Datorit\u0103 con\u021binutului ridicat \u00een leg\u0103turi duble \u00eentre atomii de carbon prezint\u0103 un poten\u021bial reduc\u0103tor mai mare dec\u00e2t vitamina C sau vitamina E, constituind prima linie de ap\u0103rare \u00eempotriva radicalilor liberi \u0219i un stabilizator optim al canalelor ionice membranare<sup>2<\/sup>.<\/p>\n<p style=\"text-align: justify;\">La om provine \u00een principal din biosintez\u0103 care are loc \u00een majoritatea \u021besuturilor, enzima -3 hidroxi-3-metil-glutaril-coenzima A <strong>reductaza<\/strong> (HMG-CoA) av\u00e2nd un rol important \u00een acest proces<sup>3<\/sup>. Coenzima Q10 poate fi preluat\u0103 \u00eentr-un procent sc\u0103zut \u0219i din alimenta\u021bie, carnea, pe\u0219tele, nucile \u0219i anumite uleiuri fiind principalele surse. Fiind liposolubil\u0103, absorb\u021bia sa la nivelul intestinului sub\u021bire este stimulat\u0103 de ingestia de lipide, prin cre\u0219terea excre\u021biei de acizi biliari<sup>4<\/sup>.<\/p>\n<p style=\"text-align: justify;\">Se g\u0103se\u0219te \u00een toate membranele celulare (ubicuitar\u0103, de unde provine \u0219i numele de ubiquinon\u0103) \u0219i este transportat\u0103 de lipoproteine \u00een circula\u021bie. Aproximativ 60% din coenzima Q10 este asociat\u0103 cu lipoproteine cu densitate sc\u0103zut\u0103 (LDL), 25% cu lipoproteine cu densitate mare (HDL) \u0219i 15% cu alte lipoproteine. Este important de men\u021bionat faptul c\u0103 este prezent\u0103 \u00een organism at\u00e2t sub form\u0103 redus\u0103 c\u00e2t \u0219i sub form\u0103 oxidat\u0103, capacitatea antioxdiant\u0103 depinz\u00e2nd nu numai de concentra\u021bia sa, ci \u0219i de statusul redox<sup>1<\/sup>.<\/p>\n<p style=\"text-align: justify;\">\u00cen condi\u021bii fiziologice celulele sintetizeaz\u0103 o cantitate suficient\u0103 de coenzim\u0103 Q10 pentru asigurarea func\u021biilor sale. Nivelul de coenzim\u0103 Q 10 scade odat\u0103 cu v\u00e2rsta, iar anumite medicamente precum \u0219i diverse afec\u021biuni pot reduce de asemenea nivelul acesteia<sup>3<\/sup>.<\/p>\n<p style=\"text-align: justify;\">Deficien\u021ba primar\u0103 de coenzim\u0103 Q10 este cauzat\u0103 de muta\u021bii inactivatoare la nivelul genelor ce codific\u0103 proteine relevante pentru procesul de biosintez\u0103 a acesteia \u0219i se caracterizeaz\u0103 prin simptome neurologice (convulsii, retard de dezvoltare, ataxie, etc) \u0219i sl\u0103biciune muscular\u0103<sup>1;3<\/sup>. Au fost descrise cel pu\u021bin cinci fenotipuri distincte de deficien\u021ba primar\u0103 de coenzim\u0103 Q10:<\/p>\n<ul style=\"text-align: justify;\">\n<li>encefalomiopatie (activitate crescut\u0103 a creatinkinazei serice, mioglobinurie recurent\u0103, acidoz\u0103 lactic\u0103)<\/li>\n<li>ataxie \u0219i atrofie cerebeloas\u0103 cu debut \u00een copil\u0103rie (neuropatie, hipogonadism)<\/li>\n<li>form\u0103 infantil\u0103 multisistemic\u0103 (nistagmus, atrofie de nerv optic, surditate neurosenzorial\u0103, distonie, nefropatie rapid progresiv\u0103)<\/li>\n<li>glomerulopatie<\/li>\n<li>miopatie (intoleran\u021b\u0103 la efort, miastenie, activitate crescut\u0103 a creatinkinazei serice).<\/li>\n<\/ul>\n<p style=\"text-align: justify;\">Tratamentul cu coenzima Q10 \u00een aceste afec\u021biuni mitocondriale poate \u00eembun\u0103t\u0103\u021bi metabolismul energetic din creier \u0219i \u021besutul muscular<sup>1<\/sup>.<\/p>\n<p style=\"text-align: justify;\">Coenzima Q10 a fost implicat\u0103 \u0219i \u00een alte afec\u021biuni cum ar fi boala Parkinson, boala Alzheimer, diabetul zaharat, precum \u0219i \u00een procesul de senescen\u021b\u0103 \u0219i stresul oxidativ<sup>1<\/sup>. \u00cen ceea ce prive\u0219te afec\u021biunile cardiovasculare, i se atribuie coenzimei Q10 un rol de \u00eembun\u0103t\u0103\u021bire a func\u021biei endoteliale<sup>5<\/sup>. De asemenea, conform unor studii, administrarea de oenzim\u0103 Q10 poate ameliora func\u021bia diastolic\u0103 la copiii cu cardiomiopatie dilatativ\u0103 idiopatic\u0103<sup>6<\/sup>.<\/p>\n<p style=\"text-align: justify;\">Rela\u021bia posibil\u0103 \u00eentre deficien\u021ba de coenzim\u0103 Q10 \u0219i efectele adverse ale terapiei cu statine este intens dezb\u0103tut\u0103, \u00een special dac\u0103 suplimentarea acestui compus ar putea ameliora mialgiile induse de statine<sup>5<\/sup>. Mai multe studii au ar\u0103tat c\u0103 statinele pot sc\u0103dea nivelurile de coenzim\u0103 Q10 at\u00e2t \u00een ser c\u00e2t \u0219i \u00een \u021besutul muscular. Prin inhibarea HMG-CoA statinele interfer\u0103 cu produc\u021bia de mevalonat, un precursor \u00een biosinteza coenzimei Q10; de asemenea tratamentul hipolipemiant reduce lipoproteinele care transport\u0103 coenzima Q10 \u00een circula\u021bie<sup>7<\/sup>.
