{"id":10157,"date":"2020-04-23T03:58:15","date_gmt":"2020-04-23T00:58:15","guid":{"rendered":"https:\/\/www.synevo.ro\/product\/cultura-secretie-auriculara-cu-antibiograma-dupa-caz\/"},"modified":"2026-02-18T23:28:34","modified_gmt":"2026-02-18T21:28:34","slug":"cultura-secretie-auriculara-cu-antibiograma-dupa-caz","status":"publish","type":"product","link":"https:\/\/old.synevo.md\/ru\/shop\/cultura-secretie-auriculara-cu-antibiograma-dupa-caz\/","title":{"rendered":"\u041f\u043e\u0441\u0435\u0432 \u0430\u0443\u0440\u0438\u043a\u0443\u043b\u044f\u0440\u043d\u043e\u0433\u043e \u0441\u0435\u043a\u0440\u0435\u0442\u0430 (\u0410\u043d\u0442\u0438\u0431\u0438\u043e\u0442\u0438\u043a\u043e\u0433\u0440\u0430\u043c\u043c\u0430 \u043f\u043e \u043d\u0435\u043e\u0431\u0445\u043e\u0434\u0438\u043c\u043e\u0441\u0442\u0438)"},"content":{"rendered":"<p><strong><em>\u041e\u0431\u0449\u0430\u044f \u0438\u043d\u0444\u043e\u0440\u043c\u0430\u0446\u0438\u044f<\/em><\/strong><\/p>\n<p>Patologia infectioasa a urechii este dominata de otitele externe (infectii ale pavilionului si ale conductului auditiv extern, regiuni anatomice cu flora saprofita) si de otitele medii (infectii ale urechii medii).<\/p>\n<p>Flora saprofita a urechii externe este reprezentata de microorganismele care se intalnesc pe tegument:<\/p>\n<p style=\"padding-left: 30px;\">- <em>specii bacteriene dominante<\/em>: stafilococi coagulazo-negativi si corynebacterii;<\/p>\n<p style=\"padding-left: 30px;\">- <em>specii bacteriene mai rar intalnite<\/em>: micrococi,\u00a0 neiserii saprofite;<\/p>\n<p style=\"padding-left: 30px;\">- <em>genuri de fungi: <\/em>Alternaria, Penicillium si Candida (non albicans)<sup>3;5<\/sup>.<\/p>\n<p><span style=\"text-decoration: underline;\">Otitele externe<\/span> \u2013 au particularitati proprii infectiilor tegumentare, contaminarea putandu-se produce prin:<\/p>\n<p style=\"padding-left: 30px;\">\u2022 intepari sau raniri cu diferite corpuri tari: scobitori, ace, creioane;<\/p>\n<p style=\"padding-left: 30px;\">\u2022 erizipel determinat de Streptococcus pyogenes;<\/p>\n<p style=\"padding-left: 30px;\">\u2022 patrunderea apei in conductul auditiv, cu macerarea consecutiva a tegumentelor de la acest nivel poate determina aparitia \u201eotitei inotatorilor\u201d produsa de Pseudomonas aeruginosa (la cei care inoata in apa dulce) sau de Vibrio alginolyticus (inotatorii din oceane)<sup>5<\/sup>;<\/p>\n<p style=\"padding-left: 30px;\">\u2022 post antibioticoterapie locala indelungata pot aparea micoze auriculare determinate de Candida albicans sau \u00a0de fungi din genul Aspergillus.<\/p>\n<p><span style=\"text-decoration: underline;\">\u0421\u0440\u0435\u0434\u043d\u0438\u0435 \u043e\u0442\u0438\u0442\u044b<\/span> <em>\u00a0<\/em>\u2013 au particularitati proprii anatomiei si fiziologiei locale, fiind mai frecvente la copiii in varsta de 3 luni pana la 3 ani. Urechea medie (ca si cea interna), regiune sterila din punct de vedere bacteriologic, se poate infecta fie prin invazia florei locale si de vecinatate, fie in cadrul unor boli sistemice, cu poarta de intrare respiratorie (rujeola, scarlatina)<sup>2<\/sup>.<\/p>\n<p>Un rol important in aparitia acestora il au afectiunile rinofaringelui care, prin congestia si edemul trompei lui Eustachio, blocheaza drenajul normal al cavitatii si eliminarea contaminantilor ocazionali proveniti din microbiota locala<sup>3<\/sup>.<\/p>\n<p style=\"padding-left: 30px;\">\u2022 la copii, amigdalitele si faringitele determina aparitia otitelor medii prin propagarea inflamatiei si infectiei, care este favorizata de beanta si pozitia orizontala a trompei la aceasta varsta.<\/p>\n<p style=\"padding-left: 30px;\">\u2022 vegetatiile adenoidiene determina aparitia otitelor atat pe cale mecanica, prin volumul lor, cat si pe cale inflamatorie, prin infectiile pe care le intretin.<\/p>\n<p>Principalii agenti etiologici implicati in otitele medii sunt: Streptococcus pneumoniae, Haemophilus influenzae si Moraxella catarrhalis<sup>5<\/sup>.<\/p>\n<p>In otitele cronice, externe si medii, predomina germenii anaerobi: Peptostreptococcus spp., Bacteroides fragilis, Prevotella melaninogenica, Porphyromonas, frecvent insotiti de Staphylococcus aureus, Pseudomonas aeruginosa sau alti bacili Gram negativi<sup>1;5<\/sup>.