{"id":10130,"date":"2020-04-23T03:57:29","date_gmt":"2020-04-23T01:57:29","guid":{"rendered":"https:\/\/www.synevo.ro\/product\/hla-b27\/"},"modified":"2025-01-29T16:54:35","modified_gmt":"2025-01-29T14:54:35","slug":"hla-b27","status":"publish","type":"product","link":"https:\/\/old.synevo.md\/ru\/shop\/hla-b27\/","title":{"rendered":"HLA-B27 (\u0431\u043e\u043b\u0435\u0437\u043d\u044c \u0411\u0435\u0445\u0442\u0435\u0440\u0435\u0432\u0430)"},"content":{"rendered":"<p><strong><em>Informatii generale si recomandari<\/em><\/strong><\/p>\n<p>Testul are drept scop sa stabileasca daca este prezent sau nu pe suprafata leucocitelor antigenul HLA-B27, pentru a evalua posibilitatea existentei unei boli autoimune asociate cu acest marker<sup>5<\/sup>.<\/p>\n<p>Antigenul HLA-B27 este intalnit la 90-95% din pacientii cu spondilita ankilopoietica, detinand un rol important in aparitia bolii, in timp ce incidenta sa in populatia generala este de aproximativ 8%.<\/p>\n<p>Rudele de gradul intai HLA-B27 pozitive ale pacientilor cu spondilita ankilopoietica au intr-un procent de 10-20% simptome clinice si radiologice de boala sau numai modificari radiologice. Studiile pe gemenii monozigoti arata o concordanta a bolii de 60%; \u00a0pentru cei dizigoti concordanta este de 20-25%<sup>4<\/sup>.<\/p>\n<p>HLA-B27 este extrem de heterogen descriindu-se peste 35 subtipuri moleculare (HLA-B*2701 \u2013<\/p>\n<p>B*2736). Subtipurile HLA-B27 difera prin anumiti aminoacizi care foarte probabil sunt implicati in legarea peptidelor antigenice specifice. Subtipurile asociate in mod cert cu spondilita ankilopoietica, sunt <strong>HLA-B*2705<\/strong>, <strong>HLA-B*2702<\/strong>, <strong>HLA-B*2704<\/strong>, <strong>HLA-B*2707<\/strong>, <strong>HLA-B*2714<\/strong>. Pentru HLA-B*2706 si \u00a0HLA-B*2709, care sunt intalnite la persoane din sud-estul Asiei si Sardinia, nu s-a demonstrat asocierea cu spondilartropatiile seronegative. Celelalte subtipuri de HLA-B27 sunt prea rare pentru a fi stabilita o asociere cu boala. La subtipul HLA-B*2722 s-a constatat o eroare de secventiere, motiv pentru a care a fost retras din clasificare<sup>1<\/sup>.<\/p>\n<p>HLA-B27 detine un rol dublu in ceea ce priveste recunoasterea antigenului de catre limfocitul T. Pe de o parte are rol in alegerea repertoriului receptorului pentru antigen al limfocitului T citotoxic CD8+ in timus, selectand in special unele grupe de celule T CD8+ cu proprietati artritogene. Pe de alta parte, HLA-B27 este implicat in legarea si \u00a0prezentarea\u00a0 antigenului catre limfocitul T citotoxic CD8+ in tesuturile periferice<sup>4<\/sup>.<\/p>\n<p>Intre HLA-B27 si o proteina a agentului infectios Klebsiella pneumoniae exista un mimetism molecular, ceea ce ar putea implica existenta unei patogenii bacteriene a spondilitei ankilopoietice. Este posibil ca aceasta similitudine antigenica sa permita dezvoltarea unui raspuns imun incrucisat, cu selectionarea de limfocite T CD8+ fata de celule ce exprima HLA-B27, sau sa favorizeze persistenta agentului infectios prin toleranta imuna<sup>3;4<\/sup>.<\/p>\n<p>Alte afectiuni autoimune asociate cu HLA-B27 sunt:<\/p>\n<p style=\"padding-left: 30px;\">-sindromul Reiter (artrita reactiva): incidenta HLA-B27 este de 60-80%;<\/p>\n<p style=\"padding-left: 30px;\">-artrita idiopatica juvenila: incidenta HLA-B27 este de 50% in forma de debut cu artrita asociata entesitei;<\/p>\n<p style=\"padding-left: 30px;\">-artrita psoriazica: incidenta HLA-B27 este de 50% la pacientii care asociaza afectare axiala;<\/p>\n<p style=\"padding-left: 30px;\">-bolile inflamatorii ale intestinului: incidenta HLA-B27 este de 50% la pacientii care asociaza spondilita<sup>4;5<\/sup>.<\/p>\n<p><strong><em>Pregatire pacient<\/em><\/strong> \u2013 nu este necesara o pregatire speciala<sup>2<\/sup>.<\/p>\n<p><strong><em>Specimen recoltat<\/em><\/strong> \u2013 sange venos<sup>2<\/sup>.<\/p>\n<p><strong><em>Recipient de recoltare<\/em><\/strong>\u00a0\u2013 vacutainer ce contine EDTA ca anticoagulant<sup>2<\/sup>.<\/p>\n<p><strong><em>Cantitate recoltata <\/em><\/strong>\u2013 cat permite vacuumul<sup>2<\/sup>.<\/p>\n<p><strong><em>Cauze de respingere<\/em><\/strong> <strong><em>a probei <\/em><\/strong>\u2013 folosirea heparinei ca anticoagulant; probe coagulate sau hemolizate<sup>2<\/sup>.<\/p>\n<p><strong><em>Stabilitate proba<\/em><\/strong> - <em>7 zile<\/em> la 2-8\u00baC<sup>2<\/sup>.<\/p>\n<p><strong><em>Metoda<\/em><\/strong> - <span style=\"text-decoration: underline;\">reactie de polimerizare in lant (PCR) cu detectie colorimetrica dupa hibridizarea cu sonde specifice pe stripuri;<\/span> testul detecteaza urmatoarele subtipuri HLA-B27: 02-17, 19-21, 24, 25, 27, 28, 30, 32-36<sup>2<\/sup>.<\/p>\n<p><strong><em>Valori de referinta<\/em><\/strong> \u2013 HLA-B27 negativ<sup>2<\/sup>.<\/p>\n<p><strong><em>Limite si interferente<\/em><\/strong><\/p>\n<p>HLA-B27 nu este un test screening pentru o spondilartropatie seronegativa.
