De\u015fi sunt cunoscute peste 200 specii de Aspergillus, mai pu\u0163in de 20 sunt patogene pentru om. Principalul factor de risc este neutropenia; urm\u0103toarele afec\u0163iuni sunt asociate cu risc crescut de infec\u0163ii severe:<\/p>\n<ul>\n<li>leucemie acut\u0103;<\/li>\n<li>tumori solide;<\/li>\n<li>transplant de ogane;<\/li>\n<li>transplant medular;<\/li>\n<li>imunodeficien\u0163e primare;<\/li>\n<li>afec\u0163iuni pulmonare cronice: tuberculoz\u0103, mucoviscidoz\u0103, bron\u015fit\u0103 cronic\u0103.<\/li>\n<\/ul>\n<div style=\"text-align: justify;\">Majoritatea cazurilor de aspergiloza invaziv\u0103 sunt sporadice, adesea fiind dificil de stabilit dac\u0103 infec\u0163iile au fost dob\u00e2ndite sau nu nozocomial. Este posibil ca unii pacien\u0163i s\u0103 fie coloniza\u0163i cu Aspergillus \u00eenainte de internarea \u00een spital \u015fi s\u0103 dezvolte boala invaziv\u0103 \u00een momentul instal\u0103rii neutropeniei.<\/div>\n<div style=\"text-align: justify;\">Rata mortalit\u0103\u0163ii asociate cu aspergiloza invaziv\u0103 este foarte ridicat\u0103: \u00eentre 50 \u015fi 100% pentru toate categoriile de pacien\u0163i imunodeprima\u0163i. Totu\u015fi, dac\u0103 diagnosticul este stabilit precoce, un num\u0103r semnificativ de pacien\u0163i pot fi vindeca\u0163i la ora actual\u0103<sup>2<\/sup>.<\/div>\n<p style=\"text-align: justify;\">Sunt descrise mai multe forme clinice de aspergilloza:<\/p>\n<ul style=\"text-align: justify;\">\n<li><strong>Aspergiloza pulmonar\u0103 invaziv\u0103 acut\u0103<\/strong> \u2013 poate fi focal\u0103 sau difuz\u0103; diseminarea hematogen\u0103 la distan\u0163\u0103 este o complica\u0163ie frecvent\u0103 a pacien\u0163ilor neutropenici sau cu transplant; angioinvazia reprezint\u0103 tr\u0103s\u0103tura caracteristic\u0103 a infec\u0163iei cu Aspergillus care duce la tromboz\u0103, infarct \u015fi necroz\u0103 a \u0163esutului pulmonar; debutul este adesea cu febr\u0103 \u00eenalt\u0103, care nu r\u0103spunde la tratamentul antibiotic cu spectru larg, f\u0103r\u0103 simptome respiratorii<sup>2;4<\/sup>.<\/li>\n<li><strong>Traheobron\u015fita \u015fi aspergiloza bron\u015fic\u0103 obstructiv\u0103<\/strong> \u2013 traheobron\u015fita apare adesea la persoanele cu SIDA sau cu transplant pulmonar; se caracterizeaz\u0103 predominant prin dispnee \u015fi wheezing; unii pacien\u0163i sufer\u0103 deces prin obstruc\u0163ia traheei sau a unei bronhii, \u00een timp ce al\u0163ii dezvolt\u0103 infec\u0163ie diseminat\u0103; aspergiloza bron\u015fic\u0103 obstructiv\u0103 este o condi\u0163ie non-invaziv\u0103 descris\u0103 la pacien\u0163ii cu SIDA care \u00een absen\u0163a tratamentului poate evolua c\u0103tre traheobron\u015fit\u0103.<\/li>\n<li><strong>Sinuzita acut\u0103 invaziv\u0103<\/strong> \u2013 boala rapid progresiv\u0103 a pacien\u0163ilor neutropenici \u015fi a celor cu transplant; \u00een 25% din cazuri infec\u0163ia se poate propaga la nivel cerebral fiind letal\u0103;<\/li>\n<li><strong>Aspergiloza cerebral\u0103<\/strong> \u2013 apare mai frecvent ca urmare a disemin\u0103rii hematogene de la focarele pulmonare; la pacien\u0163ii cu transplant de celule stem reprezint\u0103 o cauz\u0103 obi\u015fnuit\u0103 de abces cerebral; pot s\u0103 apar\u0103 semne neurologice de focar datorit\u0103 trombozei arterelor cerebrale cu zone multiple de infarct.