Informa\u021bii generale<\/em><\/strong><\/p>\n

Anticorpii anticardiolipinici apar\u021bin grupului anticorpilor antifosfolipidici specifici pentru fosfolipidele anionice (\u00eenc\u0103rcate negativ), componente ale membranelor celulare1<\/sup>. Fosfolipidele anionice (de exemplu, fosfatdilserina) sunt localizate pe suprafa\u021ba citoplasmatic\u0103 a membranei \u00een timp ce fosfolipodele neutre (cum ar fi fosfadilcolina) se g\u0103sesc predominant la suprafa\u021ba extern\u0103.<\/p>\n

Fosfolipidele membranare particip\u0103 la anumite procese celulare importante cum ar fi schimbul transmembranar de metaboli\u021bi \u0219i transferul de semnale moleculare; servesc de asemenea ca platforme pentru constituirea complexelor proteine-lipide. Activarea celular\u0103 este \u00eenso\u021bit\u0103 adesea de mobilizarea fosfolipidelor anionice c\u0103tre suprafa\u021ba extern\u0103 a membranei. De exemplu, \u00een cursul procesului de coagulare fosfadilserina este translocat\u0103 de pe fa\u021ba intern\u0103 a membranei trombocitare oferind o suprafa\u021ba de asamblare pentru complexul protrombinazei care catalizeaz\u0103 generarea de trombin\u0103.<\/p>\n

Complexele formate de fosfolipidele anionice \u0219i proteinele plasmatice endogene furnizeaz\u0103 epitopi pentru anticorpii naturali. \u00cen mod normal, plasma con\u021bine concentra\u021bii sc\u0103zute de astfel de autoanticorpi de tip IgG ce prezint\u0103 o afinitate moderat\u0103. Nivelurile patologice sunt consecin\u021ba pierderii toleran\u021bei imunologice \u0219i a produc\u021biei excesive de autoanticorpi2<\/sup>.<\/p>\n

Cardiolipina este un fosfolipid acid, derivat din glicerol \u0219i a fost pentru prima dat\u0103 izolat\u0103 \u00een 1941 din cordul bovin, de unde provine \u0219i denumirea sa1<\/sup>.<\/p>\n

Anticorpii anticardiolipinici sunt adesea prezen\u021bi la persoanele cu sindrom antifosfolipidic (Hughes). Acesta constituie o boal\u0103 autoimun\u0103 caracterizat\u0103 prin repetate tromboze arteriale \u0219i venoase, sarcini pierdute recurent \u0219i\/sau trombocitopenie asociate cu prezen\u021ba anticorpilor anticardiolipinici, a anticoagulantului lupic sau a anticorpilor anti-\u03b22 glicoproteina-1. Exist\u0103 2 forme clinice de sindrom antifosfolipidic: primar (53% din cazuri) sau \u00een asociere cu alte afec\u021biuni autoimune (LES, sindrom Sj\u00f6gren, sclerodermie, boal\u0103 mixt\u0103 de \u021besut conjunctiv, artrit\u0103 reumatoid\u0103, artrit\u0103 psoriazic\u0103, vasculite, boala Crohn, etc.)3<\/sup>.<\/p>\n

Majoritatea anticorpilor anticardiolipinici recunosc drept \u021bint\u0103 antigenic\u0103 \u03b22 glicoproteina 1, exprimat\u0103 pe suprafa\u021ba celulelor endoteliale \u0219i a trombocitelor. Autoanticorpii anti-\u03b22 glicoproteina 1 activeaz\u0103 celula endotelial\u0103, monocitele \u0219i trombocitele, av\u00e2nd un efect procoagulant4<\/sup>.<\/p>\n

Testul de aglutinare VDRL folosit de mult timp \u00een diagnosticul sifilisului, se bazeaz\u0103 pe detectarea anticorpilor anticardiolipinici. Un VDRL repetat pozitiv \u00een absen\u021ba TPHA exclude diagnosticul de sifilis \u0219i impune continuarea investiga\u021biilor \u00een vederea identific\u0103rii unui posibil sindrom antifosfolipidic.<\/p>\n

Testele imunoenzimatice pentru detectarea anticorpilor anticardiolipinici sunt de 100 ori mai sensibile dec\u00e2t testul VDRL \u0219i genereaz\u0103 mult mai multe rezultate pozitive.<\/p>\n

