Numeroasele studii efectuate au ar\u0103tat c\u0103 muta\u021biile genelor BRCA1 \u0219i BRCA2 care afecteaz\u0103 linia germinal\u0103 sunt responsabile de marea majoritate a cancerelor ereditare de s\u00e2n \u0219i ovar1;7.<\/p>\n<p style=\"text-align: justify;\">BRCA1 (Breast Cancer 1 Gene) \u0219i BRCA2 (Breast Cancer 2 Gene) sunt gene supresoare ale tumorilor care codific\u0103 proteine cu rol \u00een procesele de reparare a ADN-ului. Au fost comunicate peste 1200 muta\u021bii ale genei BRCA1 \u0219i peste 1300 muta\u021bii ale genei BRCA2. De\u0219i persoanele cu HBOC mo\u0219tenesc o alel\u0103 BRCA1 sau BRCA2 defectuoas\u0103 de la unul dintre p\u0103rin\u021bi, ace\u0219tia prezint\u0103 o a doua alel\u0103 func\u021bional\u0103; \u00een cazul \u00een care \u0219i cea de-a doua alel\u0103 devine nefunc\u021bional\u0103 se dezvolt\u0103 procesul neoplazic, prin acumularea de muta\u021bii adi\u021bionale. Se estimeaz\u0103 c\u0103 prevalen\u021ba muta\u021biilor BRCA \u00een popula\u021bia general\u0103 este de 1\/300 &#8211; 1\/800. \u00cen popula\u021biile \u00eentemeiate de un grup ancestral mic, anumite muta\u021bii BRCA1 sau BRCA2 se pot \u00eent\u00e2lni mai frecvent \u0219i sunt denumite \u201emuta\u021bii fondatoare\u201d. Aceste tipuri de muta\u021bii au fost identificate predominant la evreii Ashkenazi (din Europa de Est), canadienii francezi \u0219i la islandezi1.<\/p>\n<p style=\"text-align: justify;\"><strong>Gena BRCA 1 <\/strong>localizat\u0103 la nivelul cromozomului 17q21 este format\u0103 din 22 de exoni \u0219i codific\u0103 o protein\u0103 de 1863 de aminoacizi. Proteina BRCA1 este o fosfoprotein\u0103 nuclear\u0103 cu rol important \u00een men\u021binerea stabilit\u0103\u021bii genomice \u0219i \u00een supresia tumoral\u0103; \u00eempreun\u0103 cu alte proteine supresoare ale tumorilor sau cu rol \u00een transmiterea semnalului celular, formeaz\u0103 un complex cu subunit\u0103\u021bi multiple denumit BASC (BRCA1-asociated genome surveillance complex); de asemenea se asociaz\u0103 cu ARN polimeraza II intervenind astfel \u00een transcriptie, repararea ADN-lui dublu catenar, precum \u0219i \u00een recombinare.<\/p>\n<p style=\"text-align: justify;\">Muta\u021biile genei BRCA1, ce conduc la erori \u00een replicarea ADN-ului \u0219i proliferarea necontrolat\u0103 a celulelor epiteliale, sunt r\u0103spunz\u0103toare de aproximativ 40% din cazurile de cancer mamar ereditar \u0219i mai mult de 80% din cazurile de cancer mamar \u0219i ovarian ereditare. Splicing-ul alternativ de\u021bine rol \u00een modularea localiz\u0103rii subcelulare (citoplasm\u0103, nucleu) \u0219i func\u021bionarea normal\u0103 a genei BRCA1. Au fost descrise mai multe variante de produ\u0219i de transcrip\u021bie unele fiind asociate cu muta\u021bii<\/p>\n<p style=\"text-align: justify;\">cauzatoare de boal\u0103. A fost identificat\u0103 \u0219i o pseudogen\u0103, localizat\u0103 tot pe cromozomul 17. \u00cen literatura de specialitate este specificat c\u0103 aceast\u0103 regiune de pe cromozomul 17 con\u021bine o duplica\u021bie \u00een tandem (YBRCA1), de aproximativ 30kb, care duce la apari\u021bia a 2 copii ale exonilor 1 \u0219i 2 ai BRCA1, ale exonilor 1 \u0219i 3 ai genei adiacente denumit\u0103 1A1-3B, precum \u0219i a unei regiuni intergenice de 295 bp3. Prin secven\u021biere s-a demonstrat c\u0103 ace\u0219ti exoni duplica\u021bi constituie pseudogene neprocesate.