Boala apare \u00een principal la adul\u021bii de v\u00e2rst\u0103 mijlocie, fiind de 3-4 ori mai frecvent\u0103 la femei.<\/p>\n<p>Riscul de a dob\u00e2ndi aceast\u0103 boal\u0103 este mai mare la popula\u021bia neagr\u0103. Scurtarea frenului lingual, sindromul Raynaud (stadiul 1: ischemia m\u00e2inilor \u0219i picioarelor; stadiul 2: cianoza local\u0103 cauzat\u0103 de hipoxie; stadiul 3: hiperemie reactiv\u0103), poliartralgiile \u0219i poliartritele sunt cele mai precoce semne de scleroz\u0103 sistemic\u0103. \u00centr-o etap\u0103 ulterioar\u0103 pacien\u021bii dezvolt\u0103 edeme la nivelul m\u00e2inilor \u0219i picioarelor, pielea devine rigid\u0103, \u00eengro\u0219at\u0103 \u00a0\u0219i \u00een stadiile finale atrofic\u0103, ceroas\u0103 \u0219i sub\u021bire. Apar de asemenea deform\u0103ri ale m\u00e2inilor iar degetele r\u0103m\u00e2n fixate \u00eentr-o pozi\u021bie \u00eendoit\u0103. Musculatura fe\u021bei devine rigid\u0103 (aspect de \u201cicoan\u0103 bizantin\u0103\u201d), ceea ce conduce la microstomie \u0219i tulbur\u0103ri de \u00eenchidere a fantei palpebrale. \u00cen final se dezvolt\u0103 fibroza organelor viscerale (tractul digestiv, inima, pl\u0103m\u00e2nii, rinichii) \u0219i a vaselor sanguine\u00a0 ca urmare a depozitelor excesive de colagen. Insuficien\u021ba\u00a0 cardiac\u0103 asociat\u0103 cu\u00a0 hipertensiune pulmonar\u0103 \u0219i fibroz\u0103 cardiac\u0103\u00a0 reprezint\u0103 principala cauz\u0103 de deces \u00een scleroza sistemic\u0103.<\/p>\n<p>\u00cen func\u021bie de distribu\u021bia cutanat\u0103 sunt descrise dou\u0103 forme \u00a0de scleroz\u0103 sistemic\u0103:<\/p>\n<p>&#8211;\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 <em>limitat\u0103<\/em>, cu o evolu\u021bie mai lent\u0103, \u00een care leziunile cutanate sunt distribuite la nivelul extremit\u0103\u021bilor distale;<\/p>\n<p>&#8211;\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 <em>difuz\u0103<\/em>, \u00een care leziunile sunt distribuite difuz la nivelul trunchiului \u0219i fe\u021bei.<\/p>\n<p>Sindromul CREST constituie un subtip de scleroz\u0103 sistemic\u0103 caracterizat clinic prin: calcific\u0103ri cutanate, sindrom Raynaud, tulbur\u0103ri de motilitate a esofagului, sclerodactilie \u0219i telangiectazii.<\/p>\n<p>Criteriile de clasificare elaborate de ACR (American College of\u00a0 Rheumatology) pentru scleroza sistemic\u0103 sunt urm\u0103toarele:<\/p>\n<ul>\n<li>Criteriu major: afectarea cutanat\u0103 care se extinde proximal c\u0103tre articula\u021biile metacarpofalangiene sau metatarsofalangiene;<\/li>\n<li>Criterii minore: sclerodactilie; cicatrici erozive ale pulpei degetelor; fibroz\u0103 pulmonar\u0103 bazal\u0103 bilateral\u0103.<\/li>\n<\/ul>\n<p>Pentru stabilirea diagnosticului este necesar\u0103 prezenta criteriului major sau a dou\u0103 criterii minore<sup>2;3<\/sup>. Determinarea autoanticorpilor specifici reprezint\u0103 un element important pentru diagnosticul serologic al sclerozei sistemice (inclusiv \u00een stadii precoce), monitorizarea evolu\u021biei bolii, estimarea prognosticuuil \u0219i diferen\u021bierea de alte \u00a0colagenoze<sup>1<\/sup>.