Trebuie men\u021bionat c\u0103 valorile medii GFR \u00eenregistreaz\u0103 sc\u0103deri odat\u0103 cu \u00eenaintarea \u00een v\u00e2rst\u0103.<\/p>\n<p style=\"text-align: justify;\">O valoare GFR &lt;60 mil\/min\/1.73 m<sup>2<\/sup> se asociaz\u0103 cu un risc crescut de complica\u021bii ale bolii cronice de rinichi, comparativ cu persoanele av\u00e2nd CKD \u0219i GFR conservat. \u00a0Aceste complica\u021bii includ: toxicitate medicamentoas\u0103, complica\u021bii metabolice \u0219i endocrine, risc \u00eenalt de boli cardiovasculare \u0219i deces.<\/p>\n<p style=\"text-align: justify;\">\u00cen popula\u021bia pediatric\u0103 se aplic\u0103 defini\u021bia CKD de la adult, cu urm\u0103toarele excep\u021bii:<\/p>\n<div style=\"text-align: justify;\">\u2022 criteriul de durat\u0103 &gt; 3 luni nu este valabil la copiii \u2264\u00a0 3luni;<\/div>\n<div style=\"text-align: justify;\">\u2022 criteriul referitor la valoarea GFR &lt;60 mil\/min\/1.73 m<sup>2 <\/sup>nu se aplic\u0103 la copiii &lt; 2 ani.<\/div>\n<p style=\"text-align: justify;\">La nou-n\u0103scut se \u00eent\u00e2lnesc valori GFR &lt;60 mil\/min\/1.73 m<sup>2<\/sup> \u0219i numai dup\u0103 v\u00e2rsta de 2 ani se ating valorile adultului. Majoritatea copiilor &lt; 2 ani cu CKD prezint\u0103 anomalii de dezvoltare a aparatului urinar; deoarece creatinina seric\u0103 se \u00eencadreaz\u0103 \u00een general \u00een intervalul de referin\u021b\u0103 corespunz\u0103tor v\u00e2rstei diagnosticul de CKD se stabile\u0219te pe baza anomaliilor structurale detectate imagistic.<\/p>\n<p style=\"text-align: justify;\">GFR este m\u0103surat prin clearance-ul plasmatic \u0219i urinar al unor markeri endogeni sau exogeni ai filtr\u0103rii glomerulare sau poate fi estimat cu ajutorul unor ecua\u021bii din concentra\u021bia seric\u0103 a unui marker endogen (creatinina sau cistatina C). Inulina este un marker exogen ideal dar pot fi utiliza\u021bi \u00a0\u0219i markeri alternativi: iotalamat, EDTA, DTPA \u0219i iohexol. M\u0103surarea clearance-ului cu ajutorul markerilor exogeni este complex\u0103, costisitoare \u0219i dificil de aplicat \u00een practica clinic\u0103 de rutin\u0103.<\/p>\n<p style=\"text-align: justify;\">Clearance-ul urinar al unui marker endogen, cum ar fi creatinina, poate fi calculat dintr-o prob\u0103 de urin\u0103 colectat\u0103 \u00eentr-un anumit interval de timp (de exemplu, urina din 24 ore) \u0219i o prob\u0103 de s\u00e2nge recoltat\u0103 \u00een intervalul de colectare a urinei, f\u0103r\u0103 a fi necesar\u0103 administrarea unui marker exogen. \u0218i aceast\u0103 procedur\u0103 prezint\u0103 dificult\u0103\u021bi \u00een aplicarea de rutin\u0103; \u00a0\u00een plus, poate supraestima GFR din cauza secre\u021biei tubulare a creatininei\u00a0 dar \u0219i a colect\u0103rii incorecte a probei de urin\u0103.<\/p>\n<p style=\"text-align: justify;\">Din aceste motive, metodele de clearance sunt rezervate pentru situa\u021biile \u00een care estimarea GFR prin intermediul ecua\u021biilor matematice prezint\u0103 o acurate\u021be mai mic\u0103.<\/p>\n<p style=\"text-align: justify;\">Ghidul KDIGO (Kidney Disease Improving Global Outcomes) recomand\u0103 ca test de prim\u0103 linie \u00een investigarea func\u021biei renale estimarea GFR pe baza unei ecua\u021bii ce utilizeaz\u0103 concentra\u021bia creatininei serice.