<\/p>\n<p style=\"text-align: justify;\">Aceste constat\u0103ri pot nu numai s\u0103 \u00eengreuneze analiza muta\u021biilor BRCA1, ci s\u0103 aib\u0103 \u0219i implica\u021bii asupra regl\u0103rii normale sau anormale a transcrip\u021biei, transla\u021biei \u0219i func\u021biei BRCA13;4.<\/p>\n<p style=\"text-align: justify;\">Muta\u021biile genei BRCA1 se caracterizeaz\u0103 prin apari\u021bia prematur\u0103 a codonilor STOP la nivelul cadrului de citire (open reading frame, ORF), produsul rezultat fiind o protein\u0103 trunchiat\u0103. Cele mai frecvente muta\u021bii sunt 185delAG (c.68_69del AG) \u0219i 5385insC (c.5266dupC) prezente \u00een procent de 1%, respectiv, 0.15% la evreii Ashkenazi. Au fost identifica\u021bi mii de purt\u0103tori ai dele\u021biei 185delAG, care determin\u0103 apari\u021bia codonului STOP la aminoacidul 39 din secven\u021ba proteic\u01032. O muta\u021bie asem\u0103n\u0103toare este 188del11 (c.71_81del) care schimb\u0103 cadrul de citire \u00een exonul 3 (la nivelul codonilor 39 \u0219i 36), rezult\u00e2nd de asemenea o proteina trunchiat\u0103.<\/p>\n<p style=\"text-align: justify;\">Un alt element caracteristic al genei BRCA1 \u00eel constituie secven\u021bele Alu repetitive.\u00a0 Au fost identificate 45 de rearanj\u0103ri genetice, care includ dele\u021bii \u0219i duplica\u021bii \u00eentr-unul sau mai mul\u021bi exoni. Genomul uman con\u021bine peste 1 milion de c\u00f3pii de elemente Alu (un element la fiecare 5kb), care aparent mediaz\u0103 rearanj\u0103rile cromozomiale \u0219i recombinarea omoloag\u0103, rezult\u00e2nd duplica\u021bii, inversii sau dele\u021bii. Aproximativ 41.5% din secven\u021bele intronice ale genei BRCA1 sunt constituite din elemente Alu, privite de multe ori ca factori genetici de instabilitate4;8.<\/p>\n<p style=\"text-align: justify;\">De\u0219i sunt prezente numeroase elemente repetitive, rearanj\u0103rile sunt mai pu\u021bin frecvente la nivelul genei <strong>BRCA2<\/strong>, situat\u0103 pe cromozomul 13q12-q13; cadrul de citire \u00eencepe la nucleotidul 229 \u0219i codific\u0103 o protein\u0103 de 390 kDa. Gena are 27 exoni, cel mai mare exon fiind, ca \u0219i \u00een cazul genei BRCA1, exonul 11. Ini\u021bierea transla\u021biei are loc \u00een exonul 2, gena fiind foarte bogat\u0103 \u00een nucleotide AT. BRCA2 func\u021bioneaz\u0103 tot\u00a0 ca o protein\u0103 de reparare a ADN-ului prin interac\u021biunea cu RAD 51 (supresor tumoral)3;8. Prin examinarea modific\u0103rilor posttransla\u021bionale ale BRCA2, s-a stabilit c\u0103 proteina este hiperfosforilat\u0103 de Polo-like kinaza 1 (PLK1) \u00een metafaz\u0103 \u0219i defosforilat\u0103 c\u00e2nd celula iese din metafaz\u0103 \u0219i intr\u0103 \u00een interfaz\u0103. RAD 51 interac\u021bioneaz\u0103 cu domeniul C-terminal al BRCA (BRCT), o secven\u021b\u0103 