Investigarea muta\u021biilor genei ELA2 – Tipul de mo\u0219tenire este autosomal dominant. Genele responsabile de dezvoltarea bolii-gena ela2 – serin\u0103 proteaz\u0103 (elastaz\u0103, exprimat\u0103 \u00een neutrofile) – este localizat\u0103 pe cromozomul 19 \u00een regiunea 19p13.3).<\/p>\n

\u00cen plus, muta\u021biile genelor GFII (factorul de activare a elastazei neutrofile) \u0219i 6-CSFR care codific\u0103 receptorul G-CSF sunt rareori detectate. Sindromul Kostmann. Sindromul Kostman este un tip de neutropenie care are un tip de mo\u0219tenire autosomal recesiv.<\/p>\n

Un grup eterogen de boli bazate pe o \u00eenc\u0103lcare a granulocitopoiezei la nivelul promielocitelor cu o sc\u0103dere a num\u0103rului de neutrofile sub 0,2* 109\/l \u00een s\u00e2ngele periferic.<\/p>\n

Gena ELA2 codific\u0103 o serin\u0103 proteaz\u0103 care face parte din granulele azurofile ale neutrofilelor. Boala se manifest\u0103 \u00een primele zile de via\u021b\u0103. Neutropenia este \u00eemp\u0103r\u021bit\u0103 \u00een mai multe forme \u00een func\u021bie de tipul de mo\u0219tenire \u0219i gene, muta\u021bii \u00een care duc la boal\u0103:<\/p>\n

Principala manifestare clinic\u0103 a bolii este infec\u021biile bacteriene severe. Tratamentul bolii se bazeaz\u0103 pe utilizarea factorului de stimulare a coloniilor de granulocite (GCSF). O caracteristic\u0103 a cursului clinic al acestei boli este sc\u0103derea periodic\u0103 a num\u0103rului de neutrofile din s\u00e2ngele periferic sub 0,2 * 109 \/ l, urmat\u0103 de o cre\u0219tere la valori aproape normale (frecven\u021ba variaz\u0103 de la 15 la 35 de zile). Mai des, boala se manifest\u0103 \u00een copil\u0103rie. Boala este mo\u0219tenit\u0103 de un tip autosomal dominant \u0219i este cauzat\u0103 de muta\u021bii ale genei ELA2. Au fost \u00eenregistrate o serie de cazuri sporadice ale bolii datorit\u0103 apari\u021biei de noi muta\u021bii. La examinarea m\u0103duvei osoase pe fondul hiperplaziei mieloide, un num\u0103r crescut de mieloblaste relev\u0103 o \u00eentrerupere a matur\u0103rii la nivelul promielocitelor, se g\u0103se\u0219te adesea eozinofilie. Examenul citogenetic relev\u0103 cariotipul normal al celulelor m\u0103duvei osoase.<\/p>\n

To\u021bi pacien\u021bii cu neutropenie prezint\u0103 un risc crescut de dezvoltare a sindromului mielodisplazic \u0219i a leucemiei mieloblastice acute.<\/p>\n

Frecven\u021ba apari\u021biei nu este stabilit\u0103. Boala este rar\u0103.<\/p>\n

Efectuarea unui diagnostic clinic necesit\u0103 o serie de studii de laborator \u0219i consult\u0103ri cu medici cu \u00eenalt\u0103 calificare: un hematolog \u0219i un genetician, lu\u00e2nd \u00een considerare manifest\u0103rile clinice complexe \u0219i istoricul familial, care pot ajuta la determinarea uneia sau altei forme de neutropenie. Confirmarea diagnosticului este detectarea unei muta\u021bii a genei responsabile de dezvoltarea bolii.<\/p>\n