<\/p>\n<p><strong><span style=\"text-decoration: underline;\">Anticorpii anti-Scl-70<\/span><\/strong> sunt \u00eendrepta\u021bi \u00eempotriva unei proteine non-histonice de 70 kDa, izolat\u0103 ini\u021bial din nuclei de celule hepatice provenite de la \u0219obolan. Ulterior, s-a demonstrat c\u0103 aceast\u0103 protein\u0103 este un produs de degradare al ADN topoizomerazei I, o clas\u0103 de enzime localizate \u00een nucleoplasm\u0103 \u0219i nucleoli, av\u00e2nd func\u021bia de a men\u021bine integritatea informa\u021bional\u0103 a ADN (prin\u00a0 capacitatea de a scinda \u0219i de a reuni la loc unul din cele dou\u0103 lan\u021buri ADN).<\/p>\n<p>Anticorpii anti-Scl-70 se g\u0103sesc la 40% din pacien\u021bii cu scleroz\u0103 sistemic\u0103 forma difuz\u0103 \u0219i la 10% din pacien\u021bii cu scleroz\u0103 sistemic\u0103 forma limitat\u0103. Prezen\u021ba anti-Scl-70 se asociaz\u0103, \u00een majoritatea cazurilor, cu o evolu\u021bie sever\u0103 \u0219i rapid\u0103 a bolii, cu afectarea extensiv\u0103 a teritoriului cutanat \u0219i a organelor interne.<\/p>\n<p>Ca \u0219i \u00een cazul anticorpilor anti-centromer prezen\u021ba anticorpilor anti-Scl-70 la pacien\u021bii cu sindrom Raynaud poate fi un factor predictiv pentru dezvoltarea ulterioar\u0103 a sclerozei sistemice. Anticorpii anti-Scl 70 se asociaz\u0103 cu alela DRB1*11 \u00een toate grupurile etnice<sup>1;2;3<\/sup>.<\/p>\n<p><strong><span style=\"text-decoration: underline;\">Anticorpii anti-centromer<\/span><\/strong> au fost descri\u0219i cu pattern-ul caracteristic \u00a0o dat\u0103 cu introducerea substratului Hep2 \u00een tehnica de imunofluorescen\u021b\u0103 indirect\u0103, ceea ce nu a fost posibil utiliz\u00e2nd \u021besuturile animale. Anticorpii sunt \u00eendrepta\u021bi \u00eempotriva a 6 proteine centromerice (A-F) localizate \u00een zona de ancorare a fibrelor fusului mitotic \u00een cursul mitozei.Toate serurile care con\u021bin anticorpi anti-centromer reac\u021bioneaz\u0103 cu CENP-B (80 kDa).<\/p>\n<p>Anticorpii anti-CENP B constituie un marker serologic pentru\u00a0 sindromul CREST, un subtip de scleroz\u0103 sistemic\u0103<\/p>\n<p>(prevalen\u021b\u0103 70-90%). Acest tip de autoanticorpi mai poate ap\u0103rea \u00een sindromul Raynaud \u0219i \u00een ciroza biliar\u0103<\/p>\n<p>primitiv\u0103.<\/p>\n<p>Studiile clinice au ar\u0103tat c\u0103 pacien\u021bii cu scleroz\u0103 sistemic\u0103 av\u00e2nd anticorpi anti\u2013centromer pozitivi \u0219i anti-Scl 70 negativi prezint\u0103 o rat\u0103 de supravie\u021buire la 10 ani cu mult mai bun\u0103. \u00cen general, prezen\u021ba anticorpilor anti-centromer determin\u0103 un prognostic mai bun al bolii dec\u00e2t al\u021bi autoanticorpi asocia\u021bi cu scleroza sistemic\u0103. Frecventa anticorpilor anti-centromer la pacien\u021bii cu scleroz\u0103 sistemic\u0103 poate varia \u00een func\u021bie de originea etnic\u0103 (crescut\u0103 la caucazieni, sc\u0103zut\u0103 la hispanici \u0219i afro-americani)<sup>1;3;5<\/sup>.<\/p>\n<p><strong><span style=\"text-decoration: underline;\">Anticorpii anti-ARN-polimeraza III (RNAP III)<\/span><\/strong> se \u00eent\u00e2lnesc \u00eempreun\u0103 cu anti-RNAP I \u0219i anti-RNAP II la 20% din pacien\u021bii cu scleroz\u0103 sistemic\u0103, av\u00e2nd valoare diagnostic\u0103 \u0219i prognostic\u0103. Specificitatea anti-RNAP III \u0219i RNAP I este similar\u0103 \u00eens\u0103 mai bun\u0103 dec\u00e2t cea a anticorpilor anti-RNAP II, ace\u0219tia din urm\u0103 put\u00e2ndu-se \u00eent\u00e2lni \u00een sindroame de tip overlap LES\/scleroz\u0103 sistemic\u0103. Anticorpii\u00a0 anti-RNAP I \u0219i anti-RNAP III coexist\u0103 \u00eentotdeauna. Anticorpii anti-RNAP apar mai frecvent \u00een forma difuz\u0103 a sclerozei sistemice\u00a0 \u0219i sunt \u00eendrepta\u021bi \u00eempotriva unor proteine nucleolare \u0219i nucleare. Complexul RNAP I este localizat \u00een nucleoli, \u00een timp ce proteinele RNAP II \u0219i III se g\u0103sesc \u00een nucleoplasm\u0103.