Acestea se clasific\u0103 \u00een 5 categorii: benigne, varia\u021bii cu semnifica\u021bie necunoscut\u0103 (VOUS) posibil benigne, VOUS cu semnifica\u021bie incert\u0103, VOUS posibil patogenice \u0219i clar patogenice.<\/p>\n<p style=\"text-align: justify;\">Aceste rearanjamente genomice submicroscopice sunt larg r\u0103sp\u00e2ndite \u00een \u00eentregul genom \u0219i reprezint\u0103 un factor important pentru evolu\u021bie, diferen\u021biere fenotipic\u0103, dar \u0219i pentru susceptibilitate c\u0103tre anumite afec\u021biuni.<\/p>\n<p><img decoding=\"async\" class=\"alignleft size-medium wp-image-17437\" src=\"\/wp-content\/uploads\/2015\/10\/Fig.3.jpg\" alt=\"Fig.3\" width=\"404\" height=\"247\" \/><\/p>\n<p><em>Legend\u0103:<\/em><\/p>\n<p>Ro\u0219u = pierdere de material genetic<\/p>\n<p>Albastru = c\u00e2\u0219tig de material genetic<\/p>\n<p>Maro = rearanjament complex<\/p>\n<p><em>Fig.3 Diferite tipuri de varia\u021bii ale num\u0103rului de copii (CNV)<\/em><\/p>\n<p><strong>C\u00e2te tipuri de arrayCGH exist\u0103?<\/strong><\/p>\n<p style=\"text-align: justify;\">Cariotipul molecular arrayCGH este utilizat at\u00e2t \u00een cercetare, c\u00e2t \u0219i \u00een diagnostic. Metodologic, acesta poate interoga nediscriminativ \u00eentregul genom, poate \u021binti un anumit num\u0103r de regiuni cu semnifica\u021bie clinic\u0103 aparte sau poate combina ambele metode.<\/p>\n<p style=\"text-align: justify;\">\u00cen func\u021bie de ceea ce con\u021bin microcipurile de ADN fixate pe lam\u0103, exist\u0103 microarray cu BAC (Bacterial Artificial Chromosome), cu oligonucleotide sau metoda particular\u0103 cu SNP (Single Nucleotide Polymorphisms). Fiecare metod\u0103 are particularit\u0103\u021bile \u0219i aplica\u021biile sale.<\/p>\n<p style=\"text-align: justify;\">Cariotipul molecular arrayCGH postnatal, efectuat \u00een cadrul laboratorului Synevo Rom\u00e2nia, presupune 60.000 de sonde reprezentate de oligonucleotide constituite din 60 de baze.<\/p>\n<p style=\"text-align: justify;\"><strong>Care sunt indica\u021biile cariotipului molecular arrayCGH postnatal?<\/strong><\/p>\n<p style=\"text-align: justify;\">\u00cen diagnosticul postnatal, cariotipul molecular arrayCGH este test de prim\u0103 inten\u021bie pentru:<\/p>\n<ul style=\"text-align: justify;\">\n<li>\n<div>Pacien\u021bi cu malforma\u021bii congenitale multiple \/ tr\u0103s\u0103turi dismorfice ne\u00eencadrabile \u00eentr-un sindrom genetic cunoscut<\/div>\n<\/li>\n<li>\n<div>Nou-n\u0103scu\u021bi cu malforma\u021bii congenitale multiple la care nu poate fi ob\u021binut cariotipul<\/div>\n<\/li>\n<li>\n<div>Copii cu \u00eent\u00e2rziere \u00een dezvoltarea psihomotorie<\/div>\n<\/li>\n<li>\n<div>Pacien\u021bi cu dizabilitate intelectual\u0103<\/div>\n<\/li>\n<li>\n<div>Pacien\u021bi cu tulbur\u0103ri din spectru autist<\/div>\n<\/li>\n<\/ul>\n<p style=\"text-align: justify;\">Alte aplica\u021bii, cum ar fi retardul de cre\u0219tere, \u00eent\u00e2rzierea \u00een achizi\u021bia limbajului sunt \u00een curs de investigare.