Diferentele individuale in activiatea CYP450 au ca rezultat modificari in metabolizarea medicamentelor, de la absenta totala a metabolizarii pana la degradarea ultrarapida a acestora. Aceste variatii pot determina fie reactii adverse semnificative, fie lipsa raspunsului terapeutic la doze standard de medicament<sup>3;4<\/sup>.<\/p>\n<p>CYP2C19 constituie una din enzimele CYP450 ce este responsabila de metabolizarea a 15% dintre medicamentele administrate, printre care se numara clopidogrel, propafenona, omeprazol, sertralina, citalopram, diazepam<sup>3<\/sup>.<\/p>\n<p>Au fost identificate polimorfisme specifice al genei care codifica enzima CYP2C19 ce induc alterari ale activitatii enzimatice. Frecventa acestor polimorfisme variaza semnificativ in cadrul principalelor grupuri etnice. Persoanele heterozigote, cu o alela normala si una polimorfica vor prezenta un fenotip metabolic intermediar, intre cel de \u201cmetabolizator extensiv\u201d si cel de \u201cmetabolizator lent\u201d.<\/p>\n<p>Din aceste motive, dozajul medicamentelor ce sunt metabolizate de catre enzima CYP2C19 necesita o ajustare individuala pentru a obtine efectul terapeutic scontat, cu un risc minim de reactii adverse. Pacientii care sunt \u201cmetabolizatori lenti\u201d pot beneficia de modificarea dozelor sau de inlocuirea cu medicamente similare ce nu sunt metabolizate la nivelul CYP2C19.<\/p>\n<p><span style=\"text-decoration: underline;\">Principalele clase de medicamente metabolizate de catre CYP2C19<\/span>:<\/p>\n<p style=\"padding-left: 30px;\">\u2022 antiagregante: clopidogrel (Plavix);<\/p>\n<p style=\"padding-left: 30px;\">\u2022 anticonvulsivante: mefenitoin, fenitoin, diazepam, primidona;<\/p>\n<p style=\"padding-left: 30px;\">\u2022 antidepresive: amitriptilina, citalopram, S-citalopram, clomipramina;<\/p>\n<p style=\"padding-left: 30px;\">\u2022 citostatice: ciclofosfamida;<\/p>\n<p style=\"padding-left: 30px;\">\u2022 antiretrovirale: nelfinavir;<\/p>\n<p style=\"padding-left: 30px;\">\u2022 inhibitori de pompa protonica: lansoprazol, pantoprazol, omeprazol;<\/p>\n<p style=\"padding-left: 30px;\">\u2022 altele: progesteron, propranolol, R-warfarina (izomerul mai putin activ), proguanil.<\/p>\n<p>Coadministrarea poate scadea rata de eliminare a altor medicamente metabolizate de catre CYP2C19.<\/p>\n<p><span style=\"text-decoration: underline;\">Medicamente care cresc activittaea CYP2C19<\/span>:<\/p>\n<p style=\"padding-left: 30px;\">\u2022 carbamazepina;<\/p>\n<p style=\"padding-left: 30px;\">\u2022 prednison;<\/p>\n<p style=\"padding-left: 30px;\">\u2022 rifampicina.<\/p>\n<p>Coadministrarea acestor medicamente induce o sinteza crescuta CYP2C19 si mareste astfel rata de eliminare a medicamentelor\u00a0 metabolizate de catre CYP2C19.<\/p>\n<p><span style=\"text-decoration: underline;\">Medicamente care reduc activittaea CYP2C19<\/span>:<\/p>\n<p style=\"padding-left: 30px;\">\u2022 cloramfenicol;<\/p>\n<p style=\"padding-left: 30px;\">\u2022 cimetidina;<\/p>\n<p style=\"padding-left: 30px;\">\u2022 felbamat;<\/p>\n<p style=\"padding-left: 30px;\">\u2022 fluoxetin;<\/p>\n<p style=\"padding-left: 30px;\">\u2022 fluvoxamin;<\/p>\n<p style=\"padding-left: 30px;\">\u2022 indometacin;<\/p>\n<p style=\"padding-left: 30px;\">\u2022 carbamazepina;<\/p>\n<p style=\"padding-left: 30px;\">\u2022 prednison;<\/p>\n<p style=\"padding-left: 30px;\">\u2022 ketoconazol;<\/p>\n<p style=\"padding-left: 30px;\">\u2022 lansoprazol, omeprazol;<\/p>\n<p style=\"padding-left: 30px;\">\u2022 modafinil;<\/p>\n<p style=\"padding-left: 30px;\">\u2022 probenecid;<\/p>\n<p style=\"padding-left: 30px;\">\u2022 ticlopidina;<\/p>\n<p style=\"padding-left: 30px;\">\u2022 topiramat.<\/p>\n<p>Coadministrarea va reduce rata de eliminare a medicamentelor\u00a0 metabolizate de catre CYP2C19 si va creste astfel riscul de reactii adverse, in special la persoanele heterozigote<sup>4<\/sup>.