Informa\u0163<\/em><\/strong>ii generale <\/em><\/strong><\/p>\n

Gangliozidele sunt componente\u00a0glicolipidice prezente \u00een membrana extern\u0103 a celulelor nervoase periferice. Gangliozidele apar\u0163in grupului de glicolipide acide care con\u0163in lipide (ceramide), oligozaharide \u015fi acid sialic.<\/p>\n

Structuri asem\u0103n\u0103toare gangliozidelor apar de asemenea pe suprafa\u0163a unor microorganisme. Neuropatiile inflamatorii apar cel mai adesea \u00een urma infec\u0163iilor cu Campylobacter jejuni, cu Citomegalovirus, cu virusul Epstein-Barr, cu Mycoplasma pneumoniae sau Haemophilus influenzae. Anticorpii \u00eendrepta\u0163i \u00eempotriva gangliozidelor aflate \u00een structura microorganismelor pot determina reac\u0163ii \u00eencruci\u015fate cu gangliozidele din structura mielinei \u015fi a fibrelor nervoase induc\u00e2nd procese inflamatorii cu demielinizari consecutive. Ca rezultat al acestei reac\u0163ii \u00eencruci\u015fate cu structurile microbiene, anticorpii anti GM1 <\/sub>de tip IgM pot fi \u00eent\u00e2lni\u0163i \u015fi la persoane s\u0103n\u0103toase (o singur\u0103 determinare cu valoare crescut\u0103 nu este patognomonic\u0103 pentru neuropatie)2<\/sup> .<\/p>\n

Neuropatii motorii periferice pot s\u0103 apar\u0103 la pacien\u0163i cu gamapatie monoclonal\u0103 IgM, a c\u0103ror paraproteine IgM reac\u0163ioneaz\u0103 cu gangliozidele GM1<\/sub> si GD1b<\/sub>. De\u015fi anticorpii anti GM1<\/sub> \u015fi GD1b<\/sub> pot fi implica\u0163i \u00eentr-o varietate de sindroame neurologice, prezen\u0163a lor \u00een titruri crescute este mai caracteristic\u0103 afec\u0163iunilor neuronale motorii dec\u00e2t altor tipuri de afec\u0163iuni neuronale4<\/sup>.<\/p>\n

Recomand\u0103ri pentru determinarea anticorpilor anti-gangliozide <\/em><\/strong>\u2013 evaluarea pacien\u0163ilor cu diverse neuropatii motorii \u015fi senzoriale3<\/sup>.<\/p>\n

Preg\u0103tire pacient <\/em><\/strong>- <\/em>nu necesit\u0103 o preg\u0103tire deosebit\u01031,2<\/sup>.<\/p>\n

Metoda <\/em><\/strong>- Test imunodot<\/span>\u00a0pentru determinarea calitativ\u0103 a anticorpilor IgG \u015fi\/sau IgM fa\u0163\u0103 de gangliozide (test screening \u2013 nu diferen\u0163iaz\u0103 anticorpii IgG de cei IgM)<\/b>; stripurile sunt alc\u0103tuite dintr-o membran\u0103 pe care sunt trasate 13 linii diferite corespunz\u0103toare diverselor autoantigene \u2013 o linie serve\u015fte drept control pozitiv, iar celelalte 12\u00a0con\u0163in urm\u0103toarele gangliozide \u00eenalt purificate: GM1, GM2, GM3, GM4, GD1a, GD1b, GD2, GD3, GT1a, GT1b, GQ1b \u015fi sulfatide. \u00cen cursul primei etape de incubare autoanticorpii din proba pacientului se leag\u0103 de antigenele \u0163int\u0103 imobilizate la nivelul membranei (faza solid\u0103). Dup\u0103 o etap\u0103 de sp\u0103lare, strip-ul este incubat cu 2 solu\u0163ii de conjugat (anti-IgG \u015fi respectiv anti-IgM uman) marcat cu peroxidaz\u0103. Conjugatul se va ata\u015fa la por\u0163iunea IgG \u015fi\/sau IgM a complexului antigen-anticorp. Dup\u0103 \u00eendep\u0103rtarea prin sp\u0103lare a conjugatului nelegat se adaug\u0103 substratul \u2013 tetrametilbenzidin\u0103. Dac\u0103 este prezent conjugatul legat, peroxidaza va transforma solu\u0163ia incolor\u0103 a substratului \u00eentr-o linie de precipitare corespunz\u0103toare autoantigenului \u0163int\u0103 de pe strip. Dup\u0103 o etap\u0103 de uscare, vor fi vizualizate \u015fi interpretate benzile ob\u0163inute.<\/p>\n

Valori de referin\u0163\u0103 <\/em><\/strong>\u2013 anticorpi anti-gangliozide: GM1, GM2, GM3, GM4, GD1a, GD1b, GD2, GD3, GT1a, GT1b, GQ1b \u015fi sulfatide\u00a0\u2013 negativ.<\/p>\n

Totu\u015f anticorpi anti-GM1<\/sub> tip IgM pot fi pozitivi la persoane s\u0103n\u0103toase datorit\u0103 reactivit\u0103\u0163ii \u00eencruci\u015fate cu antigenele microbiene. De aceea diagnosticul clinic nu se va baza doar pe aceast\u0103 determinare ci va lua \u00een considerare coroborarea tuturor datelor clinice \u015fi paraclinice2<\/sup>.<\/p>\n
