Informa\u0163ii generale \u015fi recomand\u0103ri pentru efectuarea testului<\/em><\/strong><\/p>\n

Boala Crohn este o afec\u0163iune inflamatorie cronic\u0103 sever\u0103 a tractului gastro-intestinal, de etiologie necunoscut\u0103, ce afecteaz\u0103 peste 1 milion de pacien\u0163i \u00een Statele Unite \u015fi Europa. Tabloul clinic \u00een boala Crohn este polimorf, condi\u0163ionat de factori precum: topografia leziunilor, intensitatea manifest\u0103rilor inflamatorii, asocierea unor complica\u0163ii intestinale (stenoze, fistule) sau extraintestinale1<\/sup>. Fiecare caz are un aspect cu totul particular.<\/p>\n

Diareea, durerile abdominale \u015fi sc\u0103derea ponderal\u0103 sunt semnele comune ale bolii. Simptomele sistemice \u00eent\u00e2lnite sunt febra, anorexia, letargia \u015fi corespund perioadelor de activitate a bolii2<\/sup>.<\/p>\n

Diagnosticul diferen\u0163ial cu celelalte boli inflamatorii cronice intestinale, \u00een special cu rectocolita ulcero-hemoragic\u0103, este dificil din cauza simptomatologiei asem\u0103n\u0103toare. Exist\u0103 mai multe metode neinvazive ce pot fi utilizate \u00een diagnosticul diferen\u0163ial. Diagnosticul serologic cuprinde o list\u0103 \u00eentreag\u0103 de anticorpi a c\u0103ror importan\u0163\u0103 este diferit\u0103 \u00een precizarea diagnosticului.<\/p>\n

Anticorpii anti-Saccharomyces cerevisiae se asociaz\u0103 cu boala Crohn av\u00e2nd o sensibilitate de 64% \u015fi o specificitate de 77%.<\/p>\n

ASCA au fost g\u0103si\u0163i la 20% dintre rudele de gradul I ale pacien\u0163ilor cu boal\u0103 Crohn, suger\u00e2nd c\u0103 ar putea fi un marker subclinic intrafamilial.<\/p>\n

Anticorpii anti-Saccharomyces cerevisiae au drept \u0163int\u0103 antigenic\u0103 un poliozid derivat de manoz\u0103 extras din levuri. Cauza prezen\u0163ei ASCA \u00een boala Crohn poate fi imunizarea contra levurilor endogene sau alimentare sau reactivitatea \u00eencruci\u015fat\u0103 cu peretele celular al altor microorganisme sau cu glico-conjuga\u0163i umani ce reac\u0163ioneaz\u0103 ca autoantigene1<\/sup>.<\/p>\n

ASCA reprezint\u0103 de asemenea un marker pentru evolu\u0163ia \u015fi prognosticul bolii. Pacien\u0163ii cu ASCA pozitiv prezint\u0103 risc crescut pentru formele stenozante \u015fi fistulizante, \u00een timp ce pacien\u0163ii cu ASCA negativ prezint\u0103 frecvent forme inflamatorii.<\/p>\n

Determinarea concomitent\u0103 a anticorpilor pANCA \u015fi ASCA permite diferen\u0163ierea pacien\u0163ilor cu rectocolit\u0103 ulcero-hemoragic\u0103 de cei cu boala Crohn \u015fi precizarea tratamentului3<\/sup>. \u00cen plus, studiile efectuate de Lawrence \u015fi colaboratorii s\u0103i au ar\u0103tat c\u0103 testarea combinat\u0103 a anticorpilor anti-acini pancreatici, ASCA \u015fi pANCA permite o diferen\u0163iere mai bun\u0103 a colitei ulceroase de boala Crohn, \u00een special \u00een forma colonic\u0103 izolat\u0103, \u00een compara\u0163ie cu testarea individual\u0103 ASCA \u015fi pANCA4<\/sup>.<\/p>\n

Un rezultat p-ANCA negativ asociat cu niveluri crescute pentru ASCA (oricare din cele trei tipuri de anticorpi) are o valoare predictiv\u0103 pozitiv\u0103 pentru boala Crohn de 90%.<\/div>\n
Aproximativ 40% din pacien\u0163ii cu boal\u0103 Crohn au niveluri crescute at\u00e2t pentru ASCA IgA c\u00e2t \u015fi pentru ASCA IgG6<\/sup>.<\/div>\n