<\/p>\n<p style=\"text-align: justify;\">Folosirea ecua\u021biilor de estimare furnizeaz\u0103 o evaluare mai direct\u0103 a ratei de filtrare glomerular\u0103 dec\u00e2t concentra\u021bia seric\u0103 a creatininei utilizat\u0103 ca test singular. Concentra\u021bia seric\u0103 a creatininei este influen\u021bat\u0103 at\u00e2t de GFR c\u00e2t \u0219i de alte procese fiziologice denumite \u201edeterminan\u021bi non-GFR\u201d: generarea creatininei de c\u0103tre mu\u0219chi, dieta proteic\u0103, secre\u021bia tubular\u0103 a creatininei de c\u0103tre transportori anionici organici, eliminarea extrarenal\u0103 a creatininei (prin tractul gastrointestinal). Ecua\u021biile de estimare includ al\u0103turi de creatinina seric\u0103 variabile clinice \u0219i demografice cu rol de surogat pentru \u201edeterminan\u021bii non-GFR\u201d ai creatininei serice. Ecua\u021biile matematice au fost dezvoltate cu ajutorul tehnicilor de regresie pentru a modela rela\u021bia observat\u0103 dintre nivelul seric al creatininei \u0219i valoarea GFR m\u0103surat\u0103 \u00een popula\u021bia studiat\u0103. De men\u021bionat c\u0103 \u00een marea majoritate a cazurilor subiec\u021bii analiza\u021bi au fost pacien\u021bi cu CKD \u0219i GFR sc\u0103zut.<\/p>\n<p style=\"text-align: justify;\">Ghidul KDIGO 2012 recomand\u0103 ca estimarea GFR la adult s\u0103 se fac\u0103 cu ajutorul ecua\u021biei CKD-EPI (Chronic Kidney Disease Epidemiology Collaboration) 2009 care utilizeaz\u0103 4 variabile &#8211; concentra\u021bia creatininei serice, v\u00e2rst\u0103, sex \u0219i ras\u0103 \u2013 \u0219i ajusteaz\u0103 GFR la suprafa\u021ba corporal\u0103 (mil\/min\/1.73 m<sup>2<\/sup>): 141 x min(SCr\/k, 1)<sup>\u03b1<\/sup> x max(SCr\/k, 1)<sup>&#8211;<\/sup><sup>1.209<\/sup> x 0.993<sup>v\u00e2rst\u0103<\/sup> x 1.018 [la femei] x 1.159 [la rasa neagr\u0103],\u00a0 unde SCr este creatinina seric\u0103 (exprimat\u0103 \u00een mg\/dL), k este 0.7 pentru femei \u0219i 0.9 pentru b\u0103rba\u021bi, \u03b1este -0.329 pentru femei \u0219i -0.411 pentru b\u0103rba\u021bi, min este minimul raportului SCr\/k sau 1 \u0219i \u00a0max este maximul \u00a0raportului SCr\/k sau 1<sup>3<\/sup>.<\/p>\n<p style=\"text-align: justify;\">Ecua\u021bia CKD-EPI a fost dezvoltat\u0103 pentru a \u00eembun\u0103t\u0103\u021bi acurate\u021bea ecua\u021biei MDRD (Modification of Diet in Renal Disease) care a fost introdus\u0103 \u00een 1999 \u00een urma studierii unei popula\u021bii cu func\u021bie renal\u0103 suboptimal\u0103 \u0219i care a fost recunoscut\u0103 c\u0103 subestimeaz\u0103 GFR la valori normale (&gt;90 mL\/min\/1.73m<sup>2<\/sup>). Ecua\u021bia CKD-EPI a fost stabilit\u0103 \u00een urma analizei a 8254 de date, provenite din 6 \u0219ase studii \u0219i de la 4 popula\u021bii, valorile ini\u021biale ale creatininei serice fiiind recalibrate la metoda enzimatic\u0103 Roche. S-a ar\u0103tat ulterior c\u0103 noua ecua\u021bie prezint\u0103 o acurate\u021be similar\u0103 ecua\u021biei MDRD la valori eGFR &lt;60 ml\/min\/1.73m<sup>2 <\/sup>\u00a0\u0219i o acurate\u021be considerabil mai mare la sugrupul de persoane cu eGFR &gt;60 ml\/min\/1.73m<sup>2<\/sup>.<\/p>\n<p style=\"text-align: justify;\">Aplicarea ecua\u021biei CKD-EPI \u00een studiul AusDiab (Australian, Diabetes, Obesity and Lifestyle)a furnizat o prevalen\u021b\u0103 estimat\u0103 a CKD mai mic\u0103 dec\u00e2t cea bazat\u0103 pe utilizarea ecua\u021biei MDRD (11.5% versus 13.4%). Extrapol\u00e2nd aceste rezultate la nivelul \u00eentregii popula\u021bii, \u00eenseamn\u0103 c\u0103 mai pu\u021bine persoane vor fi \u00eencadrate ca av\u00e2nd CKD \u0219i astfel resursele existente vor fi utilizate mai eficient<sup>4<\/sup>.