Informa\u021bii generale \u0219i recomand\u0103ri pentru efectuarea anticorpilor anti-anhidraz\u0103 carbonic\u0103 II<\/em><\/strong><\/p>\n

Anhidrazele carbonice (engl. carbonic anhydrases, CA) fac parte dintr-o familie de metaloenzime ce con\u021bin zinc, care scindeaz\u0103 reversibil acidul carbonic \u00een dioxid de carbon \u0219i ap\u0103. Sunt implicate \u00eentr-o varietate de procese biologice precum respira\u021bia, echilibrul acido-bazic, calcificarea, resorb\u021bia osoas\u0103 precum \u0219i \u00een formarea salivei, sucului gastric, umorii apoase \u00a0<\/sup>\u0219i lichidului cefalo-rahidian (LCR)1<\/sup>. La om au fost descrise 14 izoenzime av\u00e2nd distribu\u021bii tisulare \u0219i localiz\u0103ri celulare diferite2<\/sup>.<\/p>\n

Izoenzima II a CA (CA-II) cu localizare citoplasmatic\u0103 este exprimat\u0103 de celulele epiteliale de la nivelul ductelor pancreatice, biliare, ale glandelor salivare, precum \u0219i de celulele intercalare ale tubilor colectori renali3<\/sup>.<\/p>\n

Mai multe studii au indicat prezen\u021ba de anticorpi ce \u021bintesc CA-II \u00een unele boli autoimune, cum ar fi sindromul Sj\u00f6gren, lupusul eritematos sistemic4<\/sup>, artrita reumatoid\u01035<\/sup>, boala Graves6<\/sup>, ciroza biliar\u0103 primitiv\u01037<\/sup>.\u00a0 \u00cen artrita reumatoid\u0103 s-a presupus c\u0103 stresul oxidativ eritrocitar crescut ar putea fi implicat \u00een apari\u021bia acestor autoanticorpi5<\/sup>. \u00cen sindromul Sj\u00f6gren anticorpii anti-CAII ar fi responsabili de pH-ul urinar crescut \u0219i acidoza tubular\u0103 renal\u01038<\/sup>. La pacien\u021bii cu ciroz\u0103 biliar\u0103 primitiv\u0103 care nu prezint\u0103 anticorpi antimitocondriali au fost propu\u0219i ca test diagnostic7<\/sup>.<\/p>\n

O asociere important\u0103 a anti-CAII este cu pancreatita autoimun\u0103, o form\u0103 de pancreatit\u0103 cronic\u0103 idiopatic\u0103 ce genereaz\u0103 colestaz\u0103 obstructiv\u0103 \u0219i care r\u0103spunde favorabil la corticoterapie. Comparativ cu pancreatita cronic\u0103 alcoolic\u0103, puseele de acutizare, calcific\u0103rile \u0219i pseudochisturile sunt rare \u00een aceast\u0103 afec\u021biune, \u00een schimb prezen\u021ba unei mase pancreatice este \u00eenregistrat\u0103 destul de frecvent, ceea ce face dificil diagnosticul diferen\u021bial cu cancerul de pancreas. Av\u00e2nd \u00een vedere faptul c\u0103 prevalen\u021ba anti-CAII \u00een pancreatita autoimun\u0103 este de 31-33% \u0219i c\u0103 ace\u0219tia se coreleaz\u0103 cu manifest\u0103rile clinice ale bolii, Japan Pancreas Society a recomandat ca autoanticorpii s\u0103 fie inclu\u0219i \u00een criteriile de diagnostic ale pancreatitei autoimune. Cu toate acestea, anti-CAII nu sunt specifici pentru aceast\u0103 afec\u021biune, prezen\u021ba lor fiind \u00eenregistrat\u0103 \u0219i \u00een 10% din cazurile de pancreatit\u0103 acut\u0103 alcoolic\u0103. Pentru a cre\u0219te specificitatea diagnosticului s-a propus ca anti-CAII s\u0103 fie testa\u021bi \u00eempreun\u0103 cu IgG4. Nivelul seric de IgG4 este crescut \u00een 90% din cazurile de pancreatit\u0103 acut\u0103 \u00eens\u0103 poate fi detectat cu frecven\u021b\u0103 mai redus\u0103 \u0219i \u00een cancerul pancreatic \u0219i pancreatita cronic\u0103 alcoolic\u0103. S-a constatat c\u0103 exist\u0103 o asociere str\u00e2ns\u0103 \u00eentre nivelurile de IgG4, anti-CAII, simptomatologia compatibil\u0103 cu pancreatita autoimun\u0103 \u0219i infiltratul limfoplasmocitic depistat la examenul histopatologic9<\/sup>.<\/p>\n
