Informa\u0163<\/em><\/strong>ii generale <\/em><\/strong><\/p>\n

Anticorpii anti-neuronali sunt folosi\u0163i pentru diagnosticul sindroamelor neurologice paraneoplazice (care sunt complica\u0163ii nonmetastatice ale unui cancer sistemic)2<\/sup>.<\/p>\n

Anticorpi anti-neuronali sunt detecta\u0163i \u00een s\u00e2ngele sau LCR-ul majorit\u0103\u0163ii pacien\u0163ilor cu sindrom neurologic paraneoplazic, dar lipsesc \u00een s\u00e2ngele \u015fi LCR-ul indivizilor s\u0103n\u0103to\u015fi. Au fost descri\u015fi peste 14 tipuri de anticorpi anti-neuronali. Trei dintre ace\u015ftia sunt mai frecvent utiliza\u0163i \u00een diagnostic:<\/p>\n

-Anti-Hu IgG<\/span> determin\u0103 fie un sindrom encefalomielitic, fie o neuropatie senzitiv\u0103. Ace\u015fti anticorpi nu se \u00eent\u00e2lnesc la indivizii s\u0103n\u0103to\u015fi \u015fi foarte rar apar la pacien\u0163ii f\u0103r\u0103 sindrom neurologic paraneoplazic. Apar \u00een sindroamele neurologice paraneoplazice asociate cancerului pulmonar cu celule mici (se afirm\u0103 c\u0103 prezen\u0163a acestor anticorpi trebuie s\u0103 n\u0103 fac\u0103 s\u0103 c\u0103utam cancerul pulmonar, chiar dac\u0103 \u00eentr-o prim\u0103 faz\u0103 acesta este greu de descoperit). Prezen\u0163a unori titruri sc\u0103zute\u00a0de anticorpi anti\u2013Hu se asociaz\u0103 cu o faz\u0103 precoce \u015fi un r\u0103spuns bun la chimioterapie. Unii dintre pacien\u0163i se pot prezenta cu gastroparez\u0103 sau obstruc\u0163ie intestinal\u0103. Aproximativ 15% dintre pacien\u0163i prezint\u0103 \u015fi o alt\u0103 tumor\u0103 asociat\u0103 cancerului pulmonar. Ace\u015fti anticorpi au mai fost detecta\u0163i \u015fi la copii cu tulbur\u0103ri de motilitate intestinal\u0103, ataxie cerebeloas\u0103, encefalite asociate sau nu cu neuroblastom3<\/sup>.<\/p>\n

-Anti-Ri IgG<\/span> reprezint\u0103 cei mai comuni anticorpi anti-neuronali paraneoplazici. Determinarea titrului lor \u00een s\u00e2nge sau LCR se face \u00een prezen\u0163a unor afec\u0163iuni neurologice de cauz\u0103 neprecizat\u0103 (autoimun\u0103 sau paraneoplazic\u0103). Un rezultat pozitiv sugereaz\u0103 o malignitate ocult\u0103. Cel mai frecvent se asociaz\u0103 clinic cu opsoclonus-mioclonus ocular. Majoritatea pacien\u0163ilor prezint\u0103 asociat un cancer mamar. Mai rar se asociaz\u0103 cu un cancer ginecologic sau pulmonar. Tratamentul malignit\u0103\u0163ii poate duce la sc\u0103derea titrului acestor anticorpi \u015fi la ameliorarea manifest\u0103rilor neurologice3<\/sup>.<\/p>\n

-Anti-Yo IgG <\/span>\u00a0sunt denumi\u0163i \u015fi anticorpi anti-celule Purkinje deoarece afecteaz\u0103 acest tip de celule din cerebel. Determinarea titrului\u00a0 lor \u00een s\u00e2nge sau LCR se face la pacien\u0163ii care prezint\u0103 diverse simptome neurologice mai ales ataxie, nistagmus \u015fi disartrie. Sunt sugestivi mai ales pentru tumorile maligne\u00a0ginecologice, mamare \u015fi pulmonare3<\/sup>.<\/p>\n

Recomand\u0103ri pentru determinarea anticorpilor anti-neuronali <\/em><\/strong>\u2013 diagnosticul unor afec\u0163iuni neurologice de cauz\u0103 neprecizat\u0103 (paraneoplazic\u0103 sau imun\u0103) 3<\/sup>.\u00a0<\/em><\/strong><\/p>\n

Preg\u0103tire pacient <\/em><\/strong>- <\/em>nu este necesar\u0103 o preg\u0103tire special\u01032<\/sup>.<\/p>\n

Specimen recoltat <\/em><\/strong>- <\/em>\u0432\u0435\u043d\u043e\u0437\u043d\u0430\u044f \u043a\u0440\u043e\u0432\u044c1,2,3<\/sup>.<\/p>\n

Recipient de recoltare <\/em><\/strong>- <\/em>vacutainer f\u0103r\u0103 anticoagulant, cu\/f\u0103r\u0103 gel separator1<\/sup>,2.<\/sup><\/p>\n

Prelucrare necesar\u0103 dup\u0103 recoltare<\/em><\/strong> \u2013 se separ\u0103 serul prin centrifugare \u00een maxim 45 minute de la recoltare folosindu-se pentru separare o eprubet\u0103 de plastic2<\/sup>; se lucreaz\u0103 serul proasp\u0103t; dac\u0103 acest lucru nu este posibil, serul se p\u0103streaz\u0103 la 2-8\u00b0C, la -20\u00b0C sau la -70\u00b0C1<\/sup>.<\/p>\n

Volum prob\u0103<\/em><\/strong> \u2013 minim 2 mL ser1<\/sup>.<\/p>\n

Cauze de respingere<\/em><\/strong> a probei<\/em><\/strong><\/p>\n

\u2022 specimen hemolizat1<\/sup><\/div>\n
\u2022 specimen expus la temperatur\u0103 ridicat\u01031<\/sup><\/div>\n
\u2022 specimen contaminat bacterian1<\/sup><\/div>\n
\u2022 ser lipemic2<\/sup><\/div>\n
\u2022 proba recoltat\u0103 pe anticoagulant sau contaminat\u0103 cu anticoagulant2<\/sup><\/div>\n

Stabilitate prob\u0103 <\/em><\/strong>- <\/em>serul separat este stabil 14 zile 2-8\u00b0C; dou\u0103 luni<\/em> la -20\u00b0C 1,2<\/sup>. Nu decongela\u0163i\/recongela\u0163i1<\/sup>.<\/p>\n

Metod\u0103 <\/em><\/strong>- imunofluorescen\u0163a indirect\u0103<\/span>5<\/sup>.<\/p>\n

Interpretarea rezultatelor<\/em><\/strong><\/p>\n

Ob\u0163inerea unor rezultate pozitive la determinarea acestor anticorpi anti-neuronali impune c\u0103utarea unei tumori maligne responsabile de sindromul neuronal paraneoplazic, chiar dac\u0103 la o prim\u0103 examinare aceast\u0103 tumor\u0103 nu este evident\u0103 (\u00een special pentru tumorile pulmonare, mamare sau ginecologice)2<\/sup>.<\/p>\n

Limite \u015fi interferen\u0163e<\/em><\/strong><\/div>\n
Anticorpi anti-neuronali anti-Hu IgG pot avea valori pozitive (f\u0103r\u0103 a fi obligatorie prezen\u0163a unei malignit\u0103\u0163i) la\u00a0copiii cu tulbur\u0103ri de motilitate intestinal\u0103, ataxie cerebeloas\u0103, encefalite3<\/sup>.<\/div>\n
