Virusul varicelo-zosterian (VZV) face parte din familia Herpesviridae; contine un ADN dublu catenar localizat central si inconjurat de o anvelopa.<\/p>\n

VZV produce leziuni veziculare dermice ce se exprima clinic ca varicela<\/em> (infectie primara) sau herpes zoster<\/em> (reactivare). Complicatiile varicelei sunt rare si afecteaza predominat SNC si plamanul. Anomaliile neurologice includ ataxia cerebeloasa acuta<\/em>, encefalita<\/em>, meningita<\/em>, mielita transversa<\/em>.<\/p>\n

Ataxia cerebeloasa<\/em> (prevalenta 1 la 4000 cazuri) se dezvolta cel mai adesea in decurs de 1 saptamana de la debutul exantemului, dar poate sa apara chiar si dupa 21 zile. Se manifesta prin: febra, vertij, varsaturi, ataxie, tulburari de vorbire sau nistagmus. La copii constituie de obicei o complicatie benigna care se remite intr-un interval de 2-4 saptamani.<\/p>\n

O complicatie mai grava este encefalita<\/em>, care este insotita de o mortalitate de 5-20% si de sechele neurologice la 15% dintre supravietuitori. Pacientii se prezinta cu cefalee progresiva, febra, varsaturi, convulsii si tulburari de constienta, care evolueaza minimum 2 saptamani.<\/p>\n

Meninigita<\/em> afecteaza copiii si adultii tineri si evolueaza ca o meningita obisnuita cu cefalee, febra, rigiditate nucala si fotofobie1;5<\/sup>.<\/p>\n

Sinteza intratecala de anticorpi specifici poate fi observata incepand cu saptamana a 2-a de la debutul infectiei. Detectarea prin PCR a VZV-ADN in LCR constituie o metoda sensibila pentru diagnosticul rapid al infectiilor SNC asociate cu VZV, in timp ce demonstrarea productiei intratecale de anticorpi specifici constituie un element adjuvant pentru diagnosticul retrospectiv al acestor infectii2;4<\/sup>. Cresteri ale sintezei intratecale de anticorpi virus-specifici sunt intalnite si in scleroza multipla, ca parte a reactiei MRZ4<\/sup>. Prezenta intratecala a anticorpilor specifici trebuie evaluata luand in considerare variabilele esentiale ale lichidului cefalo-rahidian, cum ar fi: celularitatea, concentratia totala a anticorpilor IgG, concentratia albuminei. Pentru o cuantificare mai veridica a sintezei intratecale de imunoglobuline, trebuie aplicata o functie hiperbolica pentru a distinge intre fractiunile proteice derivate din sange si, respectiv, SNC prezente in LCR (diagrama lui Reiber)3<\/sup>.<\/p>\n

Recomandari pentru determinarea VZV \u2013 <\/em><\/strong>IgG in LCR\/ser <\/strong>\u2013 diagnosticul sindroamelor SNC asociate cu VZV; utilitate in diagnosticul sclerozei multiple (ca parte a reactiei MRZ) impreuna cu determinarea benzilor oligoclonale 3;4<\/sup>.<\/p>\n

Specimen recoltat<\/em><\/strong> \u2013 se recolteaza obligatoriu 2 probe: a) lichid cefalorahidian recoltat prin punctie lombara si b) sange venos3<\/sup>.<\/p>\n

Recipient de recoltare<\/em><\/strong>\u00a0\u2013 a) recipient steril; b) vacutainer fara anticoagulant cu\/fara gel separator3<\/sup>.<\/p>\n

Prelucrare\u00a0 necesara dupa recoltare <\/em><\/strong>\u2013 se separa serul prin centrifugare3<\/sup>.<\/p>\n

Volum minim proba<\/em><\/strong> \u2013 1) 2 mL; 2) 1mL3<\/sup>.<\/p>\n

Cauze de respingere a probei <\/em><\/strong>\u2013 ser hemolizat; plasma in loc de ser3<\/sup>.<\/p>\n

Stabilitate proba <\/em><\/strong>- 7 zile<\/em> la\u00a0 2-80<\/sup>C atat pentru ser cat si pentru LCR3<\/sup>.<\/p>\n

Metoda <\/em><\/strong>- nefelometrica<\/span> (pentru determinarea albuminei, anticorpilor totali IgG in ser si LCR); ELISA<\/span> (pentru determinarea anticorpilor specifici anti-VZV in ser si LCR)3<\/sup>.<\/p>\n

Pentru anticorpii IgG, indicele anticorpilor specifici (index specific antibodies = ASI) ASI este calculat conform formulei:<\/p>\n

ASI = Qspec \/ QIgG.<\/p>\n

Qspec = coeficientul anticorpilor IgG specifici = anticorpi IgG specifici in LCR\/anticorpi IgG specifici in ser.<\/p>\n

QIgG = coeficientul anticorpilor IgG totali = anticorpi IgG in LCR\/anticorpi IgG in ser.<\/p>\n

Daca este necesar (QIgG > Qlim), se va efectua corectia pentru sinteza locala de IgG policlonale:<\/p>\n

ASI = Qspec \/ Qlim.<\/p>\n

Qlim reprezinta fractia anticorpilor\u00a0 IgG din LCR care provin numai din sange si se calculeaza cu ajutorul unei formule din indicele albuminic individual (Qalb).<\/p>\n

In mod normal ASI <1.3. Valorile ASI 1.3-1.5 constituie un rezultat echivoc. O valoare a ASI mai mare de 1.5 indica o productie locala crescuta de anticorpi specifici2;3<\/sup>.<\/p>\n
