NIFTY ofer\u0103, de asemenea, op\u021biunea pentru testarea trisomiilor 9, 16 \u0219i 22, a sindromului Van der Woude, a sindroamelor de dele\u021bii Prader-Willi\/Angelman, Jacobsen \u0219i DiGeorge (22q11.2).<\/p>\n<p style=\"text-align: justify;\">ADN-ul fetal liber circulant ce reprezint\u0103 aproximativ 3-13% din ADN-ul matern liber total, deriv\u0103 probabil \u00een mare parte din placent\u0103 \u0219i dispare \u00een decurs de c\u00e2teva ore dup\u0103 na\u0219terea f\u0103tului<sup>1<\/sup>.<\/p>\n<p style=\"text-align: justify;\">Prezen\u021ba ADN-ului fetal \u00een plasma matern\u0103 a fost demonstrat\u0103 pentru prima dat\u0103 de c\u0103tre Lo et al. \u00een 1997, utiliz\u00e2nd ca model secven\u021bele ADN ale cromozomului Y provenite de la un f\u0103t de sex masculin<sup>2<\/sup>, fiind ulterior confirmat\u0103 \u0219i de c\u0103tre al\u021bi cercet\u0103tori<sup>3;4<\/sup>. Aceast\u0103 descoperire a ar\u0103tat c\u0103 ADN-ul matern din plasm\u0103 reprezint\u0103 un amestec himeric de ADN matern \u0219i fetal. \u00cenainte de demonstrarea acestui himerism al ADN-ului, probele de plasm\u0103 matern\u0103 nu reprezentau o surs\u0103 pentru analiza molecular\u0103, investigatorii fiiind interesa\u021bi doar de celulele fetale nucleate din circula\u021bia matern\u0103<sup>5<\/sup>.<\/p>\n<p style=\"text-align: justify;\">Tentativele ini\u021biale de detectare a aneupoidiiilor fetale utiliz\u00e2nd ADN-ul fetal liber circulant au necesitat folosirea unor markeri ADN sau ARN placentari multiplii, ceea ce a f\u0103cut ca screening-ul s\u0103 fie costisitor \u0219i laborios. Recent, c\u00e2teva grupuri de cercet\u0103tori au validat tehnologia de secven\u021biere genomic\u0103 paralel\u0103 masiv\u0103 ce poate cuantifica milioane de fragmente ADN \u00een probe biologice \u0219i poate depista cu mare acurate\u021be trisomiile 13, 18 \u0219i 21. Utilz\u00e2nd probe de s\u00e2nge arhivate provenite de la paciente cu risc crescut de aneuploidii fetale care au efectuat diagnostic prenatal invaziv, c\u00e2teva studii de validare desf\u0103\u0219urate pe scar\u0103 larg\u0103 au demonstrat c\u0103 rata de detec\u021bie a trisomiilor 13, 18 \u0219i 21 este &gt; 98%, cu un procent redus de rezultate fals-pozitive. De\u0219i nu exist\u0103 studii prospective bazate pe aceast\u0103 tehnologie, se pare c\u0103 analiza ADN-ului fetal liber circulant constituie cel mai eficient instrument de screening pentru aneuploidii fetale la femeile cu risc crescut<sup>1<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><strong>Testul NIFTY\u2122<\/strong> (Non-Invasive Fetal Trisomy Test) a fost dezvoltat de BGI, prima companie care a furnizat diagnostic prenatal neinvaziv \u0219i care a acumulat o experien\u021b\u0103 important\u0103 bazat\u0103 pe num\u0103rul cel mai mare de teste efectuate. Astfel, conform unei statistici din Aprilie 2014, BGI Diagnostics a efectuat 292 248 teste cu o acurate\u021be de &gt;99%.<\/p>\n<p style=\"text-align: justify;\">Testul examineaz\u0103 s\u00e2ngele periferic matern \u0219i utilizeaz\u0103 tehnologia de secven\u021biere de ultim\u0103 genera\u021bie (NGS \u2013 Next Generation Sequencing) pentru detectarea, dup\u0103 s\u0103pt\u0103m\u00e2na a 10-a** de sarcin\u0103, a celor 3 aneupoidii cromozomiale fetale majore: trisomie 21, 18 \u0219i 13.<\/p>\n<p style=\"text-align: justify;\">Facultativ, testul ofer\u0103 \u0219i alte op\u021biuni de testare, care includ: trisomiile 9, 16, 22, identificarea sexului genetic, aneuploidii ale cromozomilor de sex, sindroame de dele\u021bii:<\/p>\n<div class=\"table-mobile\">\n<table class=\"wp-block-table w-100\" style=\"border-left: 2px solid #191497; text-align: center;\" width=\"187\">\u041d\u0435\u0442<\/td>\n<\/tr>\n<\/tbody>\n<\/table>\n<\/div>\n<p style=\"text-align: justify;\">Investiga\u021bia fiind neivaziv\u0103 este evitat riscul de avort \u0219i infec\u021bie intrauterin\u0103 asociat cu testarea prenatal\u0103 invaziv\u0103 (prin amniocentez\u0103 sau prelevarea de vilozit\u0103\u021bi coriale) iar detectarea aneuploidiilor fetale permite interven\u021bia precoce<sup>6<\/sup>.