Folose\u0219te o tehnologie brevetat\u0103 bazat\u0103 pe cercetarea \u0219i dezvoltarea de ultim\u0103 genera\u021bie din domeniul geneticii \u0219i a diagnostic\u0103rii moleculare. A fost dezvoltat de o echip\u0103 \u0219tiin\u021bific\u0103 cu o experien\u021b\u0103 de peste 25 de ani \u00een domeniul diagnosticului prenatal, medicinei moleculare, genomicii, transcriptomicii, metilomicii \u0219i bioinformaticii.<\/p>\n<p style=\"text-align: justify;\">ADN-ul fetal liber circulant ce reprezint\u0103 aproximativ 3-13% din ADN-ul matern liber total, deriv\u0103 probabil \u00een mare parte din placent\u0103 \u0219i dispare \u00een decurs de c\u00e2teva ore dup\u0103 na\u0219terea f\u0103tului<sup>1<\/sup>. Testul <strong>VERACITY<\/strong> m\u0103soar\u0103 cu acurate\u021be ADN-ul extracelular liber fetal din s\u00e2ngele matern pentru a detecta prezen\u021ba aneuploidiilor fetale.<\/p>\n<p style=\"text-align: justify;\">Prezen\u021ba ADN-ului fetal \u00een plasma matern\u0103 a fost demonstrat\u0103 pentru prima dat\u0103 de c\u0103tre Lo et al. \u00een 1997, utiliz\u00e2nd ca model secven\u021bele ADN ale cromozomului Y provenite de la un f\u0103t de sex masculin<sup>2<\/sup>, fiind ulterior confirmat\u0103 \u0219i de c\u0103tre al\u021bi cercet\u0103tori<sup>3;4<\/sup>. Aceast\u0103 descoperire a ar\u0103tat c\u0103 ADN-ul matern din plasm\u0103 reprezint\u0103 un amestec himeric de ADN matern \u0219i fetal. \u00cenainte de demonstrarea acestui himerism al ADN-ului, probele de plasm\u0103 matern\u0103 nu reprezentau o surs\u0103 pentru analiza molecular\u0103, investigatorii fiind interesa\u021bi doar de celulele fetale nucleate din circula\u021bia matern\u0103<sup>5<\/sup>.<\/p>\n<p style=\"text-align: justify;\">Testul <strong>VERACITY <\/strong>examineaz\u0103 s\u00e2ngele periferic matern pentru detectarea, dup\u0103 s\u0103pt\u0103m\u00e2na a 10-a de sarcin\u0103, a celor 3 aneupoidii cromozomiale fetale majore: trisomie 21, 18 \u0219i 13<sup>6<\/sup>, precum \u0219i prezen\u021ba cromozomului Y \u0219i este disponibil pentru sarcini monofetale \u0219i gemelare, inclusiv pentru cele concepute prin fertilizare in vitro (FIV)<sup>6<\/sup>.<\/p>\n<p><img decoding=\"async\" class=\"aligncenter wp-image-35138 size-medium\" src=\"\/wp-content\/uploads\/2019\/04\/Veracity-poza.jpg\" alt=\"veracity\" width=\"380\" height=\"404\" \/><\/p>\n<p><img decoding=\"async\" class=\"size-medium wp-image-35138 aligncenter\" src=\"\/wp-content\/uploads\/2019\/04\/Veracity-poza.jpg\" alt=\"-\" width=\"380\" height=\"404\"><\/p>\n<p style=\"text-align: justify;\">Investiga\u021bia fiind non-invaziv\u0103 evit\u0103 riscul unui avort \u0219i infec\u021bie intrauterin\u0103 asociat cu testarea prenatal\u0103 invaziv\u0103 (prin amniocentez\u0103 sau prelevarea de vilozit\u0103\u021bi coriale). Detectarea \u00eens\u0103, a aneuploidiilor fetale permite interven\u021bia precoce<sup>6<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><strong><em>Recomand\u0103ri de