Metabolitul oxcarbazepinei este monohidroxi-carbamazepina (MHC) \u00a0care ac\u021bioneaz\u0103 similar oxcarbazepinei<sup>1<\/sup>. Oxcarbazepina este folosit\u0103 ca monoterapie sau ca tratament adjuvant\u00a0 \u00een cadrul convulsiilor par\u021biale cu sau f\u0103r\u0103 generalizare secundar\u0103 \u0219i la copii mai mari de 6 ani<sup>2<\/sup>.<\/p>\n<p>Epilepsia benign\u0103 centrotemporal\u0103 este una dintre cele mai comune tipuri de epilepsie idopatic\u0103, cu debutul la v\u00e2rsta de 6-10 ani; Oxcarbazepina a demonstrat efecte moderat pozitive \u00een tratamentul acestei patologii, efecte protectoare cognitive, dar \u0219i efecte date de capacitatea sa de a ac\u021biona ca stabilizator al dispozi\u021biei<sup>3<\/sup>. Timpul de \u00eenjum\u0103t\u0103\u021bire prin eliminare este de 1 p\u00e2n\u0103 la 2,5 ore pentru OCBZ \u0219i de 8 p\u00e2n\u0103 la 10 ore pentru MHC.<\/p>\n<p>Intervalul terapeutic (10-35 mcg\/mL) se bazeaz\u0103 pe concentra\u021biile metabolitului s\u0103u, acest test m\u0103sur\u00e2nd exclusiv metabolitul acestuia. \u00cen practica clinic\u0103, doza de OCBZ ar trebui s\u0103 fie ajustat\u00e2 individual pentru fiecare pacient, pentru a atinge raspunsul terapeuticul dorit.<\/p>\n<p>Oxcarbazepina prezint\u0103 avantaje comparativ cu carbamazepina, datorit\u0103 efectelor adverse reduse<sup>4<\/sup>. Printre efectele adverse ale oxcarbazepinei se num\u0103r\u0103 ame\u021beli, somnolen\u021b\u0103, grea\u021b\u0103, ataxie, tulburaru de vedere, dureri abdominale, tremor, dispepsie \u0219i tulbur\u0103ri ale mersului. Printre reac\u021biile adverse rare se num\u0103r\u0103: sindromul Stevens-Johnson, eurp\u021bia maculo-papular\u0103 \u0219i necroliza toxic\u0103 epidermic\u0103, care necesit\u0103 \u00eentreruperea tratamentului<sup>5,6<\/sup>. Oxcarbazepin\u0103 prezint\u0103 efect teratogen minor<sup>1<\/sup>.<\/p>\n<p><strong><em>Recomand\u0103ri pentru determinarea oxcarbazepinei<\/em><\/strong><\/p>\n<ul>\n<li>Monitorizarea eficien\u021bei terapeutice<\/li>\n<li>Evaluarea unei posibile toxicit\u0103\u021bi a medicamentului<\/li>\n<\/ul>\n<p><strong><em>Preg\u0103tire pacient<\/em><\/strong><strong>\u00a0-\u00a0<\/strong>S\u00e2ngele trebuie recoltat\u00a0 \u00eenainte de urm\u0103toarea doz\u0103 programat\u0103<sup>7<\/sup>.<\/p>\n<p><strong><em>Specimen recoltat<\/em><\/strong><strong>\u00a0-\u00a0<\/strong>ser<sup>7<\/sup><\/p>\n<p><strong><em>Recipient de recoltare<\/em><\/strong><strong>\u00a0-\u00a0<\/strong>vacutainer ro\u0219u f\u0103r\u0103 anticoagulant<sup>7<\/sup><\/p>\n<p><strong><em>Prelucrare necesar\u0103 dup\u0103 recoltare<\/em><\/strong><strong>\u00a0-\u00a0<\/strong>\u00een termen de 2 ore de la recoltare, specimenul trebuie centrifugat<sup>7<\/sup><\/p>\n<p><strong><em>Volum prob\u0103<\/em><\/strong><strong>\u00a0-<\/strong>\u00a0minim 0.5 mL<sup>7<\/sup><\/p>\n<p><strong><em>Stabilitate prob\u0103<\/em><\/strong><strong>\u00a0-\u00a0<\/strong>Recomandat la temperaturi cuprinse intre 2-8\u00b0C timp de 28 zile, la temperatur\u0103 ambiental\u0103 pentru 28 de zile, iar la congelator pentru 28 de zile.<sup>7<\/sup><\/p>\n<p><strong><em>Metod\u0103<\/em><\/strong><strong>\u00a0-\u00a0<\/strong>Spectrometrie de mas\u0103 \u00een tandem cu cromatografie lichid\u0103 (HTLC-MS\/MS)<sup>7<\/sup><\/p>\n<p><strong><em>Valori de referin\u021b\u0103<\/em><\/strong><strong>\u00a0-\u00a0<\/strong>10-35 mcg\/mL<sup>7<\/sup><strong>\u00a0<\/strong><\/p>\n<p><strong><em>Limite \u0219i interferen\u021be<\/em><\/strong><\/p>\n<p>Nivelurile toxice nu au fost bine stabilite.<sup>7<\/sup><\/p>\n<p>Acest test nu poate fi efectuat din s\u00e2nge integral, ci doar din ser.<sup>7<\/sup><\/p>\n<p><strong><em>Aten\u021bionare<\/em><\/strong><\/p>\n<p>Intervalele terapeutice se bazeaz\u0103 pe specimene extrase imediat \u00eenainte de administrarea dozei urmatoare. Majoritatea indivizilor arat\u0103 un r\u0103spuns optim la terapia cu oxcarbazepin\u0103 atunci c\u00e2nd nivelurile serice ale metaboli\u021bi (m\u0103sura\u021bi cu acest test) sunt cuprinse \u00a0\u00eentre 10 \u0219i 35 mcg\/mL. Unele persoane pot r\u0103spunde bine \u00een afara acestui interval, sau pot prezenta toxicitate \u00een intervalul terapeutic.
