Func\u0163ia biologic\u0103 a colagenului fibrilar este de a furniza rezisten\u0163a la \u00eentindere a osului. Aceast\u0103 rezisten\u0163\u0103 este conferit\u0103 de o serie de leg\u0103turi covalente \u00eencruci\u015fate (\u201ecollagen crosslinks\u201d) care se dezvolt\u0103 \u00eentre moleculele individuale de colagen dintr-o fibr\u0103.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-size: 12pt;\">Molecula de colagen de tip I cu structur\u0103 triplu helicoidal\u0103 prezint\u0103 dou\u0103 situsuri de ata\u015fare a leg\u0103turilor \u00eencruci\u015fate (denumite <em>telopeptide<\/em>), unul \u00een regiunea amino (N) \u2013 terminal\u0103 \u015fi cel\u0103lalt \u00een regiunea carboxi (C) \u2013 terminal\u0103. Leg\u0103turile se realizeaz\u0103 efectiv prin intermediul unor polimeri \u00eencruci\u015fa\u0163i forma\u0163i, fie din trei reziduuri de hidroxilizin\u0103 (<em>piridinolina PYD<\/em>), fie din dou\u0103 reziduuri de hidroxilizin\u0103 \u015fi unul de lizin\u0103 (<em>deoxipiridinolina DPYD<\/em>). DPYD este prezent aproape \u00een \u00eentregime la nivelul osului (<em>vezi figura 14.4.1<\/em>).<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-size: 12pt;\">Infrastructura de colagen a osului sufer\u0103 un proces continuu de remodelare care implic\u0103, at\u00e2t un proces de resorb\u0163ie osoas\u0103 mediat\u0103 de osteoclaste c\u00e2t \u015fi un proces de formare osoas\u0103 de care sunt responsabile osteoblastele.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-size: 12pt;\">\u00cen cursul procesului de resorb\u0163ie osoas\u0103 doar 40% dintre leg\u0103turile \u00eencruci\u015fate sunt eliberate sub form\u0103 de compu\u015fi de piridinium (PYD si DPYD), restul de 60% fiind ata\u015fate de telopeptidele N- \u015fi C-terminale. Din circula\u0163ie telopeptidele NTx \u015fi CTx sunt excretate\u00a0\u00een urin\u0103<sup>1;4;5<\/sup>.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-size: 12pt;\">Secven\u0163ele de aminoacizi \u015fi orientarea \u03b1 2N-telopeptidului fac ca acest produs de degradare al colagenului uman de tip I s\u0103 constituie un marker specific pentru resorb\u0163ia osoas\u0103. Astfel nivelurile urinare de NTx se coreleaz\u0103 cu rata resorb\u0163iei osoase.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-size: 12pt;\">\u00a0\u00a0<a href=\"\/ru\/n-telopeptidul-urinar\/ntx\/\" rel=\"attachment wp-att-9166\"><img decoding=\"async\" class=\"alignnone size-full wp-image-9166\" src=\"\/wp-content\/uploads\/2010\/05\/NTX.jpg\" alt=\"-\" width=\"399\" height=\"239\">\u00a0\u00a0<\/a><a href=\"https:\/\/www.synevo.md\/n-telopeptidul-urinar\/ntx\/\" rel=\"attachment wp-att-9166 nofollow noopener\" target=\"_blank\"><img decoding=\"async\" class=\"alignnone size-full wp-image-9166\" src=\"\/wp-content\/uploads\/2010\/05\/NTX.jpg\" alt=\"-\" width=\"399\" height=\"239\"><\/a><\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-size: 12pt;\">Fig. 14.4.1 Leg\u0103turile \u00eencruci\u015fate DPYD\u00a0\u00een colagenul osului <em>(Adaptare dup\u0103 emedicine.medscape.com)<sup>5<\/sup><\/em><\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-size: 12pt;\">\u00cen mod fiziologic, valori ridicate ale markerilor de resorb\u0163ie apar \u00een copil\u0103rie, \u00een perioda de cre\u015ftere osoas\u0103, precum \u015fi \u00een faza de vindecare a fracturilor. In aceste situa\u0163ii \u00eens\u0103, cre\u015fterile markerilor de resorb\u0163ie \u015fi cele ale markerilor de formare osoas\u0103 sunt echilibrate \u015fi nu au valoare diagnostic\u0103. In schimb, multe afec\u0163iuni cum ar fi hipertiroidia, toate cazurile de hiperparatiroidism, majoritatea formelor de osteomalacie \u015fi rahitism (chiar dac\u0103 nu sunt asociate de hiperparatiroidism), hipercalcemia indus\u0103 de afec\u0163iuni maligne, boala Paget, mielomul multiplu, metastazele osoase, precum \u015fi diverse boli congenitale ale \u0163esutului osos pot determina un turn-over osos accelerat \u015fi dezechilibrat. Majoritatea pacien\u0163ilor cu osteopenie sau osteoporoz\u0103 au un turn-over osos redus, dar dezechilibrat, cu predominanta resorb\u0163iei osoase. Acest fapt poate s\u0103 conduc\u0103 la cre\u015fteri u\u015foare ale NTX urinar; cre\u015fterile mari nu sunt caracteristice acestor condi\u0163ii, de aceea dac\u0103 se ob\u0163in valori &gt;1.5-2 ori dec\u00e2t limita superioar\u0103 a valorilor de referin\u0163\u0103 trebuie s\u0103 fie suspectat\u0103 o osteomalacie asociat\u0103 sau un diagnostic alternativ. Dac\u0103 sunt excluse alte condi\u0163ii \u00eenso\u0163ite de turn-over osos crescut la un pacient cu osteopenie\/osteoporoz\u0103 se va considera c\u0103 acesta are un risc crescut de progresie accelerat\u0103 a bolii.<\/span><\/p>\n<div style=\"text-align: justify;\"><span style=\"font-size: 12pt;\">Femeile cu menopauz\u0103 precoce \u015fi niveluri ale NTx &gt; 67 nmol\/mmol creatinina beneficiaz\u0103 cel mai mult la terapia hormonal\u0103 de substitu\u0163ie.<\/span><\/div>\n<div style=\"text-align: justify;\"><span style=\"font-size: 12pt;\">O reducere cu \u2265 50% a nivelului NTx urinar fa\u0163\u0103 de nivelul bazal la 3-6 luni de la ini\u0163ierea terapiei antiresorbtive indic\u0103, cu probabilitate mare, un r\u0103spuns adecvat.<\/span><\/div>\n<div style=\"text-align: justify;\"><span style=\"font-size: 12pt;\">Pacien\u0163ii cu afec\u0163iuni tratabile, cum ar fi hipertiroidia, vor prezenta o normalizare a valorilor Ntx urinar \u00een decurs de 3-6 luni de la vindecarea complet\u0103<sup>3<\/sup>.<\/span><\/div>\n<p style=\"text-align: justify;\"><span style=\"font-size: 12pt;\"><strong><em>Limite \u015fi interferen\u0163e<\/em><\/strong><\/span><\/p>\n<div style=\"text-align: justify;\"><span style=\"font-size: 12pt;\">Deoarece exist\u0103 fluctua\u0163ii diurne ale valorilor NTX se recomand\u0103 efectuarea testului din urina recoltat\u0103 \u00een 24 ore.