unic\u0103 de aminoacizi care se repet\u0103 de 8 ori \u00een regiunea central\u0103 a BRCA2. \u00cendep\u0103rtarea acestei regiuni duce la inhibarea apoptozei (BRCT fiind implicat\u0103 \u00een apoptoz\u0103 pe calea caspazei). Majoritatea muta\u021biilor sunt alter\u0103ri ale cadrului de citire \u0219i muta\u021bii non-sens localizate \u00een exonul 11, cea mai frecvent\u0103 fiind muta\u021bia 6174delT \u00eent\u00e2lnit\u0103 cu o frecven\u021b\u0103 de aproximativ 1% \u00een popula\u021bia evreilor Ashkenazi6.<\/p>\n<p style=\"text-align: justify;\">Celulele care nu au proteinele normale BRCA1 \u0219i BRCA2 acumuleaz\u0103 anomalii cromozomiale: rupturi, aneuploidii severe, amplificarea centrozomului; instabilitatea cromozomial\u0103 generat\u0103 poate constitui baza patogenic\u0103 a dezvolt\u0103rii tumorilor mamare. La femeile care mo\u0219tenesc o muta\u021bie inactivatoare, deficitul unei proteine<\/p>\n<p style=\"text-align: justify;\">BRCA este critic pentru apari\u021bia bolii, fiind generat at\u00e2t de alela inactivat\u0103 mo\u0219tenit\u0103, c\u00e2t \u0219i de pierderea somatic\u0103 a alelei s\u0103lbatice \u00een celulele epiteliale mamare sau ovariene. \u00cen plus s-a constatat c\u0103 expresia genelor este suprareglat\u0103 \u00een timpul pubert\u0103\u021bii \u0219i sarcinii, fiind asociat\u0103 cu cre\u0219terea secre\u021biei de estrogeni; aceast\u0103 observa\u021bie sugereaz\u0103 c\u0103 estrogenii ar putea stimula expresia BRCA1 \u0219i\/sau BRCA2. A fost propus \u0219i un model de tumorigenez\u0103 la femeile purt\u0103toare de muta\u021bii BRCA\u00a0 ale liniei germinale: la pubertate, ca urmare a nivelului crescut de estrogeni, celulele epiteliale ale glandei mamare prolifereaz\u0103 rapid; prezen\u021ba unei muta\u021bii BRCA inactivatoare \u00een contextul unei replic\u0103ri crescute reduce mult capacitatea de\u00a0 reparare a ADN-ului \u0219i cre\u0219te frecven\u021ba alter\u0103rilor genomice somatice la nivelul elementelor repetitive din genele BRCA1 \u0219i BRCA2. Majoritatea celulelor care prezint\u0103 ambele tipuri de muta\u021bii nu supravie\u021buiesc \u00een ciclul celular urm\u0103tor datorit\u0103 incapacit\u0103\u021bii de reparare a ADN-ului, \u00eens\u0103 \u00een cazul \u00een care se produc muta\u021bii adi\u021bionale la nivelelor unor gene care controleaz\u0103 ciclul celular (\u201echeckpoint genes\u201d), unele celule BRCA-nule se imortalizeaz\u0103 \u0219i pot constitui punctul de plecare \u00een dezvoltarea tumorilor9.<\/p>\n<p style=\"text-align: justify;\"><u>Penetran\u021ba<\/u> muta\u021biilor BRCA1 \u0219i BRCA2, definit\u0103 ca probabilitatea de a dezvolta cancer atunci c\u00e2nd este prezent\u0103 o muta\u021bie, este variabil\u0103, fiind influen\u021bat\u0103 probabil de etnicitate, v\u00e2rst\u0103, tipul de cancer.