{"id":10080,"date":"2020-04-23T03:56:01","date_gmt":"2020-04-23T00:56:01","guid":{"rendered":"https:\/\/www.synevo.ro\/product\/bartonella-henselae-serologie\/"},"modified":"2025-02-05T15:57:39","modified_gmt":"2025-02-05T13:57:39","slug":"bartonella-henselae-serologie","status":"publish","type":"product","link":"https:\/\/old.synevo.md\/ru\/shop\/bartonella-henselae-serologie\/","title":{"rendered":"Bartonella henselae - \u0441\u0435\u0440\u043e\u043b\u043e\u0433\u0438\u044f"},"content":{"rendered":"<p><strong><em>\u041e\u0431\u0449\u0430\u044f \u0438\u043d\u0444\u043e\u0440\u043c\u0430\u0446\u0438\u044f <\/em><\/strong><\/p>\n<p><em>Bartonella henselae<\/em> este un bacil gram negativ mic, mobil, aerob, ce se transmite la om prin \u00a0inoculare (zgarietura sau muscatura pisicilor) sau in urma contactului cu saliva sau secretiile animalului, la nivelul unor leziuni cutanate preexistente.<\/p>\n<p>Boala ghearelor de pisica (boala zgarieturilor de pisica) se caracterizeaza printr-o leziune cutanata primara, limfadenopatie locala, febra si evolutie autolimitanta.<\/p>\n<p>Initial considerata o afectiune rara, este acum o cauza comuna a limfadenopatiei la copiii si adultii tineri. 80% dintre copiii cu varste intre 3 si 12 ani prezinta anticorpi anti-<em>Bartonella<\/em> in ser.<\/p>\n<p>Dupa o perioada de incubatie de 3-10 zile (2 zile \u2013 8 saptamani), la locul inoculului apar leziuni maculo-papulare pruriginoase, care se transforma ulterior in vezicule, pustule si apoi cruste care se detaseaza in cateva zile. Dupa 1-3 saptamani, manifestarile cutanate sunt insotite de limfadenopatie regionala ce poate cuprinde unul sau mai multi ganglioni limfatici. Mai frecvent afectati sunt ganglionii situati axilar si epitrohlear (46%),\u00a0 cervical, submandibular (26%), urmati de cei femurali si inghinali (17,5%), preauriculari (7%) si claviculari (2%). In 10-20% din cazuri, limfadenoptia poate fi generalizata. La persoanele imunocompetente, boala se remite spontan in decurs de 2-4 luni si doar in cazuri rare poate avea o evolutie cronica, ce dureaza pana 2 ani.<\/p>\n<p>Leziunile loco-regionale sunt adesea insotite de manifestari generale: febra, frisoane, cefalee, mialgii, artralgii sau fenomene digestive.<\/p>\n<p>In 5-25% din cazuri boala ghearelor de pisica se manifesta prin forma atipice:<\/p>\n<ul>\n<li><span style=\"text-decoration: underline;\">sindromul oculoglandular Parinaud<\/span>, caracterizat prin conjunctivita granulomatoasa si limfadenopatie preauriculara;<\/li>\n<li><span style=\"text-decoration: underline;\">neuroretinita <\/span>\u2013 pierderea vederii unilateral, fara durere, cu scotoame centrale si refacere dupa 1-3 luni;<\/li>\n<li><span style=\"text-decoration: underline;\">encefalopatie acuta<\/span> \u2013 cefalee cu progresie rapida spre letargie si coma. Evolutia este, in general, autolimitanta, fara sechele neurologice;<\/li>\n<li><span style=\"text-decoration: underline;\">endocardita;<\/span><\/li>\n<li><span style=\"text-decoration: underline;\">hepatita granulomatoasa;<\/span><\/li>\n<li><span style=\"text-decoration: underline;\">granulomul splenic;<\/span><\/li>\n<li><span style=\"text-decoration: underline;\">leziuni osteolitice;<\/span><\/li>\n<li><span style=\"text-decoration: underline;\">eruptii cutanate, eritem nodos si vasculita.<\/span><\/li>\n<\/ul>\n<p>La pacientii imunocompromisi\u00a0 sever, infectia cu <em>Bartonella henselae<\/em> se manifesta cu febra\u00a0 persistenta sau recidivanta cu bacteriemie, angiomatoza bacilara sau pieloza bacilara.