<\/p>\n<p>Clopidogrelul este un antiagregant palchetar larg utilizat in tratamentul pacientilor care au suferit in antecedente un eveniment cardiovascular, cat si pentru profilaxia trombozei in urma implantarii unui stent. Totusi, studiile arata ca acest medicament este mai putin eficient la circa 30% dintre caucazieni, 40% dintre africani si &gt;55% dintre persoanele de origine est-asiatica in asociere cu fenotipul CYP2C19 de \u201cmetabolizator lent\u201d<sup>5<\/sup>.<\/p>\n<p>Clopidogrelul este un promedicament care este metabolizat in compusul sau activ de catre anumite enzime CYP450, dintre care CYP2C19 detine rolul esential<sup>2;4;5<\/sup>. Alelele care determina pierderea sau reducerea severa a activitatii enzimatice (*2, *3, *4, *6, *7, *8) sunt asociate cu o inhibitie plachetara mai redusa si cu un risc crescut de complicatii cardiovasculare, cum ar fi infarctul miocardic, tromboza de stent, accidente vasculare cerebrale si\/sau deces, in comparatie cu persoanele homozigote pentru fenotipul \u201esalbatic\u201d (*1\/*1). Pe de alta parte, persoanele purtatoare ale alelei *17 sunt considerate \u201cmetabolizatori rapizi\u201d si prezinta un risc crescut de sangerari in cursul administrarii de clopidogrel<sup>6<\/sup>.<\/p>\n<p><strong><em>Recomandari pentru genotiparea CYP2C19<\/em><\/strong><\/p>\n<p style=\"padding-left: 30px;\">-identificarea activitatii enzimatice alterate inaintea inceperii tratamentului (preferabil!) cu un medicament metabolizat de catre CYP2C19;<\/p>\n<p style=\"padding-left: 30px;\">-identificarea pacientilor care prezinta rezistenta la tratamentul cu clopidogrel sau, dimpotriva, au un risc mare de sindrom hemoragipar<sup>4<\/sup>.<\/p>\n<p><strong><em>Pregatire pacient<\/em><\/strong> \u2013 nu este necesara o pregatire speciala; pe biletul de trimitere catre laborator va fi precizat medicamentul sau medicamentele pentru care se solicita testarea<sup>1<\/sup>.<\/p>\n<p><strong><em>Specimen recoltat<\/em><\/strong> \u2013 sange venos<sup>1<\/sup>.<\/p>\n<p><strong><em>Recipient de recoltare<\/em><\/strong>\u00a0\u2013 vacutainer ce contine EDTA ca anticoagulant<sup>1<\/sup>.<\/p>\n<p><strong><em>Cantitate recoltata <\/em><\/strong>\u2013 cat permite vacuumul<sup>1<\/sup>.<\/p>\n<p><strong><em>Cauze de respingere<\/em><\/strong> <strong><em>a probei <\/em><\/strong>\u2013 folosirea heparinei ca anticoagulant; probe coagulate sau hemolizate<sup>1<\/sup>.<\/p>\n<p><strong><em>Stabilitate proba<\/em><\/strong> - <em>7 zile<\/em> la 2-8\u00baC; <em>1 an<\/em> la -20 \u00baC<sup>1<\/sup>.<\/p>\n<p><strong><em>Metoda<\/em><\/strong> - <span style=\"text-decoration: underline;\">reactie de polimerizare in lant (PCR) cu detectie colorimetrica dupa hibridizarea cu sonde specifice pe stripuri<\/span><sup>1<\/sup>.<\/p>\n<p><strong><em>Raportarea si interpretarea rezultatelor<\/em><\/strong><\/p>\n<p>Se va comunica genotipul CYP2C19 obtinut si fenotipul de metabolizare asociat (conform tabelului de mai sus)<sup>1<\/sup>.<\/p>\n<p><strong><em>Limite si interferente<\/em><\/strong><\/p>\n<p>Testul detecteaza doar alelele mentionate in tabel; alte alele mai rare asociate cu alterarea activitatii enzimatice nu vor fi identificate<sup>1<\/sup>.<\/p>\n<p>Inaintea de initierea unei terapii noi trebuie tinut cont de faptul ca exista si alti factori care pot influenta metabolismul medicamentelor cum ar fi: dieta, absorbtia digestiva, medicatia asociata si, nu in ultimul rand, complianta inadecvata a pacientului<sup>2<\/sup>.<\/p>\n<p><span style=\"font-size: small;\">\u00a0<\/span><\/p>\n<p><span style=\"font-size: small;\">\u0411\u0438\u0431\u043b\u0438\u043e\u0433\u0440\u0430\u0444\u0438\u044f<\/span><\/p>\n<p><span style=\"font-size: small;\">1.