<\/p>\n<p><strong><span style=\"text-decoration: underline;\">Anticorpii anti-fibrilarin\u0103 (anti-U3RNP)<\/span><\/strong> sunt \u00eendrepta\u021bi \u00eempotriva\u00a0 unei proteine de 34 kDa, subunitate a\u00a0 familiei de proteine\u00a0 snRNP (ribonucleoproteine nucleare mici) implicate \u00een procesarea ARN-ului mesager. Anticorpii anti-fibrilarin\u0103 apar la aproximativ 4% din pacien\u021bii cu scleroz\u0103 sistemic\u0103, fiind specifici pentru forma difuz\u0103 de boal\u0103 \u0219i asociindu-se cu manifest\u0103rile de miozit\u0103, hipertensiune pulmonar\u0103 \u0219i afectare renal\u0103.<\/p>\n<p>Ace\u0219ti autoanticorpi au mai fost descri\u0219i \u0219i la pacienti cu LES, \u00a0boal\u0103 mixt\u0103 de \u021besut conjunctiv \u0219i sindrom Raynaud.<\/p>\n<p><strong><span style=\"text-decoration: underline;\">Anticorpii anti-Th\/To<\/span><\/strong> au fost identifica\u021bi mai recent \u00a0\u0219i sunt \u00eendrepta\u021bi \u00eempotriva componentelor ribonucleoproteazei\u00a0 MRP \u0219i complexului ribonucleazei P, care se g\u0103sesc la nivelul nucleolului. Anticorpii Th\/To sunt prezen\u021bi la 2-5% din pacien\u021bii cu scleroz\u0103 sistemic\u0103, fiind mai frecven\u021bi \u00een popula\u021bia japonez\u0103. De asemenea, ace\u0219ti anticorpi au fost descri\u0219i la pacien\u021bi cu LES, sindrom Raynaud \u0219i polimiozit\u0103. Anticorpii anti-Th\/To nu se asociaz\u0103 \u00een general cu anticorpii anti-centromer<sup>1;2<\/sup>.<\/p>\n<p><strong><span style=\"text-decoration: underline;\">Anticorpii anti-NOR 90<\/span><\/strong> recunosc un component de 90kDa al regiunii\u00a0 cromozomiale\u00a0 \u00een jurul c\u0103reia se formeaz\u0103 nucleolul (Nucleolus organizer region). Antigenul NOR 90 a mai fost descris \u0219i ca hUBF (human upstream binding factor) cu 2 izoforme &#8211; factorul de transcriptie al ARN polimerazei I. Anti-NOR 90 sunt \u00eent\u00e2lni\u021bi la pacien\u021bi cu scleroz\u0103 sistemic\u0103, sindrom Raynaud, LES, artrit\u0103 reumatoid\u0103 si diferite malignit\u0103\u021bi<sup>2<\/sup>.<\/p>\n<p><strong><span style=\"text-decoration: underline;\">Anticorpii anti PM-Scl<\/span><\/strong> sunt \u00eendrepta\u021bi \u00eempotriva proteinelor\u00a0 complexului nucleolar PM-Scl, compus din 11-16 polipeptide \u00een care componentele antigenice au greut\u0103\u021bi moleculare cuprinse \u00eentre 20 kDa \u0219i 110 kDa. Principalele antigene sunt 2 polipeptide de 75 \u0219i respectiv 100 kDa, cunoscute <span style=\"text-decoration: underline;\">ca PM-Scl 75<\/span> \u0438 <span style=\"text-decoration: underline;\">PM-Scl 100<\/span>. Cele 2 antigene sunt independente unul de cel\u0103lat \u0219i nu reac\u021bioneaz\u0103 \u00eencruci\u0219at. Acest complex de proteine este implicat \u00een biosinteza ribozomilor. Anticorpii anti-PM-Scl se asociaz\u0103 frecvent cu sindroamele overlap sclerodermie-polimiozit\u0103\/dermatomiozit\u0103.