Diabetul zaharat de tip 1 este caracterizat printr-o distrugere mediat\u0103 imunologic a celulelor beta pancreatice produc\u0103toare de insulin\u0103. \u00cen timp ce \u00een patogenez\u0103 au rol hot\u0103rator limfocitele T autoreactive, pentru diagnostic prezint\u0103 importan\u0163\u0103 autoanticorpii specifici. Riscul de \u00eemboln\u0103vire este influen\u0163at de predispozi\u0163ia genetic\u0103 (tiparul HLA), factorii de mediu \u015fi o disfunc\u0163ie a sistemului imun. Procesul autoimun fa\u0163\u0103 de celulele beta evolueaz\u0103 asimptomatic timp de luni sau ani, p\u00e2n\u0103 c\u00e2nd num\u0103rul celulelor devine insuficient pentru a men\u0163ine valoarea glicemiei \u00een limite normale.\u00a0<\/em><\/strong><\/p>\n

Autoanticorpii fa\u0163\u0103 de citoplasma\u00a0celulelor pancreatice\u00a0au fost descri\u015fi ini\u0163ial \u00een 1970. Sunt detecta\u0163i prin imunofluorescen\u0163\u0103 indirect\u0103 folosind ca substrat celule pancreatice umane de grup sanguin O. ICA sunt anticorpi policlonali care reac\u0163ioneaz\u0103 cu toate celulele insulare (de exemplu alfa, beta, gamma, delta \u015fi PP). Autoantigenele lipidice \u015fi proteice recunoscute de ICA includ \u015fi aloglicoconjugatul, GAD \u015fi IA2. <\/sub>La fel ca al\u0163i autoanticorpi insulari, nici ICA nu par s\u0103 joace un rol etiologic \u00een distrugerea celulelor beta, dar sunt markeri importan\u0163i de autoimunitate.<\/p>\n

La debutul diabetului de tip 1, se consider\u0103 c\u0103 70-90% dintre pacien\u0163i sunt ICA-pozitivi1,6<\/sup>. Frecven\u0163a acestora scade pe parcursul evolu\u0163iei bolii, astfel c\u0103 doar 5-10% din diabetici r\u0103m\u00e2n pozitivi dup\u0103 10 ani de la debut6<\/sup>.<\/p>\n

Recomand\u0103ri pentru determinarea anticorpilor anti-celule insulare pancreatice <\/em><\/strong><\/p>\n

\u2013\u00a0diferen\u0163ierea pacien\u0163ilor cu diabet de tip 1 de cei de tip 2;<\/div>\n
\u2013\u00a0evaluarea riscului de a dezvolta un diabet zaharat la rudele de gradul I ale unui diabetic;<\/div>\n
\u2013\u00a0evaluarea riscului la pacientele cu diabet gesta\u0163ional2;4;5<\/sup>.<\/div>\n

Preg\u0103tire pacient <\/em><\/strong>- <\/em>\u00e0 jeun (pe nem\u00e2ncate) \u2013 minim 7 ore4<\/sup>.<\/p>\n

Interpretarea rezultatelor<\/em><\/strong><\/p>\n

Frecven\u0163a anticorpilor anti-celule insulare pancreatice \u00een popula\u0163ia general\u0103 este sc\u0103zut\u0103 (0.59% conform unor studii6<\/sup>) \u015fi, de asemenea, la pacien\u0163ii cu diabet tip 2. Prezen\u0163a lor confirm\u0103 autoimunitatea \u00een cazurile de diabet la debut \u015fi pot diferen\u0163ia diabetul de tip 1 de cel de tip 2: pacien\u0163ii cu anticorpi anti-celule insulare pancreatice sunt \u00eencadra\u0163i ca av\u00e2nd diabet tip 1. Totu\u015fi, absen\u0163a lor nu exclude diabetul de tip 1. Un titru mai mare ar fi asociat cu un risc mai mare de \u00eemboln\u0103vire6<\/sup>. De asemenea num\u0103rul anticorpilor pozitivi ar fi mai important dec\u00e2t combina\u0163ia lor6<\/sup>.<\/p>\n

Limite \u015fi interferen\u0163e<\/em><\/strong><\/p>\n

Anticorpii anti-celule insulare pancreatice pot fi g\u0103si\u0163i \u015fi \u00een alte boli autoimune (artrita reumatoid\u0103, boli tiroidiene autoimune). De\u015fi \u00een trecut determinarea anticorpilor anti-celule insulare pancreatice reprezenta \u201estandardul de aur\u201d, tehnica este dificil de standardizat datorit\u0103 variabilit\u0103\u0163ii \u0163esuturilor pancreatice\u00a0de la donatori diferi\u0163i7<\/sup>.<\/p>\n
