Aceste sindroame au la baz\u0103 mecanisme imunologice al c\u0103ror efect se manifest\u0103 la distan\u021ba de tumor\u0103 primar\u0103 \u0219i\/sau site-ul metastatic<sup>1<\/sup>.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"color: #000000; font-size: 12pt;\">\u00cen literatur\u0103 sunt utilizate dou\u0103 tipuri de nomenclaturi pentru anticorpii antineuronali: una dintre acestea este bazat\u0103 pe primele 2 litere ale pacientului la care a fost descris\u00a0prima dat\u0103 anticorpul respectiv (Hu de la Hull, Yo de la Young, Ma de la Margret), iar cealalt\u0103 pe ini\u021bialele colora\u021biei imunohistochimice (ANNA = anticorpi antinucleari antineuronali)<sup>2<\/sup>.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"color: #000000; font-size: 12pt;\">\u00cen func\u021bie de tipul tumorii celulele tumorale exprim\u0103 antigene care induc formarea de autoanticorpi specifici. Ace\u0219ti autoanticorpi reac\u021bioneaz\u0103 \u00eencruci\u0219at cu antigene identice localizate la nivelul \u021besutului nervos \u0219i sunt probabil implica\u021bi \u00een dezvoltarea\u00a0sindroamelor paraneoplazice<sup>1<\/sup>.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"color: #000000; font-size: 12pt;\">Din momentul \u00een care au fost descri\u0219i pentru prima dat\u0103 anticorpii anti-Hu (1985) este bine cunoscut faptul c\u0103 mai mult de 2\/3 din\u00a0 pacien\u021bii cu sindroame paraneoplazice\u00a0prezint\u0103 \u00een ser autoanticorpi\u00a0 specifici. Reac\u021biile\u00a0antigen-anticorp nu numai c\u0103 demonstreaz\u0103 etiologia paraneoplazic\u0103 \u00een aproape 100% din cazuri, dar sunt de asemenea str\u00e2ns asociate cu tumori specifice, care p\u00e2n\u0103 atunci nu au fost detectate la ace\u0219ti pacien\u021bi<sup>3<\/sup>. Astfel, depistarea anticorpilor anti-neuronali poate fi foarte util\u0103 \u00een diagnosticul precoce al tumorilor<sup>2<\/sup>.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"color: #000000; font-size: 12pt;\">PNS\u00a0apar \u00een aproximativ 15% din afec\u021biunile neoplazice, \u00een particular\u00a0\u00een tumorile pulmonare \u0219i gastrice. Sindroamele\u00a0paraneoplazice oto-neuro-oftalmologice rezult\u0103 de obicei din encefalita de trunchi cerebral\u00a0 \u0219i\/sau degenerescen\u021ba cerebeloas\u0103. Acestea se pot dezvolta ca urmare a r\u0103spunsului imun fa\u021b\u0103 de proteine onconeurale (ca de exemplu, anti-Hu, anti-Yo, anti-Ri) sau alte antigene necunoscute.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"color: #000000; font-size: 12pt;\">PNS apar cel mai adesea asociate cu carcinomul\u00a0 pulmonar cu celule mici (SCLC), carcinomul mamar sau ovarian. <b>Sindromul\u00a0 miastenic Lambert-Eaton (LEMS)<\/b>, unul din cele mai frecvente PNS, prezint\u0103 cea mai mare valoare predictiv\u0103 \u00een ceea ce prive\u0219te existen\u021ba unei tumori de baz\u0103 (\u00een acest caz SCLC). In majoritatea PNS, \u00een special <b>encefalita limbic\u0103 paraneoplazic\u0103<\/b> (<b>PLE),<\/b> <b>degenerescen\u021ba cerebeloas\u0103 paraneoplazic\u0103<\/b> (<b>PCD<\/b>), <b>sindromul opsoclonus-mioclonus-ataxie <\/b>(<b>POMA<\/b>), <b>sindromul \u201estiff man\u201d <\/b>(<b>SPS<\/b>) \u0219i <b>retinopatia paraneoplazic\u0103 (amauroza subacuta)<\/b>, detec\u021bia anticorpilor specifici are valoare predictiv\u0103 pozitiv\u0103 crescut\u0103, corel\u00e2ndu-se mai bine cu prezen\u021ba tumorii de baz\u0103 dec\u00e2t simptomele clinice propriu-zise<sup>4;5<\/sup>.<b><\/b><\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"color: #000000; font-size: 12pt;\">Spre exemplu, PCD\u00a0 este determinat\u0103\u00a0 de mecanismele autoimune care apar \u00een unele neoplazii, mai ales \u00een stadiile precoce ale carcinoamelor mamare, ovariene, pulmonare precum \u0219i \u00een limfomul Hodgkin. Reac\u021bia imun\u0103 \u00eendreptat\u0103 \u00eempotriva antigenelor tumorale, identice cu antigenele\u00a0 cerebeloase, duce la formarea de anticorpi anti-Hu, anti-Yo, anti-Ri ce pot fi detecta\u021bi at\u00e2t \u00een s\u00e2nge c\u00e2t \u0219i\u00a0\u00een lichidul cefalorahidian. Asocierea existent\u0103 \u00eentre afec\u021biunea neoplazic\u0103 \u0219i modific\u0103rile cerebeloase este demonstrat\u0103 prin reac\u021bia de imunofluorescen\u021b\u0103 indirect\u0103 \u00een care anticorpii\u00a0anti-Yo reac\u021bioneaz\u0103 at\u00e2t cu celulele Purkinje cerebeloase c\u00e2t \u0219i cu celulele carcinomatoase<sup>2<\/sup>.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"color: #000000; font-size: 12pt;\">PNS nu numai c\u0103 acompaniaza tumorile maligne, dar pot\u00a0 fi \u00eent\u00e2lnite \u0219i \u00een afec\u021biuni\u00a0autoimune ca diabetul zaharat tip 1, c\u00e2nd se poate demonstra apari\u021bia \u00een paralel\u00a0a anticorpilor specifici\u00a0 anti-substan\u021b\u0103 cenu\u0219ie\u00a0\u0219i a\u00a0celor \u00eendrepta\u021bi \u00eempotriva celulelor insulare pancreatice. Astfel, detectarea autoanticorpilor fa\u021b\u0103 de enzima glutamat-decarboxilaz\u0103 (GAD) este important\u0103 at\u00e2t pentru depistarea precoce a diabetului zaharat tip 1 c\u00e2t \u0219i pentru diagnosticul unor afec\u021biuni neurologice, cum ar fi sindromul \u201estiff man\u201d asociat cu limfomul Hodgkin<sup>6<\/sup>.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"color: #000000; font-size: 12pt;\">Conform\u00a0recomand\u0103rilor Societ\u0103\u021bii\u00a0Germane de Neurologie, determinarea anticorpilor din sindroamele paraneoplazice (anticorpi antineuronali) ar trebui efectuat\u0103 utiliz\u00e2nd\u00a0cel pu\u021bin dou\u0103 metode: screening prin\u00a0 imunofluorescen\u021b\u0103 indirect\u0103 \u0219i confirmare\u00a0prin tehnica imunoblot. Rezultatele ob\u021binute au relevan\u021b\u0103 diagnostic\u0103 numai dac\u0103 sunt concordante prin cele dou\u0103 metode \u0219i se integreaz\u0103 \u00een tabloul clinic al pacien\u021bilor<sup>8;9<\/sup><span style=\"font-family: Times New Roman;\">.<\/span><\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"color: #000000; font-size: 12pt;\">I\u00ce tabelul
