Pentru descrierea mecanismelor de modulare a acestor func\u0163ii sunt vizate \u00een principal cele 3 interleukine de tip 1 care se pot lega de 2 tipuri de receptori: citokinele IL-1\u03b1, IL-1b \u015fi IL-1RA (din familia receptorilor de tip IL-1\/Toll-like receptor) \u015fi receptorii IL-1RI \u015fi IL-1RII. Caracteristicile func\u0163ionale ale acestor citokine \u015fi receptori difer\u0103, determin\u00e2nd consecin\u0163e variate, necesare modul\u0103rii efectelor, astfel:<\/p>\n<ul>\n<li style=\"text-align: justify; padding-left: 30px;\">IL-1\u03b1 \u015fi IL-1b sunt citokinele active biologic, cu structuri \u015fi roluri similare, reglate ins\u0103 diferit;<\/li>\n<li style=\"text-align: justify; padding-left: 30px;\">IL-1RA (<em>engl<\/em>. IL-1 receptor antagonist) este, \u00een fapt, un antagonist natural al receptorului pentru IL-1;<\/li>\n<li style=\"text-align: justify; padding-left: 30px;\">IL-1\u03b1 se poate lega doar de IL-1RI, iar IL-1b \u015fi IL-1RA se pot lega de ambii receptori;<\/li>\n<li style=\"text-align: justify; padding-left: 30px;\">receptorul IL-1RII (67 kDa), care <strong>nu<\/strong> transmite semnale intracelulare, este considerat un receptor \u201ecapcan\u0103\u201d (<em>engl. \u201e<\/em>decoy\u201d). De IL-1RII se pot lega doar IL-1b \u015fi IL-1RA;<\/li>\n<li style=\"text-align: justify; padding-left: 30px;\">doar receptorul IL-1RI (80 kDa) transmite semnale intracelulare. De receptorul IL-1RI se pot lega toate cele 3 citokine IL-1 \u015fi transmite semnale intracelulare primite de la IL-1\u03b1 sau IL-1b<sup>7;8;9<\/sup>.<\/li>\n<\/ul>\n<p style=\"text-align: justify;\">Ata\u015farea IL-1\u03b1 sau IL-1b de IL-1RI determin\u0103 asocierea receptorului cu o protein\u0103 accesorie (IL-1RI RAcP), critic\u0103 pentru inducerea semnalului ce determin\u0103 activarea factorilor nucleari de transcrip\u0163ie, prin intermediul unor proteine MAP (<em>engl,<\/em> macrotubule associate proteins) asociate c\u0103ii kinazice<sup>8<\/sup>.<\/p>\n<p style=\"text-align: justify;\">Receptorul activ, IL-1RI, este larg r\u0103sp\u00e2ndit, spre deosebire de IL-1RII care se afl\u0103 pe suprafa\u0163a doar a c\u00e2torva tipuri celulare (neutrofile, monocite, celule B). Prin captarea \u015fi sechestrarea celor 2 citokine active (IL-1b si IL-1RA) receptorul IL-1RII are rol antiinflamator<sup>7<\/sup>. Ambii receptori pot fi cliva\u0163i, cu apari\u0163ia rapid\u0103 \u00een circula\u0163ie a receptorilor proteici solubili (un alt mecanism tampon al semnaliz\u0103rii induse de citokinele IL-1)<sup>9<\/sup>.<\/p>\n<p style=\"text-align: justify;\">Ambele citokine active biologic (IL-1\u03b1 sau IL-1b) sunt sintetizate sub form\u0103 de precursori proteici, activi doar \u00een cazul pre-IL-1\u03b1; precursorul pre-IL-1b r\u0103m\u00e2ne \u00een citoplasm\u0103 p\u00e2n\u0103 la clivarea sa de c\u0103tre caspaza 1 (<em>engl.<\/em> IL-1b converting enzyme, ICE)<sup>7;9<\/sup>.<\/p>\n<p style=\"text-align: justify;\">Sursa major\u0103 a citokinelor IL-1 este reprezentat\u0103 de linia celulelor fagocitare mononucleare (macrofage), de\u015fi pot fi sintetizate de asemenea \u00een multe alte celule (celule endoteliale, keratinocite, osteoblaste, neutrofile, celule gliale). Agen\u0163ii stimulan\u0163i ai secre\u0163iei de IL-1 sunt reprezenta\u0163i de endotoxine, alte citokine, microorganisme, antigene variate<sup>7<\/sup>.<\/p>\n<p style=\"text-align: justify;\">Produc\u0163ia de IL-1\u00a0\u00een cursul r\u0103spunsului imun este responsabil\u0103 de spectrul manifest\u0103rilor clinice asociate \u201est\u0103rii de boal\u0103\u201d, precum febra, letargia, somnolen\u0163a \u015fi anorexia (urmare a ac\u0163iunii IL-1 asupra sistemului nervos central)<sup>7;8<\/sup>.