C1q este o protein\u0103 de 400 kD format\u0103 din 18 catene polipeptidice, cu regiuni \u00een triplu helix \u015fi regiuni globulare, la nivelul c\u0103rora se g\u0103sesc situsurile de legare cu imunoglobulinele. C1q interac\u0163ioneaz\u0103 prin domeniul s\u0103u globular cu domenii specifice ale IgG \u015fi IgM numai dac\u0103 acestea formeaz\u0103 complexe imune. C1q nu are activitate proteazic\u0103 \u015fi se asociaz\u0103 cu 2 molecule C1r \u015fi 2 C1s, \u00een prezen\u0163a Ca<sup>2+<\/sup>. \u00cen mod normal, activarea spontan\u0103 a complexului C1s \u2013 C1r este stopat\u0103 de un factor inhibitor \u2013 C1 inh (inhibitor de serin proteaz\u0103). \u00cen urma ata\u015f\u0103rii complexului antigen \u2013 anticorp, C1q sufer\u0103 o modificare conforma\u0163ional\u0103 care duce la eliberarea C1 \u2013 inh \u015fi la activarea autocatalitic\u0103 a unei molecule de C1r.<\/p>\n<p style=\"text-align: justify;\">O serie de molecule, cum ar fi: proteina C reactiv\u0103, \u03b2 amiloidul, heparina, unele bacterii gram negative, virusuri, mycoplasme, protozoare, componente celulare (ADN, membrana mitocondrial\u0103, filamente de citoschelet) au de asemenea capacitatea de a activa complementul pe calea clasic\u0103 prin interac\u0163iunea direct\u0103 cu componenta C1q.<\/p>\n<p style=\"text-align: justify;\">Deficien\u0163ele congenitale de la nivelul componentelor C1-C4 duc la incapacitatea de a genera peptide\u00a0necesare elimin\u0103rii complexelor imune, de a atrage neutrofile sau de a genera activitate catalitic\u0103. Deficitul de C1 este asociat cu o inciden\u0163\u0103 crescut\u0103 a bolilor cu complexe imune (lupus eritematos sistemic, polimiozita, glomerulonefrita \u015fi purpura Henoch-Schonlein). LES este manifestarea cea mai comun\u0103 de deficien\u0163\u0103 de C1, iar manifest\u0103rile clinice nu se deosebesc de cele din LES f\u0103r\u0103 deficit de complement, \u00eens\u0103 v\u00e2rsta de debut este de multe ori \u00eenainte de pubertate.<\/div>\n<div style=\"text-align: justify;\">Niveluri sc\u0103zute de C1 au fost, de asemenea, raportate la pacien\u0163ii cu niveluri anormale de imunoglobuline (hipogamaglobulinemie Bruton, hipogamaglobulinemie form\u0103 comun\u0103 \u015fi imunodeficien\u0163\u0103 sever\u0103 combinat\u0103), dar acest lucru este cel mai probabil cauza unui catabolism crescut.<\/div>\n<div style=\"text-align: justify;\">M\u0103surarea activit\u0103\u0163ii C1q este un indicator al prezen\u0163ei componentei C1. Absen\u0163a C1q \u00een prezen\u0163a unor valori normale ale C3 \u015fi C4 sunt \u00een concordan\u0163\u0103 cu o deficien\u0163\u0103 C1.
