{"id":10249,"date":"2020-04-23T05:00:52","date_gmt":"2020-04-23T02:00:52","guid":{"rendered":"https:\/\/www.synevo.ro\/product\/borrelia-igg-igm-in-lcr-ser\/"},"modified":"2025-02-06T11:17:58","modified_gmt":"2025-02-06T09:17:58","slug":"borrelia-igg-igm-in-lcr-ser","status":"publish","type":"product","link":"https:\/\/old.synevo.md\/ru\/shop\/borrelia-igg-igm-in-lcr-ser\/","title":{"rendered":"\u0411\u043e\u0440\u0440\u0435\u043b\u0438\u0438 IgG, IgM \u0432 \u0441\u043f\u0438\u043d\u043d\u043e\u043c\u043e\u0437\u0433\u043e\u0432\u043e\u0439 \u0436\u0438\u0434\u043a\u043e\u0441\u0442\u0438\/\u0441\u044b\u0432\u043e\u0440\u043e\u0442\u043a\u0435"},"content":{"rendered":"<p><strong><em>\u041e\u0431\u0449\u0430\u044f \u0438\u043d\u0444\u043e\u0440\u043c\u0430\u0446\u0438\u044f<\/em><\/strong><\/p>\n<p>Borrelia burgdorferi (B.b.) face parte din ordinul Spirochaetales, familia Spirochaetaceae, genul Borrelia si a fost izolata in 1982 din intestinul capusei Ixodes dammini de catre Dr. Willy Burgdorfer, al carui nume il poarta<sup>2<\/sup>.<\/p>\n<p>Studiul diferentelor antigenice dintre tulpinile izolate in America si Europa a permis recunoasterea implicarii in patologia umana a trei genospecii care alcatuiesc complexul Borrelia burgdorferi sensu lato: <em>Borrelia burgdorferi sensu stricto<\/em> (ce cuprinde toate tulpinile izolate in America de Nord), <em>B. garinii<\/em> si <em>B. afzelii<\/em>. In Europa circula toate cele trei genospecii, dar predomina B. garinii si B. afzelii<sup>1;2;5;9;10<\/sup>.<\/p>\n<p>Boala Lyme\u00a0este o\u00a0afectiune multisistemica aparuta dupa muscatura de capusa infectata cu Borrelia burgdorferi.<\/p>\n<p>Manifestarile neurologice pot aparea din primul stadiu al bolii. Aproximativ 15% dintre pacientii netratati dezvolta in primele 3 luni <em>meningita acuta aseptica<\/em>. Simptomele apar, de obicei, la 2-10 saptamani dupa infectare si se caracterizeaza prin: cefalee, redoare de ceafa sau fotofobie. Afectarea meningiana poate fi insotita de radiculoneuropatii craniane sau periferice, perechea VII de nervi cranieni fiind cea mai afectata. Paralizia faciala bilaterala poate fi intalnita la 35% din pacienti. Neuropatia craniana mai poate afecta perechile: II, III, IV, V, VI, VIII. Cand este afectat nervul optic pacientii prezinta edem papilar si reflex Argyll-Roberson abolit.<\/p>\n<p>In stadiul 2 al bolii apar manifestari ale <em>sindromului Bannwarth<\/em> (meningopolinevrita de capusa, meningoradiculita limfocitara sau meningita limfocitara cronica), ce includ nevrita craniana fara cefalee si pleiocitoza limfocitara.<\/p>\n<p>Stadiul 3 al bolii se caracterizeaza prin: encefalopatie, neuropatie senzitiva periferica si encefalomielita.<\/p>\n<p><em>Encefalopatia borreliozica<\/em> este cea mai frecventa forma si se manifesta ca o stare confuzionala usoara insotita de tulburari de memorie, de concentrare, de somn, de personalitate sau de vorbire, ce apare la o distanta de luni sau ani de la infectare. Depresia si iritabilitatea sunt de asemenea manifestari comune.<\/p>\n<p><em>Encefalomielita<\/em> indusa de\u00a0 Borrelia este o manifestare rara, dar severa si lent progresiva, care apare in cadrul formei diseminate de boala. Simptomele pot progresa treptat sau intr-un model recurenta \u2013 remisie, cu o ameliorare partiala dupa atacuri. Manifestarile clinice cele mai comune sunt hemipareza, ataxia, convulsiile, tulburarile cognitive, disfunctiile vezicii urinare si pierderea auzului. Mielita este prezenta la 50% dintre pacientii. Parapareza sau tetrapareza spastica progresiva sunt forme comune de boala in acest stadiu.<\/p>\n<p>Pacientii cu <em>neuropatie periferica<\/em> raporteaza de obicei parestezii intermitente, iar cea mai frecventa constatare in urma examinarii este scaderea sensibilitatii vibratorii a extremitatilor.<\/p>\n<p>Manifestarile psihice sunt controversate si includ depresie, anxietate, psihoze\u00a0 (schizofrenie, tulburare bipolara si dementa)<sup>3;8<\/sup>.