Este o enzim\u0103 citoplasmatic\u0103 tetrameric\u0103, compus\u0103 din subunit\u0103\u0163i H (Heart)\u00a0\u015fi M (Muscle), larg distribuit\u0103 \u00een organism, fiind \u00eent\u00e2lnit\u0103 cu prec\u0103dere \u00een rinichi, miocard, musculatura scheletic\u0103, creier, eritrocite, ficat \u015fi pl\u0103m\u00e2ni.<\/p>\n<p style=\"text-align: justify;\">LDH este un indicator nespecific de lezare tisular\u0103. Pentru a identifica sursa cre\u015fterii nivelului enzimatic sunt necesare izolarea \u015fi cuantificarea celor 5 frac\u0163iuni diferite ale LDH-ului rezultate prin combinarea \u00een diferite propor\u0163ii a celor dou\u0103 tipuri de monomeri H \u015fi M: LDH-1 (H4), LDH-2 (H3M), LDH-3 (H2M2), LDH-4 (HM3) \u015fi LDH-5 (M4).<\/p>\n<p style=\"text-align: justify;\">Specificitatea tisular\u0103 deriv\u0103 din faptul c\u0103\u00a0\u00een anumite \u0163esuturi exist\u0103 sinteza specific\u0103 de diferite subunit\u0103\u0163i\u00a0\u00een raporturi bine definite. Astfel celulele cardiace, eritrocitele \u015fi rinichii sintetizeaz\u0103 preferen\u0163ial subunit\u0103\u0163i H (LDH-1 -H4),\u00a0\u00een timp ce hepatocitele sintetizeaz\u0103 aproape exclusiv subunit\u0103\u0163i M. Musculatura striat\u0103 scheletal\u0103 produce, de asemenea,\u00a0\u00een mare m\u0103sur\u0103, subunit\u0103\u0163i M, astfel \u00eenc\u00e2t LDH-5 cre\u015fte at\u00e2t\u00a0\u00een afec\u0163iuni hepatice c\u00e2t \u015fi musculare. LDH-1 \u015fi LDH-5 sunt cel mai adesea utilizate pentru a indica patologie cardiac\u0103 sau hepatic\u0103. LDH-2 se g\u0103se\u015fte\u00a0\u00een eritrocite, inim\u0103, rinichi, sistemul reticuloendotelial, LDH-3 se \u00eent\u00e2lne\u015fte preferen\u0163ial\u00a0\u00een pl\u0103mani, limfocite \u015fi pancreas, iar LDH-4\u00a0\u00een musculatura striat\u0103, ficat, rinichi sau pancreas<sup>2;3<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><strong><em>Recomand\u0103ri pentru determinarea izoenzimelor LDH<\/em><\/strong><\/p>\n<ul style=\"text-align: justify;\">\n<li>Diagnosticul afec\u0163iunilor cardiace, hepatice, musculare, renale, pulmonare sau hematologice;<\/li>\n<li>Diagnosticul diferen\u0163ial al bolilor cardiace, hepatice, musculare, renale, pulmonare sau hematologice;<\/li>\n<li>Diagnosticul IMA \u00een combina\u0163ie cu creatinkinaza totala (CK) \u015fi CK-MB;<\/li>\n<li>Investigarea cauzelor inexplicabile de cre\u015fteri LDH;<\/li>\n<li>Detectarea formelor macro-LDH<sup>3<\/sup>.<\/li>\n<\/ul>\n<p style=\"text-align: justify;\"><strong><em>Preg\u0103tire pacient <\/em><\/strong>\u2013 \u00e0 jeun (pe nem\u00e2ncate) sau postprandial<sup>1<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><strong><em>Specimen recoltat <\/em><\/strong>- <em>s\u00e2<\/em>nge venos<sup>1<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><strong><em>Recipient de recoltare <\/em><\/strong>\u2013 vacutainer f\u0103r\u0103 anticoagulant cu\/f\u0103r\u0103 gel separator<sup>1<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><strong><em>Prelucrare necesar\u0103 dup\u0103 recoltare <\/em><\/strong>\u2013 se separ\u0103 serul prin centrifugare<sup>4<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><strong><em>Volum prob\u0103<\/em><\/strong> \u2013 2 ml ser<sup>4<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><strong><em>Cauze de respingere<\/em><\/strong> <strong><em>a probei <\/em><\/strong>\u2013 specimen hemolizat<sup>4<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><strong><em>Stabilitate prob\u0103 <\/em><\/strong>\u2013 1 saptamana la 15-28\u00b0C. <strong>Probele NU se trimite refrigerate sau congelate<\/strong><sup>4<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><strong><em>Metod\u0103 <\/em><\/strong>- <span