Gamapatiile monoclonale cuprind: mielom multiplu (MM), mielom multiplu cu lan\u0163uri u\u015foare (LCMM), macroglobulinemia Waldenstrom (WM), mielom nonsecretant (NSMM), mielom multiplu cu evolu\u0163ie lent\u0103 (SMM), gamapatie monoclonal\u0103 cu semnifica\u0163ie nedeterminat\u0103 (MGUS), amiloidoz\u0103 primar\u0103 sistemic\u0103 (AL) \u015fi boala depozitelor de lan\u0163uri u\u015foare (LCDD).<\/p>\n<p style=\"text-align: justify;\">Imunoglobulinele monoclonale secretate de aceste celule servesc ca marker de proliferare clonal\u0103, iar cuantificarea lor poate fi folosit\u0103 pentru\u00a0monitorizarea bolii<sup>2;3;5:6<\/sup>.<\/p>\n<p style=\"text-align: justify;\">Moleculele de imunoglobuline sunt formate din dou\u0103 lan\u0163uri grele identice (\u03b1, \u03b4, \u03b5, \u03b3 sau \u03bc), care definesc clasa de anticorpi, \u015fi dou\u0103 lan\u0163uri u\u015foare identice (\u03ba sau \u03bb). Fiecare lan\u0163 u\u015for este legat covalent de un lan\u0163 greu, iar cele dou\u0103 lan\u0163uri grele sunt legate covalent \u00eentre ele la nivelul regiunii \u201ebalama\u201d.<\/p>\n<p style=\"text-align: justify;\">La persoanele s\u0103n\u0103toase, majoritatea lan\u0163urilor u\u015foare din ser exist\u0103 sub forma legat\u0103. Cu toate acestea, un nivel plasmatic redus de lan\u0163uri u\u015foare libere poate fi detectat \u00een serul persoanelor normale din cauza supraproduc\u0163iei \u015fi secre\u0163iei de lan\u0163uri u\u015foare de la nivelul plasmocitelor. \u00cen ser, lan\u0163ul u\u015for liber kappa exist\u0103 \u00een principal ca monomer (greutate molecular\u0103 de 22.5 kDa), iar lan\u0163ul u\u015for liber lambda ca dimer legat covalent, cu o greutate molecular\u0103 de aproximativ 45 kDa. Produc\u0163ia unor cantit\u0103\u0163i mari de lan\u0163uri u\u015foare monoclonale poate dep\u0103\u015fi \u00eens\u0103 capacitatea de reabsorb\u0163ie a rinichilor \u015fi acestea pot fi cuantificate cu u\u015furin\u0163\u0103 \u00een urin\u0103. Eviden\u0163ierea lan\u0163urilor u\u015foare monoclonale urinare (proteinurie Bence Jones) a fost folosit\u0103 ca marker de diagnostic pentru mielomul multiplu \u00eenc\u0103 din 1847, cand a fost descris de Dr. H. Bence Jones.<\/p>\n<p style=\"text-align: justify;\">Detectarea gamapatiilor monoclonale sau a discraziilor plasmocitelor implic\u0103, de obicei, electroforeza proteinelor serice \u015fi imunoelectroforeza at\u00e2t \u00een ser c\u00e2t \u015fi\u00a0\u00een urin\u0103. Cu toate acestea, bolile monoclonale cu lan\u0163uri u\u015foare (LCMM, AL, LCDD) \u015fi NSMM nu au, de cele mai multe ori, proteine serice monoclonale\u00a0\u00een concentra\u0163ie suficient de mare pentru a fi detectate \u015fi cuantificate de electroforez\u0103. De aceea, recent a fost descris\u0103 o metod\u0103 nefelometric\u0103 specific\u0103 pentru lan\u0163urile u\u015foare libere, care nu identific\u0103 \u015fi formele legate de lan\u0163urile grele. Acest test este mai sensibil dec\u00e2t electroforeza pentru detectarea lan\u0163urilor u\u015foare libere monoclonale. La unii pacien\u0163i cu NSMM, AL sau LCDD testul ofer\u0103 o identificare pozitiv\u0103 a unui lan\u0163 u\u015for monoclonal\u00a0\u00een ser atunci c\u00e2nd IF este negativ\u0103. In plus, cuantificarea lan\u0163urilor u\u015foare libere a fost corelat\u0103 cu activitatea bolii la pacien\u0163ii cu NSMM \u015fi AL<sup>4;5<\/sup>.