<\/p>\n<p style=\"text-align: justify;\">Pentru evaluarea func\u021biei renale \u00een popula\u021bia pediatric\u0103 (&lt;18 ani) ghidul KDIGO recomand\u0103 utilizarea unei ecua\u021bii de estimare a GFR bazat\u0103 pe creatinina seric\u0103 care include \u00een\u0103l\u021bimea, cum ar fi ecua\u021bia \u201eBedside\u201d Schwartz:\u00a0 eGFR (ml\/min\/1.73m<sup>2<\/sup>) = 41.3 x (\u00een\u0103l\u021bimea\/SCr), unde \u00een\u0103l\u021bimea este exprimat\u0103 \u00een metri iar SCr (creatinina seric\u0103) este exprimat\u0103 \u00een mg\/dL.<\/p>\n<p style=\"text-align: justify;\">Aceast\u0103 ecua\u021bie deriv\u0103 din analiza efectuat\u0103 de c\u0103tre Schwartz et al \u00eentr-o popula\u021bie pediatric\u0103 av\u00e2nd CKD, \u00een care GFR a fost m\u0103surat prin clearance-ul plasmatic al iohexolului ca metod\u0103 \u201egold standard\u201d. Studiul lor a demonstrat importan\u021ba variabilei\u00a0 \u00een\u0103l\u021bime\/SCr\u00a0 care a furnizat cea mai bun\u0103 corela\u021bie cu GFR m\u0103surat cu iohexol<sup>1<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><strong><em>Recomand\u0103ri pentru determinarea ratei filtr\u0103rii glomerulare <\/em><\/strong><\/p>\n<p style=\"text-align: justify;\">Estimarea GFR pe baza creatininei serice este util\u0103 pentru:<\/p>\n<ul>\n<li style=\"text-align: justify;\">screening-ul pacien\u021bilor cu factori de risc pentru CKD: diabet zaharat tip I \u0219i II; hipertensiune arterial\u0103 \u2013 afec\u021biuni cardiace; istoric familial de boal\u0103 renal\u0103;<\/li>\n<li style=\"text-align: justify;\">monitorizarea pacien\u021bilor cu CKD \u00een vederea stadializ\u0103rii bolii \u0219i stabilirii prognosticului<sup>5<\/sup>.<\/li>\n<\/ul>\n<p style=\"text-align: justify;\">La pacien\u021bii cu boal\u0103 cronic\u0103 de rinichi ghidul KDIGO recomand\u0103 monitorizarea GFR \u00a0(\u00eempreun\u0103 cu albuminuria) cel pu\u021bin o dat\u0103 pe an. Testul va fi efectuat mai frecvent \u00een situa\u021biile \u00een care exist\u0103 un risc mai mare de progresie a bolii (rat\u0103 sc\u0103zut\u0103 a filtr\u0103rii glomerulare \u0219i albuminurie crescut\u0103) \u0219i\/sau \u00een cazurile \u00een care \u00a0determinarea va influen\u021ba deciziile terapeutice (glomerulonefrite sau niveluri crescute de albuminurie). De asemnea, frecven\u021ba monitoriz\u0103rii trebuie s\u0103 fie individualizat\u0103 \u00een func\u021bie de istoricul \u0219i de etiologia bolii renale<sup>1<\/sup>.<\/p>\n<p style=\"text-align: justify;\">Testul efectuat \u00een laboratorul nostru include determinarea <em>creatininei serice \u0219i estimarea GFR pe baza ecua\u021biei CKD-EPI la adult (\u2265 18 ani) \u0219i a ecua\u021biei Bedside Schwartz la copil <\/em>(pentru popula\u021bia pediatric\u0103 este obligatoriu s\u0103 se comunice laboratorului valoarea \u00een\u0103l\u021bimii)<sup>6<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><strong><em>Modul de recoltare a probelor <\/em><\/strong><em>&#8211;<\/em> similar cu cel descris la <strong>Creatinina seric\u0103<\/strong><sup>6<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><strong><em>Valori de referin\u021b\u0103 \u0219i interpretarea rezultatelor<\/em><\/strong><\/p>\n<p style=\"text-align: justify;\">Categoriile de GFR conform ghidului KDIGO 2012 sunt urm\u0103toarele:<\/p>\n<p style=\"text-align: justify;\">G1 \u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u2265 