In ultimile doua decenii numeroase studii au incercat sa stabileasca o asociere intre anumite variatii ale secventelor ADN (polimorfisme genetice) si riscul de boala coronariana. Printre aceste variatii se numara si polimorfismul nucleotidic unic al unei gene care codifica o proteina din familia kinezinelor (\u201ekinesin family member 6\u201d, KIF-6), care are drept rezultat inlocuirea triptofanului cu arginina in pozitia 719 a secventei de aminoacizi (Trp719Arg).<\/p>\n<p>Gena KIF6, localizata pe cromozomul 6p21.2, codifica o kinezina, membra a familiei de proteine motorii implicate in transportul \u201ecargo-ului\u201d (incarcaturii) celular \u2013 organite, complexe proteice, ARNm \u2013 de-a lungul microtubulilor. Kinezinele sunt constituite dintr-un domeniu motor, conservat, cu rol in transport, ATP-dependent si un domeniu neconservat care se leaga de \u201ecargo\u201d, avand o structura \u201ccoil-coiled\u201d ce faciliteaza interactiunile proteice (<em>vezi figura<\/em>). Conform datelor actuale, gena KIF6 nu este exprimata la nivel vascular, ci la un nivel redus in tesutul conjunctiv, colon, ochi, faringe, creier, tegument, testicul, organe si tesuturi care nu sunt implicate in ateroscleroza. Polimorfismul Trp719Arg este localizat la nivelul primului nucleotid al codonului 719 si este definit prin schimbul non-conservativ de aminoacizi, care inlocuieste un rest nepolar cu un rest bazic, afectand situsul de legare al \u201ecargo\u201d -ului. Este cunoscut faptul ca multe kinezine sunt implicate in patogeneza unor afectiuni cronice ca diabetul zaharat de tip 2 si boala\u00a0 Alzheimer.<\/p>\n<p><a href=\"\/ru\/polimorfismul-genei-kif-6-tratament-cu-statine\/kif6\/\" rel=\"attachment wp-att-9007\"><img decoding=\"async\" class=\"alignnone size-full wp-image-9007\" src=\"\/wp-content\/uploads\/2010\/08\/KIF6.jpg\" alt=\"-\" width=\"480\" height=\"361\"><\/a><\/p>\n<p><a href=\"https:\/\/www.synevo.md\/polimorfismul-genei-kif-6-tratament-cu-statine\/kif6\/\" rel=\"attachment wp-att-9007 nofollow noopener\" target=\"_blank\"><img decoding=\"async\" class=\"alignnone size-full wp-image-9007\" src=\"\/wp-content\/uploads\/2010\/08\/KIF6.jpg\" alt=\"-\" width=\"480\" height=\"361\"><\/a><\/p>\n<p><span style=\"font-size: small;\">Structura proteinei KIF6 (Adaptare <em>dupa <\/em><em>Asbury<\/em> <em>et al<\/em>. <em>Science<\/em> <strong>302<\/strong>: 2130, 2003)<\/span><\/p>\n<p>Aceasta varianta a genei KIF6 este prezenta la ~60% din populatia caucaziana; 7 studii prospective (3 studii de cohorta si 4 trialuri randomizate, de tip \u201edublu-orb\u201d cu doua tipuri de statine) care au inclus peste 50 000 persoane au demonstrat o asociere stransa intre varianta genetica Trp719Arg KIF-6 si cresterea cu 55% a riscului de boala coronariana, in mod independent de alti factori de risc cardiovascular; pe de alta parte, acestea au aratat ca terapia intensiva cu statine reduce semnificativ numarul de evenimente coronariene la purtatorii acestei variante dar nu si la non-purtatori<sup>1;2<\/sup>.<\/p>\n<p>Un exemplu de astfel de studiu este WHS (Women Health Study) la care au participat 25 283 femei caucaziene cu varsta \u2265 45 ani, initial sanatoase ce au fost testate in vederea depistarii polimorfismului rs20455 (Trp719Arg) si monitorizate timp de 12 ani pentru evenimente cardiovasculare. In aceasta perioada, un numar de 953 femei au dezvoltat pentru prima data un eveniment coronarian (infarct miocardic, revascularizare coronariana, deces de cauza cardiovasculara) sau accident vascular cerebral. Studiul a aratat ca purtatoarele alelei Arg719 a genei KIF6 prezinta un risc cu 34% mai mare de infarct miocardic si cu 24% de boala coronariana ischemica in general, comparativ cu femeile care nu prezinta aceasta alela. Riscul asociat cu prezenta alelei Arg719 a fost estimat dupa ajustarea in functie de varsta si de existenta factorilor de risc traditionali. Nu a fost demonstrata asocierea variantei genetice KIF6 cu riscul crescut de accident vascular cerebral<sup>6<\/sup>.