Are localizare transmembranar\u0103 \u015fi este alc\u0103tuit\u0103 dintr-un singur lan\u0163 polipeptidic cu dou\u0103 situsuri catalitice ce con\u0163in zinc. Clivajul proteolitic elibereaz\u0103 din membrana celular\u0103 \u00een spa\u0163iul extracelular enzima func\u0163ional\u0103, circulant\u0103<sup>1<\/sup>.<\/p>\n<p style=\"text-align: justify;\">Majoritatea ACE (~90%) se g\u0103se\u015fte legat\u0103 \u00een \u0163esuturi \u015fi doar o mic\u0103 parte circul\u0103 liber\u0103 \u00een plasm\u0103. Sursa principal\u0103 a enzimei este endoteliul pulmonar.<\/p>\n<p style=\"text-align: justify;\">Enzima particip\u0103 \u00een cascada sistemului renin\u0103-angiotensin\u0103-aldosteron ca r\u0103spuns la hipovolemie, fiind responsabil\u0103 de conversia angiotensinei I \u00een angiotensina II, vasoconstrictor puternic care cre\u015fte tensiunea arterial\u0103. De asemenea angiotensin convertaza este responsabil\u0103 \u015fi de inactivarea bradikininei (\u00een sistemul kalikrein\u0103-kinin\u0103)<sup>1;2<\/sup>.<\/p>\n<p style=\"text-align: justify;\">Activitatea ACE este crescut\u0103 \u00een sarcoidoz\u0103, boal\u0103 granulomatoas\u0103 sistemic\u0103 care afecteaz\u0103 \u00een mod obi\u015fnuit pl\u0103m\u00e2nul. \u00cen sarcoidoz\u0103 se pare c\u0103 ACE este produs\u0103 de celulele epiteloide \u015fi macrofagele forma\u0163iunilor granulomatoase. Pacien\u0163ii cu tuberculoz\u0103 activ\u0103 sau boal\u0103 Hodgkin \u2013 afec\u0163iuni care pot genera modific\u0103ri radiologice similare sarcoidozei \u2013 nu prezint\u0103 valori ACE crescute, element important pentru diagnosticul diferen\u0163ial<sup>2<\/sup>.<\/p>\n<p style=\"text-align: justify;\">Activitatea ACE reflect\u0103 <u>severitatea sarcoidozei<\/u>: \u00een stadiul I testul este pozitiv \u00een 68% din cazuri, \u00een stadiul II 86% din pacien\u0163i prezint\u0103 valori crescute, iar \u00een stadiul III se ob\u0163in rezultate pozitive \u00een 91% din cazuri<sup>3<\/sup>. Pe m\u0103sur\u0103 ce boala progreseaz\u0103 spre fibroz\u0103, activitatea ACE scade<sup>1<\/sup>. De asemenea valorile ACE reflect\u0103 <u>activitatea bolii<\/u>, la pacien\u0163ii\u00a0afla\u0163i \u00een remisiune sub corticoterapie \u00eenregistr\u00e2ndu-se sc\u0103deri dramatice ale ACE<sup>3<\/sup>.<\/p>\n<p style=\"text-align: justify;\">Monitorizarea activit\u0103\u0163ii ACE mai poate avea utilitate \u00een evaluarea riscului de afectare pulmonar\u0103 la pacien\u0163ii care au primit tratamente citostatice, \u00een special cu bleomicin\u0103.<\/p>\n<p style=\"text-align: justify;\">Valori ACE crescute mai pot fi \u00eent\u00e2lnite \u00een sindromul Melkersson-Rosenthal, o afec\u0163iune foarte rar\u0103 caracterizat\u0103 histologic prin prezen\u0163a unor granuloame f\u0103r\u0103 necroz\u0103 cazeoas\u0103, asem\u0103n\u0103toare cu cele din sarcoidoz\u0103. Activitatea ACE se normalizeaz\u0103 dup\u0103 un tratament eficient cu metotrexat<sup>2<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><strong>Neurosarcoidoza <\/strong>este o complica\u021bie rar\u0103, dar sever\u0103 a sarcoidozei ce afecteaz\u0103 aproximativ 5-10% din pacien\u021bi. Cele mai