<\/p>\n<p>Sinteza intratecala de anticorpi anti-Borrelia IgG si IgM este de obicei observata la 3-6 saptamani de la infectie, la 60-90% dintre pacienti<sup>7<\/sup>. Prezenta intratecala a anticorpilor specifici trebuie evaluata luand in considerare variabilele esentiale ale lichidului cefalo-rahidian, cum ar fi: celularitatea, concentratia totala a anticorpilor IgG si IgM, concentratia albuminei. Pentru o cuantificare mai veridica a sintezei intratecale de imunoglobuline, trebuie aplicata o functie hiperbolica pentru a distinge intre fractiunile proteice derivate din sange si, respectiv, SNC prezente in LCR (diagrama lui Reiber)<sup>6<\/sup>.<\/p>\n<p>Studii recente propun ca 4 din urmatoarele 5 criterii sa fie prezente pentru a stabili diagnosticul de neuroborrelioza:<\/p>\n<ul>\n<li>lipsa antecedentelor de neuroborrelioza;<\/li>\n<li>anticorpi anti-Borelia burgdorferi in LCR;<\/li>\n<li>index de specificitate a anticorpilor (ASI) pozitiv;<\/li>\n<li>raspuns favorabil clinic dupa tratament antibiotic corespunzator;<\/li>\n<li>absenta unui diagnostic alternativ<sup>8<\/sup>.<\/li>\n<\/ul>\n<p><strong><em>Recomandari pentru determinarea Borrelia burgdorferi \u2013 IgG in LCR\/ser <\/em><\/strong>\u2013 diagnosticul infectiilor SNC asociate cu Borrelia burgdorferi<sup>6<\/sup>.<\/p>\n<p><strong><em>Pregatire pacient <\/em><\/strong>\u2013 se aduce la cunostinta pacientului procedura de recoltare<sup>6<\/sup>.<\/p>\n<p><strong><em>Specimen recoltat<\/em><\/strong> \u2013 se recolteaza obligatoriu 2 probe: a) lichid cefalorahidian recoltat prin punctie lombara si b) sange venos<sup>6<\/sup>.<\/p>\n<p><strong><em>Recipient de recoltare<\/em><\/strong>\u00a0\u2013 a) recipient steril; b) vacutainer fara anticoagulant cu\/fara gel separator<sup>6<\/sup>.<\/p>\n<p><strong><em>Prelucrare\u00a0 necesara dupa recoltare <\/em><\/strong>\u2013 se separa serul prin centrifugare<sup>6<\/sup>.<\/p>\n<p><strong><em>Volum minim proba<\/em><\/strong> \u2013 1) 2 mL; 2) 1mL<sup>6<\/sup>.<\/p>\n<p><strong><em>Cauze de respingere a probei <\/em><\/strong>\u2013 ser hemolizat; plasma in loc de ser<sup>6<\/sup>.<\/p>\n<p><strong><em>Stabilitate proba <\/em><\/strong>- <em>7 zile<\/em> la\u00a0 2-8<sup>0<\/sup>C atat pentru ser cat si pentru LCR<sup>6<\/sup>.<\/p>\n<p><strong><em>Metoda <\/em><\/strong>- <span style=\"text-decoration: underline;\">nefelometrica<\/span> (pentru determinarea albuminei, anticorpilor totali IgG si IgM in ser si LCR); sinteza intratecala de imunoglobuline IgG\/IgM absenta;<strong>\u00a0<\/strong><\/p>\n<p style=\"padding-left: 30px;\">-bariera hematoencefalica integra; prezenta sinteza intratecala de imunoglobuline IgG\/IgM<sup>6<\/sup>.<strong>\u00a0<\/strong><\/p>\n<p>Pentru anticorpii IgG indicele anticorpilor specifici ASI (<em>index specific antibodies<\/em>) este calculat conform formulei:<\/p>\n<p>ASI = Qspec \/ QIgG.<\/p>\n<p>Qspec = coeficientul anticorpilor IgG specifici = anticorpi IgG specifici in LCR\/anticorpi IgG specifici in ser.<\/p>\n<p>QIgG = coeficientul anticorpilor IgG totali = anticorpi IgG in LCR\/anticorpi IgG in ser.<\/p>\n<p>Daca este necesar (QIgG &gt; Qlim), se va efectua corectia pentru sinteza locala de IgG policlonale:<\/p>\n<p>ASI = Qspec \/ Qlim.<\/p>\n<p>Qlim reprezinta fractia anticorpilor \u00a0IgG din LCR care provin numai din sange si se calculeaza cu ajutorul unei formule din indicele albuminic individual (Qalb).<\/p>\n<p>Pentru anticorpii IgM indicele anticorpilor specifici ASI este calculat conform formulei:<\/p>\n<p>ASI = Qspec \/ QIgM.<\/p>\n<p>Qspec = coeficientul anticorpilor IgM specifici = anticorpi IgM specifici in LCR\/anticorpi IgM specifici in ser.<\/p>\n<p>QIgM = coeficientul anticorpilor IgM totali = anticorpi IgM in LCR\/anticorpi IgM in ser.<\/p>\n<p>In mod normal ASI &lt;1.3. Valorile ASI = 1.3-1.5 constituie un rezultat echivoc.. O valoare a ASI mai mare de 1.5 indica o productie locala crescuta de anticorpi specifici<sup>4:6<\/sup>.<\/p>\n<p>Studii recente au aratat ca ASI are o specificitate de 96% si sensibilitate de 80% pentru diagnosticul neuroborreliozei.<\/p>\n<p>Valori ale ASI &gt;4 sunt sugestive pentru formele neurologice de borrelioza<sup>7<\/sup>.<\/p>\n<p><span style=\"font-size: small;\">\u00a0<\/span><\/p>\n<p><span style=\"font-size: small;\">\u0411\u0438\u0431\u043b\u0438\u043e\u0433\u0440\u0430\u0444\u0438\u044f<\/span><\/p>\n<p><span style=\"font-size: small;\">1.
