Informa\u0163<\/em><\/strong>ii generale \u015fi <\/em><\/strong>recomand\u0103ri pentru determinarea anticorpilor anti-aquaporina 4<\/em><\/strong>\u00a0<\/em><\/strong><\/p>\n

Neuromielita optic\u0103 (NMO, sindromul Devie) este o afec\u0163iune inflamatorie de cauz\u0103 autoimun\u0103 caracterizat\u0103 prin producerea unei nevrite optice bilaterale, urmat\u0103 \u00eentr-un interval de zile sau s\u0103pt\u0103m\u00e2ni de o mielit\u0103 transvers\u0103. Ocazional nevrita optic\u0103 poate fi unilateral\u0103 sau poate urma unui acces ini\u0163ial de mielit\u0103. Leziunile din SNC pot fi necrozante \u015fi severe. Modific\u0103rile LCR pot fi variabile, dar \u00een unele cazuri constau \u00een pleiocitoz\u0103 polimorfonuclear\u0103 \u015fi \u00eentr-o cre\u015ftere a con\u0163inutului de proteine.<\/p>\n

A existat o controvers\u0103 \u00eendelungat\u0103 asupra faptului dac\u0103 NMO reprezint\u0103 un subtip de scleroz\u0103 multipl\u0103 sau o afec\u0163iune distinct\u0103. Recent, a fost identificat un autoanticorp NMO IgG cu specificitate foarte mare pentru neuromielita optic\u0103 (a fost depistat \u00een exclusivitate \u00een serul pacien\u0163ilor cu aceast\u0103 boal\u0103), iar antigenul \u0163int\u0103 este reprezentat de proteina canalului de ap\u0103 aquaporina 4 (AQP 4) exprimat\u0103 la nivelul astrocitelor.<\/p>\n

Studiile histopatologice au sugerat c\u0103 leziunile astrocitare mediate autoimun (\u00eempotriva AQP 4) sunt implicate \u00een patogenia NMO, care se distinge astfel de scleroza multipl\u0103 \u2013 o afec\u0163iune demielinizant\u0103 primar\u0103. Dovezile din ce \u00een ce mai numeroase ale efectelor citotoxice sau func\u0163ionale exercitate in vitro<\/em> asupra astrocitelor de c\u0103tre anti-AQP 4 sus\u0163in implicarea acestora \u00een patogenia bolii.<\/p>\n

\u00cen ceea ce prive\u015fte tratamentul NMO, s-a constatat c\u0103 agen\u0163ii imunosupresori cum ar fi prednisolonul \u015fi\/sau azatioprina reduc rata rec\u0103derilor de boal\u0103, \u00een timp ce eficacitatea terapeutic\u0103 a interferonului beta r\u0103m\u00e2ne incert\u0103. Astfel, NMO difer\u0103 de scleroza multipl\u0103 \u015fi prin abordarea terapeutic\u0103. Din acest motiv, determinarea anticorpilor anti-AQP 4 este indispensabil\u0103 pentru diagnosticul \u015fi tratamentul NMO2<\/sup>.<\/span><\/p>\n

Preg\u0103tire pacient <\/em><\/strong>- <\/em>nu este necesar\u0103 o preg\u0103tire special\u01031<\/sup>.<\/p>\n

Interpretarea rezultatelor<\/em><\/strong><\/p>\n

Anti-AQP 4 prezint\u0103 o sensibilitate \u015fi o specificitate pentru NMO de 91% \u015fi respectiv 100%. Titrurile crescute de autoanticorpi se asociaz\u0103 cu cecitate \u015fi leziuni cerebrale extinse eviden\u0163iate prin RMN. Exist\u0103 de asemenea o corela\u0163ie pozitiv\u0103 \u00eentre titrurile de anticorpi \u015fi extinderea leziunilor medulare. Titrurile scad dup\u0103 admistrarea de metilprednisolon \u00een doze mari \u015fi se men\u0163in sc\u0103zute \u00een perioada de remisiune clinic\u0103 sub imunosupresie3<\/sup>.<\/div>\n
