Lu\u00e2nd \u00een considerare doar Statele Unite ale Americii,\u00a0Centers for Disease Control and Prevention\u00a0(CDC) a raportat \u00een 2013 1,401,906 cazuri noi de infec\u021bie cu\u00a0Chlamydia trachomatis\u00a0\u0219i 820,000 cazuri noi de infec\u021bie cu\u00a0Neisseria gonorrhoeae<sup>3<\/sup><\/p>\n<p>Chlamydia este o bacterie Gram negativ\u0103 de dimensiuni foarte mici, obligatoriu intracelular\u0103 datorit\u0103 faptului c\u0103 nu \u00ee\u015fi poate sintetiza ATP-ul, fiind cultivabil\u0103 doar \u00een celulele vii eucariote (ou\u0103 embrionate sau culturi celulare).<\/p>\n<p>Clasific\u0103ri recente \u00eempart Chlamydiile \u00een 4 familii<em>,<\/em>\u00a0dintre care cea mai important\u0103 \u00een patologia uman\u0103 este familia Chlamydiaceae, care con\u0163ine dou\u0103 genuri:<\/p>\n<ul>\n<li>genul Chlamydia \u2013 cel mai important reprezentant fiind C. trachomatis;<\/li>\n<li>genul Chlamydophila \u2013 C. pneumoniae, C. psittaci, C. pecorum, C. abortus, etc.<\/li>\n<\/ul>\n<h6><em>Chlamydia trachomatis<\/em>\u00a0este \u00eent\u00e2lnit\u0103 aproape \u00een exclusivitate la om. Sunt descrise 15 serovaruri implicate \u00een diverse infec\u0163ii<sup>1;2<\/sup>.<\/h6>\n<h6>Infec\u0163ia cu Chlamydia trachomatis este cea mai frecvent\u0103 boal\u0103 cu transmitere sexual\u0103 de cauz\u0103 bacterian\u0103. Grupa de v\u00e2rst\u0103 cea mai afectat\u0103 este 15-25 ani, prevalen\u0163a infec\u0163iei fiind \u00eentre 2 \u015fi 25%.<\/h6>\n<p>Perioada de incuba\u0163ie a bolii variaz\u0103 \u00een general \u00eentre 1 \u015fi 3 s\u0103pt\u0103m\u00e2ni, ajung\u00e2nd \u00een unele cazuri la 6 s\u0103pt\u0103m\u00e2ni. O mare parte a infec\u0163iilor evolueaz\u0103 asimptomatic: 75% din infec\u0163iile la femei \u015fi 50% din infec\u0163iile la b\u0103rba\u0163i. \u00cen cazurile simptomatice la b\u0103rbat pot ap\u0103rea uretrit\u0103 \u015fi proctit\u0103 \u00een faza acut\u0103; \u00een lipsa tratamentului se poate instala cronicizarea cu epididimit\u0103, prostatit\u0103 \u015fi artrit\u0103 reactiv\u0103. Simptomele clinice la femeie constau \u00een uretrit\u0103, cervicit\u0103 \u015fi bartolinit\u0103 care se pot complica cu: endometrit\u0103, salpingit\u0103, pelviperitonit\u0103, perihepatit\u0103 precum \u015fi artrit\u0103 reactiv\u0103.<\/p>\n<p>Caracterul asimptomatic al multor infec\u0163ii cu Chlamydia trachomatis este responsabil de subdiagnosticarea acestora, cu importante consecin\u0163e mai ales la femei datorit\u0103 corela\u0163iei dovedite \u00eentre infec\u0163ia netratat\u0103 \u015fi complica\u0163ii cum ar fi sarcina ectopic\u0103 \u015fi infertilitatea.<\/p>\n<p>Fiind o boal\u0103 cu transmitere sexual\u0103 evaluarea partenerilor este obligatorie pentru a preveni reinfec\u0163iile.<\/p>\n<p>Ca \u015fi \u00een cazul altor boli cu transmitere sexual\u0103 infec\u0163ia cu Chlamydia trachomatis poate facilita transmiterea infec\u0163iei HIV. \u00cen plus, nou-n\u0103scu\u0163ii se pot infecta la trecerea prin canalul cervical \u015fi pot dezvolta conjunctivit\u0103, faringit\u0103 \u015fi pneumonie<sup>3;4;5<\/sup>.<\/p>\n<p>Diagnosticul infec\u0163iilor cu Chlamydia trachomatis include izolarea agentului patogen \u00een culturi celulare, depistarea antigenului prin metode imunologice \u015fi determinarea ADN-ului prin tehnici moleculare. Deoarece culturile celulare sunt laborioase \u015fi necesit\u0103 timp nu se folosesc de rutin\u0103 fiind apanajul unor laboratoare speciale<sup>3;5<\/sup><\/p>\n<p>Numeroasele studii au indicat c\u0103 tehnicile de amplificare ADN detecteaz\u0103 cu aproximativ 15% mai multe \u00eenfec\u0163ii comparativ cu celelalte metode care nu se bazeaz\u0103 pe culturi celulare, cu o specificitate &gt;99%. \u00cen plus, testul ADN poate utiliza probe care nu sunt compatibile cu metodele bazate pe culturi cum ar fi cele de urin\u0103 la b\u0103rba\u0163i<sup>6<\/sup>.