<\/p>\n<p style=\"text-align: justify;\">Validarea clinic\u0103 a testului NIFTY a avut loc \u00een dou\u0103 faze. \u00cen prima faz\u0103 s-a efectuat un studiu dublu-orb \u00een care au fost analizate 3464 probe cu scopul de a stabili valoarea predictiv\u0103 a testului NIFTY pentru trisomiile 21, 18 \u0219i 13 \u00eentr-o popula\u021bie cu risc crescut \u0219i de a evalua sensibilitatea \u0219i specificitatea acestuia \u00een compara\u021bie cu rezultatul cariotipului din lichid amniotic (considerat drept testul \u201egold-standard\u201d). Au fost demonstrate o sensibilitate \u0219i o specificitate de 100% \u0219i respectiv 99.97% pentru trisomiile 21, 18 \u0219i 13. \u00cen a doua faz\u0103 Testul NIFTY a fost validat pe scar\u0103 larg\u0103 \u00eentr-un studiu multicentric ce a analizat 11 105 cazuri. Au fost ob\u021binute rezultate pozitive \u00een 190 de cazuri, fiind incluse 143 cazuri cu trisomie 21 \u0219i 47 de cazuri cu trisomie 18. Nu s-au constatat rezultate fals-negative; au fost raportate 2 rezultate fals-pozitive pentru trisomia 21 \u0219i respectiv trisomia 18. Testul a demonstrat astfel o sensibilitate de 100% \u0219i o specificitate de 99.96% pentru detec\u021bia trisomiilor 18 \u0219i 21, iar concluzia cercet\u0103torilor a fost c\u0103 introducerea acestui test screening \u00een practica medical\u0103 curent\u0103 ar putea evita aproximativ 98% dintre procedurile de diagnostic prenatal invaziv<sup>7<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><strong>Recomand\u0103ri pentru efectuarea testului NIFTY<\/strong><\/p>\n<ul style=\"text-align: justify;\">\n<li>v\u00e2rsta matern\u0103 avansat\u0103 (&gt;35 ani);<\/li>\n<li>semne ecografice sugestive pentru anomalii cromozomiale fetale;<\/li>\n<li>identificarea unui risc crescut de anomalii cromozomiale la na\u0219tere \u00een urma screening-ului de prim trimestru, trimestru II sau integrat;<\/li>\n<li>prezen\u021ba la unul dintre p\u0103rin\u021bi a unei transloca\u021bii robertsoniene echilibrate care predispune la aneupoidii fetale;<\/li>\n<li>paciente care au contraindica\u021bie pentru metodele invazive de diagnostic prenatal: placenta previa, iminen\u021b\u0103 de avort sau infectii HBV\/HIV;<\/li>\n<li>paciente supuse feriliz\u0103rii in vitro sau care au suferit anterior avort habitual;<\/li>\n<li>testul reprezint\u0103 o op\u021biune pentru orice gravid\u0103 a c\u0103rei v\u00e2rst\u0103 gesta\u021bionl\u0103 este cuprins\u0103 \u00eentre 10** \u0219i 24 s\u0103pt\u0103m\u00e2ni de sarcin\u0103 \u0219i care dore\u0219te s\u0103 efectueze un test neivaziv pentru depistarea aneupoidiilor fetale<sup>1;6<\/sup>.<\/li>\n<\/ul>\n<p style=\"text-align: justify;\"><strong>Preg\u0103tire pacient\u0103:<\/strong><\/p>\n<p>Se recomand\u0103 ca medicul trimi\u021b\u0103tor s\u0103 efectueze o consiliere corespunz\u0103toare pretestare.<\/p>\n<p><strong>Pacienta trebuie s\u0103 prezinte o ecografie care s\u0103 fac\u0103 dovada v\u00e2rstei gesta\u021bionale.<\/strong><\/p>\n<p>Testul poate fi efectuat \u00eentre s\u0103pt\u0103m\u00e2na a 10-a** \u0219i s\u0103pt\u0103m\u00e2na a 24-a de sarcin\u0103.\u00a0Se recomand\u0103 ca medicul trimi\u021b\u0103tor s\u0103 furnizeze informa\u021bii ecografice detaliate care estimeaz\u0103 s\u0103pt\u0103m\u00e2nile de sarcin\u0103, \u00een special pentru sarcina incipient\u0103. De asemenea, se recomand\u0103 consult genetic \u00eenainte \u0219i dup\u0103 efectuarea testului NIFTY dac\u0103 sarcina este &gt; 18 s\u0103pt\u0103m\u00e2ni<sup>6;8<\/sup><\/p>\n<p>\u00cenaintea recolt\u0103rii probei pacientele vor completa obligatoriu 3 formulare:\u00a0<a href=\"https:\/\/www.synevo.md\/wp-content\/uploads\/2020\/06\/NIFTY-Patient-Consent-Form-.pdf\" rel=\"nofollow noopener\" target=\"_blank\">de cerere, de consim\u021b\u0103m\u00e2nt \u0219i de asigurare<\/a><a href=\"\/wp-content\/uploads\/2014\/11\/NIFTY-Patient-Consent-Form-Upgrade-Nov-2017.pdf\">\u00a0<\/a>(ce includ informa\u021bii despre utilitatea test\u0103rii neinvazive prenatale, precum \u0219i limitele acesteia).