<\/p>\n<p style=\"text-align: justify;\">\u00cen tabelul de mai jos este prezentat riscul cumulat de cancer de s\u00e2n, \u00een func\u021bie de decada de via\u021b\u0103, la femeile purt\u0103toare de muta\u021bii BRCA1 sau BRCA2, provenite din familiile cu risc crescut :<\/p>\n<table class=\"wp-block-table w-100\">\n<tbody>\n<tr>\n<td rowspan=\"2\" width=\"190\"><strong>\u0412\u043e\u0437\u0440\u0430\u0441\u0442<\/strong><\/td>\n<td colspan=\"2\" width=\"386\"><strong>Risc cumulat<\/strong><\/td>\n<\/tr>\n<tr>\n<td width=\"197\"><strong>BRCA1<\/strong><\/td>\n<td width=\"190\"><strong>BRCA2<\/strong><\/td>\n<\/tr>\n<tr>\n<td width=\"190\">30 ani<\/td>\n<td width=\"197\">3.2%<\/td>\n<td width=\"190\">4.6%<\/td>\n<\/tr>\n<tr>\n<td width=\"190\">40 ani<\/td>\n<td width=\"197\">19.1%<\/td>\n<td width=\"190\">12%<\/td>\n<\/tr>\n<tr>\n<td width=\"190\">50 ani<\/td>\n<td width=\"197\">50.8%<\/td>\n<td width=\"190\">46%<\/td>\n<\/tr>\n<tr>\n<td width=\"190\">60 ani<\/td>\n<td width=\"197\">54.2%<\/td>\n<td width=\"190\">61%<\/td>\n<\/tr>\n<tr>\n<td width=\"190\">70 ani<\/td>\n<td width=\"197\">85%<\/td>\n<td width=\"190\">86%<\/td>\n<\/tr>\n<\/tbody>\n<\/table>\n<p style=\"text-align: justify;\">Cancerul mamar la b\u0103rba\u021bi a fost observat \u00een familiile cu muta\u021bii BRCA2, unele dintre acestea prezent\u00e2nd \u00een exclusivitate afectarea persoanelor de sex masculin. \u00centr-un studiu efectuat pe 26 familii care au prezentat cel pu\u021bin un caz de cancer de s\u00e2n la b\u0103rba\u021bi, 77% au demonstrat o asociere cu muta\u021bii BRCA2 (Ford, 1998). Dac\u0103 \u00eens\u0103 nu s-a luat \u00een considerare istoricul familial, prevalen\u021ba muta\u021biilor BRCA2 ale liniei germinale la pacien\u021bii cu cancer mamar a fost de numai 4-14% (Couch, Friedman, 1994). Pentru b\u0103rba\u021bii cu muta\u021bii BRCA2 riscul cumulat de cancer mamar p\u00e2n\u0103 la v\u00e2rsta de 80 ani a fost estimat la 6.9%7.<\/p>\n<p style=\"text-align: justify;\">La femeile cu muta\u021bii BRCA1 sau BRCA2 riscul de cancer ovarian este de 39-46% \u0219i, respectiv, de 12-20%. Cancerul ovarian prezint\u0103 un fenotip histologic distinct la aceast\u0103 categorie de paciente, \u00eenregistr\u00e2ndu-se \u00een principal tumori seroase sau endometrioide de grad \u00eenalt1.<\/p>\n<p style=\"text-align: justify;\">\u00cen afara cancerului ereditar de s\u00e2n \u0219i ovar muta\u021biile BRCA1 \u0219i BRCA2 pot fi asociate \u0219i cu alte forme de cancer. Astfel, s-a estimat c\u0103 riscul de cancer de prostat\u0103 este\u00a0 de 3 ori mai mare la b\u0103rba\u021bii purt\u0103tori de muta\u021bii BRCA1 \u00een compara\u021bie cu popula\u021bia general\u0103. Mai mult, cazuri de cancer de pancreas, prostat\u0103, esofag, laringe, stomac, colon, vezic\u0103 biliar\u0103, duct biliar, melanom au fost raportate \u00een familiile cu muta\u021bii BRCA2 ce predispun la cancer. De asemenea \u00een anemia Fanconi s-a constatat inactivarea bialelic\u0103 BRCA27.<\/p>\n<p style=\"text-align: justify;\">Testarea genetic\u0103 este important\u0103 \u00een practica clinic\u0103 pentru identificarea persoanelor care prezint\u0103 un risc semnificativ de cancer de s\u00e2n sau de ovar \u0219i aplicarea strategiilor adecvate de screening \u0219i preven\u021bie1;7.