Rezisten\u021ba se poate dezvolta atunci c\u00e2nd compu\u0219i antimicrobieni sunt utiliza\u021bi pentru a trata o infec\u021bie, \u00eens\u0103 are loc o muta\u021bie- o modificare a uneia dintre genele din structura acestui microorganism.<\/p>\n<p>Aceast\u0103 schimbare duce la apari\u021bia unei popula\u021bii microbiene mixte \u00een organismul persoanei infectate \u2013 o combina\u021bie de microorganisme care sunt rezistente la medicamente \u0219i altele \u00eenc\u0103 sensibile la ac\u021biunea acestora. Microorganismele ce nu au suferit muta\u021bii vor fi anihilate, in timp ce, cele care prezint\u0103 muta\u021bii se vor \u00eenmul\u021bi rapid \u0219i vor deveni predominente. Acest fenomen poart\u0103 numele de \u201epresiune selectiv\u0103\u201d, \u00a0medicamentul fiind cel care declan\u0219eaz\u0103 \u201eselec\u021bia\u201d, permi\u021b\u00e2nd astfel proliferarea formelor genetice rezistente ale microorganismului vizat. Atunci c\u00e2nd se \u00eent\u00e2mpl\u0103 acest fenomen, antimicrobianul utilizat devine ineficient \u00een tratarea infec\u021biei<sup>1,2<\/sup>.<\/p>\n<p>Exist\u0103 24 de medicamente antiretrovirale, grupate \u00een 6 clase autorizate pentru tratamentul infec\u021biei cu virusul imunodeficien\u021bei umane (HIV): 6 inhibitori nucleozidici \u0219i un nucleotidic de revers transcriptaz\u0103 (INRT), 5 inhibitori non-nucleozidici de revers transcriptaz\u0103 (INNTI), 9 inhibitori de proteaz\u0103 (IP), un inhibitor de fuziune, un antagonist al receptorului 5 al chemokinei CC (CCR5) \u0219i un inhibitor al integrazei<sup>3,4<\/sup>.<\/p>\n<p>HIV demonstreaz\u0103 o rat\u0103 mare de apari\u021bie a muta\u021biilor, acumul\u00e2nd aproximativ o muta\u021bie nucleotidic\u0103 per ciclu de replicare. De\u0219i pacien\u021bii sunt de obicei infecta\u021bi fie cu o singur\u0103 clon\u0103, fie cu un numar minim de clone salbatice, se estimeaz\u0103 c\u0103 \u00een cazurile netratate sunt produ\u0219i zilnic un numar de peste 1000 de virioni.<\/p>\n<p>Capacitatea de a genera rapid noi variante, permite HIV s\u0103 evite ac\u021biunile de ap\u0103rare exercitate de c\u0103tre sistemul imunitar, favoriz\u00e2nd astfel dezvoltarea rezisten\u021bei la medicamentele antiretrovirale. Dezvoltarea rezisten\u021bei antivirale este considerat\u0103 esen\u021bial\u0103 pentru a demonstra c\u0103 un compus antiretroviral inhib\u0103 \u00een mod direct HIV, comparativ cu celulele gazd\u0103, \u00een care virusul se reproduce.<\/p>\n<p>Rezisten\u021ba la medicamente poate fi dob\u00e2ndit\u0103 fie prin presiunea de selec\u021bie a medicamentelor (rezisten\u021b\u0103 dob\u00e2ndit\u0103), fie transmis\u0103 de la persoan\u0103 la persoan\u0103 (rezisten\u021b\u0103 transmisibil\u0103)<sup>5<\/sup>. Sunt rare virusurile ce demonstreaz\u0103 o rezistent\u0103 fa\u021b\u0103 de medicamente, \u00een mod natural (deci, care nu au fost supuse presiunii selective ale medicamentului)<sup>5,6<\/sup>.<\/p>\n<p>Dezvoltarea unor combina\u021bii antiretrovirale suficient de puternice pentru a preveni apari\u021bia rezisten\u021bei la medicamente HIV a fost esen\u021bial\u0103 pentru dezvoltarea unei terapii antiretrovirale \u00a0de succes. Cu toate acestea, dob\u00e2ndirea \u0219i transmiterea rezisten\u021bei la medicamente anti-HIV se dovedesc a fi obstacole continue \u00een calea succesului terapiei antiretrovirale. Pacien\u021bii care dob\u00e2ndesc sau sunt infecta\u021bi \u00een principal cu HIV rezistent la medicamente, au mai pu\u021bine op\u021biuni de tratament \u0219i prezint\u0103 un risc crescut de morbiditate \u0219i mortalitate, \u00een special \u00een \u021b\u0103rile \u00een curs de dezvoltare, unde op\u021biunile pentru terapia antiretroviral\u0103 sunt limitate<sup>7<\/sup>.