Sultiamul ac\u021bioneaz\u0103 prin inhibarea anhidrazelor carbonice; inhibarea anhidrazei carbonice II citosolice stabilizeaz\u0103 neuronii responsabili de generare de convulsii. Ac\u021biunea sa antiepileptic\u0103 este dat\u0103 \u0219i de inhibarea canalelor de sodiu<sup>1<\/sup>.<\/p>\n<p>Sultiamul utilizat ca monoterapie reduce apari\u021bia convulsiilor \u0219i reduce desc\u0103rc\u0103rile electroencefalografice la persoanele cu epilepsie benign\u0103 din copil\u0103rie (focar centrotemporal), \u00een\u00a0 epilepsia copil\u0103riei benign\u0103, epilepsia localizat\u0103 simptomatic\u0103, epilepsia mioclonic\u0103 juvenil\u0103, sindrom West \u0219i la adul\u021bii cu epilepsie rezistent\u0103 la medicamente \u0219i dificult\u0103\u021bi de \u00eenv\u0103\u021bare<sup>2,3<\/sup>.<\/p>\n<p>Sultiam este ini\u021bial dozat \u00eentr-un interval de concentratie medie. Doza se calculeaz\u0103 pe greutatea corporal\u0103, 5-10 mg\/kg de greutate corporal\u0103, \u00eemp\u0103r\u021bit\u0103 \u00een 3 doze individuale. Pentru un t\u00e2n\u0103r care c\u00e2nt\u0103re\u0219te aproximativ 50 kg, aceasta ar fi de 250-500 mg pe zi. Sultiamul are un timp de \u00eenjum\u0103t\u0103\u021bire \u00eentre 2 \u0219i 16 ore, iar printre efectele adverse se num\u0103r\u0103: deteriorarea memoriei, a capacit\u0103\u021bii de \u00eenv\u0103\u021bare, diminuarea aten\u021biei, acidoz\u0103 metabolic\u0103 \u0219i respiratorie \u0219i cristalurie<sup>1,5,6<\/sup>. Disconfort gastric ca efect secundar poate sa apara la aproximativ 10% dintre pacien\u021bi.<\/p>\n<p>Pot aparea furnic\u0103turi la nivelul m\u00e2inilor, picioarelor \u0219i fe\u021bei, respira\u021bie accelerat\u0103, ame\u021beli, cefalee, palpita\u021bii, vedere dubl\u0103, sughi\u021b \u0219i pierdere \u00een greutate, tulburari cardiace \u0219i pierderea poftei de m\u00e2ncare, dependent de doz\u0103.<\/p>\n<p>\u00cen principiu, administrarea tuturor medicamentelor antiepileptice poate fi supus\u0103 dozarii, p\u00e2n\u0103 la pragul la care acestea nu mai au un efect anticonvulsiv, sau p\u00e2n\u0103 la concentra\u021bii \u00een sange la care apar reac\u021bii adverse. C\u00e2nd se \u00eent\u00e2mpl\u0103 acest lucru, doza este redus\u0103 la ultimul nivel tolerat.