<\/p>\n<h4>Recomand\u0103ri pentru analiza muta\u021biilor genelor BRCA1 \u0219i BRCA2<\/h4>\n<p style=\"text-align: justify;\">Se recomand\u0103 testarea genetic\u0103 la persoanele cu un risc \u226520-25% de a avea o predispozi\u021bie mo\u0219tenit\u0103 pentru cancerul mamar sau ovarian:<\/p>\n<ul style=\"text-align: justify;\">\n<li>femei cu antecedente personale at\u00e2t de cancer mamar, c\u00e2t \u0219i ovarian;<\/li>\n<li>femei cu cancer ovarian ce au o rud\u0103 apropiat\u0103 fie cu cancer ovarian, fie mamar cu debut \u00eenainte de menopauz\u0103 sau ambele;<\/li>\n<li>femei cu cancer ovarian ce apar\u021bin grupului etnic al evreilor Ashkenazi;<\/li>\n<li>femei cu cancer ovarian depistat la v\u00e2rsta de 50 ani sau mai devreme, ce au o rud\u0103 apropiat\u0103 fie cu cancer ovarian, fie cu cancer mamar la b\u0103rbat dezvoltat la orice v\u00e2rst\u0103;<\/li>\n<li>femei ce apar\u021bin grupului etnic al evreilor Ashkenazi cu cancer mamar diagnosticat la o v\u00e2rst\u0103 \u226440 ani;<\/li>\n<li>femei av\u00e2nd o rud\u0103 apropiat\u0103 ce prezint\u0103 o muta\u021bie BRCA1 sau BRCA2 cunoscut\u0103.<\/li>\n<\/ul>\n<p style=\"text-align: justify;\">Testarea genetic\u0103 este op\u021bional\u0103 la persoanele cu un risc \u22655-10% de a avea o predispozi\u021bie mo\u0219tenit\u0103 pentru cancerul mamar sau ovarian:<\/p>\n<ul style=\"text-align: justify;\">\n<li>femei cu cancer mamar diagnosticat la o v\u00e2rst\u0103 \u226440 ani;<\/li>\n<li>femei cu cancer ovarian, peritoneal primar sau de tromp\u0103 uterin\u0103, histologic de grad \u00eenalt \u0219i de tip seros, dezvoltat la orice v\u00e2rst\u0103;<\/li>\n<li>femei cu cancer mamar bilateral (\u00een special dac\u0103 prima tumor\u0103 a fost diagnosticat\u0103 la o v\u00e2rst\u0103 \u226450 ani);<\/li>\n<li>femei cu cancer mamar diagnosticat la o v\u00e2rst\u0103 \u226450 ani, av\u00e2nd o rud\u0103 apropiat\u0103 cu cancer mamar dezvoltat \u226450 ani;<\/li>\n<li>femei ce apar\u021bin grupului etnic al evreilor Ashkenazi cu cancer mamar diagnosticat la o v\u00e2rst\u0103 \u226450 ani;<\/li>\n<li>femei cu cancer mamar dezvoltat la orice v\u00e2rst\u0103, av\u00e2nd cel pu\u021bin 2 rude apropiate cu cancer mamar, indiferent de v\u00e2rsta la care a fost depistat (\u00een special dac\u0103 cel pu\u021bin unul din cazuri a fost diagnosticat la o v\u00e2rst\u0103 \u226450 ani);<\/li>\n<li>femei s\u0103n\u0103toase av\u00e2nd o rud\u0103 apropiat\u0103 care \u00eentrune\u0219te unul din criteriile mai sus men\u021bionate.<\/li>\n<\/ul>\n<p style=\"text-align: justify;\">Men\u021biuni:<\/p>\n<ul style=\"text-align: justify;\">\n<li>cancerul primar de tromp\u0103 uterin\u0103 sau peritoneal primar fac parte din spectrul de manifest\u0103ri caracteristice HBOC;<\/li>\n<li>o rud\u0103 apropiat\u0103 este definit\u0103 ca orice rud\u0103 de gradul I (mam\u0103, sor\u0103, fiic\u0103) sau de gradul II (bunic\u0103, nepoat\u0103, m\u0103tu\u0219\u0103).<\/li>\n<\/ul>\n<p style=\"text-align: justify;\">Dac\u0103 este posibil, testarea genetic\u0103 trebuie ini\u021biat\u0103 la persoana afectat\u0103; deoarece num\u0103rul muta\u021biilor BRCA1 \u0219i BRCA2 este foarte mare se va efectua secven\u021bierea\u00a0complet\u0103 a ambelor gene.
