Diabetul zaharat tip 1 apare \u00een urma distrugerii celulelor beta ale pancreasului, ceea ce conduce la un deficit absolut de insulin\u0103. Simptomele clinice ale diabetului nu apar p\u00e2n\u0103 c\u00e2nd nu au fost distruse 80% p\u00e2n\u0103 la 90% din aceste celule5<\/sup>. \u00cenaintea manifest\u0103rii clinice, diabetul de tip 1 se caracterizeaz\u0103 prin apari\u0163ia autoanticorpilor circulan\u0163i fa\u0163\u0103 de o varietate de antigene insulare, ce sunt reprezentate de decarboxilaza acidului glutamic (GAD), tirozin fosfataza \u015fi insulin\u01031<\/sup>. Distrugerea autoimun\u0103 a celulelor beta pancreatice produc\u0103toare de insulin\u0103 este considerat\u0103 a fi cauza primar\u0103 a diabetului2,3<\/sup>.<\/p>\n

Prezen\u0163a acestor autoanticorpi demonstreaz\u0103 o activitate autoimun\u0103 precoce \u015fi determinarea lor poate ajuta clinicianul \u00een predic\u0163ia, diagnosticul \u015fi managementul pacien\u0163ilor diabetici5<\/sup>.<\/p>\n

IA2<\/sub> face parte din familia proteinei tirozinfosfataza, parte component\u0103 a membranei celulare. Proteinele tirozinfosfataze (PTPs) sunt o mare familie de enzime cu rol \u00een semnalizarea intra-\u015fi intercelular\u0103, antagoniste ale tirozinkinazelor (PTKs). In timp ce PTK fosforileaz\u0103 tirozina, PTP \u00eendep\u0103rteaz\u0103 fosfatul de pe substrat. Deoarece statusul de fosforilare a unei proteine \u00eei poate modula func\u0163ia, PTK \u015fi PTP conlucreaz\u0103 pentru a regla func\u0163ionarea proteinelor \u00een ceea ce prive\u015fte r\u0103spunsul fa\u0163\u0103 de o varietate de semnale, inclusiv hormoni, mitogeni \u015fi oncogene8<\/sup>. Ca \u015fi GAD, IA2<\/sub> se g\u0103se\u015fte \u00een \u0163esutul nervos \u015fi alte \u0163esuturi endocrine. Anticorpii fa\u0163\u0103 de IA2 <\/sub>se g\u0103sesc la 50-75% dintre copiii diabetici de tip 1 \u015fi 30-50% din adul\u0163i \u00eenaintea apari\u0163iei bolii. Deoarece riscul de diabet cre\u015fte cu prezen\u0163a fiec\u0103rui anticorp, valoarea predictiv\u0103 a anticorpilor IA2 <\/sub>cre\u015fte c\u00e2nd este \u00eenso\u0163it\u0103 de prezen\u0163a anticorpilor fa\u0163\u0103 de insulin\u0103 sau fa\u0163\u0103 de GAD1,3.7<\/sup>.<\/p>\n\n\n\n\n\n\n\n

\u00a0Interpretarea rezultatelor<\/em><\/strong><\/p>\n

Eviden\u0163ierea anticorpilor indic\u0103 evolu\u0163ia unui proces autoimun. Studii efectuate pe termen lung arat\u0103 c\u0103 o testare combinat\u0103 a diferi\u0163ilor anticorpi (GADA+IA 2 <\/sub>\u00a0sau GADA+IAA) se coreleaz\u0103 \u00een peste 85% din cazuri cu risc sau debut de diabet de tip13<\/sup>. Specificitatea anticorpilor atinge 98%1<\/sup>. Un rezultat negativ la copii face improbabil diagnosticul de diabet de tip 1.<\/p>\n

O problem\u0103 special\u0103 o reprezint\u0103 diabetul LADA. Acesta are \u00een comun cu diabetul de tip 1 caracteristicile fiziologice privind disfunc\u0163ia metabolic\u0103, genetic\u0103 \u015fi autoimunitatea. Pacien\u0163ii dezvolt\u0103 un diabet dup\u0103 30 ani ca urmare a evolu\u0163iei lente a unui proces autoimun. \u00a0Ace\u015fti pacien\u0163i sunt diagnostica\u0163i frecvent ca av\u00e2nd diabet de tip 2 \u015fi trata\u0163i cu antidiabetice orale1,7<\/sup>. Totu\u015fi, dup\u0103 luni sau ani se ajunge la nevoia de insulin\u0103. Se estimeaz\u0103 c\u0103 20% dintre pacien\u0163ii diagnostica\u0163i ca av\u00e2nd diabet tip 2 (mai ales \u00eentre 25-44 ani) ar avea de fapt LADA1<\/sup>. Testarea anticorpilor specifici ar trebui f\u0103cut\u0103 la to\u0163i adul\u0163ii care nu sunt obezi \u015fi aparent prezint\u0103 diabet zaharat tip 2.<\/p>\n

Limite \u015fi interferen\u0163e<\/em><\/strong><\/p>\n

Deoarece anticorpii specifici nu pot fi determina\u0163i \u00eentotdeauna, un rezultat negativ nu exclude diagosticul de diabet de tip 1 (diabet zaharat tip 1 idiopatic). Mai ales \u00een cazul adul\u0163ilor, cca. 20-30% nu prezint\u0103 anticorpi, de\u015fi exist\u0103 un deficit absolut de insulin\u0103. In acest caz diagnosticul poate fi stabilit prin determinarea secre\u0163iei pancreatice reziduale \u015fi prin teste genetice1<\/sup>.<\/p>\n