<\/li>\n<li><strong>Aspergiloza cutanat\u0103<\/strong> \u2013 \u00een forma primar\u0103 apar leziuni la nivelul inser\u0163iei cateterului intravenos; forma secundar\u0103 rezult\u0103 ca urmare a disemin\u0103rii hematogene \u015fi se caracterizeaz\u0103 prin leziuni care evolueaz\u0103 c\u0103tre ulcere necrotice.<\/li>\n<li><strong>Aspergilomul pulmonar<\/strong> \u2013 masa amorf\u0103 dens\u0103 de micelii care se g\u0103se\u015fte uneori \u00een cavit\u0103\u0163ile pulmonare reziduale (dup\u0103 tuberculoz\u0103, sarcoidioz\u0103, bronsiectazii, pneumoconioz\u0103 sau spondilit\u0103 ankilopoietic\u0103); singura complica\u0163ie grav\u0103 este hemoptizia.<\/li>\n<li><strong>Aspergiloza pulmonar\u0103 cronic\u0103 necrotizant\u0103 <\/strong>\u2013 condi\u0163ie indolent\u0103 descris\u0103 la pacien\u0163ii de v\u00e2rst\u0103 mijlocie sau \u00eenaintat\u0103 cu afec\u0163iuni pulmonare subdiacente; se caracterizeaz\u0103 clinic prin tuse cronic\u0103 productiv\u0103, hemoptizie, pierdere ponderal\u0103; aproximativ 50% dintre pacien\u0163i dezvolt\u0103 unul sau mai multe aspergiloame \u00een interiorul cavit\u0103\u0163ilor pulmonare necrotice; \u00een absen\u0163a tratamentului evolu\u0163ia este c\u0103tre fibroz\u0103 pulmonar\u0103.<\/li>\n<li><strong>Sinuzita cronic\u0103 invaziv\u0103<\/strong> \u2013 condi\u0163ie lent progresiv\u0103 asem\u0103n\u0103toare aspergilozei pulmonare cronice necrotizante; poate s\u0103 apar\u0103 \u015fi la persoane imunocompetente, dar cel mai adesea este \u00eent\u00e2lnit\u0103 la pacien\u0163ii care au primit corticoterapie sau au diabet zaharat; clinic se caracterizeaz\u0103 prin polipoz\u0103 nazal\u0103 \u015fi secre\u0163ie muco-purulent\u0103; \u00een absen\u0163a tratamentului se poate propaga la orbit\u0103 \u015fi conduce la tulbur\u0103ri de vedere.<\/li>\n<li><strong>Aspergilom al sinusurilor paranazale <\/strong>\u2013 masa dens\u0103 de micelii care este \u00eent\u00e2lnit\u0103 uneori la pacien\u0163ii investiga\u0163i pentru sinuzit\u0103 cronic\u0103, obstruc\u0163ie nazal\u0103, durere facial\u0103 sau alte condi\u0163ii<sup>2<\/sup>.<\/li>\n<li><strong>Infec\u0163ia auricular\u0103 <\/strong>\u2013 otomicoza este o infec\u0163ie a urechii externe caracterizat\u0103 prin dureri locale, hipoacuzie \u015fi o secre\u0163ie \u00eenso\u0163it\u0103 de o excrescen\u0163\u0103 verde sau neagr\u0103 \u00een conductul auditiv; rareori infec\u0163ia se propag\u0103 dincolo de urechea extern\u0103, cu excep\u0163ia celei survenite la imunodeprima\u0163i<sup>4<\/sup>.<\/li>\n<li><strong>Endocardita <\/strong>\u2013 apare mai ales la persoanele care au suferit interven\u0163ii pe cord deschis sau ca o complica\u0163ie la utilizatorii de droguri intravenoase; principala caracteristic\u0103 o constituie vegeta\u0163iile mari friabile.<\/li>\n<li><strong>Osteomielita <\/strong>\u2013 este o condi\u0163ie rar\u0103 survenit\u0103 mai ales la copiii cu boal\u0103 granulomatoas\u0103 cronic\u0103; sunt afectate coastele \u015fi coloana vertebral\u0103 datorit\u0103 propag\u0103rii infec\u0163iei de la o leziune pulmonar\u0103 adiacent\u0103.<\/li>\n<li><strong>Endoftalmita <\/strong>\u2013 este o condi\u0163ie rar\u0103 descris\u0103 la consumatorii de droguri intravenoase, pacien\u0163i cu endocardit\u0103 sau cu transplant de organe; clinic pacien\u0163ii prezint\u0103 dureri oculare \u015fi tulbur\u0103ri de vedere.