Acest precursor con\u021bine peptide de extensie terminal\u0103: propeptid\u0103 amino-terminal\u0103 (PINP) \u0219i propeptid\u0103 carboxi-terminal\u0103 (PICP). Aceste extensii de propeptide sunt \u00eendep\u0103rtate de enzime specifice, \u00eenainte de formarea moleculelor de colagen. Ambele propeptide pot fi g\u0103site \u00een circula\u021bie, iar concentra\u021bia lor reflect\u0103 rata de sintez\u0103 a colagenului de tip I si implicit a tesutului osos.<\/p>\n<p>De\u0219i propeptidele de colagen tip I pot ap\u0103rea \u0219i din alte \u021besuturi (piele, vase de sange, cartilaje fibroase \u0219i tendoanele), majoritatea \u021besuturilor nonscheletice prezint\u0103 distructie\/regenerare mai lente dec\u00e2t cea osoas\u0103 \u0219i contribuie foarte pu\u021bin la rezerva circulant\u0103 a PINP. Este considerat cel mai sensibil biomarker al form\u0103rii osoase \u0219i este deosebit de util pentru monitorizarea terapiilor de sinteza osoas\u0103 \u0219i a terapiilor antiresorptive; se recomand\u0103 ca testul s\u0103 fie efectuat \u00eenainte de initierea terapiei anti-osteoporoza \u0219i repetat la 3- 6 luni.<\/p>\n<p><b><i>Informa\u021bii generale \u2013\u00a0 PINP (propeptid N-terminal al procolagenului de tip I)<\/i><\/b><\/p>\n<p>Propeptidele procolagen tip I sunt derivate din colagenul tip I, care este cel mai comun tip de colagen identificat \u00een osul mineralizat. \u00cen os, colagenul este sintetizat de osteoblaste sub form\u0103 de procolagen. Acest precursor con\u021bine o secven\u021b\u0103 semnal scurt\u0103 \u0219i peptide de extensie terminal\u0103: propeptid\u0103 amino-terminal\u0103 (PINP) \u0219i propeptid\u0103 carboxi-terminal\u0103<sup>1,2.<\/sup>.<\/p>\n<p>Aceste extensii de propeptide sunt \u00eendep\u0103rtate de proteinaze specifice \u00eenainte de formarea moleculelor de colagen. Ambele propeptide pot fi identificate \u00een circula\u021bie, iar concentra\u021bia lor reflect\u0103 viteza de sintez\u0103 a colagenului de tip I. PINP este considerat cel mai sensibil marker al form\u0103rii osoase \u0219i este util pentru monitorizarea terapiilor ce stimuleaz\u0103 formarea osoas\u0103 \u0219i a terapiilor antiresorptive; se recomand\u0103 ca testul s\u0103 fie efectuat la momentul ini\u021bial \u00eenainte de \u00eenceperea terapiei osteoporozei \u0219i efectuat din nou, incepand de la 3 p\u00e2n\u0103 la 6 luni<sup>2,3<\/sup>.<\/p>\n<p>Concentra\u021bia seric\u0103 a propeptidei amino-terminale de procolagen de tip I (P1NP) este direct propor\u021bional\u0103 cu cantitatea de colagen nou produs de osteoblaste. Bolile osoase metabolice se caracterizeaz\u0103 prin dezechilibre la nivel osos care apar ca urmare a unei defazari \u00eentre formarea osoas\u0103 \u0219i resorb\u021bie<sup>3<\/sup>. Ca indicator al produc\u021biei de colagen de tip I, P1NP poate fi util \u00een evaluarea remodel\u0103rii scheletului \u00een condi\u021bii normale \u0219i anormale.<\/p>\n<p>P1NP s-a folosit pentru monitorizarea fluctua\u021biei biosintezei osoase la femeile aflate \u00een postmenopauz\u0103\u0219i pentru a monitoriza efectul terapiei antiresorptive \u0219i anabolice a metabolismului osos<sup>4<\/sup>. P1NP s-a dovedit, de asemenea, util \u00een monitorizarea tratamentului pacien\u021bilor care primesc terapie cu teriparatid \u0219i pentru monitorizarea bolii Paget a oaselor<sup>5<\/sup>. La pacien\u021bii care iau bifosfona\u021bi, s-a demonstrat c\u0103 nivelul PINP scade cu p\u00e2n\u0103 la 70% fa\u021b\u0103 de valoarea ini\u021bial\u0103 dup\u0103 6 luni de tratament. Tratamentul cu terapie de substitu\u021bie hormonal\u0103 arat\u0103, de asemenea, o sc\u0103dere a nivelurilor PINP, dar \u00eentr-un grad mai mic dec\u00e2t terapia cu bifosfona\u021bi.<\/p>\n<p>La pacien\u021bii trata\u021bi cu teriparatid\u0103 (hormonul paratiroidian uman recombinant), nivelurile PINP cresc de la momentul ini\u021bial, reflect\u00e2nd efectul stimulator al teriparatidei asupra osteoblastelor \u0219i form\u0103rii osoase. Nivelurile PINP s-au dovedit a cre\u0219te semnificativ \u00eenc\u0103 de la o lun\u0103 dup\u0103 tratamentul cu teriparatid, ating\u00e2nd maximul la 6 luni dup\u0103 tratament. Cre\u0219teri de 10 mcg\/L au fost raportate la 77% p\u00e2n\u0103 la 79% dintre pacien\u021bii trata\u021bi cu teriparatid\u0103 dup\u0103 3 luni de tratament<sup>9,10<\/sup>.<\/p>\n<p><b><i>Limite si interferente<\/i><\/b><sup>10<\/sup><\/p>\n<p>Exist\u0103 varia\u021bii diurne ale nivelurilor propeptidei N-terminale intacte (PINP) ale procolagenului I, valorile fiind mai mari noaptea. C\u00e2nd se efectueaz\u0103 m\u0103sur\u0103tori \u00een serie a PINP, specimenele trebuie colectate la aceea\u0219i or\u0103 a zilei.<\/p>\n<p>PINP este metabolizat \u00een ficat.