<\/p>\n<p>Pe l\u00e2ng\u0103 ADN-ul cromozomial C. trachomatis con\u0163ine o plasmid\u0103 criptic\u0103 care este comun\u0103 tuturor serovarurilor. Din acest motiv, testele moleculare au drept \u0163int\u0103 secven\u0163e nucleotidice de la nivelul acestei plasmide. \u00cen 2006 a fost descris\u0103 pentru prima dat\u0103 varianta suedez\u0103 de Chlamydia trachomatis ce prezint\u0103 prezint\u0103 o dele\u0163ie de 377 bp \u00een interiorul plasmidei ADN \u015fi care nu este detectat\u0103 de unele teste comerciale<sup>5<\/sup>.<\/p>\n<p><em>Neisseria gonorrhoeae<\/em>, un diplococ Gram negativ, este agentul etiologic al gonoreei, una din cele mai frecvente boli cu transmitere sexual\u0103.<\/p>\n<p>La b\u0103rba\u0163i infec\u0163ia este \u00een cele mai multe cazuri simptomatic\u0103, fiind asociat\u0103 cu uretrit\u0103 anterioar\u0103 \u00eenso\u0163it\u0103 de secre\u0163ie purulent\u0103. Perioada de incuba\u0163ie este de 1-10 zile. La femei principala localizare a infec\u0163iei este endocervixul. Se descrie o prevalen\u0163\u0103 crescut\u0103 a coinfec\u0163iilor cu Chlamydia trachomatis \u015fi Trichomonas vaginalis precum \u015fi asocierea cu vaginoza bacterian\u0103. Multe dintre femei r\u0103m\u00e2n asimptomatice \u015fi din acest motiv nu se prezint\u0103 la medic. Atunci c\u00e2nd apar manifest\u0103rile clinice acestea sunt reprezentate de secre\u0163ii vaginale modificate, disurie, dispareunie, s\u00e2nger\u0103ri intermenstruale. Alte posibile localiz\u0103ri ale infec\u0163iei gonococice sunt rectul, faringele, conjunctiva; rareori se pot produce infec\u0163ii diseminate. Copiii n\u0103scu\u0163i din mame infectate pot prezenta conjunctivit\u0103.<\/p>\n<p>\u00cen absen\u0163a tratamentului antibiotic adecvat se pot dezvolta complica\u0163ii acute sau tardive, cum ar fi: epididimit\u0103 sau prostatit\u0103 la b\u0103rba\u0163i; boal\u0103 inflamatorie pelvin\u0103, perihepatit\u0103, infertilitate, sarcini ectopice la femei<sup>3;4;5<\/sup>.<\/p>\n<ol>\n<li><em>gonorrhoeae<\/em>\u00a0este un germene labil care nu supravie\u0163uie\u015fte \u00een probele ce sunt recoltate \u00een afara laboratorului. Din acest motiv, diagnosticul microbiologic bazat pe cultur\u0103 are valoare sc\u0103zut\u0103 \u00een aceste cazuri.<\/li>\n<\/ol>\n<p>Dezvoltarea tehnicilor de amplificare a acizilor nucleici a \u00eembun\u0103t\u0103\u0163it depistarea infec\u0163iilor cu gonoc, aceste tehnici prezent\u00e2nd \u00een mod evident o serie de avantaje \u00een compara\u0163ie cu metodele clasice de cultivare:<\/p>\n<ul>\n<li>deoarece este detectat ADN-ul bacterian nu este necesar ca microorganismul s\u0103 fie viabil;<\/li>\n<li>prezint\u0103 o sensibilitate mai mare;<\/li>\n<li>la b\u0103rba\u0163i testul poate fi efectuat din urin\u0103, ceea ce cre\u015fte complian\u0163a pacien\u0163ilor la investiga\u0163ie<sup>6<\/sup>.<\/li>\n<\/ul>\n<p>Ca o concluzie la cele men\u0163ionate mai sus\u00a0<em>Chlamydia trachomatis\u00a0<\/em>(CT) \u015fi\u00a0<em>Neisseria gonorrhoeae\u00a0<\/em>(GC) sunt agen\u0163ii patogeni cel mai frecvent implica\u0163i \u00een bolilele cu transmitere sexual\u0103 de cauz\u0103 bacterian\u0103. Foarte important,\u00a0 coinfec\u0163ia CT-GC este raportat\u0103 \u00een aproximativ 30% din cazuri (mai frecvent la femei) de unde \u015fi necesitatea test\u0103rii combinate a celor doi patogeni.