<\/p>\n<p>Testarea genotipic\u0103 a rezisten\u021bei la medicamente antiretrovirale cu HIV evalueaz\u0103 probabilitatea ca tulpina HIV care infecteaz\u0103 un individ s\u0103 fie rezistent\u0103 sau s\u0103 fi dezvoltat rezisten\u021b\u0103 la unul sau mai multe medicamente antiretroviale. Testul analizeaz\u0103 genele tulpinii HIV care infecteaz\u0103 persoana pentru a identifica muta\u021biile care pot determina rezisten\u021ba virusului la terapia antiretroviral\u0103. O persoan\u0103 poate fi infectat\u0103 ini\u021bial cu o tulpin\u0103 HIV rezistent\u0103 la medicamente sau rezisten\u021b\u0103 se poate dezvolta \u00een timpul tratamentului<sup>7,8<\/sup>.<\/p>\n<p>HIV sufer\u0103 muta\u021bii frecvente \u2013 chiar \u0219i \u00een absen\u021ba tratamentului medicamentos \u2013 dar nu orice muta\u021bie provoac\u0103 rezisten\u021b\u0103 la medicamentele antiretrovirale. Prin testarea rezisten\u021bei genotipice, este analizat codul genetic al HIV cu care a fost infectat pacientul, pentru a determina dac\u0103 exist\u0103 muta\u021bii genetice despre care se \u0219tie c\u0103 cauzeaz\u0103 rezisten\u021b\u0103 la terapia antiretroviral\u0103. Pentru a evita dezvoltarea rezisten\u021bei la terapia antiretroviral\u0103, se recomand\u0103 ca pacien\u021bii infecta\u021bi cu \u00a0cu HIV s\u0103 primeasc\u0103 o combina\u021bie de medicamente ce provin din dou\u0103 clase diferite de antiretrovirale.<\/p>\n<p>C\u00e2nd o persoan\u0103 este diagnosticat\u0103 ini\u021bial, testarea rezisten\u021bei la medicamente HIV este utilizat\u0103 pentru a determina dac\u0103 tulpina HIV este deja rezistent\u0103 la unul sau mai multe medicamente. Informa\u021biile sunt folosite pentru a identifica \u0219i selecta medicamentele antiretrovirale care vor fi probabil eficiente<sup>9<\/sup>.<\/p>\n<p>Testarea poate fi folosit\u0103 \u0219i dup\u0103 ce o persoan\u0103 \u00eencepe tratamentul pentru a ajuta la determinarea cauzei e\u0219ecului tratamentului medicamentos. E\u0219ecul tratamentului este identificat atunci c\u00e2nd \u00eenc\u0103rc\u0103tura viral\u0103 a unei persoane nu scade sau \u00eencepe s\u0103 creasc\u0103, \u00een ciuda tratamentului. \u00cen acest caz, tratamentul va fi schimbat.<\/p>\n<p>Rezultatele test\u0103rii rezisten\u021bei antiretrovirale pot ajuta la selectarea unui regim de medicamente diferit care poate fi eficient \u00een suprimarea virusului. Testarea este utilizat\u0103 pentru a identifica rezisten\u021ba la medicamentele antiretrovirale din clase precum: inhibitori de proteaz\u0103, inhibitori de revers transcriptaz\u0103 \u0219i inhibitori de integraz\u0103<sup>10<\/sup>.<\/p>\n<p><strong><em>Recomand\u0103ri pentru determinarea HIV-1 genotipare (rezisten\u021ba la antiretrovirale)<\/em><\/strong><\/p>\n<ul>\n<li>Testarea rezisten\u021bei genotipice HIV este recomandat\u0103 atunci c\u00e2nd o persoan\u0103 este diagnosticat\u0103 cu o infec\u021bie primar\u0103 HIV \u0219i \u00eenainte de a \u00eencepe terapia. Dac\u0103 tratamentul nu este ini\u021biat imediat dup\u0103 diagnostic, atunci trebuie luat\u0103 \u00een considerare repetarea testului de rezisten\u021b\u0103 \u00eenainte de \u00eenceperea terapiei<sup>10<\/sup>.<\/li>\n<li>Testarea poate fi, de asemenea, recomandat\u0103 atunci c\u00e2nd valorile\u00a0<a href=\"https:\/\/www.synevo.ro\/shop\/hiv-1-arn-cantitativ\/\" data-wpel-link=\"internal\" rel=\"nofollow noopener\" target=\"_blank\">\u00eenc\u0103rc\u0103turii virale HIV<\/a>\u00a0nu scad sau cresc constant \u00een timpul terapiei, indic\u00e2nd faptul c\u0103 HIV poate dezvolta rezisten\u021b\u0103. \u00cen general, un tratament este considerat un e\u0219ec dac\u0103 \u00eenc\u0103rc\u0103tura viral\u0103 cre\u0219te de mai mult de trei ori \u00een teste consecutive.