<\/p>\n<p>Asocierea dintre polimorfismul KIF6 si tratamentul cu statine reprezinta o deschidere importanta catre medicina personalizata. Astfel, aceasta constatare poate fi utila in efortul de a personaliza tratamentul si de a motiva complianta pacientilor purtatori ai alelei Arg719KIF6.<\/p>\n<p>Atat studiul CARE (Cholesterol and Recurrent Events) cat si WOSCOPS (West of Scotland Coronary Prevention Study) au indicat un beneficiu semnificativ al tratamentului cu pravastatin la purtatorii alelei Arg719KIF6, inregistrandu-se o reducere a riscului absolut de 4.89% in studiul CARE si de 5.49% in studiul WOSCOPS. La persoanele care nu prezinta aceasta varianta genetica nu s-a constatat un beneficiu semnificativ statistic al tratamentului cu pravastatin, in sensul reducerii numarului de evenimente coronariene. In plus, au fost analizate 26 polimorfisme genetice ce flancheaza regiunea KIF6, fara a se constata in cele doua studii o asociere cu afectiunile coronariene<sup>3<\/sup>.<\/p>\n<p>Studiul PROSPER (Prospective Study of Pravastatin in Elderly at Risk) \u2013 un studiu randomizat, de tip \u201edublu-orb\u201d, cu control placebo \u2013 a inclus barbati si femei &gt;70 ani cu boala coronariana preexistenta sau cu factori de risc majori si a avut drept scop sa testeze ipoteza potrivit careia tratamentul cu pravastatin ar putea reduce numarul evenimentelor cardiovasculare majore la aceasta categorie de persoane. Statusul de purtator al alelei Arg719KIF6 a fost analizat la 5752 pacienti care au participat la studiul PROSPER; a fost \u00a0constatata o diferenta semnificativa in ceea ce priveste numarul de evenimente cardiovasculare intre purtatorii variantei genetice\u00a0 Trp719Arg si non-purtatori, desi a existat o reducere similara a nivelului de LDL-colesterol in cele doua grupuri; terapia cu pravastatin a redus semnificativ evenimentele cardiovasculare in randul purtatorilor cu boala vasculara preexistenta, fara a se constata un beneficiu terapeutic la non-purtatorii cu boala vasculara preexistenta sau la cei fara boala (purtatori sau non-purtatori)<sup>4<\/sup>.<\/p>\n<p>In cadrul studiului PROVE IT-TIMI 22 (Pravastatin or Atorvastatin Evaluation and Infarction Therapy: Thrombolysis in Myocardial Infarction 22) au fost genotipati KIF6 1778 pacienti cu sindrom coronarian acut si a fost investigat efectul tratamentului cu statine in doze diferite la purtatorii Arg719 comparativ cu non-purtatorii. S-a constatat ca purtatorii Arg719KIF6 au beneficiat mai mult de tratamentul intensiv cu atorvastatin comparat cu tratamentul moderat cu pravastatin decat persoanele fara acest marker genetic (reducerea riscului absolut cu 10% vs 0.8%). Pacientii testati au prezentat reduceri similare de LDL-colesterol, trigliceride si proteina C-reactiva sub tratament, ceea ce indica faptul ca beneficiul se datoreaza unui mecanism independent<sup>5<\/sup>.<\/p>\n<p>Aceste studii trezesc interesul asupra polimorfismului genei KIF6 ca posibil marker genetic al susceptibilitatii pentru infarctul miocardic, ateroscleroza coronariana precum si pentru raspunsul clinic la statine. Cu toate acestea, sunt inca putine date legate de gena KIF6 si de proteina codificata de aceasta care sa sustina plauzabilitatea biologica a asocierii cu riscul de boala coronariana.<\/p>\n<p>Determinarea statusului de purtator al variantei Trp719Arg poate ajuta medicul clinician in evaluarea mai precisa a riscului de afectiuni cardiovasculare si in personalizarea tratamentului cu statine.
