Streptococii de grup A determina un spectru larg de infectii, cel mai frecvent faringita si amigdalita, dar si variate\u00a0 infectii cutanate si sistemice (impetigo, limfangita, celulita, fasciita necrotizanta, scarlatina, sepsis puerperal). In absenta tratamentului antibiotic adecvat se pot dezvolta complicatii nesupurative cum ar fi reumatismul articular acut (febra reumatismala) si glomerulonefrita poststreptococica5<\/sup>.<\/p>\n

Complicatiile apar dupa o perioada de latenta in care pacientul este asimptomatic; aceasta perioada variaza, fiind de aproximativ 10 zile pentru glomerulonefrita si 20 de zile pentru reumatismul articular acut. Glomerulonefrita poate sa apara atat dupa faringita streptococica cat si dupa infectii cutanate fiind asociata cu anumite serotipuri nefritogene<\/em>: M2, M12, M57, M59, M60. Febra reumatismala apare ca o complicatie a faringitei streptococice, fiind asociate cu serotipuri reumatogene<\/em>: M1, M3, M5, M6, M18 si M194<\/sup>.<\/p>\n

In culturile streptococilor de grup A au fost identificate cateva antigene bacteriene; acesti produsi extracelulari sunt in principal enzime si includ streptolizina O, streptokinaza, deoxiribonucleaze, hialuronidaza, nicotinamid-adenin nucleotidaza. Streptodornaza \u2013 deoxiribonucleaza streptococica \u2013 este o enzima ce depolimerizeaza ADN-ul; s-au identificat 4 tipuri antigenice (A, B, C, D), tipul B fiind produs in cantitate mare de Streptococcus pyogenes4;5<\/sup>.<\/p>\n

Testele care detecteaza anticorpii fata antigenele streptococice sunt utile pentru documentarea unei infectii recente la pacientii suspectati de febra reumatismala sau glomerulonefrita postreptococica. Cel mai cunoscut este testul ASLO: anticorpii anti-streptolizina O sunt produsi in 75-80% din cazurile de infectii streptococice, dar nu sunt prezenti de obicei in infectiile cutanate. Nivelurile anticorpilor devin detectabile in 1-2 saptamani de la faringita acuta si ating un maximum dupa 3-6 saptamani; daca nivelurile ridicate persista dupa 6 saptamani sau chiar se inregistreaza o crestere a acestora se va suspecta o reinfectie. Testul ASLO detecteaza si infectiile cu streptococi de grup C si G, care produc de asemenea streptolizina O. Anticorpii anti-DNA-aza B apar aproape in exclusivitate in infectiile cauzate de streptococi de grup A\u00a0 cu cateva zile mai devreme decat anticorpii fata de streptolizina O si persista 2-3 luni dupa infectie. Acest tip de anticorpi se dezvolta la 85-90% dintre pacientii cu infectii cutanate1;5<\/sup>.<\/p>\n

Desi testul ASLO are o acuratete buna, este justificata determinarea concomitenta a anticorpilor anti-DNA-aza B cel putin din doua motive. In primul rand niveluri crescute de anti-streptolizina O se inregistreaza doar la 85% dintre pacientii cu febra reumatismala; in cazul in care se utilizeaza ambele teste \u00a090-95% dintre pacienti vor avea un rezultat pozitiv la cel putin unul dintre teste. In al doilea rand, infectiile streptococice cutanate se asociaza cu un raspuns imun atenuat fata de streptolizina O, astfel ca pacientii cu glomerulonefrita dezvoltata post-impetigo pot sa nu prezinte niveluri ASLO crescute, fiind necesari anticorpii anti-DNA-aza B pentru demonstrarea infectiei din antecedente4<\/sup>.<\/p>\n

In plus, avand in vedere \u00a0faptul ca anticorpii anti-streptodornaza persista mai mult timp in ser, acest test este util si in diagnosticul coreeei reumatismale, complicatie care apare dupa o perioada de latenta mai mare de la infectia streptococica1<\/sup>.<\/p>\n

Recomandari pentru determinarea<\/em><\/strong> anticorpilor anti-streptodornaza (anti DNA-aza) B <\/em><\/strong>-<\/em>diagnosticul infectiilor streptococice recente1;3;4;5<\/sup>.<\/p>\n

Interpretarea rezultatelor<\/em><\/strong><\/p>\n

Determinarea anticorpiilor anti-streptodornaza este o metoda sensibila in diagnosticul glomerulonefritei post-streptococice si in infectiile cutanate grave, cum este de exemplu fasciita necrozanta.
\nCand ASLO si anticorpii anti-DNA-aza B sunt efectuate in acelasi timp, 95% din infectiile streptococice anterioare sunt detectate. In cazul in care ambele teste sunt negative in mod repetat, simptomele pacientului nu sunt cauzate de o boala poststreptococica1;4;5<\/sup>.<\/p>\n

Limite si interferente<\/em><\/strong><\/p>\n

Tratamentul antibiotic reduce nivelul anticorpilor anti-DNA-aza B1<\/sup>.<\/p>\n