<\/p>\n<p><strong><em>Recoltarea si transportul probelor<\/em><\/strong><\/p>\n<p>In <em>otitele externe<\/em> produsul patologic este reprezentat de <em>exsudatul <\/em>leziunii tegumentare a pavilionului sau a conductului auditiv extern. Spre deosebire de acesta, prelevatul de electie pentru examenul bacteriologic al <em>otitelor medii<\/em> il reprezinta <em>exsudatul aspirat prin timpanocenteza<\/em>. In caz de perforare spontana a timpanului prelevatul se recolteaza pe un fin tampon steril<sup>3;4<\/sup><sup>;5<\/sup>.<\/p>\n<p>Se recomanda recoltarea produsului patologic pe 2 tampoane:<\/p>\n<ul>\n<li>un tampon simplu din care se vor efectua doua frotiuri pentru examenul microscopic<\/li>\n<li>un tampon prevazut cu mediu de transport pentru efectuarea culturii.<\/li>\n<\/ul>\n<p>In\u00a0 spitale se poate recurge la insamantarea extemporanee cu lanteta de punctie, direct pe mediile de cultura.<\/p>\n<p>Indiferent de modalitatea de prelevare, produsul patologic este imersat in mediu de transport si trimis imediat la laborator. Transportul probelor catre laborator se face in maximum 2 ore de la prelevare. Desi probele prelevate pe tampoane in tuburi ce contin mediu de transport pot fi pastrate pana la 24 h, este recomandat ca insamantarile pe mediile de cultura sa se faca imediat ce probele ajung la laborator, pentru o mai sigura recuperare a microorganismelor urmarite<sup>4<\/sup>.<\/p>\n<p><strong><em>Diagnostic de laborator<\/em><\/strong><\/p>\n<p>Examenul microscopic: se efectueaza doua frotiuri, unul cu coloratie Gram pentru determinarea morfologiei germenilor si unul cu coloratie Giemsa, pentru aprecierea reactiei inflamatorii<sup>3<\/sup><sup>;5<\/sup>.<\/p>\n<p>Insamantarea pe medii de cultura<em>: <\/em>agar Columbia cu 5% sange de berbec, agar chocolate, agar Mac Conkey <sup>5<\/sup>.<\/p>\n<p>Insamantarea se face cu dispersie, pentru a permite o apreciere semicantitativa a cresterii. Se incubeaza aerob in atmosfera cu 5% CO<sub>2<\/sub>, 24 h la 37\u00b0C, cu prelungire pana la 48 h daca la prima citire a placilor nu se observa coloniile caracteristice germenului urmarit. Coloniile caracteristice se repica in vederea obtinerii culturii pure pentru identificare si antibiograma<sup>4<\/sup>.<\/p>\n<p>Medicul clinician poate orienta investigatiile suplimentar catre <em>cultura de fungi <\/em>si\/sau<em> cultura anaeroba<\/em>, in functie de contextul clinic<em>.<\/em> In cazul germenilor anaerobi, rapiditatea prelevarii si transportul in conditii optime sunt esentiale in izolarea acestora.<\/p>\n<p><strong><em>Comunicarea si interpretarea rezultatelor<\/em><\/strong><em>- <\/em>se face corelat cu examenul microscopic direct.<strong><em>\u00a0<\/em><\/strong><\/p>\n<p>In cazul otitelor externe microscopia este ingreunata din cauza numerosilor germeni contaminanti prezenti la nivelul leziunii tegumentare. Coexistenta pe frotiu a reactiei inflamatorii cu prezenta germenilor (in special intraleucocitari) ghideaza microbiologul spre germenul patogen.<\/p>\n<p>In cazul otitelor interne la care produsul patologic a fost recoltat prin timpanocenteza culturile sunt, de obicei, monobacteriene, iar identificarea germenilor in vederea obtinerii antibiogramei este mult usurata.<\/p>\n<p><strong><em>Criterii pentru efectuarea antibiogramei<\/em><\/strong><\/p>\n<p>Necesitatea izolarii in cultura pura, urmata de <strong><em>identificarea la nivel de specie<\/em><\/strong> si efectuarea <strong><em>antibiogramei<\/em><\/strong> este impusa de cresterea, in ultima vreme, a numarului agentilor microbieni producatori de betalactamaza.<\/p>\n<p>Tratamentul ghidat conform antibiogramei este un tratament tintit si in acelasi timp are avantajul de a nu determina rezistenta la antibiotice.<\/p>\n<p>Mentiune: In laboratorul Synevo Bucuresti este disponibil, pentru cazurile de urgenta, un sistem automat de identificare bacteriana si de efectuare a antibiogramei, cu eliberarea rezultatului in 24 ore de la izolarea germenului in cultura pura<sup>4<\/sup>.<\/p>\n<p><span style=\"font-size: small;\">\u00a0<\/span><\/p>\n<p><span style=\"font-size: small;\">\u0411\u0438\u0431\u043b\u0438\u043e\u0433\u0440\u0430\u0444\u0438\u044f<\/span><\/p>\n<p><span style=\"font-size: small;\">1.
