Anticorpii anti-trombocitari pot fi autoimuni <\/strong>(\u00eendrepta\u0163i \u00eempotriva antigenelor trombocitare proprii) sau alloimuni<\/strong> (\u00eendrepta\u0163i \u00eempotriva antigenelor trombocitare exogene, de ex.\u00a0\u00een cazul femeilor gravide sau transfuziilor). Autoanticorpii<\/strong> reac\u0163ioneaz\u0103 aproape \u00eentotdeauna cu determinan\u0163i monomorfi, trombocitele autologe reprezent\u00e2nd cel mai bun material pentru detectarea autoanticorpilor. Alloanticorpii <\/strong>reac\u0163ioneaz\u0103 cu determinan\u0163i antigenici polimorfi localiza\u0163i pe suprafa\u0163a trombocitelor; ace\u015ftia nu reac\u0163ioneaz\u0103 cu trombocitele autologe.<\/span><\/p>\n

Anticorpii antitrombocitari pot fi \u00eendrepta\u0163i fa\u0163\u0103 de un num\u0103r de \u0163inte antigenice aflate pe membrana plasmatic\u0103 trombocitar\u0103, acestea put\u00e2nd fi antigene HLA clasa I sau antigene apar\u0163in\u00e2nd glicoproteinelor trombocitare specifice: GPIIb\/IIIa, GPIb\/IX \u015fi GPIa\/IIa.3<\/sup> Complexul glicoproteic IIb\/IIIa\u00a0de\u0163ine un rol important \u00een adezivitatea trombocitar\u0103, leg\u00e2nd fibrinogenul, fibronectina, vitronectina \u015fi factorul von Willebrand. Glicoproteina Ib\/IX este principalul receptor pentru factorul von Willebrand, iar glicoproteina Ia\/IIa este implicat\u0103 \u00een adeziunea colagenului2<\/sup>.<\/span><\/p>\n

Cele mai frecvente sunt antigenele HPA-1a\/b p\u00e2n\u0103 la HPA-5a\/b, celelalte fiind prezente foarte rar \u00een popula\u0163ie.<\/span><\/p>\n

Condi\u0163ii clinice asociate cu prezenta anticorpilor anti-trombocitari<\/em><\/strong><\/span><\/p>\n

1 \u2013 Purpura trombocitopenic\u0103 autoimun\u0103 (PTI) acut\u0103 \u015fi cronic\u0103<\/em> \u2013 este o condi\u0163ie caracterizat\u0103 prin produc\u0163ia de autoanticorpi care distrug trombocitele proprii. Majoritatea anticorpilor recunosc glicoproteine membranare plachetare, \u00een special GPIIb\/IIIa, GPIb\/IX \u015fi GPIa\/IIa. Anticorpii care recunosc aceste \u0163inte pot fi detecta\u0163i cu o frecven\u0163\u0103 redus\u0103 \u015fi\u00a0\u00een plasma\/serul pacien\u0163ilor cu PTI, dar la ace\u015fti pacien\u0163i diagnosticul de certitudine const\u0103 \u00een detectarea autoanticorpilor lega\u0163i de trombocitele proprii, respectiv\u00a0\u00een depistarea anticorpilor \u00eentr-un eluat preparat din trombocitele autologe, testul av\u00e2nd o sensibilitate de 53% \u015fi o specificitate de 72% pentru diagnosticul de PTI, conform unui studiu recent3<\/sup>. Totu\u015fi testul nu poate fi efectuat cand trombocitopenia este sever\u0103 (<10000\/\u00b5L), autoanticorpii liberi put\u00e2nd fi detecta\u0163i \u00een aceste cazuri.<\/span><\/p>\n

Un num\u0103r mic de pacien\u0163i pot prezenta autoanticorpi fa\u0163\u0103 de determinan\u0163ii func\u0163ionali localiza\u0163i mai ales pe GP IIb\/IIIa, ceea ce se traduce prin afectare func\u0163ional\u0103 important\u0103 \u00een prezen\u0163a unui num\u0103r normal de trombocite, iar la unii din ace\u015fti pacien\u0163i s-a observat tranzi\u0163ia spre PTI clasic.<\/span><\/p>\n

a) <\/em>PTI acut\u0103:<\/em> trombocitopenia, adesea sever\u0103 (<10000 trombocite\/microlitru), debuteaz\u0103 acut, la 10-14 zile dup\u0103 o infec\u0163ie viral\u0103; este complet reversibil\u0103, afect\u00e2nd cel mai frecvent copii cu v\u00e2rsta sub 10 ani. Clinic se prezint\u0103 cel mai adesea cu petesii sau cu s\u00e2nger\u0103ri posttraumatice, hemoragia cerebral\u0103 este rar \u00eent\u00e2lnit\u0103. PTI acut\u0103 poate ap\u0103rea ocazional \u015fi dup\u0103 vaccinare cu o inciden\u0163\u0103 de 1:25000 de copii\/an.\u00a0<\/em><\/span><\/p>\n