<\/p>\n<p>\u00cen vederea recolt\u0103rii probei nu este necesar\u0103 o preg\u0103tire special\u0103. Datorit\u0103 particularit\u0103\u021bilor sistemului de recoltare aceasta se va efectua \u00eens\u0103<strong>\u00a0numai la sediile Synevo.<\/strong><\/p>\n<p>Laboratorul Synevo ofer\u0103 consiliere genetic\u0103 \u00een <strong>Centrul de recoltare Grozovici<\/strong> (Dr. Grozovici, nr. 6, sector 2, Bucure\u0219ti), cu programare la 021 9666.<\/p>\n<p>Zilele de recoltare sunt:\u00a0<strong>luni &#8211; miercuri: 7.00-14.00 si joi: 7.00 &#8211; 10.00<\/strong>\u00a0pentru <u>centrele din Bucure\u0219ti<\/u> \u0438\u00a0<strong>luni \u2013 miercuri\u00a0<\/strong>pentru <u>centrele din \u021bar\u0103, pe baza de programare la sediul fiecarui laborator.<\/u><\/p>\n<p style=\"text-align: justify;\"><strong>Specimen recoltat<\/strong> &#8211; s\u00e2nge venos<sup>8<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><strong>Recipient de recoltare<\/strong> &#8211; vacutainer Streck-Cell-Free BCT<sup>8<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><strong>Prelucrare necesar\u0103 dup\u0103 recoltare<\/strong> &#8211; pentru a asigura optima omogenizare a s\u00e2ngelui efectua\u021bi 10 mi\u0219c\u0103ri u\u0219oare de inversiune a tubului8.<\/p>\n<p style=\"text-align: justify;\"><strong>Volum prob\u0103<\/strong> &#8211; minim 10 mL<sup>8<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><strong>Stabilitate prob\u0103 <\/strong>&#8211;<strong>\u00a0<\/strong>\u00een vederea asigur\u0103rii stabilit\u0103\u021bii probei p\u00e2n\u0103 la sosirea \u00een laboratorul unde se efectueaza testarea, proba va fi men\u021binut\u0103 la temperaturi de 6 &#8211; 24\u00b0C, \u00een cutia original\u0103.<\/p>\n<p style=\"text-align: justify;\">Dac\u0103 proba se recolteaz\u0103 la centrele din \u021bar\u0103 aceasta trebuie s\u0103 ajung\u0103 \u00een 24h la Laboratorul Central8.<\/p>\n<p style=\"text-align: justify;\"><strong>Metode<\/strong> &#8211; secven\u021biere de ultim\u0103 genera\u021bie (NGS) \u0219i analiz\u0103 bioinformatic\u01038.<\/p>\n<p style=\"text-align: justify;\"><strong>Interpretarea rezultatelor<\/strong><\/p>\n<div style=\"text-align: justify;\">Prin acest test se evalueaz\u0103 trisomiile 13, 18, 21, dar este, de asemenea, posibil s\u0103 se raporteze \u0219i alte aneuploidii cromozomiale sau sindroame de microdele\u021bie (5p, 1p36, 2q33.1). Rata de detec\u021bie a acestor anomalii este apropiat\u0103, dar nu egal\u0103 cu 100%.<\/div>\n<div style=\"text-align: justify;\">Un rezultat \u201erisc \u00eenalt\u201d (pozitiv) indic\u0103 faptul c\u0103 exista o mare probabilitate (&gt;99.9%) ca f\u0103tul s\u0103 prezinte o trisomie a cromozomilor 21, 18, 13. Deoarece NIFTY nu este un test diagnostic, un rezultat pozitiv trebuie confirmat prin examen citogenetic (cariotip) din lichid amniotic sau vilozit\u0103\u021bi coriale.<\/div>\n<div style=\"text-align: justify;\">De\u0219i rata de detec\u021bie este foarte mare un rezultat \u201erisc sc\u0103zut\u201d (negativ) nu poate exclude total posibilitatea unei trisomii fetale<sup>6;8<\/sup>.<\/div>\n<div style=\"text-align: justify;\">\u00cen ceea ce prive\u0219te detec\u021bia aneuploidiilor cromozomilor sexuali sensibilitatea testului este de 75% pentru sindromul Turner (XO) \u0219i apropiat\u0103 de 100% pentru anomaliile XXY sau XYY<sup>9<\/sup>.<\/div>\n<div style=\"text-align: justify;\">Testul este capabil s\u0103 identifice informa\u021bii despre sexul f\u0103tului cu o acurate\u021be de 98%.