<\/li>\n<\/ul>\n<p style=\"text-align: justify;\">Bolile induse de Aspergillus pot rezulta \u015fi ca urmare a unor mecanisme neinfec\u0163ioase: inhalarea de spori poate cauza simptome alergice at\u00e2t la persoanele atopice c\u00e2t \u015fi la cele non-atopice<sup>2<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><strong>Aspergiloza bronho-pulmonar\u0103 alergic\u0103 (ABPA)<\/strong> este o afec\u0163iune inflamatorie a pl\u0103m\u00e2nului \u015fi c\u0103ilor respiratorii care apare predominat la pacien\u0163ii cu astm bronsic \u015fi fibroz\u0103 chistic\u0103. Procesul inflamator este rezultatul unui r\u0103spuns imun fa\u0163\u0103 de colonizarea c\u0103ilor respiratorii cu Aspergillus \u015fi a unui <em>clearance<\/em> sc\u0103zut al secre\u0163iilor mucoase. In consecin\u0163\u0103, se dezvolt\u0103 obstruc\u0163ie bron\u015fic\u0103 recurent\u0103, bronsiectazii consecutive, fibroz\u0103 pulmonar\u0103 \u015fi \u00een final compromiterea func\u0163iei pulmonare.<\/p>\n<p style=\"text-align: justify;\">Se estimeaz\u0103 c\u0103 ABPA apare la 1-2% dintre pacien\u0163ii cu astm \u015fi 1-15% dintre cei cu fibroz\u0103 chistic\u0103. \u00cen timp ce boala poate s\u0103 debuteze la orice v\u00e2rst\u0103 tabloul clinic se instaleaz\u0103 \u00een principal la persoanele atopice care au fost diagnosticate cu astm bron\u015fic \u00een urm\u0103 cu 5-10 ani. Exist\u0103 \u015fi o susceptibilitate imunogenetic\u0103 la subiec\u0163ii care exprim\u0103 HLA-DR2 sau HLA-DR5.<\/p>\n<p style=\"text-align: justify;\"><em>Aspergillus fumigatus<\/em> (Af) este patogenul cel mai frecvent implicat \u00een procesul inflamator din ABPA; se produce de fapt o colonizare datorit\u0103 clearance-ului sc\u0103zut al mucusului \u015fi nu o invazie local\u0103. \u00cen cadrul acestui proces se elibereaz\u0103 peste 20 antigene care vor ini\u0163ia r\u0103spunsul celular \u015fi umoral. Limfocitele B \u015fi alte celule ale persoanelor susceptibile pot fi mai sensibile la ac\u0163iunea IL-4 datorit\u0103 polimorfismului lan\u0163ului \u03b1 al receptorului pentru IL-4; prin urmare rezult\u0103 o produc\u0163ie crescut\u0103 de IgE. \u00cen plus, exist\u0103 o produc\u0163ie crescut\u0103 de anticorpi IgA \u015fi IgG fa\u0163\u0103 de antigenele Af, \u00een special <em>Af f2<\/em>, <em>Af f3<\/em> \u0438 <em>Af f6<\/em>. \u00cen concluzie, exist\u0103 2 c\u0103i ale inflama\u0163iei \u00een ABPA:<\/p>\n<ul>\n<li>activarea de c\u0103tre alergene a celulelor Th2, secre\u0163ie de citokine asociate cu Th2, recrutarea de eozinofile \u015fi inflama\u0163ia eozinofilic\u0103 cu eliberarea produ\u015filor din granulele eozinofilelor;<\/li>\n<li>activarea de c\u0103tre proteazele Af a macrofagelor \u015fi celulelor epiteliale din c\u0103ile respiratorii cu produc\u0163ia de IL-8, recrutarea de neutrofile \u015fi inflama\u0163ia neutrofilic\u0103 cu eliberarea produ\u015filor din granulele neutrofilelor: elastaza \u015fi metaloproteinazele matricei.<\/li>\n<\/ul>\n<p style=\"text-align: justify;\">Astfel, modific\u0103rile patologice din ABPA includ: bronsiectazie central\u0103, granulomatoz\u0103 bonhocentric\u0103, bron\u015fiolit\u0103 \u015fi pneumonie eozinofilic\u0103.