b) <\/em>PTI cronic:<\/em> cu debut la orice v\u00e2rst\u0103; poate fi idiopatic\u0103 sau secundar\u0103 unor afec\u0163iuni cum ar fi bolile autoimmune (SLE) sau bolile maligne hematologice (leucemia limfatic\u0103 cronic\u0103); sunt afectate mai frecvent femeile; se caracterizeaz\u0103 prin trombocitopenie sever\u0103 care rareori se remite spontan.<\/span><\/p>\n

2 \u2013 Trombocitopenia alloimuna neonatal\u0103 (NAIT):<\/em> este echivalentul pentru trombocite a bolii hemolitice a nou-n\u0103scutului (HDN), fiind cea mai frecvent\u0103 cauz\u0103 de trombocitopenie sever\u0103 la nou-n\u0103scu\u0163ii la termen ce nu se asociaz\u0103 cu sepsis, infec\u0163ii sau boli hematologice maligne. Prevalen\u0163a este de 1:1000 \u2013 1:2000 din totalul nou-n\u0103scu\u0163ilor2<\/sup>. Spre deosebire de HDN, NAIT afecteaz\u0103 \u00een mod egal at\u00e2t primul n\u0103scut c\u00e2t \u015fi nou-n\u0103scu\u0163ii din sarcinile ulterioare1<\/sup>. Similar incompatibilit\u0103\u0163ii prin Rh, trombocitele materne sunt lipsite de un anumit determinant antigenic (2\u20133 % din femeile gravide sunt HPA-1a negative), care \u00eens\u0103 este prezent la f\u0103t, fiind mo\u015ftenit de la tat\u0103. Antigenele sunt prezente pe trombocite din s\u0103pt\u0103m\u00e2na 16 de gesta\u0163ie5<\/sup>. NAIT este rezultatul alloimunizarii materne fa\u0163\u0103 de antigenele trombocitare fetale2<\/sup>. Ca urmare alloanticorpii materni (mai ales de tip IgG) traverseaz\u0103 placenta determin\u00e2nd trombocitopenie \u015fi manifest\u0103ri hemoragice la f\u0103t\/nou-n\u0103scut \u00een primele ore de via\u0163\u0103, ce pot merge p\u00e2n\u0103 la hemoragie intracranian\u0103 (\u00een 10-25% din cazuri). In 85-90% din cazuri alloimunizarea matern\u0103 se face \u00eempotriva antigenului trombocitar HPA-1a (deteminant antigenic situat pe GP IIb\/IIIa), mamele fiind homozigote pentru alela alternativ\u0103 HPA-1b, urmat cu o fecven\u0163\u0103 mult mai mic\u0103 de anti-HPA-5b (situat pe GP Ia\/IIa) \u015fi de alloanticorpi fa\u0163\u0103 de determinan\u0163i antigenici de pe GP Ib\/IX1,2<\/sup>. In serul femeilor gravide au fost g\u0103si\u0163i \u015fi anticorpi anti-HLA, dar se pare c\u0103 ace\u015ftia nu sunt semnificativi pentru NAIT2<\/sup>.<\/span><\/p>\n

Trombocitopenia dispare spontan \u00een c\u00e2teva s\u0103pt\u0103m\u00e2ni, nu mai mult de 3 luni5<\/sup>. Num\u0103rul de trombocite materne se \u00eencadreaz\u0103 \u00een limite normale.<\/span><\/p>\n

3 \u2013 Trombocitopenia nou-n\u0103scutului<\/em> datorat\u0103 autoanticorpilor materni<\/em> se caracterizeaz\u0103 prin prezen\u0163a trombocitopeniei at\u00e2t la mam\u0103 c\u00e2t \u015fi la nou-n\u0103scut, mama prezent\u00e2nd PTI cronic.<\/span><\/p>\n

4 \u2013 Purpura posttransfuzional\u0103 (PTP)<\/em> \u2013 reac\u0163ie transfuzional\u0103 extrem de rar\u0103, ce apare aproape exclusiv la \u00a0femei de v\u00e2rsta medie sau v\u00e2rstnice, la aproximativ o s\u0103pt\u0103m\u00e2n\u0103 de la transfuzia de s\u00e2nge. Trombocitopenia poate fi sever\u0103, dar reversibil\u0103. Din motive care nu sunt clar \u00een\u0163elese trombocitele proprii sunt distruse o dat\u0103 cu trombocitele transfuzate. Sensibilizarea antigenic\u0103 s-a produs probabil prin expunerea la antigene plachetare exogene prin sarcini sau transfuzii anterioare3<\/sup>. Se caracterizeaz\u0103 prin detectarea \u00een titru mare de alloanticorpi anti-GP IIb\/IIIa \u015fi prin prezenta alloanticorpilor \u00een eluatul ob\u0163inut din trombocitele autologe.<\/span><\/p>\n

