Modelele de acumulare a porfirinei \u00een eritrocite \u0219i plasm\u0103 \u0219i excre\u021bia precursorilor hemului \u00een urin\u0103 \u0219i fecale permit detectarea \u0219i diferen\u021bierea porfirilor. Testarea nivelului de porfirin\u0103 eritrocitar\u0103 este cel mai informativ pentru pacien\u021bii cu suspiciune clinic\u0103 de protoporfirie eritropoietic\u0103 (EPP) sau protoporfirie dominant\u0103 legat\u0103 de cromozomul X (XLDPP). Prezentarea clinic\u0103 a EPP \u0219i XLDPP este identic\u0103, cu apari\u021bia simptomelor, de cele mai multe ori in copil\u0103rie. De\u0219i sunt de natur\u0103 genetic\u0103, factorii de mediu exacerbeaz\u0103 simptomele, afect\u00e2nd \u00een mod semnificativ severitatea \u0219i evolu\u021bia boalii. EPP este cauzat de sc\u0103derea activit\u0103\u021bii ferochelatazei, rezult\u00e2nd niveluri semnificativ crescute de protoporfirin\u0103 liber\u0103 \u00een\u00a0eritrocite, plasm\u0103 \u0219i fecale.<\/p>\n<p>O reducere a aportului de fier poate fi recunoscut\u0103 \u0219i prin m\u0103surarea\u00a0protoporfirinei IX\u00a0(protoporfirin\u0103 eritrocitar\u0103 liber\u0103 sau FEP) \u00een globulele ro\u0219ii periferice. Insuficien\u021ba fierului in organism limiteaz\u0103 etapa final\u0103 de sintez\u0103 a hemului, determin\u00e2nd acumularea de FEP \u00een precursorii eritrocitelor<sup>1<\/sup>. Con\u021binutul celular r\u0103m\u00e2ne neschimbat pe toat\u0103 durata de via\u021b\u0103 a eritrocitului. Valorile FEP din s\u00e2ngele periferic reflect\u0103, prin urmare, aportul de fier \u00een timpul produc\u021biei de celule ro\u0219ii. Nivelurile FEP sunt, de asemenea, crescute \u00een intoxica\u021bia cu plumb<sup>2<\/sup>. FEP este crescut la numai c\u00e2teva s\u0103pt\u0103m\u00e2ni de eritropoiez\u0103 cu deficit de fier \u0219i necesit\u0103 o perioad\u0103 similar\u0103 pentru normalizare, odat\u0103 cu restabilirea unui aport adecvat de fier<sup>3<\/sup>.<\/p>\n<p>De asemenea, FEP se determin\u0103 pentru diagnosticarea\/monitorizarea protoporfiriei eritropoietice sau protoporfiriei legat\u0103 de X<sup>4<\/sup>\u00a0(X-Linked Protoporphyria), o boal\u0103 autozomal\u0103. Protoporfiria eritropoietic\u0103 \u0219i protoporfiria legat\u0103 de X sunt fotodermatoze genetice rare, rezultate din defecte ale enzimelor c\u0103ii hemo-biosintetice. Protoporfiria eritropoietic\u0103 rezult\u0103 din deficien\u021ba par\u021bial\u0103 a ferochelatazei, iar protoporfiria legat\u0103 de X rezult\u0103 in urma unor muta\u021bii- ALAS2. Ambele tulbur\u0103ri duc la acumularea de protoporfirin\u0103 eritrocitar\u0103, care este eliberat\u0103 \u00een plasm\u0103 \u0219i preluat\u0103 de ficat \u0219i endoteliu vascular.