<\/p>\n<p><em>Angiomatoza bacilara <\/em>este o boala vasculoproliferativa ce afecteaza orice organ, dar care apare mai frecvent la nivelul pielii si a tesutului subcutanat. Se manifesta prin numeroase leziuni cutanate superficiale sau profunde, de dimensiuni variate, ce se prezinta ca noduli sau papule eritematoase sau petesiale. La nivel visceral pot apare microabcese hepatice sau splenice, leziuni litice ale maduvei, care confera un prognostic nefavorabil.<\/p>\n<p>Leziunile vasculare se caracterizeaza prin proliferare la nivelul vaselor mici, cu aparitia unor celule endoteliale cuboidale, separate de o stroma edematoasa, mucinoasa\u00a0 sau fibroasa,\u00a0 care contine neutrofile si microcolonii granulare de <em>Bartonella<\/em>.<\/p>\n<p>Diagnosticul pozitiv al infectiei cu <em>Bartonella henselae<\/em> se stabileste pe baza datelor epidemiologice, clinice si examenelor serologice<sup>1;2;3<\/sup>.<\/p>\n<p><em><strong>Recomandari pentru determinarea anticorpilor anti- Bartonella <\/strong>- <\/em>diagnosticul rapid al infectiei cu <em>Bartonella<\/em>, in special in contextul prezentei unor zgarieturi de pisica in antecedente<sup>4<\/sup>.<\/p>\n<p><strong><em>Pregatire pacient <\/em><\/strong>\u2013 preferabil \u00e0 jeun (pe nemancate)<sup>3<\/sup>.<\/p>\n<p><strong><em>Specimen recoltat <\/em><\/strong>\u2013 sange venos<sup>3<\/sup>.<\/p>\n<p><strong><em>Recipient de recoltare <\/em><\/strong>\u2013 vacutainer fara anticoagulant cu\/fara gel separator<sup>3<\/sup>.<\/p>\n<p><strong><em>Prelucrare necesara dupa recoltare <\/em><\/strong>\u2013 se separa serul prin centrifugare<sup>3<\/sup>.<\/p>\n<p><strong><em>Volum proba <\/em><\/strong>\u2013 minim 2 mL ser<sup>3<\/sup>.<\/p>\n<p><strong><em>Cauze de respingere a probei <\/em><\/strong>\u2013\u00a0 specimen intens hemolizat, hemolitic sau contaminat bacterian<sup>3<\/sup>.<\/p>\n<p><strong><em>Stabilitate proba <\/em><\/strong>- <em>2 saptamani la 2-8\u00b0C<\/em>; <em>timp indelungat<\/em> la \u2013<em>20\u00b0C<\/em>; evitati decongelarea\/recongelarea repetata<sup>3<\/sup>.<\/p>\n<p><strong><em>Metoda <\/em><\/strong>- <span style=\"text-decoration: underline;\">imunofluorescenta indirecta IFA<\/span><sup>3<\/sup>.<\/p>\n<p><strong><em>Valori de referinta<\/em><\/strong><\/p>\n<p style=\"padding-left: 30px;\">Bartonella IgM: Negativ<\/p>\n<p style=\"padding-left: 30px;\">Bartonella IgG: Negativ<sup>3<\/sup>.<\/p>\n<p><strong><em>Interpretarea rezultatelor<\/em><\/strong><\/p>\n<p><strong><span style=\"text-decoration: underline;\">IgM <\/span><\/strong><\/p>\n<p style=\"padding-left: 30px;\">Prezenta anticorpilor IgM sugereaza un diagnostic clinic de infectie recenta cu <em>Bartonella henselae<\/em>.<\/p>\n<p style=\"padding-left: 30px;\">Concentratii scazute de anticorpi IgM pot persista, ocazional, mai mult de 12 luni postinfectie.<\/p>\n<p style=\"padding-left: 30px;\">Absenta anticorpilor IgM ne indica lipsa infectiei acute cu <em>Bartonella henselae<\/em><sup>3<\/sup>.<\/p>\n<p><strong><span style=\"text-decoration: underline;\">,<\/span><\/strong><\/p>\n<p style=\"padding-left: 30px;\">Diagnosticul serologic al infectiei cu <em>Bartonella<\/em> \u00a0necesita identificarea unei seroconversii de la negativ la pozitiv intre faza acuta si convalescenta.<\/p>\n<p style=\"padding-left: 30px;\">Absenta anticorpilor IgG sugereaza lipsa unui raspuns imun la antigene bacteriene specifice<sup>2;4<\/sup>.<\/p>\n<p><span style=\"font-size: small;\">\u00a0<\/span><\/p>\n<p><span style=\"font-size: small;\">\u0411\u0438\u0431\u043b\u0438\u043e\u0433\u0440\u0430\u0444\u0438\u044f<\/span><\/p>\n<p><span style=\"font-size: small;\">1.
