Patogeneza diabetului zaharat insulino-dependent este caracterizat\u0103 printr-o lung\u0103 perioad\u0103 subclinic\u0103 \u00een care autoanticorpii circulan\u0163i fa\u0163\u0103 de antigenele tisulare sunt asocia\u0163i cu evolu\u0163ia bolii \u00een celulele pancreatice. \u00cen istoria natural\u0103 a diabetului zaharat, distrugerea celulelor beta poate fi v\u0103zut\u0103 ca trec\u00e2nd prin 5 faze:<\/p>\n

1. predispozi\u0163ie genetic\u0103;<\/div>\n
2. pozitivarea autoanticorpilor;<\/div>\n
3. r\u0103spuns anormal al insulinei la testul de toleran\u0163\u0103 la glucoz\u0103 i.v.;<\/div>\n
4. test patologic de toleran\u0163\u0103 la glucoza administrat\u0103 oral;<\/div>\n
5. diabet clinic manifest.<\/div>\n

Majoritatea cazurilor de diabet tip 1 apare sporadic, \u00een absen\u0163a unui istoric familial pozitiv. Concordan\u0163a pentru diabetul de tip 1 la gemenii identici este \u00eentre 30% \u015fi 50%. De aceea se consider\u0103 c\u0103 factorii de mediu joac\u0103 rol important \u00een dezvoltarea diabetului, av\u00e2nd o ac\u0163iune de declan\u015fare a procesului autoimun6<\/sup>.<\/p>\n

Dezvoltarea diabetului zaharat de tip 1 autoimun este \u00eenso\u0163it\u0103 de apari\u0163ia anticorpilor fa\u0163\u0103 de o varietate de antigene ale celulelor insulare pancreatice, inclusiv insulina1<\/sup>. La copiii diabetici ace\u015ftia se g\u0103sesc \u00een propor\u0163ie de 50-70%, pe c\u00e2nd la pacien\u0163ii adul\u0163i doar \u00eentr-un procent de 20-30%3<\/sup>. Odat\u0103 ce diabetul devine clinic manifest, nivelul anticorpilor anti-insulin\u0103 scade la titruri joase p\u00e2n\u0103 la nedetectabil.<\/p>\n

Pe de alt\u0103 parte, tratamentul diabetului zaharat presupune aproape \u00een mod invariabil (tipul 1) sau frecvent (tipul 2) administrarea insulinei. Anticorpi IgG pot ap\u0103rea \u015fi fa\u0163\u0103 de insulina administrat\u0103, de\u015fi folosirea aproape exclusiv\u0103 a insulinei umane recombinate face ca acest lucru s\u0103 se \u00eent\u00e2mple mai rar dec\u00e2t \u00een trecut c\u00e2nd era folosit\u0103 insulina de origine animal\u0103. La ace\u015fti pacien\u0163i, o parte din insulina injectat\u0103 este legat\u0103 de anticorpi \u015fi devine biologic neutilizat\u0103. \u00cen consecin\u0163\u0103 este nevoie de doze mari de insulin\u0103 datorit\u0103 acestei rezisten\u0163e induse de anticorpi2<\/sup>.<\/p>\n

Recomand\u0103ri pentru determinarea anticorpilor anti-insulin\u0103<\/em><\/strong><\/p>\n

\u2013\u00a0evaluarea riscului dezvolt\u0103rii unui diabet zaharat tip 1 la pacien\u0163ii predispu\u015fi genetic;<\/div>\n
\u2013\u00a0diagnosticul diferen\u0163ial \u00eentre diabetul de tip 1 \u015fi de tip 2;<\/div>\n
\u2013\u00a0evaluarea diabeticilor cu rezisten\u0163\u0103 la insulin\u0103.<\/div>\n

Preg\u0103tire pacient <\/em><\/strong>- <\/em>\u00e0 jeun (pe nem\u00e2ncate) \u2013 minim 7 ore4<\/sup>.<\/p>\n

Prelucrare necesar\u0103 dup\u0103 recoltare <\/em><\/strong>- <\/em>se separ\u0103 serul prin centrifugare; se lucreaz\u0103 serul proasp\u0103t; dac\u0103 acest lucru nu este posibil, serul se p\u0103streaz\u0103 la 2-8\u00b0C sau la -20\u00b0C4<\/sup>.<\/p>\n

Metod\u0103 <\/em><\/strong>- radioimunologic\u0103<\/span> (RIA<\/span> )4<\/sup>.<\/p>\n

Valori de referin\u0163\u0103 <\/em><\/strong>- \u00a0<\/em><\/strong><1 U\/mL4<\/sup>.<\/p>\n

Interpretarea rezultatelor<\/em><\/strong><\/p>\n

Un rezultat pozitiv indic\u0103 dezvoltarea autoimun\u0103 primar\u0103 de anticorpi anti-insulin\u0103 sau secundar\u0103 datorat\u0103 administr\u0103rii \u00eendelungate a insulinei. La pacien\u0163ii predispu\u015fi genetic la diabet zaharat (istoric familial de diabet zaharat sau HLA predispozant) detec\u0163ia diferi\u0163ilor autoanticorpi, inclusiv anticorpi anti-insulin\u0103, constituie un indicator de dezvoltare \u00een viitor a unui diabet tip 12<\/sup>.\u00a0<\/em><\/strong><\/p>\n

Limite \u015fi interferen\u0163e<\/em><\/strong><\/p>\n

Testul nu este recomandat pacien\u0163ilor care au primit recent radioizotopi \u00een scop terapeutic sau diagnostic, datorit\u0103 interferen\u0163elor posibile. O recomandare privind perioada de timp necesar\u0103 \u00eenaintea recolt\u0103rii nu se poate face, aceasta depinz\u00e2nd de izotop, de doz\u0103 \u015fi de pacient2<\/sup>.<\/p>\n