<\/p>\n<p><strong><span style=\"text-decoration: underline;\">Anticorpii anti-Ku<\/span><\/strong> sunt \u00eendrepta\u021bi \u00eempotriva unui antigen proteic nucleolar de tip heterodimer, compus din dou\u0103 subunit\u0103\u021bi cu masa molecular\u0103 de 70 \u0219i respectiv 80 kDa, component al unui complex enzimatic al proteinkinazei dependente de ADN. Acest complex de\u021bine un rol important \u00een procese celulare legate de transla\u021bie, repararea ADN-ului \u0219i rearanjarea genelor implicate \u00een sinteza imunoglobulinelor, precum \u0219i a receptorilor celulelor T.\u00a0 Anticorpii anti-Ku sunt \u00eent\u00e2lni\u021bi \u00eentr-un spectru larg de boli ale \u021besutului conjunctiv, \u00een special sindroame overlap sclerodermie-miozit\u0103, dar \u0219i LES (3-10%). Aspectul \u00een imunofluorescen\u021ba indirect\u0103 este de tip granular (p\u0103tat); de asemenea a fost descris\u0103 o asociere cu alela HLA DQw1. Prevalen\u021ba anticorpilor anti-Ku \u00een afec\u021biunile autoimune este variabil\u0103, originea etnic\u0103 fiind un element important. Spre exemplu, \u00een America de Nord \u0219i Europa au fost descrise cazuri de SLE asocia\u021bi cu anticorpi anti-Ku, dar nu \u0219i \u00een Japonia. Sindromul Sj\u00f6gren primar, ca \u0219i hipertensiunea pulmonar\u0103 primitiv\u0103 constituie alte condi\u021bii clinice frecvent asociate cu anti-Ku<sup>1;2<\/sup>.<\/p>\n<p><strong><span style=\"text-decoration: underline;\">Anticorpii anti-PDGFR<\/span><\/strong><\/p>\n<p>PDGFR (platelet derived growth factor receptor) sunt receptori de suprafa\u021b\u0103 tirozinkinazici\u00a0 pentru membrii familiei PDGF &#8211; factori care regleaz\u0103 proliferarea, diferen\u021bierea, cre\u0219terea \u0219i dezvoltarea celular\u0103. PDGFR de\u021bin un rol important \u00een dezvoltarea embrionului \u0219i\u00a0 contribuie la \u00eentre\u021binerea\u00a0 \u021besutului conjunctiv la adult. Dereglarea semnalului PDGFR\u00a0 conduce la ateroscleroz\u0103, hipertensiune pulmonar\u0103 \u0219i fibroza organelor. Fibroza rezult\u0103 din depunerea excesiv\u0103 a matricei extracelulare ca urmare a activarii fibrobla\u0219tilor (celule de natur\u0103 mezenchimal\u0103). PDGF promoveaz\u0103 expansiunea fibrobla\u0219tilor, astfel c\u0103 \u00een leziunile fibrotice se g\u0103sesc nivele crescute de PDGF. Anticorpii anti-PDGFR\u00a0 au fost descri\u0219i la pacien\u021bi cu scleroz\u0103 sistemic\u0103<sup>2<\/sup>.<\/p>\n<p><strong><span style=\"text-decoration: underline;\">Anticorpii anti-Ro52<\/span><\/strong>\u00a0 sunt \u00eendrepta\u021bi \u00eempotriva unei proteine de 52 kDa. Dup\u0103 \u00eendep\u0103rtarea prin sp\u0103lare a conjugatului nelegat se adaug\u0103 solu\u021bia de colorare (substratul enzimei) \u2013 un sistem de detec\u021bie enzimatic\u0103 colorimetric\u0103 . Dac\u0103 este prezent conjugatul legat reac\u021bia enzimatic\u0103 va genera un produs colorat albastru-\u00eenchis la nivelul benzilor ocupate de anticorpi specifici. Astfel, benzile vizualizate la nivelul stripului sunt rezultatul leg\u0103rii\u00a0 anticorpilor specifici de antigenele individuale de la nivelul strip-ului.<\/p>\n<p>Pe fiecare strip\u00a0 exist\u0103 o band\u0103 de control intern.