<\/p>\n<p style=\"text-align: justify;\">La astmatici, tabloul clinic al ABPA const\u0103 \u00een exacerbarea simptomelor respiratorii (wheezing, tuse, sput\u0103 cu dopuri de mucus de culoare maronie) \u015fi sistemice (febr\u0103, stare alterat\u0103). Diagnosticul se bazeaz\u0103 pe:<\/p>\n<ul>\n<li>eozinofilie periferic\u0103 &gt; 1000\/mm<sup>3<\/sup>;<\/li>\n<li>teste cutanate pozitive pentru antigenele Af;<\/li>\n<li>niveluri serice IgE totale crescute (&gt; 400 UI\/mL);<\/li>\n<li>niveluri serice crescute de IgE specifice \u015fi IgG specifice fa\u0163\u0103 de antigenele Af;<\/li>\n<li>bronsiectazie central\u0103.<\/li>\n<\/ul>\n<p style=\"text-align: justify;\">La pacien\u0163ii cu fibroz\u0103 chistic\u0103 diagnosticul ABPA este mai dificil \u015fi include:<\/p>\n<ul>\n<li>deteriorare clinic\u0103 acut\u0103 sau subacut\u0103, \u00een absen\u0163a oric\u0103rei alte etiologii;<\/li>\n<li>niveluri serice IgE totale crescute (&gt;500 UI\/mL);<\/li>\n<li>teste cutanate pozitive sau IgE specifice fa\u0163\u0103 de Af;<\/li>\n<li>anticorpi IgG fa\u0163\u0103 de Af sau modific\u0103ri recente ale radiografiei pulmonare, cum ar fi infiltrate care nu dispar sub tratament<sup>3<\/sup>.<\/li>\n<\/ul>\n<p style=\"text-align: justify;\">Diagnosticul aspergilozei\u00a0necesit\u0103 coroborarea datelor clinice, radiologice, microbiologice \u015fi histopatologice. Este descris\u0103 o corela\u0163ie slab\u0103 \u00eentre rezultatele culturii \u015fi boala invaziv\u0103, microorganismul fiind izolat frecvent din secre\u0163iile pacien\u0163ilor care nu au infec\u0163ii invazive. Pe de alt\u0103 parte, datorit\u0103 r\u0103sp\u00e2ndirii ubicuitare \u00een mediu, Aspergillus este un contaminant de laborator uzual. \u00cen acest context au fost dezvoltate \u015fi alte metode de diagnostic care nu se bazeaz\u0103 pe cultur\u0103. Astfel, recent a fost aprobat de c\u0103tre FDA in SUA un test imunoenzimatic de depistare a antigenului galactomannan (o molecul\u0103 ce se g\u0103se\u015fte \u00een peretele celular al speciilor de Aspergillus) pentru diagnosticul rapid al aspergilozei invazive. Sunt semnificative clinic titrurile ini\u0163iale &gt;1\/320 sau cre\u015fterile \u00een dinamic\u0103 ale titrurilor de cel pu\u0163in 3-4 ori<sup>1<\/sup>. Dup\u0103 un tratament eficient se \u00eenregistreaz\u0103 o sc\u0103dere a titrurilor de anticorpi<sup>1<\/sup>.<\/div>\n<p style=\"text-align: justify;\"><strong><em>Limite \u015fi interferen\u0163e <\/em><\/strong><\/p>\n<p style=\"text-align: justify;\">Pentru antigenul Aspergillus este raportat\u0103 o rat\u0103 de rezultate fals-pozitive de 7-14%. Numeroase alimente (paste, orez) con\u0163in galactomannan; se crede c\u0103 modific\u0103rile peretului intestinal induse de chimioterapie, iradiere sau de reac\u0163ia gref\u0103-contra gazdei (GVHD) faciliteaz\u0103 transferul antigenului din lumenul intestinal \u00een s\u00e2nge, fiind responsabil par\u0163ial de rezultatele fals-pozitive ob\u0163inute la acest test. Antibioticele semisintetice cum ar fi piperacilina, amoxicilina \u015fi augmentinul, care se bazeaz\u0103 pe compu\u015fi naturali deriva\u0163i din genul Penicillium, pot da reac\u0163ii \u00eencruci\u015fate cu anticorpii monoclonali inclu\u015fi \u00een test. Probele care con\u0163in antigen Hitoplasma pot da de asemenea reac\u0163ii \u00eencruci\u015fate. Pe de alt\u0103 parte, un rezultat negativ nu exclude o aspergiloz\u0103 invaziv\u0103.<\/p>\n<p style=\"text-align: justify;\">Pacien\u0163ii cu aspergiloz\u0103 sistemic\u0103 pot avea adesea serologie negativ\u0103, de aceea la ace\u015ftia sunt necesare alte metode de diagnostic.