frecvente manifest\u0103ri ale acestei complica\u021bii sunt: mielopatia, neuropatia cranian\u0103 (perechile VII \u0219i II de nervi cranieni sunt cele mai afectate) \u0219i encefalopatia. \u00cen lipsa tratamentului pacien\u021bii pot dezvolta simptome neurologice acute (epilepsie, cre\u0219terea presiunii intracraniene, compresie medular\u0103). Valorile crescute ale ACE \u00een lichid cefalo-rahidian (LCR) sugereaz\u0103 diagnosticul de neurosarcoidoz\u0103, dar nu reprezint\u0103 un test diagnostic (specificitatea este de 94-95%, dar sensitivitatea este &lt;60%). Un procent de 30% din pacien\u021bi nu prezint\u0103 modific\u0103ri ale LCR<sup>4<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><strong><em>Recomand\u0103ri pentru determinarea angiotensin convertazei \u00een LCR:<\/em><\/strong><\/p>\n<ul style=\"text-align: justify;\">\n<li>suspiciune clinic\u0103 de afectare a sistemului nervos central \u0219i a meningelui \u00een sarcoidoz\u0103;<\/li>\n<li>evaluarea r\u0103spunsului la tratament la pacien\u021bii cu neurosarcoidoz\u0103<sup>4;5<\/sup>.<\/li>\n<\/ul>\n<p style=\"text-align: justify;\"><strong><em>Specimen recoltat <\/em><\/strong>\u2013 LCR<sup>6<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><strong><em>Recipient de recoltare <\/em><\/strong>\u2013 tub pentru transportul lichidelor de punc\u021bie<sup>6<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><strong><em>Volum prob\u0103 <\/em><\/strong>\u2013 1ml LCR<sup>6<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><strong><em>Cauze de respingere a probei<\/em><\/strong> \u2013 specimen hemolizat sau xantocrom<sup>6<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><strong><em>Stabilitate prob\u0103 <\/em><\/strong>\u2013 LCR este stabil <em>4 ore<\/em> la temperatura camerei, refrigerat (4-8\u00b0C) <em>1 s\u0103pt\u0103m\u00e2n\u0103<\/em>, congelat <em>6 luni<\/em><sup>1;6<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><strong><em>Metod\u0103 <\/em><\/strong><em>- <\/em><u>metod\u0103 radioenzimatic\u0103<sup>6<\/sup><\/u>.<\/p>\n<p style=\"text-align: justify;\"><strong><em>Valori de referin\u0163\u0103<\/em><\/strong><sup>4<\/sup> \u2013 &lt; 2 U\/L<sup>6<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><strong><em>Interpretarea rezultatelor<\/em><\/strong><\/p>\n<p style=\"text-align: justify;\">Nivelurile ACE \u00een LCR sunt crescute la aproximativ 55% din pacien\u021bii cu neurosarcoidoz\u0103, la 5% din pacien\u021bii cu sarcoidoz\u0103 far\u0103 semne de afectare neurologic\u0103 \u0219i la 13% din pacien\u021bii cu alte boli neurologice<sup>4<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><strong><em>Limite \u0219i interferen\u021be<\/em><\/strong><\/p>\n<p style=\"text-align: justify;\">Medicamentele antihipertensive, cum ar fi inhibitorii ACE, blocantele receptorilor de angiotensin\u0103 \u0219i diureticele pot fi interfera cu testul<sup>4<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><span style=\"font-size: 12pt;\"><strong>\u0411\u0438\u0431\u043b\u0438\u043e\u0433\u0440\u0430\u0444\u0438\u044f<\/strong><\/span><\/p>\n<ol style=\"text-align: justify;\">\n<li><span style=\"font-size: 10pt;\">Naif Z.