style=\"text-decoration: underline;\">electroforez\u0103<\/span><sup>1<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><strong><em>Valori de referin\u0163\u0103<\/em><\/strong><sup>1<\/sup>:<\/p>\n<p style=\"text-align: justify; padding-left: 30px;\">LDH-1: 0.66-1.29 \u00b5mol\/sl<\/p>\n<p style=\"text-align: justify; padding-left: 30px;\">LDH-2: 1.2 \u00a0\u2013 1.71 \u00b5mol\/sl<\/p>\n<p style=\"text-align: justify; padding-left: 30px;\">LDH-3: 0.7 \u2013 1.07 \u00b5mol\/sl<\/p>\n<p style=\"text-align: justify; padding-left: 30px;\">LDH-4: 0.24 \u2013 0.5 \u00b5mol\/sl<\/p>\n<p style=\"text-align: justify; padding-left: 30px;\">LDH-5: 0.13 \u2013 0.71 \u00b5mol\/sl<\/p>\n<p style=\"text-align: justify;\"><strong><em>Interpretarea rezultatelor<\/em><\/strong><em>\u00a0<\/em><\/p>\n<p style=\"text-align: justify;\">Cre\u015fteri marcate ale activit\u0103\u0163ii lactat dehidrogenazei totale (LDH) sau modelul izomorf (LDH total semnificativ mai mare, f\u0103r\u0103 nici o cre\u015ftere \u00een procente a unei frac\u0163iuni) pot fi observate \u00een anemia megaloblastic\u0103, anemia pernicioas\u0103 netratat\u0103, boala Hodgkin, neoplazii, boli cardiorespiratorii, hipotiroidism, mononucleoza infec\u0163ioas\u0103 sau alte afec\u0163iuni inflamatorii, uremie, hipoxie sau \u015foc sever.<\/p>\n<p style=\"text-align: justify;\">In infarctul miocardic acut LDH-1 apare crescut \u00een ser dup\u0103 aproximativ 10-12 ore, atinge valori maxime \u00een 48-72 ore, apoi revine la normal \u00een 10-14 zile. Deoarece acest test este nespecific, el poate fi \u00eenlocuit cu determinarea troponinei T sau creatinkinazei-MB (CK-MB), cu excep\u0163ia cazului \u00een care IMA a avut loc cu cel pu\u0163in 24 ore \u00eenainte de testare.<\/p>\n<p style=\"text-align: justify;\">In mod normal, raportul LDH-1\/LDH-2 este subunitar (&lt;0.81). Valorile cuprinse \u00eentre 0.82-0.99 indic\u0103 o suspiciune de leziune miocardic\u0103, iar un raport &gt;1 (sau cel pu\u0163in &gt;0.9) este diagnostic pentru IMA, \u00een cazul \u00een care \u015fi criteriile clinice sunt \u00eendeplinite. Angina instabil\u0103 se asociaz\u0103 cu o cre\u015ftere a LDH-1\/LDH-2, dar cu valori normale ale LDH-ului total. Cu toate acestea, o cre\u015ftere progresiv\u0103 a LDH-1\/LDH-2 f\u0103r\u0103 inversiune complet\u0103 este important\u0103 pentru diagnosticul de IMA.<\/p>\n<p style=\"text-align: justify;\">Persisten\u0163a inversiunii raportului LDH-1\/LDH-2 \u00een zilele urm\u0103toare infarctului miocardic acut poate reprezenta un marker pentru reinfarctizare. Atunci c\u00e2nd IMA este complicat cu \u015foc, modelul izomorf precede modificarea raportului LDH-1\/LDH-2.<\/p>\n<p style=\"text-align: justify;\">Cre\u015fterile raportului LDH-1\/LDH-4 s-au dovedit a fi un indicator mai bun al IMA.<\/p>\n<p style=\"text-align: justify;\">Valori ridicate ale LDH-1 asociate cu inversarea raportului LDH-1\/LDH-2 caracterizeaz\u0103 anemia megaloblastic\u0103 (deficit de acid folic, anemia pernicioas\u0103), boala hemolitic\u0103 sau alte forme de hemoliz\u0103 <em>in vivo<\/em>, rabdomioliz\u0103, efortul fizic excesiv \u015fi ocazional, infarctul renal.<\/p>\n<p style=\"text-align: justify;\">Concentra\u0163ii plasmatice crescute ale LDH-1 pot fi int\u00e2lnite la pacien\u0163i cu seminom testicular, tumori testiculare nonseminomatoase sau disgerminom.<\/p>\n<p style=\"text-align: justify;\">Cre\u015fterea raportului LDH-5\/LDH-1 poate fi sugestiv pentru carcinomul prostatic sau alte tipuri de tumori.<\/p>\n<p style=\"text-align: justify;\">Cre\u015fteri ale formelor LDH-2, LDH-3, LDH-4 sunt nespecifice \u015fi se asociaz\u0103 cu \u015foc septic, trombembolism pulmonar, pneumonie extins\u0103, leucemie acut\u0103, limfocitoz\u0103, pancreatit\u0103 acut\u0103, neoplasme.