90 mil\/min\/1.73 m<sup>2<\/sup>: \u00a0\u00a0 GFR normal sau crescut<\/p>\n<p style=\"text-align: justify;\">G2 \u00a0\u00a0\u00a0\u00a0\u00a0\u00a060-89 mil\/min\/1.73 m<sup>2<\/sup>: Sc\u0103dere u\u0219oar\u0103*<\/p>\n<p style=\"text-align: justify;\">G3a \u00a0\u00a0\u00a0\u00a045-59 mil\/min\/1.73 m<sup>2<\/sup>: Sc\u0103dere u\u0219oar\u0103 p\u00e2n\u0103 la moderat\u0103<\/p>\n<p style=\"text-align: justify;\">G3b\u00a0\u00a0\u00a0\u00a0 30-44 mil\/min\/1.73 m<sup>2<\/sup>: Sc\u0103dere moderat\u0103 p\u00e2n\u0103 la sever\u0103<\/p>\n<p style=\"text-align: justify;\">G4 \u00a0\u00a0\u00a0\u00a0\u00a0\u00a015-29 mil\/min\/1.73 m<sup>2<\/sup>: Sc\u0103dere sever\u0103<\/p>\n<p style=\"text-align: justify;\">G5\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 &lt;15 mil\/min\/1.73 m<sup>2<\/sup>:\u00a0\u00a0\u00a0 Insuficien\u021b\u0103 renal\u0103<\/p>\n<p style=\"text-align: justify;\">*Raportat\u0103 la valoarea corespunz\u0103toare adultului t\u00e2n\u0103r.<\/p>\n<p style=\"text-align: justify;\">Valorile GFR vor fi exprimate ca num\u0103r \u00eentreg.<\/p>\n<p style=\"text-align: justify;\">Categoriile GFR de mai sus nu se aplic\u0103 la copiii &lt; 2 ani cu CKD; pentru aceast\u0103 grup\u0103 de v\u00e2rst\u0103 rezultatele ob\u021binute la calculul GFR vor fi raportate la valori GFR normale specifice v\u00e2rstei<sup>1<\/sup>.<\/p>\n<p style=\"text-align: justify;\">Cu scop orientativ, men\u021bion\u0103m mai jos valorile medii ale GFR \u0219i intervalele de referin\u021b\u0103 la copiii &lt; 117.4<\/td>\n<\/tr>\n<\/tbody>\n<\/table>\n<ul>\n<li style=\"text-align: justify;\">Ob\u021binerea unei valori GFR \u00een categoria G2 \u00een absen\u021ba altor markeri de disfunc\u021bie renal\u0103 nu constituie CKD.<\/li>\n<li style=\"text-align: justify;\">Pentru evaluarea prognosticului bolii renale categoria GFR trebuie considerat\u0103 \u00eempreun\u0103 cu categoria de albuminurie, cauza CKD, al\u021bi factori de risc \u0219i comorbidit\u0103\u021bi asociate.<\/li>\n<li style=\"text-align: justify;\">Progresia CKD este definit\u0103 prin trecerea la o categorie GFR inferioar\u0103 \u00eenso\u021bit\u0103 de o reducere GFR cu cel pu\u021bin 25% fa\u021b\u0103 de nivelul ini\u021bial; progresia rapid\u0103 este definit\u0103 prin reducerea sus\u021binut\u0103 a GFR &gt; 5 mil\/min\/1.73 m<sup>2<\/sup>\/an<sup>1<\/sup>.<\/li>\n<\/ul>\n<p style=\"text-align: justify;\"><strong><em>Limite \u0219i interferen\u021be<\/em><\/strong><\/p>\n<p style=\"text-align: justify;\">Estimarea GFR pe baza creatininei serice prezint\u0103 acurate\u021be sc\u0103zut\u0103 \u00een urm\u0103toarele situa\u021bii:<\/p>\n<div style=\"text-align: justify;\">&#8211; injuria renal\u0103 acut\u0103;<\/div>\n<div style=\"text-align: justify;\">&#8211; prezen\u021ba unor factori care afecteaz\u0103 generarea de creatinin\u0103: pierderi musculare semnificative; diet\u0103 cu un con\u021binut proteic excesiv; administrarea de suplimente cu creatin\u0103;<\/div>\n<div style=\"text-align: justify;\">&#8211; existen\u021ba unor factori care afecteaz\u0103 secre\u021bia tubular\u0103 a creatininei: administrarea de trimetoprim, cimetidin\u0103, fenofibrat poate inhiba secre\u021bia creatininei;<\/div>\n<div style=\"text-align: justify;\">&#8211; existen\u021ba unor factori care afecteaz\u0103 eliminarea extrarenal\u0103 a creatininei: dializa, inhibarea creatininazei intestinale prin administrarea de antibiotice (scade eliminarea creatininei);