<\/p>\n<p style=\"text-align: justify;\">La nivelul hepatocitului, IL-1 stimuleaz\u0103, ca \u015fi IL-6, produc\u0163ia reactan\u0163ilor de faz\u0103 acut\u0103 (peptidul amiloid, proteina C reactiva, complementul), pe baza sechestr\u0103rii locale a altor proteine (scade \u00een special eliberarea seric\u0103 a albuminei)<sup>7;8<\/sup>. Pentru a ob\u0163ine aminoacizii necesari sintezei masive a acestor noi polipeptide, IL-1 favorizeaz\u0103, de asemenea, distruc\u0163ia proteinelor musculare, resim\u0163it\u0103 ca mialgia ce acompaniaz\u0103 unele boli<sup>7;8;9<\/sup>.<\/p>\n<p style=\"text-align: justify;\">Citokinele IL-1 stimuleaz\u0103 r\u0103spunsurile imune de tip celular \u015fi umoral ac\u0163ion\u00e2nd at\u00e2t asupra limfocitelor T, via IL-2 (IL-1 stimuleaz\u0103 produc\u0163ia de IL-2 \u015fi cre\u015fterea expresiei receptorului pentru IL-2), c\u00e2t \u015fi asupra limfocitelor B (IL-1 stimuleaz\u0103 proliferarea limfocitelor B \u015fi produc\u0163ia de imunoglobuline)<sup>8<\/sup>.<\/p>\n<p style=\"text-align: justify;\">De asemenea este favorizat\u0103 acumularea locala de leucocite prin cre\u015fterea expresiei moleculelor de adeziune la nivelul endoteliului vascular<sup>8<\/sup>.<\/p>\n<p style=\"text-align: justify;\">Rolul proinflamator al citokinelor IL-1 este dat de stimularea metabolismului acidului arahidonic, cu produc\u0163ia \u015fi eliberarea consecutiv\u0103 a altor citokine (mai ales IL6 si chemokine). De asemenea, IL-1 are efecte procoagulante<sup>9<\/sup> \u015fi de stimulare a hematopoiezei (efect sinergic cu factori de cre\u015ftere locali)<sup>8;9<\/sup>.<\/p>\n<p style=\"text-align: justify;\">Citokinei IL-1 i se atribuie, pe de o parte, un rol \u00een promovarea \u015fi proliferarea unor <em>linii celulare leucemice<\/em>, iar pe de alt\u0103 parte, efecte citotoxice directe asupra unor celule infectate viral \u015fi asupra unor celule tumorale<sup>8<\/sup>.<\/p>\n<p style=\"text-align: justify;\">Inducerea sintezei de NO (monoxid de azot) de c\u0103tre celulele endoteliale reprezint\u0103 mecanismul prin care IL-1 contribuie la vasodilata\u0163ia \u015fi hipotensiunea arterial\u0103 din <em>\u015focul septic <\/em>\u015fi cel<em> cardiogen<\/em><sup>7;8<\/sup>.<\/p>\n<p style=\"text-align: justify;\">La nivelul spa\u0163iului intraarticular, IL-1 promoveaz\u0103 proliferarea sinoviocitelor, formarea depozitelor de colagen, resorb\u0163ia cartilajului \u015fi a \u0163esutului osos, condi\u0163ii asociate <em>artritei reumatoide<\/em><sup>8<\/sup>.<\/p>\n<p style=\"text-align: justify;\">De asemenea exist\u0103 dovezi privind contribu\u0163ia IL-1b la destruc\u0163ia celulelor b pancreatice \u00een ambele tipuri de <em>diabet zaharat<\/em><sup>3;6<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><strong><em>Recomand\u0103ri pentru determinarea <\/em><\/strong><strong><em>IL-1<\/em><\/strong><strong><em>b<\/em><\/strong><strong><em>\u00a0<\/em><\/strong><\/p>\n<p style=\"text-align: justify;\">Titrurile interleukinelor determinate \u00een diverse lichide biologice pot fi utilizate \u00een diagnosticarea unor afec\u0163iuni imune \u015fi \u00een monitorizarea tratamentelor, doar \u00een corela\u0163ie cu date clinice \u015fi paraclinice complementare.<\/p>\n<p style=\"text-align: justify;\"><strong><em>Preg\u0103tire pacient <\/em><\/strong>\u2013 \u00e0 jeun (pe nem\u00e2ncate) sau postprandial<sup>5<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><strong><em>Specimen recoltat <\/em><\/strong>\u2013 s\u00e2nge venos<sup>5<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><strong><em>Recipient de recoltare <\/em><\/strong><em>- <\/em>vacutainer f\u0103r\u0103 anticoagulant, cu\/f\u0103r\u0103 gel separator<sup>5<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><strong><em>Cantitate recoltat\u0103 <\/em><\/strong><em>- <\/em>minim 0.5 mL ser<sup>5<\/sup>.<\/p>\n<p style=\"text-align: justify;\"><strong><em>Cauze de respingere a probei<\/em><\/strong> \u2013 specimen intens hemolizat, icteric, lipemic sau contaminat bacterian;