testare<\/em><\/strong><\/p>\n<ul style=\"text-align: justify;\">\n<li>Femei insarcinate de toate v\u00e2rstele<\/li>\n<li>Eviden\u021bierea unui risc crescut de anomalii cromozomiale \u00een urma screening-ului efectuat prin dublu sau triplu test<\/li>\n<li>Ecografia fetal\u0103 anormal\u0103<\/li>\n<li>Na\u0219terea anterioar\u0103 a unui copil cu anomalii cromozomiale<\/li>\n<li>Fertilizare in vitro<\/li>\n<li>Paciente cu contraindica\u021bie pentru metodele invazive de diagnostic prenatal<\/li>\n<li>Istoric familial pozitiv pentru anomalii cromozomiale<sup>1;6<\/sup><\/li>\n<\/ul>\n<p style=\"text-align: justify;\"><strong>Preg\u0103tire pacient\u0103:<\/strong><\/p>\n<p style=\"text-align: justify;\">Se recomand\u0103 ca medicul trimi\u021b\u0103tor s\u0103 efectueze o consiliere corespunz\u0103toare pretestare.<\/p>\n<p style=\"text-align: justify;\"><strong>Pacienta trebuie s\u0103 prezinte o ecografie care s\u0103 fac\u0103 dovada v\u00e2rstei gesta\u021bionale.<\/strong><\/p>\n<p style=\"text-align: justify;\">Testul poate fi efectuat \u00eentre s\u0103pt\u0103m\u00e2na a 10-a** \u0219i s\u0103pt\u0103m\u00e2na a 24-a de sarcin\u0103.\u00a0Se recomand\u0103 ca medicul trimi\u021b\u0103tor s\u0103 furnizeze informa\u021bii ecografice detaliate care estimeaz\u0103 s\u0103pt\u0103m\u00e2nile de sarcin\u0103, \u00een special pentru sarcina incipient\u0103. <sup>6;8<\/sup><\/p>\n<p style=\"text-align: justify;\">\u00cenaintea recolt\u0103rii probei pacientele vor completa obligatoriu <strong>1 formular<\/strong> ce con\u021bine:\u00a0<strong><em>formular informa\u021bii prob\u0103<\/em><\/strong>, <strong><em>acordul pacientului informat<\/em><\/strong>, <strong><em>nota de informare privind prelucrarea de date cu caracter personal<\/em> <\/strong>(ce includ informa\u021bii despre utilitatea test\u0103rii neinvazive prenatale, precum \u0219i limitele acesteia).<\/p>\n<p style=\"text-align: justify;\">\u00cen vederea recolt\u0103rii probei nu este necesar\u0103 o preg\u0103tire special\u0103. Datorit\u0103 particularit\u0103\u021bilor sistemului de recoltare aceasta se va efectua \u00eens\u0103<strong>\u00a0numai la sediile Synevo.<\/strong><\/p>\n<p style=\"text-align: justify;\"><strong><em>Specimen recoltat<\/em><\/strong> \u2013 s\u00e2nge venos<sup>7<\/sup><\/p>\n<p style=\"text-align: justify;\"><strong><em>Recipient de recoltare<\/em><\/strong> \u2013 vacutainer Streck-Cell-Free BCT<sup>7<\/sup><\/p>\n<p style=\"text-align: justify;\"><strong><em>Volum prob\u0103 <\/em><\/strong><strong>&#8211; 10 mL \u00een eprubeta prezenta \u00een kitul de recoltare<\/strong><sup>7<\/sup><\/p>\n<p style=\"text-align: justify;\"><strong><em>Prelucrare necesar\u0103 dup\u0103 recoltare<\/em><\/strong> \u2013 pentru a asigura optima omogenizare a s\u00e2ngelui efectua\u021bi 10 mi\u0219c\u0103ri u\u0219oare de inversiune a tubului<sup>7<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><strong><em>Stabilitate prob\u0103<\/em><\/strong> -<strong>\u00a0<\/strong>\u00een vederea asigur\u0103rii stabilit\u0103\u021bii probei p\u00e2n\u0103 la sosirea \u00een laboratorul unde se efectueaz\u0103 testarea, proba va fi men\u021binut\u0103 la temperaturi de 6 \u2013 24\u00b0C, \u00een cutia original\u0103<sup>7<\/sup>. Stabilitatea probei este de max. 4 zile.