5 \u2013 Trombocitopenie refractar\u0103 la transfuzia<\/em> de trombocite \u2013<\/em> se caracterizeaz\u0103 prin reac\u0163ie febril\u0103 posttransfuzional\u0103 \u015fi de cre\u015fterea\u00a0 inadecvat\u0103 (sub nivelul a\u015fteptat) a num\u0103rului de trombocite. Aceast\u0103 condi\u0163ie este mai frecvent\u0103 la pacien\u0163i trata\u0163i pentru boli hematopoietice maligne. In afara produc\u0163iei de anticorpi anti-trombocitari, alte cauze posibile includ sepsisul, CID sau trombocitopenie indus\u0103 de medicamente. Trombocitopenia refractar\u0103 mediata imun este mai frecvent cauzat\u0103 de anticorpi fa\u0163\u0103 de antigenele HLA3<\/sup>.\u00a0La ace\u015fti pacien\u0163i detectarea anticorpilor liberi \u00een ser este util\u0103 pentru administrarea de concentrate trombocitare compatibile.<\/span><\/p>\n

6 \u2013 Trombocitopenie indus\u0103 de medicamente<\/em> \u2013 tip rar de trombocitopenie sever\u0103 ce se asociaz\u0103 cu dou\u0103 tipuri de anticorpi: anticorpi dependen\u0163i de medicament \u015fi autoanticorpi. Anticorpii dependen\u0163i de medicament reac\u0163ioneaz\u0103, ca \u015fi autoanticorpii, cu epitopi ai glicoproteinelor, \u00een special GP IIb\/IIIa sau GP Ib\/IX, situa\u0163i pe trombocitele autologe sau pe trombocitele provenite de la persoane s\u0103n\u0103toase, dar numai at\u00e2t timp c\u00e2t este prezent medicamentul incriminat. In unele cazuri \u00een serul pacien\u0163ilor cu trombocitopenie imun\u0103 indus\u0103 medicamentos au fost detecta\u0163i autoanticorpi a c\u0103ror reac\u0163ie cu trombocitele nu depinde de prezen\u0163a medicamentului (ex. \u00een cazul preparatelor cu aur).<\/span><\/p>\n

Medicamentele care se pot asocia cel mai frecvent cu trombocitopenie prin anticorpi dependen\u0163i de medicamente sunt urm\u0103toarele: quinidina, trimethoprim-sulfamethoxazol (anticorpi specifici fa\u0163\u0103 de metaboli\u0163ii medicamentului), quinina, diclofenac, carbamazepina, rifampicina, ibuprofen, nomifensin, ranitidina, paracetamol, vancomicina.<\/span><\/p>\n

7 \u2013 Trombocitopenia indus\u0103 de heparin\u0103<\/em> \u2013 trombocitopenie asociat\u0103 cu complica\u0163ii tromboembolice, ce apar \u00een cursul tratamentului cu heparin\u0103. Prevalen\u0163a este de 0.5\u20132% din totalul pacien\u0163ilor la care se administrez\u0103 heparina nefrac\u0163ionat\u0103 cu o durat\u0103 mai mare de 5 zile. Se caracterizeaz\u0103 prin prezen\u0163a de anticorpi ce reac\u0163ioneaz\u0103 cu epitopi ai complexului heparina\u2013factor 4 plachetar (PF4)2<\/sup>.<\/span><\/p>\n

Recomand\u0103ri pentru efectuarea anticorpilor anti-trombocitari<\/em><\/strong><\/span><\/p>\n

\u2013 Trombocitopenii care nu pot fi explicate prin afectarea trombocitopoiezei sau prin prezen\u0163a splenomegaliei.<\/span><\/div>\n

Anticorpi anti-trombocitari liberi<\/strong><\/span><\/p>\n

Testul detecteaz\u0103 anticorpii anti-HLA clasa I ca \u015fi anticorpii specifici anti-trombocitari fa\u0163\u0103 de complexele GP IIb\/IIIa (HPA-1a,1b,3a,3b,4a), GP Ib\/IX (HPA-2a,2b), GP Ia\/IIa (HPA-5a,5b) \u015fi GP IV4<\/sup>.<\/span><\/p>\n
