins &#8211; BLOT <\/em><\/strong><em>&#8211; <\/em> evaluarea pacien\u021bilor cu testul ANA IIF (imunofluorescen\u021b\u0103 indirect\u0103) pozitiv \u0219i cu semne clinice sugestive de SARD. \u00cen general, nu se recomand\u0103 efectuarea ENA \u00een prezen\u021ba unui rezultat ANA IIF negativ dec\u00e2t dac\u0103 pacientul are semne clinice evidente de SARD \u0219i, \u00een special, de sindrom Sj\u00f6gren. Conjugatul se va ata\u0219a la por\u021biunea IgG a complexului antigen-anticorp. Dup\u0103 \u00eendep\u0103rtarea prin sp\u0103lare a conjugatului nelegat se adaug\u0103 solu\u021bia de colorare (substratul enzimei) \u2013 un sistem de detec\u021bie enzimatic\u0103 colorimetric\u0103 . Dac\u0103 este prezent conjugatul legat reac\u021bia enzimatic\u0103 va genera un produs colorat albastru-\u00eenchis la nivelul benzilor ocupate de anticorpi specifici. Astfel, benzile vizualizate la nivelul stripului sunt rezultatul leg\u0103rii anticorpilor specifici de antigenele individuale de la nivelul strip-ului.<\/p>\n<p>Pe fiecare strip exist\u0103 o band\u0103 de control intern.
