Hemoglobinopatia\u00a0 Lepore (delta-beta hemoglobina hibrid) a fost descrisa in 1958 de catre Gerald & Diamond si denumita astfel dupa numele primului pacient la care a fost recunoscuta aceasta varianta de Hb;<\/em> este caracterizata printr-o hemoglobina anormala, in care lanturile \u03b1 sunt normale, iar lanturile non-\u03b1 au o structura asemanatoare lanturilor \u03b4 la capatul N-terminal si lanturilor \u03b2 la capatul C-terminal1;3;5<\/sup>.<\/p>\n

Mecanismul molecular aflat la baza acestei hemoglobinopatii este \u00a0aparitia in cursul meiozei a unui crossing-over inegal, cu deletia portiunii \u00a03\u2019 a genei \u03b4 si cea a portiunii\u00a0 5\u2019 a genei \u03b2, ceea ce da nastere la gena de fuziune \u03b4\u03b2. Gena de fuziune codifica un lant hibrid \u03b4\u03b2 care este sintetizat la o rata mult mai redusa in comparatie cu lantul \u03b2 normal, datorita instabilitatii ARN-ului mesager1;2;5<\/sup>.<\/p>\n

Hemoglobinopatia\u00a0 Lepore la heterozigoti se caracterizeaza prin prezenta Hb Lepore intr-un procent de 5-10%, cu migrare asemanatoare HbS si scaderea procentului de Hb A2<\/sub>; la\u00a0 homozigoti Hb Lepore reprezinta 10-20%, restul fiind format de HbF (Hb A si Hb A2<\/sub> lipsesc in totalitate)3<\/sup>.<\/p>\n

Deoarece situsurile de deletie sunt variabile au fost descrise mai multe tipuri de hemoglobinopatie cu fuziunea \u03b4\u03b2,\u00a0 cele mai importante fiind: Hb Lepore Boston-Washington (\u03b487Gln<\/sup>\u03b2116His<\/sup>), Hb Lepore Hollandia (\u03b422Ala<\/sup>\u03b250Thr<\/sup>) si Hb Lepore Baltimore (\u03b450Ser<\/sup>\u03b286Ala<\/sup>) care difera in ceea ce priveste regiunea la care s-a produs crossing-over-ul1;6<\/sup>.<\/p>\n

Hemoglobinopatia cea mai frecventa este Hb Lepore Boston-Washington<\/em>, intalnita mai ales la populatia din sudul Italiei, in Grecia (in special Macedonia), Romania, Turcia si Iran (Rowly et al.1969<\/em>)5<\/sup>.<\/p>\n

Trebuie mentionat faptul ca fenomenul de crossing-over inegal are loc si la nivelul genelor normale<\/p>\n

\u03b4 si \u03b2. Produsul acestor gene anti-Lepore reprezinta 15-20% din totalul de hemoglobina. In functie de localizarea fuziunii au fost descrise mai multe hemoglobine anti-Lepore<\/em>: Hb Miyada, P Congo, P Nilotic, Hb Lincoln Park, purtatorii avand un nivel normal de hemoglobina si indici eritrocitari normali2<\/sup>.<\/p>\n

Hb Lepore este importanta datorita posibilitatii interactiunii cu hemoglobina S si cu beta-talasemiile. Din punct de vedere functional poate fi considerata ca o (\u03b4\u03b2)+<\/sup>-talasemie.<\/p>\n

Tabloul hematologic la heterozigoti nu poate fi diferentiat de cel al tarei de \u03b2-talasemie. Hemoglobinopatia Lepore poate fi detectata prin analiza electroforetica a hemoglobinei; la pH alcalin hemoglobina Lepore migreaza asemanator HbS; in mediu acid Hb Lepore migreaza in dreptul HbA. HBA2 este redusa in medie la jumatate din valoarea normala; HbF poate fi uneori usor crescuta1<\/sup>. Homozigotii pentru hemoglobina Lepore, la fel ca si heterozigotii compusi pentru Hb Lepore si\u00a0 \u03b2-talasemie, prezinta tabloul clinic si hematologic al \u03b2-talasemiei majore\u00a0 sau cel al \u03b2-talasemiei intermedia1;2;5<\/sup>.<\/p>\n

Identificarea si cuantificarea Hb Lepore se poate face si prin metoda cromatografica HPLC, dar metodele de biologie moleculara sunt necesare pentru stabilirea tipului de hemoglobinopatie2;4<\/sup>.<\/p>\n

Recomandari pentru testarea genetica <\/em><\/strong><\/p>\n

-depistarea purtatorilor, mai ales la persoanele care asociaza siclemie sau \u03b2-talasemie (in scop prognostic);<\/p>\n

-diagnostic prenatal2<\/sup>.<\/p>\n