<\/p>\n<p style=\"text-align: justify;\">Studii recente au demonstrat c\u0103 determinarea \u00een ser a lan\u0163urilor u\u015foare libere ofer\u0103 un management mai bun al pacien\u0163ilor \u00een compara\u0163ie cu alte teste, astfel:<\/p>\n<ul style=\"text-align: justify;\">\n<li>prezint\u0103 o sensibilitate superioar\u0103 \u00een diagnosticul gamopatiilor monoclonale, \u00een special al bolilor cu lan\u0163uri u\u015foare monoclonale (amiloidoza primar\u0103, LCCD);<\/li>\n<li>ofer\u0103 o evaluare a riscului de progresie a MGUS c\u0103tre MM; \u00eempreun\u0103 cu tipul \u015fi m\u0103rimea proteinei monoclonale M permite stratificarea pacien\u0163ilor \u00een func\u0163ie de riscul sc\u0103zut sau crescut; pacien\u0163ii cu un raport k:\u03bb anormal prezinta un risc de progresie catre mielom de 2.5 ori mai mare;<\/li>\n<li>permite monitorizarea evolu\u0163iei bolii la pacien\u0163ii al c\u0103ror component monoclonal nu poate fi m\u0103surat electroforetic reduc\u00e2nd astfel num\u0103rul procedurilor invazive (biopsii seriate de m\u0103duv\u0103 osoas\u0103);<\/li>\n<li>prezint\u0103 sensibilitate superioar\u0103 \u00een compara\u0163ie cu testele urinare<sup>5<\/sup>.<\/li>\n<\/ul>\n<p style=\"text-align: justify;\"><strong><em>Recomand\u0103ri pentru determinarea lan\u0163urilor u\u015foare libere de tip kappa \u015fi lambda <\/em><\/strong>\u2013 evaluarea gamapatiilor monoclonale; monitorizarea pacien\u0163ilor cu boli ale lan\u0163urilor u\u015foare<sup>2;6<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><strong><em>Preg\u0103tire pacient <\/em><\/strong>\u2013 \u00e0 jeune (pe nem\u00e2ncate) sau postprandial<sup>1<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><strong><em>Specimen recoltat<\/em><\/strong> \u2013 s\u00e2nge venos<sup>1<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><strong><em>Recipient de recoltare<\/em><\/strong> \u2013 vacutainer f\u0103r\u0103 anticoagulant cu\/f\u0103r\u0103 gel separator<sup>1<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><strong><em>Volum prob\u0103<\/em><\/strong> \u2013 2 mL ser<sup>2<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><strong><em>Cauze de respingere a probei <\/em><\/strong>\u2013 ser hemolizat, lipemic<sup>1<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><strong><em>Stabilitatea probei<\/em><\/strong> \u2013 serul separat este stabil\u00a0maxim <em>4 zile<\/em> la temperatura camerei \u015fi <em>2 s\u0103pt\u0103m\u00e2ni<\/em> la 2-8\u00b0C<sup>1<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><strong><em>Metod\u0103<\/em><\/strong> - <span style=\"text-decoration: underline;\">nefelometric\u0103<\/span><sup>1<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><strong><em>Valori de referin\u0163\u0103:<\/em><\/strong><\/p>\n<div style=\"text-align: justify;\">Lan\u0163uri u\u015foare libere tip \u03ba: 3.3-19.4 mg\/L<\/div>\n<div style=\"text-align: justify;\">Lan\u0163uri u\u015foare libere tip \u03bb: 5.7-26.3 mg\/L<\/div>\n<div style=\"text-align: justify;\">Raport \u03ba:\u03bb \u2013 0.26 -1.65<sup>1<\/sup>.<\/div>\n<p style=\"text-align: justify;\"><strong><em>Interpretarea rezultatelor<\/em><\/strong><\/p>\n<div style=\"text-align: justify;\">Dac\u0103 lan\u0163urile u\u015foare \u03ba \u015fi \u03bb \u00een ser \u015fi raportul \u03ba:\u03bb sunt normale, iar electroforeza nu indic\u0103 component monoclonal, diagnosticul de gamapatie monoclonal\u0103 este exclus cu mare probabilitate.