Criteriile pentru diagnosticul de laborator al sindromului antifosfolipidic includ prezen\u021ba anticoagulantului lupic, a anticorpilor anticardiolipinici de tip IgG \u0219i\/sau IgM \u00een titruri moderate sau crescute (>40 GPL, >40 MPL sau >percentila 99 a limitei superioare de referin\u021b\u0103) sau a anticorpilor anti-\u03b22 glicoproteina-1 de tip IgG \u0219i\/sau IgM (\u00een titruri >percentila 99 a limitei superioare de referin\u021b\u0103). Pentru oricare din ace\u0219ti anticorpi trebuie s\u0103 existe cel pu\u021bin dou\u0103 rezultate pozitive ob\u021binute la un interval de minim 12 s\u0103pt\u0103m\u00e2ni. Anticorpii anticardiolipinici \u0219i anticorpii anti-\u03b22 glicoproteina-1 de tip IgA nu au fost inclu\u0219i \u00een criteriile de diagnostic datorit\u0103 lipsei lor de specificitate5<\/sup>.<\/p>\n

\u00cen general, anticorpii anticardiolipinici sunt mai sensibili dec\u00e2t anticoagulantul lupic pentru detectarea sindromului antifosfolipidic. Anticorpii sunt prezen\u021bi la 80-90% din pacien\u021bii cu sindrom antifosfolipidic, av\u00e2nd o frecven\u021b\u0103 de cinci ori mai mare \u00een aceast\u0103 afec\u021biune dec\u00e2t anticoagulantul lupic. Cu toate acestea, <\/strong>anticoagulantul lupic este considerat a fi mai specific dec\u00e2t anticorpii anticardiolipinici6<\/sup>.<\/p>\n

Recomand\u0103ri pentru determinarea anticorpilor anticardiolipinici <\/em><\/strong>– episoade de tromboz\u0103 venoas\u0103 sau arterial\u0103, survenite \u00een special la pacien\u021bi tineri, \u00een absen\u021ba oric\u0103rui diagnostic alternativ sau a altei etiologii trombotice; avorturi spontane, na\u0219tere prematur\u0103 sau na\u0219tere de f\u0103t mort (cu f\u0103t morfologic normal); evaluarea riscului trombotic la pacien\u021bii diagnostica\u021bi cu boli de \u021besut conjunctiv; evaluarea unei trombocitopenii neexplicabile; test VDRL pozitiv \u00een absen\u021ba TPHA3;5<\/sup>.<\/p>\n

Stabilitate prob\u0103 <\/em><\/strong>– <\/em>serul separat este stabil minim 7 zile<\/em> la 4\u00b0C; timp mai \u00eendelungat<\/em> la -20 \u00b0C2;7<\/sup>.<\/p>\n

Metod\u0103<\/em><\/strong> - imunochimic<\/u>\u0103 cu detec\u021bie prin chemiluminiscen\u021b\u0103 (CLIA)<\/u>. Antigenul folosit este o cardiolipin\u0103 \u00eenalt purificat\u0103 \u00eempreun\u0103 cu \u03b22 glicoproteina 1 uman\u0103 nativ\u0103. Sunt detecta\u021bi separat anticorpii anticardiolipinici de IgG \u0219i IgM. Ace\u0219tia pot fi solicita\u021bi individual, \u00eempreun\u0103 sau \u00een cadrul Profilului sindrom antifosfolipidic7<\/sup>.<\/p>\n

Pentru sindromul antifosfolipidic sunt semnificative valori >40 GPL \u0219i respectiv >40 MPL, ob\u021binute la 2 determin\u0103ri, efectuate la un interval de minimum 12 s\u0103pt\u0103m\u00e2ni5;7<\/sup>.<\/p>\n

Limite \u0219i interferen\u021be<\/em><\/strong><\/p>\n

Anticorpii anticardiolipinici pot fi depista\u021bi \u0219i la indivizi s\u0103n\u0103to\u0219i din popula\u021bia general\u0103, IgM fiind mai frecvent \u00eent\u00e2lni\u021bi (9.4%) \u00een compara\u021bie cu IgG (6,5 %). Inciden\u021ba acestor anticorpi este \u0219i mai mare \u00een sarcina normal\u0103, unde rata de detec\u021bie poate atinge 17% pentru IgM \u0219i respectiv 10.6% pentru IgG. Mul\u021bi din ace\u0219ti anticorpi sunt tranzitorii \u0219i nu se asociaz\u0103 cu sindrom antifosfolipidic6<\/sup>.<\/div>\n
De asemenea, ace\u0219ti autoanticorpi pot fi prezen\u021bi \u0219i \u00een infec\u021bii de tip hepatit\u0103 C, malarie, boal\u0103 Lyme, sifilis, infec\u021bie HIV, leucemii, tumori solide \u0219i frecvent \u00een ciroze alcoolice.<\/div>\n
Datorit\u0103 acestor factori, diagnosticul de sindrom antifosfolipidic nu trebuie stabilit pe baza unei singure determin\u0103ri. Specificitatea anticorpilor anticardiolipinici pentru sindromul antifosfolipidic cre\u0219te cu titrul \u0219i este mai mare pentru IgG dec\u00e2t pentru IgM1<\/sup>.<\/div>\n