<\/p>\n<p>Diagnosticul precoce al infec\u0163iilor constituie o provocare deoarece o mare parte din persoanele afectate nu prezint\u0103 simptome. \u00cen absen\u0163a diagnosticului \u015fi tratamentului pot conduce la infec\u0163ii diseminate la b\u0103rba\u0163i \u015fi la boal\u0103 inflamatorie pelvin\u0103 la femei cu consecin\u0163e tardive semnificative: infertilitate, sarcin\u0103 ectopic\u0103, durere pelvin\u0103 cronic\u0103<sup>4;7<\/sup>.<\/p>\n<p>\u00cen cazul ambelor infec\u0163ii este posibil\u0103 transmiterea perinatal\u0103 cu dezvoltarea conjunctivitei la nou-n\u0103scut. De asemenea, CT \u015fi GC se asociaz\u0103 cu risc crescut de transmitere a infec\u0163iei HIV<sup>7<\/sup>.<\/p>\n<p>Una din cele mai bune metode de a preveni complica\u0163iile infec\u0163iilor CT \u015fi GC este screening-ul adolescen\u0163ilor \u015fi adul\u0163ilor tineri precum \u015fi a femeilor gravide. Dac\u0103 o persoan\u0103 este diagnosticat\u0103 cu una sau ambele infec\u0163ii to\u0163i partenerii sexuali trebuie testa\u0163i \u015fi trata\u0163i corespunz\u0103tor<sup>4<\/sup>.<\/p>\n<p>Ghidul CDC (Centers for Disease Control and Prevention) recomand\u0103 ca screening-ul s\u0103 se efectueze cu teste de amplificare a acizilor nucleici cu sensibilitate \u015fi specificitate \u00eenalt\u0103, confirm\u00e2nd faptul c\u0103 metodele moleculare constituie \u00een prezent standardul de diagnostic al infec\u0163iilor CT \u015fi GC<sup>3<\/sup>.<\/p>\n<p>\u00cen laboratorul nostru se utilizeaz\u0103 o metod\u0103 real-time PCR complet automatizat\u0103 a c\u0103rei performan\u0163\u0103 a fost recent evaluat\u0103 \u00een c\u00e2teva studii. Acestea au indicat faptul c\u0103 testul are o sensibilitate \u015fi specificitate foarte bune pentru detec\u0163ia calitativ\u0103 a ADN-ului CT \u015fi NG \u00een probe urogenitale, dovedind \u00een acela\u015fi timp \u015fi o eficien\u0163\u0103 \u00eenalt\u0103:<\/p>\n<ul>\n<li>pentru CT: sensibilitate de 6-98%, specificitate de \u226599.6%;<\/li>\n<li>pentru GC: sensibilitate de 9-100%, specificitate de \u226599.8%<sup>8;9<\/sup>.<\/li>\n<\/ul>\n<p>Din punct de vedere tehnic testul are la baz\u0103 amplificarea a dou\u0103 secven\u0163e ADN \u0163int\u0103 pentru fiecare din cei doi agen\u0163i patogeni \u00een scopul reducerii num\u0103rului de rezultate fals-negative \u015fi fals-pozitive. Astfel,\u00a0 \u00een cazul CT sunt utiliza\u0163i primeri \u015fi sonde specifice pentru o secven\u0163\u0103 de aproximativ 206 nucleotide din plasmida criptic\u0103 precum \u015fi pentru o secven\u0163\u0103 de cca 182 nucleotide din cadrul ADN-ului cromozomial (gena ompA care codific\u0103 proteina major\u0103 a membranei externe). Pentru NG se folosesc primeri \u015fi sonde care \u0163intesc at\u00e2t o secven\u0163\u0103 de aproximativ 190 nucleotide din regiunea repetitiv\u0103 direct\u0103 \u00eenalt conservat\u0103\u00a0 denumit\u0103 DR-9 c\u00e2t \u015fi o secven\u0163\u0103 de ~ 215 nucleotide identificat\u0103 drept o variant\u0103 de secven\u0163\u0103 conservat\u0103 din aceast\u0103 regiune. \u00cen plus, este introdus \u00een fiecare prob\u0103 un control intern CT\/NG reprezentat de o combina\u0163ie de dou\u0103 plasmide ADN neinfec\u0163ioase care monitorizeaz\u0103 toate etapele \u00a0reac\u0163iei PCR.