<\/p>\n<p style=\"text-align: justify;\">Dac\u0103 proba se recolteaz\u0103 la centrele din \u021bar\u0103 aceasta trebuie s\u0103 ajung\u0103 \u00een 24h la Laboratorul Central.<\/p>\n<p style=\"text-align: justify;\"><strong><em>Interpretarea rezultatelor<\/em><\/strong><\/p>\n<p style=\"text-align: justify;\">Prin acest test se evalueaz\u0103 trisomiile 13, 18, 21, dar este, de asemenea, posibil s\u0103 se raporteze \u0219i alte aneuploidii cromozomiale sau sindroame de microdele\u021bie (22q11.2, 1p36, 17p11.2, 4p16.3). Rata de detec\u021bie a acestor anomalii este apropiat\u0103, dar nu egal\u0103 cu 100%.<\/p>\n<p style=\"text-align: justify;\">Un rezultat \u201erisc \u00eenalt\u201d (pozitiv) pentru o anumit\u0103 condi\u021bie genetic\u0103 dintre cele analizate indic\u0103 faptul c\u0103 exist\u0103 o mare probabilitate (&gt;99.9%) ca f\u0103tul s\u0103 fie afectat de boala genetic\u0103 respectiv\u0103. Deoarece <strong>Veracity<\/strong> nu este un test diagnostic, un rezultat pozitiv trebuie confirmat printr-un test de diagnostic prenatal din lichid amniotic sau vilozit\u0103\u021bi coriale.<\/p>\n<p style=\"text-align: justify;\">De\u0219i rata de detec\u021bie este foarte mare un rezultat \u201erisc sc\u0103zut\u201d (negativ) nu poate exclude \u00een totalitate posibilitatea unei afec\u021biuni genetice fetale.<\/p>\n<p style=\"text-align: justify;\">\u00cen ceea ce prive\u0219te detec\u021bia aneuploidiilor cromozomilor sexuali acurate\u021bea testului este de &gt;99.9%<sup>6;<em>7<\/em><\/sup>.<\/p>\n<p style=\"text-align: justify;\"><strong><em>Limite \u0219i interferen\u021be<\/em><\/strong><\/p>\n<p style=\"text-align: justify;\">\u00cen cazul \u00een care gravidei i s-au efectuat transfuzii cu s\u00e2nge, transplant sau terapie cu celule stem, exist\u0103 posibilitatea ob\u021binerii unui rezultat fals din cauza ADN-ului exogen.<\/p>\n<p style=\"text-align: justify;\">Testele <strong>VERACITY<\/strong> nu pot eviden\u021bia:<\/p>\n<ul style=\"text-align: justify;\">\n<li>transloca\u021bii cromozomiale echilibrate \u0219i alte anomalii cromozomiale structural dec\u00e2t cele men\u021bionate mai sus<\/li>\n<li>poliploidii<\/li>\n<li>mozaicisme cromozomiale fetale \u0219i\/sau placentare \u00een procent mic (adic\u0103 prezen\u021ba a dou\u0103 linii celulare cu un set diferit de cromozomi, cu o linie celular\u0103 slab reprezentat\u0103)<\/li>\n<li>defecte de metilare a ADN-ului<sup>6;<em>7<\/em><\/sup>.<\/li>\n<\/ul>\n<p style=\"text-align: justify;\"><em><strong>\u0411\u0438\u0431\u043b\u0438\u043e\u0433\u0440\u0430\u0444\u0438\u044f<\/strong><\/em><\/p>\n<ol>\n<li style=\"text-align: justify;\">The American College of Obstetricians and Gynecologists.