<\/div>\n<div style=\"text-align: justify;\">Modific\u0103ri ale raportului k:\u03bb\u00a0 sus\u0163in diagnosticul de gamapatie monoclonal\u0103, dar este nevoie de o biopsie tisular\u0103 pentru confirmarea diagnosticului.<\/div>\n<div style=\"text-align: justify;\">Valori crescute aflate la limita superioar\u0103 a intervalului de referin\u0163\u0103 pot indica o insuficien\u0163\u0103 renal\u0103 \u015fi pot necesita teste corespunz\u0103toare func\u0163iei renale.<\/div>\n<div style=\"text-align: justify;\">Concentra\u0163ii sc\u0103zute de lan\u0163uri u\u015foare de tip \u03ba, \u03bb sau ambele indic\u0103 deprecierea func\u0163iei m\u0103duvei osoase.<\/div>\n<p style=\"text-align: justify;\">Valori crescute ale nivelului de lan\u0163uri u\u015foare de tip \u03ba sau \u03bb, cu raport k:\u03bb normal pot fi din cauzate de:<\/p>\n<ul style=\"text-align: justify;\">\n<li>insuficien\u0163\u0103 renal\u0103;<\/li>\n<li>supraproduc\u0163ie de lan\u0163uri u\u015foare policlonale\u00a0\u00een condi\u0163ii inflamatorii;<\/li>\n<li>gamapatii de diferite tipuri.<\/li>\n<\/ul>\n<div style=\"text-align: justify;\">Concentra\u0163ii serice mari at\u00e2t de lan\u0163uri u\u015foare de tip \u03ba c\u00e2t \u015fi \u03bb cu un raport \u03ba:\u03bb anormal sunt determinate de o combina\u0163ie de gamapatie monoclonal\u0103 \u015fi insuficien\u0163\u0103 renal\u0103<sup>2<\/sup>.<\/div>\n<div style=\"text-align: justify;\">Evaluarea cantitativ\u0103 a lan\u0163urilor u\u015foare monoclonale este util\u0103 pentru monitorizarea evolu\u0163iei bolii. Modific\u0103ri \u00een cuantificarea lan\u0163urilor u\u015foare libere reflect\u0103 schimb\u0103ri \u00een m\u0103rimea popula\u0163iei monoclonale de plasmocite. Orice modificare mai mare de 25% sau modific\u0103ri \u00een aceea\u015fi direc\u0163ie la test\u0103ri multiple sunt semnificative pentru evolu\u0163ia bolii.<\/div>\n<div style=\"text-align: justify;\">Prezen\u0163a unei proteine monoclonale \u00een ser sau urin\u0103 sugereaz\u0103 de obicei un proces malign, de\u015fi componente monoclonale pot s\u0103 apar\u0103 \u015fi\u00a0\u00een diverse condi\u0163ii benigne sau chiar\u00a0\u00een absen\u0163a unei modific\u0103ri patologice (gamapatie monoclonal\u0103 cu semnifica\u0163ie nedeterminat\u0103, care necesit\u0103 monitorizare periodic\u0103 pentru a verifica stabilitatea componentului monoclonal)<sup>2;6<\/sup>.<\/div>\n<div style=\"text-align: justify;\">\u00cen mielomul multiplu, 99% din pacien\u0163i prezint\u0103 o protein\u0103 monoclonal\u0103 \u00een ser sau urin\u0103.<\/div>\n<div style=\"text-align: justify;\">Boala Waldenstr\u00f6m se caracterizeaz\u0103 prin prezen\u0163a obligatorie \u00een ser a unei proteine monoclonale de tip IgM.<\/div>\n<div style=\"text-align: justify;\">La aproximativ 75% din pacien\u0163ii cu mielom multiplu sau boala Waldenstr\u00f6m se depisteaz\u0103 \u00een urin\u0103 un lan\u0163 u\u015for\u00a0monoclonal (proteina Bence-Jones).<\/div>\n<div style=\"text-align: justify;\">\u00cen urina pacien\u0163ilor cu mielom multiplu sau amiloidoz\u0103 se pot g\u0103si at\u00e2t lan\u0163uri u\u015foare libere c\u00e2t \u015fi fragmente de lan\u0163uri grele.<\/div>\n<p style=\"text-align: justify;\">Un nivel ridicat de lan\u0163uri u\u015foare libere policlonale se