<\/p>\n<p>\u00cen ceea ce prive\u015fte performan\u0163a analitic\u0103, testul este capabil s\u0103 identifice al\u0103turi de cele 14 serovaruri \u015fi varianta suedez\u0103 de CT; \u00een cazul NG nu exist\u0103 reac\u0163ii \u00eencruci\u015fate cu bacteriile\u00a0<em>Neisseria<\/em>\u00a0<em>cinerea<\/em>,\u00a0<em>flava, lactamica, perflava<\/em>,<em>\u00a0subflava\u00a0<\/em>raportate la alte teste moleculare<sup>5;8<\/sup>.<\/p>\n<p>Ca tip de probe sunt utilizate tamponul endocervical la femei \u015fi urina primul jet la b\u0103rba\u0163i. Men\u0163ion\u0103m c\u0103 \u00een cazul b\u0103rba\u0163ilor testarea probelor de urin\u0103 prezint\u0103 o sensibilitate \u015fi specificitate foarte bune reprezent\u00e2nd o alternativ\u0103 non-invaziv\u0103 la analiza probelor uretrale<sup>10<\/sup>.<\/p>\n<p><strong><em>Recomand\u0103ri pentru determinarea Chlamydia trachomatis ADN<\/em><\/strong>\u00a0\u2013 diagnosticul infec\u0163iilor acute urogenitale, screening-ul infec\u0163iei la persoanele cu risc crescut; screening-ul gravidelor \u00een primul trimestru de sarcin\u0103 (pentru gravidele \u2264 25 ani c\u00e2t \u015fi pentru cele cu risc crescut de infec\u0163ii CT \u015fi NG testarea va fi repetat\u0103 \u015fi \u00een trimestrul III; \u00een cazul gravidelor la care s-a ob\u0163inut un rezultat pozitiv \u00een trimestrul I testarea va fi repetat\u0103 dup\u0103 3-6 luni)<sup>3;6<\/sup>.<\/p>\n<p><strong><em>Specimen recoltat<\/em><\/strong>\u00a0\u2013 se vor recolta probe provenite din tractul urogenital: tampon endocervical pentru femei \u015fi urin\u0103 primul jet pentru b\u0103rba\u0163i; pentru femei se vor utiliza tampoanele incluse \u00een sistemele de recoltare furnizate de laborator (<em>vezi figura 1<\/em>).<\/p>\n<p><strong><u>Recoltarea probei endocervicale<\/u><\/strong><\/p>\n<p>\u00cenainte de recoltarea propriu-zis\u0103 se va \u00eendep\u0103rta mucusul \u00een exces de la nivelul orificiul colului cu ajutorul unuia din cele dou\u0103 tampoane incluse \u00een kit; se va introduce apoi cel\u0103lalt tampon \u00een canalul cervical, se va roti bl\u00e2nd de 5 ori \u00eentr-o singur\u0103 direc\u0163ie dup\u0103 care se va retrage u\u015for, se va evita contactul cu mucoasa vaginal\u0103 \u015fi se va introduce \u00een tubul de transport; se va rupe tamponul \u00een dreptul semnului astfel c\u0103 numai cap\u0103tul tamponului va r\u0103m\u00e2ne \u00een tubul de transport ce va \u00eenchis etan\u015f cu capacul. De asemenea prezen\u0163a s\u00e2ngelui \u00eentr-un procent mai mare de 5% (v\/v) \u00een probele endocervicale \u015fi mai mare de 0.35% (v\/v) \u00een probele de urin\u0103 transferate poate conduce la rezultate invalide sau fals-negative.<\/p>\n<p>Valoarea predictiv\u0103 a testului este influen\u0163at\u0103 de prevalen\u0163a infec\u0163iei \u00eentr-o anumit\u0103 popula\u0163ie sau grup. Astfel, \u00een grupurile cu prevalen\u0163\u0103 crescut\u0103 a bolilor cu transmitere sexual\u0103 valoarea predictiv\u0103 pozitiv\u0103 este crescut\u0103. \u00cen schimb \u00een grupurile cu prevalen\u0163\u0103 sc\u0103zut\u0103 a acestor categorii de infec\u0163ii rezultatele pozitive trebuie s\u0103 fie atent interpretate \u00een contextul clinic general al pacientului, lu\u00e2nd \u00een considerare \u015fi posibilitatea existen\u0163ei unor rezultate fals pozitive.<\/p>\n<p>Performan\u0163a stabilit\u0103 pentru probele de urin\u0103 se bazeaz\u0103 pe testarea primului jet de urin\u0103.