<\/p>\n<p style=\"text-align: justify;\">Nivele serice ridicate ale frac\u0163iunii LDH-5 sunt determinate de leziuni ale musculaturii striate (traumatisme), de boli hepatice (congestie hepatic\u0103, insuficien\u0163\u0103 cardiac\u0103 congestiv\u0103, hepatit\u0103, ciroz\u0103, alcoolism, hepatita determinat\u0103 de clorpromazina, intoxica\u0163ia cu tetraclorura de carbon) precum \u015fi de afec\u0163iuni cutanate (arsuri). Unele studii au ar\u0103tat c\u0103 un raport LDH-4\/LDH-5 &lt;1 sus\u0163ine diagnosticul de carcinom hepatocelular, \u00een timp ce o valoare supraunitar\u0103 a raportului indic\u0103 metastaze hepatice \u00een &gt; 90% dintre cazuri.<\/p>\n<p style=\"text-align: justify;\">Cre\u015fterea LDH-5 este mai semnificativ\u0103 dac\u0103 se asociaz\u0103 \u015fi cu cre\u015fteri ale raportul LDH-5\/LDH-4. Cu toate acestea, teste suplimentare, cum ar fi: ALT, GGT, electroforeza proteinelor serice \u015fi timpul de protrombin\u0103\u00a0\u00a0pot fi utile \u00een stabilirea unui diagnostic de certitudine.<\/p>\n<p style=\"text-align: justify;\">Macroenzimele sunt complexe cu greutate molecular\u0103 mare, formate dintr-o enzim\u0103 \u015fi imunoglobuline A sau G, ce pot fi detectate \u00een ser. Aceste complexe se caracterizeaz\u0103 printr-o migrare anormal\u0103 a benzilor electroforetice de izoenzim\u0103 sau prin extinderea anormal\u0103 a unei benzi. De\u015fi nu pare s\u0103 provoace sau s\u0103 fie asociat\u0103 cu nici un simptom sau boal\u0103, prezen\u0163a de macro-LDH\u00a0 poate determina o cre\u015ftere a LDH-ului total.<\/p>\n<p style=\"text-align: justify;\">Recent s-a descoperit o a \u015fasea izoenzim\u0103, localizat\u0103\u00a0\u00een apropierea catodului \u015fi denumit\u0103 LDH-6. Aceasta nu este un complex cu imunoglobulin\u0103 \u015fi a fost descris\u0103 la subiec\u0163ii cu boli hepatice \u015fi presupune un prognostic nefavorabil<sup>2;3<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><strong><em>Limite \u015fi interferen\u0163e<\/em><\/strong><\/p>\n<div style=\"text-align: justify;\">Un specimen hemolizat nu este acceptabil deoarece eritrocitele con\u0163in mai mult LDH dec\u00e2t serul \u015fi determin\u0103 valori fals crescute. Activitatea LDH este unul dintre indicatorii cei mai sensibili ai hemolizei <em>in vitro<\/em>. Inghe\u0163area sau depozitarea prelungit\u0103 la 4<sup>0<\/sup>C (&gt; 12 ore) poate cauza modific\u0103ri ale frac\u0163iunii LDH-5<sup>1;3<\/sup>.<\/div>\n<p style=\"text-align: justify;\">\u2022 Medicamente<\/p>\n<p style=\"text-align: justify;\"><strong><span style=\"text-decoration: underline;\">Cre\u015fteri<\/span><\/strong><strong>:<\/strong> acebutolol, acid valproic, amiodarona, anestezice, aspirina, azitromicina, azlocilina, captopril, cefalosporine, cimetidina, clorpromazina, dicumarol, etanol, etretinat, flufenazin, foscarnet, furosemid, heparina, imipramina, interferon, ketoconazol, labetalol, metotrexat, metoprolol, nitrofurantoin, antiinflamatoare nesteroidiene (ex.: diflunisal, ketoprofen, piroxicam), penicilamina, piperacilina, plicamicina, propoxifen, simvastatin, sulfonamide, ticarciclina.<\/p>\n<p style=\"text-align: justify;\"><strong><span style=\"text-decoration: underline;\">Sc\u0103deri<\/span><\/strong><strong>:<\/strong> acid ascorbic, amikacina, anticonvulsivante, clofibrat, enalapril, hidroxiuree, metronidazol.<\/p>\n<p style=\"text-align: justify;\"><span style=\"font-size: small;\">\u00a0<\/span><\/p>\n<p style=\"text-align: justify;\"><strong><span style=\"font-size: small;\">\u0411\u0438\u0431\u043b\u0438\u043e\u0433\u0440\u0430\u0444\u0438\u044f<\/span><\/strong><\/p>\n<div style=\"text-align: justify;\"><span style=\"font-size: small;\">1.