<\/p>\n<p style=\"text-align: justify;\"><strong>Exemplu clinic din cazuistica Synevo <\/strong><strong>\u0420\u0443\u043c\u044b\u043d\u0438\u044f<\/strong><\/p>\n<p style=\"text-align: justify;\"><strong>\u00a0<\/strong><em>Tr\u0103s\u0103turi fenotipice generale ale pacientului:<\/em> dizabilitate intelectual\u0103 neexplicat\u0103, u\u0219or dismorfism craniofacial, ochi situa\u021bi mai ad\u00e2nc, spr\u00e2ncene drepte, menton ascu\u021bit.<\/p>\n<p style=\"text-align: justify;\"><em>Test utilizat pentru stabilirea diagnosticului: <\/em>cariotip molecular arrayCGH.<\/p>\n<p style=\"text-align: justify;\"><em>Diagnostic: s<\/em>indrom de microdele\u021bie 1p36.<\/p>\n<p style=\"text-align: justify;\"><img decoding=\"async\" class=\"size-medium wp-image-17438 aligncenter\" src=\"\/wp-content\/uploads\/2015\/10\/Fig.4.jpg\" alt=\"Fig.4\" width=\"404\" height=\"165\" \/><\/p>\n<p style=\"text-align: justify;\"><strong>Care sunt avantajele metodei arrayCGH? <\/strong><\/p>\n<ul style=\"text-align: justify;\">\n<li>Utilitate clinic\u0103 crescut\u0103. Rata detec\u021biei de CNV patogenice la copiii cu retard de dezvoltare, dizabilitate intelectual\u0103 \u0219i anomalii congenitale multiple este de 18-20%, fa\u021b\u0103 de 3% prin cariotipul clasic. Ulterior, testarea altor membri ai familiei probanzilor afecta\u021bi permite identificarea st\u0103rii de purt\u0103tor \u0219i caracterul mo\u0219tenit sau <em>de novo<\/em> al anomaliei.<\/li>\n<li>Rezolu\u021bie medie de detec\u021bie de aproximativ 60kb. Ofer\u0103 o perspectiv\u0103 asupra \u00eentregului genom la o rezolu\u021bie \u00eenalt\u0103, de 10 ori mai mare dec\u00e2t cea a cariotipului clasic.<\/li>\n<li>Un singur test arrayCGH este echivalentul simultan a mii de experimente FISH \/ MLPA.<\/li>\n<li>Detecteaz\u0103 duplica\u021bii \u0219i dele\u021bii submicroscopice, rearanjamente cromozomiale neechilibrate, diferite grade de mozaicism.<\/li>\n<li>Nu necesit\u0103 cultur\u0103 celular\u0103, precum cariotipul clasic.<\/li>\n<\/ul>\n<p><img decoding=\"async\" class=\"size-large wp-image-17440 aligncenter\" src=\"\/wp-content\/uploads\/2015\/10\/Tabel.jpg\" alt=\"Tabel\" width=\"1024\" height=\"186\" \/><\/p>\n<p><strong>Care sunt limitele metodei arrayCGH?<\/strong><\/p>\n<ul style=\"text-align: justify;\">\n<li>\n<div>Nu identific\u0103 modific\u0103ri cromozomiale echilibrate (transloca\u021bii, inversii, inser\u021bii), muta\u021bii punctiforme, mozaicismul de nivel sc\u0103zut. Aceste modific\u0103ri sunt identificate prin alte teste genetice din portofoliul laboratorului Synevo Rom\u00e2nia.<\/div>\n<\/li>\n<li>\n<div>Nu furnizeaz\u0103 informa\u021bii despre natura structural\u0103 a anomaliilor identificate.<\/div>\n<\/li>\n<li>\n<div>Poate detecta CNV cu semnifica\u021bie neclar\u0103, care necesit\u0103 testare \u0219i analize ulterioare sau CNV cu semnifica\u021bie necunoscut\u0103, gener\u00e2nd rezultate neinterpretabile \u00een contextul cuno\u0219tin\u021belor actuale.<\/div>\n<\/li>\n<\/ul>\n<div style=\"text-align: justify;\"><strong>Care este rolul medicului genetician? <\/strong><\/div>\n<p style=\"text-align: justify;\">Consultul genetic \u0219i consim\u021b\u0103m\u00e2ntul informat al pacientului sunt necesare \u00een demersul stabilirii protocolului adecvat.
